1.Association between depressive symptoms and risk of stroke and all-cause mortality in young and middle-aged adults

BackgroundCurrent studies on the relationship between depressive symptoms and the risks of stroke and all-cause mortality mostly focus on the elderly population or high-risk groups with chronic diseases. The long-term impact of depressive symptoms in young and middle-aged people on the risks of stroke and all-cause mortality still lacks systematic assessment, and the relevant evidence is insufficient. ObjectiveTo explore the association between depressive symptoms and the risks of stroke and all-cause mortality in young and middle-aged adults, thereby expanding the risk evidence in this age group and providing epidemiological basis for early identification of high-risk individuals and the development of targeted intervention strategies. MethodsBased on data from the National Health and Nutrition Examination Survey (NHANES) database (2009–2018), a total of 14 947 young and middle-aged respondents aged 18 to 64 were included as the research subjects. Logistic regression and restricted cubic spline (RCS) models were employed to examine the association between depressive symptoms and stroke. Cox proportional hazards model, RCS model, and Kaplan-Meier (K-M) survival curves were used to clarify the relationship between depressive symptoms and all-cause mortality risk. To verify the robustness of main findings, sensitivity analysis was conducted stratified by gender, hypertension, diabetes, hyperlipidemia, body mass index (BMI), and age. ResultsLogistic regression analysis revealed that compared with the non-depressed group, the risk of stroke in the mild depression group increased by 53.50% (OR=1.535, 95% CI: 1.147–2.053), while the moderate-to-severe depression group showed the highest risk with a 132.90% increase (OR=2.329, 95% CI: 1.733–3.129). RCS analysis indicated an approximately linear dose-response relationship between PHQ-9 score and the risk of stroke (Pfor non-linearity=0.089), with stroke risk increasing as PHQ-9 scores rose. As the PHQ-9 score increased, the risk of stroke also showed an upward trend. Cox regression analysis demonstrated that compared with the non-depressed group, the risk of all-cause mortality in the mild depression group increased by 48.80% (HR=1.488, 95% CI: 1.164–1.902), and the moderate-to-severe depression group had a 70.60% increased risk (HR=1.706, 95% CI: 1.310–2.221). RCS analysis showed a linear dose-response relationship between PHQ-9 scores and all-cause mortality risk (Pfor non-linearity=0.440). K-M curves indicated that as the severity of depressive symptoms increases, the risk of all-cause mortality gradually rises (P<0.05). Sensitivity analysis confirmed that the associations between depression symptoms and risks of stroke and all-cause mortality were not modified by gender, hypertension, diabetes, hyperlipidemia, BMI, or age, demonstrating robust findings. ConclusionAmong the young and middle-aged population, both mild and moderate-to-severe depression were associated with increased risks of stroke and all-cause mortality, with the moderate-to-severe depression group showed more pronounced risk elevations. A positive dose-response relationship was observed between depressive symptoms and these risks.
2.Mechanism of aloin ameliorating atherosclerosis through regulating macrophage polarization

OBJECTIVE To investigate the mechanism by which aloin (ALO) ameliorates atherosclerosis (AS). METHODS Eight C57BL/6J mice were assigned to the control group (CON group) and fed a standard diet; thirty-two apolipoprotein E-knockout (APOE-/-) mice were randomly divided into model group (MOD group), ALO low-dose and high-dose groups [ALO-L group, ALO-H group, 20, 40 mg/(kg·d)], and atorvastatin positive control group [ATO group, 4 mg/(kg·d)], with 8 mice in each group, establishing the AS model through feeding with a high-fat diet. The mice were administered the drug via gavage or given an equal volume of deionized water for 8 consecutive weeks. The lipid levels in the serum of mice were measured, and the pathological structural changes in their aortas were observed. The expressions of macrophage polarization markers (CD86+ , CD206+) in the aorta were determined, along with the mRNA expressions of inducible nitric oxide synthase (iNOS), tumor necrosis factor-α (TNF-α), arginase-1 (Arg-1), and interleukin-10 (IL-10), as well as the protein expressions of iNOS and Arg- 1, and the phosphorylation levels of nuclear factor κB p65 (NF-κB p65) and signal transduction and activator of transcription 3 (STAT3) proteins. Additionally, a macrophage polarization model was established using lipopolysaccharide (LPS)-induced RAW264.7 cells, and the effect of ALO (400 μmol/L) on the cellular polarization phenotype was investigated. RESULTS Compared with the MOD group, administration groups all showed significant improvement in dyslipidemia (except for high-density lipoprotein cholesterol in the serum of ALO-L group) (P<0.05); aortic intimal structure improved significantly, plaque area was reduced significantly (P<0.01); the CD86+ relative fluorescence intensity in the aorta decreased significantly, the CD206+relative fluorescence intensity increased significantly (P<0.01), while the expressions of iNOS and TNF-α mRNA were down-regulated significantly (P<0.05); mRNA expressions of Arg-1 and IL-10, and protein expression of Arg-1 were increased significantly in ALO-H group and ATO group (P<0.05); the protein expressions of iNOS, and the phosphorylation levels of NF-κB p65 and STAT3 protein were decreased significantly (P<0.05). In vitro experiments further confirmed that ALO significantly reduced the proportion of LPS-induced M1-type macrophages but increased the proportion of M2-type macrophages (P<0.01). CONCLUSIONS ALO inhibits M1-type macrophage polarization and promotes M2-type polarization, ameliorates dyslipidemia and reduces arterial plaque formation in AS model mice, improve the structure of the aortic intima potentially through suppression of the NF-κB/STAT3 signaling pathway.

