1.Effect of remote ischemic preconditioning on preoperative heart rate variability in patients undergoing heart valve surgery: A randomized controlled trial

Objective To explore the effect of remote ischemic preconditioning (RIPC) on preoperative heart rate variability in patients with heart valves. Methods Patients scheduled to undergo on-pump cardiac valve surgery in the Department of Cardiovascular Surgery, General Hospital of Northern Theater Command, between January and July 2022 were initially enrolled. Eligible patients were randomly assigned at a 1 : 1 ratio to either the RIPC group or the control group. Relevant indicators of heart rate variability [standard deviation of NN interval (SDNN), standard deviation of mean value of NN interval in every five minutes (SDANN), mean square root of difference between consecutive NN intervals (RMSSD), percentage of adjacent RR interval>50 ms (PNN50), low frequency (LF) component, high frequency (HF) component and LF/HF] at 8 hours in the morning on the surgical day between two groups were compared. Results A total of 118 patients were initially assessed. After screening, 58 patients were excluded, and 60 patients provided written informed consent and were enrolled in the trial, with 30 allocated to the RIPC group and 30 to the control group. Seven patients in the control group and 5 patients in the RIPC group were subsequently excluded due to missing heart rate variability data resulting from cancelled operations. Finally, 23 patients in the control group and 25 patients in the RIPC group were included in the analysis. There was no statistical difference in baseline characteristics between the two groups, and there was no significant difference in heart rate variability 24 hours before intervention (P>0.05). After the intervention measures were taken, the comparison of the results of heart rate variability at 8 hours on the day of operation showed that SDNN and SDANN of patients in the RIPC group were higher than those in the control group, with statistical differences (P<0.05). Conclusion RIPC can stabilize the preoperative heart rate variability of patients undergoing cardiac valve surgery.
2.Acupuncture and Moxibustion in Combination with Pediatric Tuina in the Treatment of Pediatric Obesity Based on the Theory of "Yang Governs and Yin Follows"

Based on the theory of "yang governs and yin follows", it is believed that pediatric obesity is primarily involves the key pathogenesis of constraint and stagnation due to yang deficiency, and the excessive accumulation of yin turbid. The root cause lies in the spleen-stomach yang deficiency and impaired digestion and transformation, while the branch manifestation is characterized by constraint and stagnation in sanjiao (三焦) and excessive accumulation of yin turbid. This emphasizes the imbalance between the governing function of yang qi and the transformation mechanism of yin essence in the overall pathological process. Accordingly, the treatment approach of reinforcing yang and unblocking stagnation, inhibiting yin and resolving turbid has been proposed. And a comprehensive treatment plan is suggested, including abdominal tuina combined with spinal manipulation, timed acupuncture according to the the eightfold method of the sacred tortoise, syndrome differentiation-based acupuncture, and mild moxibustion. This approach aims to provide an integrated approach for the prevention and treatment of pediatric obesity.
3.Development status and research progress of meridian diagnosis technology from the perspective of patents.

OBJECTIVE:
To discuss the development status of meridian diagnosis technology through the patent analysis, so as to obtain the hot spots of patents and provide the ideas for the research and development of new equipment.
METHODS:
The patent data of meridian diagnosis technology were searched from Chinese Think Tank of Traditional Chinese Medicine. The patent analysis and SWOT method were used to deeply explore the patent application of meridian diagnosis technology.
RESULTS:
Among 100 patents included, the inventions were mainly in Chinese mainland, and the invention institution was from Chengdu University of TCM. Since 2015, the number of patent application for meridian diagnosis technology was increasing, and up to a peak from 2020 to 2021. The main technological programs cover electrical, temperature and optical detection; and the application functions focus on the precise positioning of acupoints, and the diagnosis of zangfu diseases.
CONCLUSION
From the perspective of patents, it is suggested that the development of meridian diagnosis technology in China should pay attention to reducing the cost, improving the function, driving the standard formulation, strengthening safety measure, deepening human-machine interaction, exploring biological effect and optimizing the experimental platform. Through continuous innovation and breakthrough, the meridian diagnosis technology can be further matured and widely applied, so as to realize its comprehensive upgrade.

