1.Surveillance report on healthcare associated infections in a tertiary children's hospital from 2019 to 2023

OBJECTIVE To get the current situation of healthcare associated infection(HAI)and implement targe-ted management through analyzing the trends of HAI incidence rate,the distribution of clinical departments,the sites of HAI and the composition of pathogens in a children's hospital in a five-year period.METHODS Data on HAI cases were collected from all inpatients in Children's Hospital of Soochow University from 2019 to 2023 through the real-time monitoring system of XINGLIN Healthcare acquired Infection Surveillance;Cochran-Armit-age test was used to check for the significant changes of distribution of HAI cases.RESULTS A total of 383,376 hospitalized patients were monitored from 2019 to 2023,of which HAI occurred on 6670 cases and 7209 case-times with an incidence rate of 1.74%and an incidence case rate of 1.88%.The top five departments of HAI inci-dence rates were cardiac,neonatal,surgical and pediatric intensive care unit and department of hematology.HAI mainly occurred in blood and alimentary system,upper and lower respiratory tract.A total of 2668 strains of path-ogenic bacteria were isolated,of which 1346 strains were gram-negative bacteria,1140 strains were gram-positive bacteria and 182 strains were fungi.The top three gram-negative bacteria were Klebsiella pneumoniae,Escherich-ia coli and Pseudomonas aeruginosa;the top three gram-negative pathogens were Streptococcus,Staphylococcus aureus and Staphylococcus epidermidis;and the top three fungi were Candida albicans,Candida parapsilosis and Candida tropicalis.CONCLUSIONS The HAI incidence rate of this hospital steadily declines in the past five years,however the same changes are not observed in the departments with high incidence of HAI.Attention should be paid to the raising bloodstream infections and detection rate of S.epidermidis.

3.Observation on Therapeutic Effects of Acupuncture plus Psychological Intervention for Postpartum Depression

Objective: To observe the therapeutic effect of acupuncture plus psychological intervention for postpartum depression.
Methods: By random number table, 85 patients with postpartum depression were divided into a treatment group and a control group. Forty-three cases in the treatment group were treated by acupuncture plus psychological intervention, once every day, five sessions per week, and rest at weekend. Forty-two cases in the control group were treated by oral administration of Fluoxetine Hydrochloride, 20 mg, once per day. The two groups were treated continuously for six weeks. The change of the score in Hamilton depression scale (HAMD) was observed and the therapeutic effect was summarized.
Results:The total effective rate was 90.7%in the treatment group and 90.5% in the control group. The difference in the total effective rate between two groups was not statistically significant (P>0.05). In the intra-group comparisons of HAMD scores two, four and six weeks after treatment in both groups with those before treatment, the differences were statistically significant (all P<0.05). In comparison between the two groups at the same time point, the differences in HAMD scores were not statistically significant (all P>0.05). Regarding the adverse events, 5 cases had nausea, 3 cases had dizziness, and 6 cases had poor appetite in the control group; no obvious adverse events happened in the treatment group.
Conclusion:Acupuncture plus psychological intervention for postpartum depression is as same as oral administration of Fluoxetine Hydrochloride in therapeutic effects, but it does not have adverse reaction.
4.Effect of self-made pillow for good limb position on the recovery of patients with hemiplegia after stroke

Objective To study the effect of self-made pillow for good limb position on the recovery of patients with hemiplegia after stroke .Methods Seventy-four patients with hemiplegia after stroke ( duration ≤3 months) were randomly divided into the observation (38 cases) and the control group (36 cases).Both groups were given conventional rehabilitation therapy and nursing , but the observation group used self-made pillow 48 hours of stable vital signs while the control group used normal pillows .Fugl-Meyer asassessment ( FMA) and Barthel index were used to evaluate patients'motor function and ability of daily living one month after treatment . Results After one month, the FMA score and Barthel Index (BI) score was respectively (80.21 ±6.06)and (63.03 ±8.10) in the observation group, higher than (75.08 ±7.84)and(56.25 ±10.38)in the control group, and the differences were statistically significant (t=3.135,3.141, respectively;P<0.01).Conclusions Self-made type pillow plays an important role in the recovery of motor function and ability of daily living for patients with hemiplegia after stroke .
5.Effects of febrile seizures in children with abnormal EEG tracing and intelligence summary(Clinical analysis of 112 cases)

Objective To study the Effects of febrile seizures in children with abnormal EEG and intelligence,the size of age at onset,duration of the seizure,positive family history,some variation of the epilepsy and mental retardation.Methods Do so within 24 hours of admission EEG 1,two weeks after a review of the EEG.This information painting some patients who do test PPVT test.Results Information in the 28 cases of abnormal EEG activities in the context of epileptiform discharges were eight cases.Select a 2.5-year-old to 5-year-old all abnormal EEG tests were done painting,PPVT test: eight cases of mental retardation,edge-six cases,four cases of mental retardation.Conclusion Effects of febrile seizures first attack aged less than 1 year old or more than six years old,a perinatai brain injury history,with EEG slow wave,a sharp variation can be affected for the slow payment of epilepsy and mental retardation.

Building up a harmonious type of physician-patient relationship in hospitals is an important contents in the construction of harmonious society.Major factors that jeopardize the physician-patient relationship include insufficiency of mutual communication,lack of human sympathy in medical treatment service,neglect of patients' right of informed consent.This article also puts forward some measures to solve these contradictions,such as reinforcing ethical training,improving physician-patient relationship,respecting patients' right of informed consent,and setting a psychological department for concerned issues,etc.
