Background and aims The mechanism of cholestatic liver injury (CLI) is unclear, and effective therapies are lacking. While peroxisome proliferator-activated receptor alpha (PPARα) agonists show potential hepatoprotective effect and pyroptosis is implicated in hepatocellular damage, how PPARα activation mitigates lithocholic acid (LCA)-induced pyroptosis remains unknown. Methods The hepatoprotective effect of PPARα agonists was evaluated in a mouse model of intrahepatic cholestasis induced by LCA. Liver injury was assessed via serum biochemistry, hematoxylin and eosin and TUNEL staining, and electron microscopy. Pyroptosis pathways were analyzed using real-time quantitative polymerase chain reaction, Western blot, and co-immunoprecipitation. Results Combined morphological, histopathological, and biochemical analyses confirmed that PPARα activation protects against CLI. Compared with LCA treatment alone, PPARα activation significantly attenuated the elevation of serum lactate dehydrogenase (LDH), the increased TUNEL-positive cells, and the formation of hepatocyte membrane pores. Mechanistically, PPARα activation suppressed both NOD-like receptor protein 3 (NLRP3) inflammasome-mediated pyroptosis and apoptosis protease-activating factor-1 (APAF-1)/CASPASE-3/GSDME-mediated pyroptosis. Furthermore, PPARα agonist pretreatment inhibited activation of the nuclear factor-kappa B (NF-κB) and forkhead box O1 (FOXO1) signaling pathways. Conclusions PPARα protects against LCA-induced CLI by inhibiting both NLRP3 inflammasome-mediated pyroptosis associated with NF-κB and APAF-1/CASPASE-3/GSDME-mediated pyroptosis associated with the FOXO1 signaling pathway.

OBJECTIVES: To investigate the clinical features of children with STAT gene mutations, and to explore corresponding immunotherapy strategies. METHODS: A retrospective analysis was performed for the clinical data of 10 children with STAT gene mutations who were admitted to the Department of Pediatrics of the First Affiliated Hospital of Guangzhou Medical University, from October 2015 to October 2024. Exploratory immunotherapy was implemented in some refractory cases, and the changes in symptoms, imaging manifestations, and cytokine levels were assessed after treatment. RESULTS: For the 10 children, the main clinical manifestations were recurrent rash since birth (7/10), cough (8/10), wheezing (5/10), expectoration (4/10), and purulent nasal discharge (4/10). Genotyping results showed that there was one child with heterozygous loss-of-function (LOF) mutation in the STAT1 gene, four children with heterozygous LOF mutation in the STAT3 gene, and five children with heterozygous gain-of-function (GOF) mutation in the STAT3 gene. Two children with LOF mutation in the STAT3 gene showed decreased interleukin-6 levels and improved clinical symptoms and imaging findings after omalizumab treatment. Three children with GOF mutation in the STAT3 gene achieved effective disease control after treatment with methylprednisolone (0.5 mg/kg per day). Two children with GOF mutation in the STAT3 gene received treatment with JAK inhibitor and then showed some improvement in symptoms. CONCLUSIONS STAT gene mutation screening should be considered for children with recurrent rash and purulent respiratory tract infections. Targeted immunotherapy may improve prognosis in patients with no response to conventional treatment.

Objective To investigate the relationship between plasma dipeptidyl peptidase 4 (DDP4) level and the progression of abdominal aortic aneurysm. Methods Forty-three patients with abdominal aortic aneurysm admitted to Baoji Central Hospital from January 2020 to May 2021 were selected. The follow-up was conducted at least 12 months,the absolute measurement of the maximum anterior-posterior diameters of the aneurysm and the rate of progression were obtained based on the first and last computed tomography angiography data,and the patients were divided into the progression group (with the rate of progression≥0.5 mm/year) and the progression-free group (with the rate of progression<0.5 mm/year) according to the rate of the abdominal aortic aneurysm progression. The clinical characteristics of the two groups were compared. Plasma DDP4 levels were detected by enzyme-linked immunosorbent assay. The correlation between plasma DDP4 and the rate of abdominal aortic aneurysm progression was analyzed by Pearson correlation and multiple linear regression. The predictive performance of plasma DDP4 on abdominal aortic aneurysm progression was analyzed by Logistic regression model and receiver operating characteristic (ROC) curve. Results The thickness of intracavitary thrombus (P=0.046) and the rate of abdominal aortic aneurysm progression (P<0.001) in the progression group were greater/faster than those in the the progression-free group,and the plasma level of DDP was significantly higher than that in the progression-free group (P<0.001). Multivariate Logistic regression analysis showed that plasma DDP4 level was an independent predictor of abdominal aortic aneurysm progression (P<0.05). Pearson correlation analysis showed that plasma DDP4 level was positively correlated with the rate of abdominal aortic aneurysm progression (r=0.517,P<0.001). After adjusting for other confounding factors,multiple linear regression showed that there was still a significantly positive correlation between plasma DDP4 level and abdominal aortic aneurysm progression rate (β=0.312,95％CI:0.002 to 0.484,P=0.048). The area under the curve of baseline plasma DDP4 level for predicting abdominal aortic aneurysm progression was 0.840 (95％CI:0.704 to 0.976),with corresponding cutoff value of 13.89 pg/mL,specificity of 85.7％,and sensitivity of 79.3％. Conclusion Elevated plasma DDP4 levels in patients with abdominal aortic aneurysm are associated with a higher risk of aneurysm progression. Plasma DDP4 is expected to be a predictive biomarker for abdominal aortic aneurysm progression.