4.Dose-effect relationship between the number of acupuncture sessions and efficacy for cervical vertigo: a Meta-regression analysis based on randomized controlled trials.

OBJECTIVE:
To explore the dose-effect relationship between the number of acupuncture sessions and the efficacy for cervical vertigo (CV).
METHODS:
Literature regarding randomized controlled trials (RCTs) of acupuncture for CV was retrieved from CNKI, Wanfang, VIP, Web of Science, and PubMed databases from inception to June 28th, 2024. Studies were included if patients were treated solely with acupuncture and the core prescription included Baihui (GV20)-Fengchi (GB20)-neck-jiaji (EX-B2). Outcomes included the evaluation scale for cervical vertigo symptoms and function (ESCV) score and the mean blood flow velocity of vertebrobasilar arteries. The Cochrane risk of bias assessment tool 2.0 was used to evaluate study quality. Dose-effect Meta-regression analysis was performed using the robust-error Meta-regression (REMR) method in Stata 17.0 software.
RESULTS:
Nineteen RCTs were included with a total of 747 patients in the experimental groups. After 10 sessions of acupuncture, the ESCV score increased to 20.29 (95% CI: 16.77, 23.80), with a pre-post ESCV difference of 4.60 (95% CI: 2.59, 6.60) and an improvement rate of 0.36 (95% CI: 0.26, 0.46). After 20 sessions of acupuncture, the ESCV score increased to 21.55 (95% CI: 18.87, 24.22), with a difference of 5.42 (95% CI: 3.87, 6.97) and an improvement rate of 0.39 (95% CI: 0.31, 0.48). After 10 sessions of acupuncture, the improvement rates for left vertebral artery (LVA), right vertebral artery (RVA), and basilar artery (BA) mean blood flow velocities were 0.08 (95% CI: 0.05, 0.12), 0.09 (95% CI: 0.05, 0.12), and 0.11 (95% CI: 0.06, 0.15), respectively. After 14 sessions of acupuncture, the improvement rates reached their peaks: LVA [0.09 (95% CI: 0.06, 0.12)], RVA [0.10 (95% CI: 0.07, 0.13)], and BA [0.12 (95% CI: 0.07, 0.16)].
CONCLUSION
A nonlinear dose-effect relationship existed between the number of acupuncture sessions and the efficacy for CV. Fourteen sessions were recommended as the optimal number of acupuncture treatments.

5.Comparison of chemical constituents in traditional decoction and formula granule decoction of Wendan Decoction

AIM To compare the chemical constituents in traditional decoction and formula granule decoction of classical famous prescription Wendan Decoction.METHODS The HPLC fingerprints were established,after which the contents of adenosine,synephrine,liquiritin,naringin,hesperidin,6-gingerol and adenosine cyclophosphate were determined,cluster analysis,principal component analysis and multidimensional scaling analysis were adopted in the investigation of component differences,and the equivalent of formula granules was adjusted.RESULTS The similarities of HPLC fingerprints for 10 batches of traditional decoctions were higher than those of HPLC fingerprints for 9 batches of formula granule decoctions(P<0.01).Adenosine,synephrine,liquiritin,hesperidin and cyclic adenosine monophosphate demonstrated higher contents in traditional decoctions than those in formula granule decoctions(P<0.05),6-gingerol displayed lower content than that in the latter produced by manufacturers A,C(P<0.05),which was higher than that in the latter produced by manufacturer B(P<0.01).Various batches of traditional decoctions and formula granule decoctions could be obviously distinguished,adenosine,synephrine and hesperidin exhibited great influences on the classification of principal component analysis,and the quality of formula granule decoctions produced by manufacturer C was closer to that of traditional decoctions.After equivalent correction,the contents of various constituents in formula granule decoctions produced by manufacturers A,C showed no significant differences as compared with those in traditional decoction(P>0.05).CONCLUSION The formula granules of Wendan Decoction from different manufacturers exist quality differences,so the preparation process and extraction process of this preparation should be optimized to improve quality,and equivalent ratio should be adjusted according to actual requirements to ensure its scientific and rational clinical application.
6.Application of cymene care solution in prognostic management of chronic periodontitis