4.Exploration of the pathogenic mechanism of a novel c.661_664dup (p.P222Lfs*60) variant of SOX10 gene.

OBJECTIVE:
To explore the pathogenic mechanism of a child with Waardenburg syndrome type 4C due to a c.661_664dup (p.P222Lfs*60) variant of SOX10 gene through in vitro experiments.
METHODS:
A child diagnosed at the Handan First Hospital was selected as the study subject. Clinical data of the child was collected. Peripheral blood samples were collected from the child and his parents. Following extraction of genomic DNA, trio-whole exome sequencing was carried out. Pathogenicity of candidate variant was determined by bioinformatic analysis and reference to the guidelines from the American College of Medical Genetics and Genomics (ACMG). Candidate variant was verified by Sanger sequencing. Expression plasmids of wild-type SOX10 and the c.661_664dup (p.P222Lfs*60) variant were constructed and transiently transfected into 293T cells to determine the expression at the RNA and protein levels. The 293T cells transiently transfected with the wild-type/mutant SOX10 were treated with 10 ug/mL cycloheximide (CHX) for 0, 4, 8, 24 h, respectively, and the degradation rate of target protein was detected by Western blotting assay. This study has been approved by the Ethics Committee of Handan First Hospital (Ethics No. HDYY-LW-25053).
RESULTS:
The child was found to harbor a heterozygous c.661_664dup (p.P222Lfs*60) variant of the SOX10 gene, which was unreported previously. The variant did not significantly alter the expression of SOX10 at the mRNA level but the protein level. After the CHX treatment, the degradation of mutant SOX10 protein had slowed down.
CONCLUSION
The mutant SOX10 may affect the expression of downstream genes by affecting the degradation rate of its protein product.

5.Diagnostic Value of Combined Detection of Serum AIF-1, GDF15, and DCLK1 in Breast Cancer

Objective To explore the diagnostic value of the combined detection of serum allograft inflammatory factor-1 (AIF-1), growth differentiation factor-15 (GDF15), and doublecortin-like kinase 1 (DCLK1) in breast cancer. Methods A total of 114 female breast cancer patients (breast cancer group) and 114 female volunteers (control group) who underwent physical examination were selected. Enzyme-Linked immunosorbent assay was applied to detect serum levels of AIF-1, GDF15, and DCLK1. Multivariate Logistic regression was used to analyze the risk factors affecting breast cancer. ROC curve was used to analyze the diagnostic value of serum AIF-1, GDF15, DCLK1 levels in breast cancer. Results The serum levels of AIF-1, GDF15, and DCLK1 in the breast cancer group were obviously higher than those of the control group (P<0.05). The expression levels of serum AIF-1, GDF15, and DCLK1 were significantly elevated in breast cancer patients with stage Ⅲ-Ⅳ, moderate and poor differentiation, lymph-node metastasis, and Ki-67 positive (P<0.05). Serum AIF-1, GDF15, and DCLK1 were risk factors for breast cancer, and the areas under the curve (AUC) for single and combined diagnosis of breast cancer for all three were 0.834, 0.753, 0.861, and 0.930, respectively. Additionally, the combined diagnosis AUC values were significantly higher than those assessed alone (Zcombination-AIF-1=3.479, Zcombination-GDF15=5.147, and Zcombination-DCLK1=3.121; all P<0.05). Conclusion In breast cancer patients, the serum AIF-1, GDF15, and DCLK1 levels increase. They are risk factors for breast cancer, and the combined diagnosis of the three is more effective.

