Objective To evaluate the cost–utility of lecanemab and donanemab in the treatment of early Alzheimer’s disease (AD) in China. Methods A Markov model was developed from a societal perspective to compare standard of care (SOC), lecanemab plus SOC, and donanemab plus SOC in terms of costs, quality-adjusted life years (QALY), and incremental cost–utility ratios (ICUR). Deterministic and probabilistic sensitivity analyses were conducted to assess parameter uncertainty. Results Both intervention strategies increased QALY compared with SOC but were associated with substantially higher costs, and neither was cost-effective under the current willingness-to-pay threshold. Conclusion Lecanemab and donanemab may be more appropriately considered within a multi-tiered healthcare reimbursement framework in China rather than relying solely on basic medical insurance.

Objective This study explores the clinical correlation between different low-density lipoprotein cholesterol(LDL-C)levels and prognosis, providing evidence-based guidance for the development of personalized lipid-lowering goals. Methods Patients who underwent elective percutaneous coronary intervention(PCI)treatment at Beijing Anzhen Hospital from January 2020 to June 2023 and received lipid-lowering therapy with the addition of Evolocumab were selected.Based on the results of blood lipid rechecks 3 to 6 months after surgery, the patients were divided into five groups:low-density lipoprotein<0.5 mmol/L,0.5 to<1.0 mmol/L,1.0 to<1.4 mmol/L,1.4 to<1.8 mmol/L,and above 1.8 mmol/L.All patients were followed up for more than one year, and clinical conditions and major adverse cardiovascular events(MACE)were recorded. Results A total of 1 106 patients undergoing PCI were enrolled;after propensity score matching and exclusion of patients lost to follow up,550 remained(110 per group).During 12 months of follow-up,58 patients(10.5%)experienced a MACE,with incidence rising step-wise across LDL-C categories. In multivariable Cox models adjusted for age, sex, diabetes, hypertension, baseline LDL-C,follow-up LDL-C,estimated glomerular filtration rate(eGFR),and left ventricular ejection fraction, the hazard ratios[HR(95%CI)]for MACE,relative to the<0.5 mmol/L group, were 1.810(0.507-6.454,P=0.361),3.036(0.945-9.749,P=0.062),5.228(1.737-15.735,P=0.003),7.708(2.633-22.565,P<0.001)for LDL-C levels of 0.5 to<1.0,1.0 to<1.4,1.4 to<1.8 and≥ 1.8 mmol/L,respectively.A restricted cubic spline model demonstrated a significant non-linear positive association between LDL-C and MACE(P-overall≤0.001;P-non-linear=0.008).Stratified analyses by age, sex, hypertension and diabetes showed consistent HR with no significant interactions(all P>0.05).There were no statistically significant differences among the groups in the incidence of bleeding events, elevated creatinine levels,or abnormal liver function(all P>0.05).Conclusions In patients using PCSK9 after PCI,there is a significant positive correlation between LDL-C levels and the risk of MACE,and no correlation was observed between different LDL-C levels and the risk of adverse events such as bleeding.

Objective To explore the pathological features of bronchiolar adenoma(BA)and its specific high-resolution computed tomography(HRCT)signs, and to differentiate BA from minimally invasive adenocarcinoma(MIA)using a non-invasive preoperative method. Methods A total of 80 patients with BA and 130 patients with MIA were retrospectively selected, and the clinical information and HRCT features were compared. All cases were divided into development set and test set at a 7︰3 ratio.Logistic regression analysis was used to screen the independent predictors of MIA and construct a model. Results There were significant differences in age, lobe distribution, density, vacuole sign,tumor-related vessels number(TVN),and distance to pleura(DTP)between BA and MIA patients(P<0.05).Age,density,TVN,DTP and long diameter were identified as independent predictors of MIA.A model was constructed, with area under the curve(AUC)of 0.887 and 0.884 in the development and test sets, respectively. Conclusion The model based on HRCT morphological features of BA and MIA demonstrates superior diagnostic performance compared to individual CT morphological features.

Objective To clarify the occurrence, location distribution, and risk factors of microperforations in surgical gloves used in dermatovenereology surgeries. Methods A total of 898 sterilized surgical gloves worn by right-handed medical staff during dermatovenereology surgeries in Suqian Hospital of Jiangsu Province Hospital from May 2022 to April 2024 were selected as the research objects. The occurrence and location distribution of microperforations in all sterilized surgical gloves after surgery were collected.Univariate analysis and binary Logistic regression analysis were conducted on the factors that might lead to the occurrence of microperforations. Results Among the 898 gloves selected in this study,61 gloves(6.79%)had microperforations;the incidence of microperforations in the gloves worn on the left hand was significantly higher than that in the gloves worn on the right hand(P<0.05);microperforations were prone to occur on the palmar surfaces of the index finger and thumb of the gloves. The results of univariate and binary Logistic regression analyses showed that the use of special instruments, surgery duration≥60 minutes, ingrown nails surgery, and worn by the chief surgeon were the risk factors for the occur-rence of microperforations in sterilized surgical gloves(OR>1,P<0.05),while wearing double-layer gloves was the protective factor to avoid the occurrence of microperforations(OR<1,P<0.05).Conclusion The sterilized surgical gloves are more likely to occur microperforations if involved special instruments in surgery, surgery duration≥60 minutes, and ingrown nails surgery, and worn by the chief surgeon, while wearing double-layer gloves can reduce the incidence of microperforations.

Xiaoaiping (XAP) Injection demonstrates the anti-prostate cancer (PCa) effects, yet the underlying mechanism remains unclear. This study aims to investigate the impact of XAP on PCa and elucidate its mechanism of action. PCa cell proliferation was evaluated using a cell counting kit-8 (CCK-8) assay. Cell apoptosis was assessed through Hoechst staining and Western blotting assays. Proteomics technology was employed to identify key molecules and significant signaling pathways modulated by XAP in PCa cells. To further validate potential key genes and important pathways, a series of assays were conducted, including acridine orange (AO) staining, transmission electron microscopy, and immunofluorescence assays. The molecular mechanism of XAP against PCa in vivo was examined using a PC3 xenograft mouse model. Results demonstrated that XAP significantly inhibited cell proliferation in multiple PCa cell lines. In C4-2 and prostate cancer cell line-3 (PC3) cells, XAP induced cellular apoptosis, evidenced by reduced B-cell lymphoma 2 (Bcl-2) levels and elevated Bcl-2-associated X (Bax) levels. Proteomic, immunofluorescence, and quantitative reverse transcription-polymerase chain reaction (qRT-PCR) investigations revealed a strong correlation between forkhead box O3a (FoxO3a) autophagic degradation and the anti-PCa action of XAP. XAP hindered autophagy by reducing the expression levels of autophagy-related protein 5 (Atg5)/autophagy-related protein 12 (Atg12) and enhancing FoxO3a expression and nuclear translocation. Furthermore, XAP exhibited potent anti-PCa action in PC3 xenograft mice and triggered FoxO3a nuclear translocation in tumor tissue. These findings suggest that XAP induces PCa apoptosis via inhibition of FoxO3a autophagic degradation, potentially offering a novel perspective on XAP injection as an effective anticancer therapy for PCa.