40 patients with choronic periodontitis underwent periodontal basic treatment were randomly divided into 2 groups(n=20).The patients in control group used special toothpaste and toothbrush to brush their teeth after meals,those in the experimental group brushed their teeth with special toothpaste and toothbrush in the morning,evening and after meals,and wore personalized film pressing trays containing cymene care solution while sleeping at night.Gingival bleeding,periodontal pocket depth and attachment loss were ob-served after 4,6 and 10 weeks respectively.The personalized tray combined with cymbidium reduced the depth of periodontal pocket(P<0.05)and the rate of probing bleeding sites(P<0.05)more effectively,and showed no statistical significance in the change of attachment loss(P<0.05).Cymene care solution is effective in the improvement of periodontal health.
7.Association between glycated albumin/hemoglobin A1c ratio and type 2 diabetes mellitus with metabolic associated fatty liver disease

Objective To investigate the association between glycated albumin/hemoglobin A1c(GA/HbA1c)and type 2 diabetes mellitus(T2DM)complicated with metabolic associated fatty liver disease(MAFLD).Methods A total of 502 hospitalized T2DM patients were enrolled between January 2022 and December 2023 and divided into T2DM group(n=301)and combined with MAFLD(MAFLD,n=201)group.Clinical data were collected,and Logistic regression analyses were performed to evaluate the relationship between GA/HbA1c and MAFLD.Mediation analysis was conducted to assess the role of blood lipids.Results The MAFLD group had higher levels of drinking,waist-to-hip ratio,body mass index,fasting insulin,fasting C-peptide,insulin resistance index,alanine aminotransferase,aspartate aminotransferase,triglycerides(TG),small dense low-density lipoprotein(sdLDL),and blood uric acid compared to the T2DM group(P<0.05).The age,DM duration,GA,GA/HbA1c,and high-density lipoprotein cholesterol(HDL-C)were lower in the MAFLD group than in the T2DM group(P<0.05).Logistic regression analysis showed that after adjusting for confounding factors,GA/HbA1c was an influencing factor for the occurrence of MAFLD in T2DM patients.Mediation analysis revealed that TG,sdLDL,and HDL-C had significant mediating effects,accounting for 19.3%,12.4%,and 8.5%of the risk association,respectively.Conclusions GA/HbA1c is influencing factor of MAFLD in T2DM patients,with blood lipids showing significant mediating effects,suggesting that GA/HbA1c may serve as a novel indicator for assessing MAFLD risk.
8.Isolation,identification and virulence evaluation of clinical strains of Actinobacil-lus pleuropneumoniae serotype 15 in pigs