To explore the application of humanistic care in children's fever clinics,the children's fever clinics of Guangzhou Women and Children's Medical Center conducted relevant research using narrative medicine as a guiding concept.This paper elaborated on the medical humanistic dilemmas and narrative care needs in children's fever clinics,and focused on exploring the practice paths of narrative medicine in pediatric with Chinese characteristics from five dimensions,including the cultivation of nursing staff's abilities of professional narrative and humanistic care,the establishment of health lectures featuring narrative patient education,the alleviation of medical anxiety for children and their families,related support of narrative nursing,and caring services.The aim was to improve the narrative care ability of nurses in children's fever clinics,develop methods for pediatric patients that can eliminate the fear of seeking medical treatment,and protect children's physical and mental health.A carrier of care,support,and emotional expression can be provided for parents.The foundation for nursing staff to achieve professional growth through narrative reflection can be laid.Thus,it can assist in establishing a life narrative community relationship between doctors and patients,and jointly explore the meaning of life.

Objective To investigate the effect of erector spinae plane block on ultrasound-based hemodynamic parameters of the arteries of the four limbs and biochemical stress indicators in patients with thoracoscopic lobectomy. Methods A total of 120 patients with thoracoscopic lobectomy were randomly divided into study group and control group, with 60 cases in each group.The study group received erector spinae plane block during surgery, while the control group received conventional anesthesia measures.Pulmonary function indicators[peak expiratory flow rate (PEFR), forced vital capacity (FVC), forced expiratory volume in the first second (FEV1)], ultrasound-based hemodynamic parameters of the arteries of the four limbs (maximum systolic velocity, minimum diastolic velocity, mean velocity, arterial pulsatility index, and arterial resistance index), stress indicators[cortisol (Cor), norepinephrine (NE), angiotensin Ⅱ(Ang Ⅱ), and adrenocorticotropic hormone (ACTH)], and analgesic effect were compared between the two groups at different time points[before anesthesia induction (T0), after extubation (T1), and after drainage tube removal (T2)]. Objective To investigate the effects of esketamine on hypoxic-ischemic myocardial injury in neonatal rats based on glycogen synthase kinase-3β/NOD-like receptor thermal protein domain-containing protein 3 (GSK-3β/NLRP3) pathway. Methods Thirty neonatal rats were randomly divided into sham operation group, model group and esketamine group, with 10 rats in each group. The rats in the sham operation group underwent a median incision in the neck to expose the bilateral common carotid arteries; the rats in the model group and the esketamine group underwent ligation of the common carotid arteries combined with a hypoxic environment to establish a model ofischemia and hypoxia; the rats in the esketamine group were given esketamine intervention (50 mg/kg). Left ventricular ejection fraction (LVEF), left ventricular fractional shortening (LVFS), left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD), serum creatine kinase isoenzyme (CK-MB), cardiac troponin I (cTnI), lactate dehydrogenase (LDH), tumor necrosis factor-α (TNF-α), interleukin (IL)-6 and IL-1β levels, myocardial injury, myocardial cell apoptosis and apoptosis protein caspase 1/3/9 levels, neutrophil infiltration in myocardial tissue, and changes in GSK-3β and NLRP3 protein levels in myocardial tissue were detected in each group. Objective To investigate the effects of perioperative goal-directed fluid therapy (GDFT) on hemodynamics, postoperative recovery, and complications in elderly patientswith gastric cancer radical surgery complicated by hypertension. A simple, fast, and visual method for detecting antibodies against peste des petits ruminants virus (PPRV) using colloidal gold strips was developed. In this study, the pET-32a-N was transformed into Escherichia coli Rosetta (DE3) for expression. Hybridoma cell lines were generated by fusing SP2/0 myeloma cells with splenocytes from immunized mice with the expressed and purified N protein of PPRV. The PPRV N protein was labeled with colloidal gold particles as the gold-labeled antigen. The N protein served as the gold standard antigen and as the test (T) line-coated antigen, while the monoclonal antibody served as the quality control (C) line-coated antibody to assemble the colloidal gold immunochromatographic test strips for detecting antibodies against the N protein of PPRV. Hybridoma cell line designated as 1F1 was able to stably secrete the monoclonal antibody against the N protein of PPRV. The titer of 1F1 monoclonal antibody in ascites was 1:128 000 determined by indirect enzyme-linked immunosorbent assays (ELISA), and the immunoglobulin subtype of the monoclonal antibody was IgG1, with kappa chain. The obtained monoclonal antibody was able to specifically recognize the N protein of PPRV, as shown by Western blotting and indirect immunofluorescent assay (IFA). The developed colloidal gold test strip method was able to detect PPRV antibodies specifically, and there was no difference between different batches of the test strips. Testing of a total of 122 clinical sera showed that the compliance rate of the test strip with ELISA test was 97.6%.The test strip assay developed in this study has good specificity, reproducibility, and sensitivity, and it can be used for the rapid detection of PPRV antibodies.