Objective:To evaluate the efficacy of shockwave therapy,acupuncture,hyperthermia,biofeedback therapy,elec-trical nerve stimulation,magnetotherapy and ultrasound therapy in the treatment of chronic prostatitis/chronic pelvic pain syndrome(CP/CPPS),and to provide evidence-based support for clinical decision-making.Methods:Two researchers independently searched PubMed,Web of Science,Embase,Cochrane Library,CNKI,Wanfang,VIP and Chinese Biomedical Literature databases for randomized controlled trials(RCTs)on the effects of different interventions on CP/CPPS from the establishment of the databases to August 2024.We evaluated the quality of the included literature and extracted the relevant data according to the Cochrane Handbook for Systematic Reviews of Interventions,followed by network meta-analysis using Revman 5.3,R 4.33 and Stata17 software.Results:A total of 25 RCTs involving 1 794 cases were included.The results of network meta-analysis showed that electrical nerve stimulation,shockwave therapy,biofeedback therapy,magnetotherapy,ultrasound therapy and acupuncture were significantly superior to conventional medication and placebo in the total NIH-CPSI scores(P＜0.05),and so were electrical nerve stimulation and shock-wave therapy to acupuncture and hyperthermia(P＜0.05),magnetic therapy to hyperthermia,and ultrasound therapy to placebo(P＜0.05).Shockwave therapy,biofeedback therapy,electrical nerve stimulation,magnetotherapy and ultrasound therapy achieved re-markably better clinical efficacy than conventional medication and placebo in the treatment of CP/CPPS,and so did shockwave therapy than electrical nerve stimulation,hyperthermia,ultrasonic therapy,magnetotherapy and acupuncture.Conclusion:For the treat-ment of CP/CPPS,electrical nerve stimulation is advantageous over the other interventions in improving total NIH-CPSI scores,and shockwave therapy is advantageous in relieving pain symptoms and clinical efficacy.This conclusion,however,needs to be further veri-fied by more high-quality clinical studies.

Objective To explore the possible therapeutic mechanism of Gastrodin Injection combined with ganglioside in the adjuvant treatment of spinal cord injury(SCI)based on NOD-like receptor thermal protein domain associated protein 3(NLRP3)signaling pathway.Methods A total of 108 patients with SCI were randomly divided into an observation group and a control group,with 54 cases in each group.Both groups were given routine rehabilitation training.Additionally,the control group was treated with intravenous drip of ganglioside,and the observation group was treated with intravenous drip of Gastrodin Injection combined with ganglioside.A course of treatment covered 30 days and the two groups were treated for 2 continuous courses.The time for the recovery of muscle strength,time to leave a sickbed for walking and time to stay in hospital for observation were compared between the two groups.The American Spinal Injury Association(ASIA)score and the serum levels of brain-derived neurotrophic factor(BDNF),central nervous specific protein(S-100β),interleukin(IL)-1β,IL-18,NLRP3,cystein-containing aspartate specific protease 1(Caspase-1)and apoptosis-associated speck-like protein containing a CARD(ASC)in the two groups were observed before and after treatment.After treatment,the clinical efficacy and safety of the two groups were evaluated.Results(1)After two courses of treatment,the total effective rate of the observation group was 83.33%(45/54),and that of the control group was 66.67%(36/54).The intergroup comparison(tested by chi-square test)showed that the curative effect of the observation group was significantly superior to that of the control group(P＜0.05).(2)After treatment,the time for the recovery of muscle strength,time to leave a sickbed for walking and time to stay in hospital for observation in the observation group were significantly shorter than those in the control group(P＜0.01).(3)After one and 2 courses of treatment,the ASIA scores of pain sense,motor sense and tactile sensation in the two groups were significantly higher than those before treatment(P＜0.05),and the scores after 2 courses of treatment were higher than those after one course of treatment(P＜0.05).The intergroup comparison showed that the increase of ASIA scores of pain sense,motor sense and tactile sensation in the observation group after one and 2 courses of treatment was significantly superior to that in the control group(P＜0.05).(4)After one and 2 courses of treatment,the serum BDNF level in the two groups was higher than that before treatment(P＜0.05)and the levels of NLRP3,ACS,Caspase-1 protein and serum IL-1β,IL-18 and S-100β were lower than those before treatment(P＜0.05),and the changes in the levels of above indicators in the two groups after 2 courses of treatment were more obvious than those after one course of treatment(P＜0.05).The intergroup comparison showed that the increase of serum BDNF level and the decrease of the levels of protein NLRP3,ACS,Caspase-1 and serum IL-1β,IL-18 and S-100β in the observation group were significantly superior to those in the control group(P＜0.05).(5)During the treatment,no drug-related adverse reactions occurred in the two groups.Conclusion Gastrodin Injection combined with ganglioside exerts certain effect for the treatment of SCI patients.The combined therapy is effective on relieving the neuroinflammatory response and promoting the recovery of neurological function by regulating the NLRP3 inflammasomes,and its curative effect is superior to that of ganglioside alone.