In recent years,the prevalence of porcine infectious pleuropneumonia caused by Actinoba-cillus pleuropneumoniae(APP)has significantly increased in large-scale pig farms in China.The i-solation,identification,and biological characteristic testing of newly prevalent strains have impor-tant clinical significance.This study isolated four suspected APP strains(named XB2T-56,JMTLP-443,YD-THB-755,SYZJ-291)from lung samples of pig farms suspected of outbreaks of porcine infectious pleuropneumonia in Hubei Province.They were identified by PCR amplification,and multiplex PCR serological typing,the biological properties were tested by detecting the growth a-bility,hemolytic activity and biofilm forming ability.The drug resistance of the strains was ana-lyzed by drug sensitivity testing,and the virulence of one strain(XB2T-56)in mice and piglets was evaluated.The PCR identification result showed that all four clinical isolates were APP serotype 15 strains.These four strains grew slower than serotype 1 standard strains,had weaker hemolytic activity,but could form stronger biofilms.These strains are all less sensitive to cephalosporin,tylo-sin,sulfamethoxazole,and doxycycline,but sensitive to cefotaxime and enrofloxacin.The mice and piglet infection experiments showed that the isolated strain XB2T-56 has strong virulence.Artifi-cial intranasal infection causes acute death in piglets,with typical symptoms of pleuropneumonia in the lungs,and serotype 15 strains can be isolated from the lesion site.This study indicates that the APP serotype 15 strain has strong virulence,providing new date for the clinical epidemiological characteristics of APP.
9.Quantitative evaluation of knee laxity after partial anterior cruciate ligament injury with a novel digital arthrometer

BACKGROUND:Quantitative measurement of knee laxity after partial anterior cruciate ligament injury is crucial for timely diagnosis,post-treatment,and rehabilitation.There is a lack of research on the use of articular ligament digital arthrometer in the diagnosis of partial anterior cruciate ligament injury.OBJECTIVE:To investigate the diagnostic value of a new articular ligament digital arthrometer in partial anterior cruciate ligament injury.METHODS:Totally 30 patients with partial anterior cruciate ligament injury and 20 healthy volunteers were included.The anterior tibial translation values under 80,120,and 150 N loads were measured by a homebred digital arthrometer.The anterior tibial translation and side-to-side difference were compared between the partial anterior cruciate ligament group and control group.The receiver operating characteristic curve was analyzed and area under curve was calculated,and the diagnostic sensitivity and specificity of the digital arthrometer were determined.RESULTS AND CONCLUSION:(1)There was no significant difference in age,sex,body mass index,and course of disease between the partial anterior cruciate ligament injury group and control group(P>0.05).(2)The side-to-side difference values of the partial anterior cruciate ligament injury group were significantly higher than those of the control group(P<0.05),and the anterior tibial translation values of the injured side were significantly higher than those of the healthy side in the partial anterior cruciate ligament injury group(P<0.05).(3)When the load was applied at 80 N,the diagnostic accuracy was highest(cutoff value=10.45 mm);the area under curve was 0.813(95%CI:0.708-0.919),and the sensitivity and specificity were 76.7%and 70.0%,respectively.(4)It is indicated that the digital arthrometer can objectively and quantitatively evaluate knee laxity after partial anterior cruciate ligament injury,and provide objective diagnostic basis for patients with partial anterior cruciate ligament iniurv.
10.Nationwide external quality assessment for nucleic acid detection of influenza A virus

Objective To understand and evaluate the overall status of nucleic acid detection efficacy for influenza A virus in the na-tionwide clinical laboratories of China,and discover and identify the potential issues to further improve the detection quality.Methods During 2024,the National Center for Clinical Laboratories distributed five samples to nationwide 1 367 participating laboratories.The detection efficacy of each participating laboratory was evaluated by calculating the overall percent agreement(OPA)of the test results using different detection reagents.Results The results of OPA,positive percent agreement(PPA)and negative percent agreement(NPA)of the five samples were 99.87%(6 826/6 835),99.89%(5 462/5 468),and 99.78%(1 364/1 367),respectively.No statistical difference of PPAs was observed between the H3N2 samples with different concentrations,between H1N1(2009)and H3N2 samples with equivalent concentration(1.0×104 copies/mL),and between seasonal H1N1 and H3N2 samples with equivalent concen-tration(1.0×105 copies/mL)(P>0.05).Conclusion The results indicated the clinical laboratories in China exhibited robust efficacy in the molecular detection for two prevalent influenza A virus subtypes,i.e.,H1N1(2009)and H3N2.However,false-negative and false-positive results were encountered in a few laboratories.
