Objective:To investigate the safety and efficacy of Fuzheng Kangfu (FK) formula combined with chemotherapy in the treatment of pancreatic cancer. Methods:A retrospective analysis was conducted on 95 pancreatic cancer patients who received conventional chemotherapy between June 2021 and June 2024. The patients were divided into the integrated traditional Chinese medicine treatment group (treatment group) and the chemotherapy-only group (control group) according to whether receiving FK formula during chemotherapy. Baseline characteristics, clinical indicators before and after treatment, incidence of adverse reactions, and overall survival were compared between the two groups. Results:After propensity score matching, 39 patients were included in each group. Compared with the control group, the treatment group presented significantly higher globulin levels [28.60 (24.70, 32.15) g/L vs 24.90 (22.40, 29.95) g/L], alongside lower levels of D-dimer [1 200 (720, 1 690) μg/L vs 2 230 (1 040, 3 330) μg/L], creatinine (64.29±15.97 μmol/L vs 77.71±12.26 μmol/L), urea nitrogen [3.43(2.58, 4.67) mmol/L vs 4.87 (3.80, 6.24) mmol/L], and fasting blood glucose [4.68(3.95, 5.73) mmol/L vs 6.91 (5.27, 7.70) mmol/L]. Furthermore, the treatment group exhibited significantly greater decreases in C-reactive protein (CRP), alanine aminotransferase (ALT), aspartate aminotransferase (AST), indirect bilirubin, D-dimer, fibrinogen, and urea nitrogen levels compared to the control group. Conversely, the decreases of hemoglobin level and body mass index in the control group were more pronounced than that of the treatment group. The incidence of chemotherapy-related adverse reactions such as leukopenia (7.7% vs 33.3%), anemia (15.4% vs 35.9%), liver dysfunction (28.2% vs 53.8%), gastrointestinal reactions (5.1% vs 20.5%), and neurotoxicity (0.0% vs 15.4%) in the treatment group was notably lower than that in the control group. The treatment group showed prolonged median survival (14 month vs 9 month). The 1, 1.5 and 2-year survival rates of the treatment group were higher than the control group (56.76% vs 42.95%, 35.77% vs 32.72% and 23.85% vs 19.63%). All aforementioned differences were statistically significant (all P value <0.05). Conclusions:The integration of the FK formula with chemotherapy in the management of pancreatic cancer could effectively mitigate clinical manifestations, enhance quality of life, extend survival duration, and diminish chemotherapy-induced adverse effects.

Pyridaben is a broad-spectrum acaricide widely used in agriculture, accidental or self-administration of large doses of pyridaben can cause multiple organ failure in patients. Due to its damage to multiple organs and no specific antidote, the mortality rate is high. This paper reports two patients who took a large amount of pyridaben, developed severe metabolic acidosis, hyperlactatemia, toxic encephalopathy, and liver, kidney, heart and digestive tract damage. After timely gastric lavage, catharsis, organ support andblood purification treatment, the condition improved and discharged. It is expected to provide clinical ideas for the treatment of pyridaben poisoning.

【Objective】 To compare the efficacy and safety of thulium laser enucleation of the prostate (ThULEP) and holmium laser enucleation of the prostate (HoLEP) in the treatment of benign prostatic hyperplasia (BPH). 【Methods】 Randomized controlled trials (RCTs) and clinical controlled trials (CCTs) were searched in PubMed, Embase, Cochrane Library, CNKI and Wanfang Database from Jan.1,2010 to May 30,2022. Two reviewers independently screened literature, extracted data, and assessed the risk of bias of included studies. All divergences were resolved by a third researcher. RevMan 5.4 software was used for Meta analysis. 【Results】 A total of 7 studies were included, involving 1 726 patients, 750 in the ThULEP group and 976 in the HoLEP group. Meta analysis showed that, compared with HoLEP group, the ThULEP group had shorter catheter indwelling time [MD=-0.10, 95%CI (-0.17--0.03), P=0.004] , shorter hospital stay [MD=-0.43, 95%CI (-0.60--0.25), P<0.000 01] , lower IPSS score 12 months after surgery [MD=-1.13, 95%CI (-1.95- -0.30), P=0.007] , lower QoL score 12 months after surgery [MD=-1.00, 95%CI (-1.19- -0.81), P<0.001] ,lower transfusion rate [OR=0.11, 95%CI (0.03-0.36), P=0.000 3] and lower incidence of urinary incontinence [OR=0.24, 95%CI (0.09-0.66), P=0.006] . 【Conclusion】 ThULEP may have similar efficacy and safety as HoLEP in the treatment of BPH, and has more advantages in some aspects.

Objective:To evaluate the efficacy and safety of clip-with-endoloop method during endoscopic submucosal dissection (ESD) in treatment of early gastric angle cancer and precancerous lesions. Methods:A total of 59 patients with early gastric angle cancer or precancerous lesions underwent ESD from January 2018 to December 2018 were randomly divided into the routine ESD group ( n=28) and the clip-with-endoloop group ( n=31). The frequency of supplementary submucosal injection, ESD procedure time, area of the resected specimen, dissection time, submucosal dissection speed, complete resection rate and complications were compared between the two groups. Results:The frequency of supplementary submucosal injection in the clip-with-endoloop group was less than that in the routine ESD group (2.3±1.1 VS 3.7±1.4, t=4.557, P<0.001). There was no significant difference in the area of the resected specimen between the two groups (12.7±2.6 cm 2 VS 11.7±2.7 cm 2,t=1.485, P=0.143). The ESD procedure time (72.4±24.7 min VS 93.6±28.9 min, t=3.043, P=0.004) and dissection time (67.7±23.3 min VS 88.2±28.3 min, t=3.054, P=0.003) in the clip-with-endoloop group were significantly shorter compared with those in the routine ESD group. The submucosal dissection speed in the clip-with-endoloop group was higher than that in the routine ESD group (20.2±3.2 mm 2/min VS 14.3±3.4 mm 2/min, t=6.879, P<0.001). The complete resection rate was 100.0% in the both groups. No perforation or postoperative bleeding occurred in the two groups. The incidence of intraoperative bleeding in the clip-with-endoloop group was lower than that in the routine ESD group [19.4% (6/31) VS 35.7% (10/28), χ2=1.992, P=0.158]. Conclusion:Clip-with-endoloop method makes ESD procedures easier and faster, with a lower possibility of intraoperative bleeding in treatment of early gastric angle cancer.