Abstract: Objective　To monitor and analyze the avian influenza virus contamination in the environment outside the poultry-related places in Guangxi, and to assess the risk of human infection with avian influenza viruses, so as to provide a scientific basis for the prevention and control of avian influenza in Guangxi. Methods　From 2021 to 2022, environmental samples from 5 kinds of poultry-related sites were collected monthly in 14 cities of Guangxi. The real-time fluorescent quantitative RT-PCR method was used to detect the nucleic acids of generic influenza A viruses, and H5, H7, and H9 subtypes of avian influenza virus. The detection results of avian influenza virus in the environment of the poultry-related sites in Guangxi were collected for retrospective analysis. SPSS 22.0 was used for data analysis, and the chi-square test was used to compare the rates. Results　From 2021 to 2022, a total of 5 960 environmental samples were collected in 14 cities, of which 3 918 were positive for influenza A virus nucleic acid, with a positive rate of 65.74%; Among the positive samples, 281 were positive for H5 subtype (7.17%), 2 508 were positive for H9 subtype (64.01%), 552 were positive for H5+H9 subtype (14.09%), 577 were positive for type A but not H5/H7/H9 (14.73%), and no subtype H7 was detected. The positive rate of influenza A in poultry-related environment samples was higher throughout the year in Guangxi; except for Wuzhou, which had a similar number of H5, H9, and A non-H5/H7/H9 subtypes, the H9 subtype was predominantly detected in other cities. There was significant variability in positive rates among different regions, with the highest being in Hezhou City (90.32%, 653/723) and the lowest in Yulin City (28.96%, 75/259). The positive rate of different specimen types ranged from 50.32% to 74.94%. There were statistically significant differences in the positive rates of samples from different types of samples (χ2=163.08, P<0.001), different months (χ2=172.69, P<0.001), different regions (χ2=498.86, P<0.001), different monitoring sites (χ2=370.01, P<0.001). Conclusions　There is severe contamination of avian influenza virus in the poultry-related environment in Guangxi, predominantly with the H9 and H5 subtypes. Therefore, the relevant authorities in Guangxi should strengthen the monitoring, management, and disinfection of poultry-related premises.

【Objective】 To investigate the effects of preoperative lipid metabolism level on the postoperative prognosis of non-muscular invasive bladder cancer (NMIBC). 【Methods】 Clinical data of NMIBC patients who underwent surgical treatment in our hospital during Mar.2014 and May 2021 were retrospectively analyzed. Based on receiver operating characteristic (ROC) curve, the optimal cutoff values of all lipid metabolism indicators were determined and patients were classified accordingly. The independent risk factors for postoperative recurrence were identified with Cox regression model. The survival was analyzed with Kaplan-Meier, and recurrence-free survival (RFS) was compared using log-rank tests. A recurrence risk prediction model was established based on the high-density lipoprotein (HDL) and other clinic pathological factors and the accuracy of prediction was evaluated with the area under the ROC curve (AUC). 【Results】 Cox multivariate analysis showed HDL, tumor number, tumor size and histological grade were independent risk factors for recurrence (P<0.05). Kaplan-Meier analysis showed that RFS was significantly longer in the high-HDL group than in the low-HDL group (P<0.001). Incorporating HDL, tumor number, tumor size, histological grade, and tumor stage into the recurrence risk model, the AUC was 0.706, and internal cross validation showed the AUC was 0.711. 【Conclusion】 Preoperative HDL is an independent risk factor affecting the RFS of patients with NMIBC, and combining it with clinic pathological factors will improve the prediction of tumor recurrence.

【Objective】 To share the technical key points and experience of transvesical robot-assisted radical prostatectomy (TvRARP). 【Methods】 The clinical data of 13 patients with prostate cancer (PCa) receiving TvRARP during Nov.2021 and May 2022 were collected. The operation time, estimated blood loss, blood transfusion rate, catheter removal time, postoperative length of hospital stay, immediate urinary continence rate, postoperative IIEF-5 score and perioperative complications were evaluated. 【Results】 The operation time was (142±39) min, estimated intraoperative blood loss was (76±40) mL, and no transfusion was needed. The median postoperative IIEF-5 score was 16 (12-22), hospital stay 3 (2-5)days, and catheter removal time 7(5-14)days. Of all 13 patients, 12(92.3%) achieved immediate urinary continence at the removal of catheter. There were no postoperative complications of Clavien Ⅲ and above. Clavien Ⅰ-Ⅱ complications were observed in 4 patients (30.8%). 【Conclusion】 TvRARP is feasible and safe for selected patients with clinically localized PCa, which can ensure promising postoperative urinary continence and preserve erectile functional.
