1.Molecular and Phenotypic Characterization of Fluid-Derived Patient-Derived Cell and Organoid Models in Advanced Gastric Cancer

Purpose:
Patient-derived cells (PDCs) and patient-derived organoids (PDOs) are complementary preclinical models widely used in translational cancer research. However, their molecular and functional differences have not been systematically characterized. This study established and analyzed paired PDC and PDO models derived from the same gastric cancer ascites to delineate platform-dependent molecular and functional profiles.
Materials and Methods:
Malignant ascites or pleural fluid obtained from 6 patients with advanced gastric cancer were used to establish paired PDC and PDO models. All pairs underwent comprehensive multi-omics profiling, integrating genomic, transcriptomic, and proteomic data. Phenotypic characterization included morphological, histological, proliferative, and cell cycle analyses. Drug sensitivity assays were performed using 4 chemotherapeutic agents commonly used to treat gastric cancer.
Results:
The 6 paired PDC and PDO models exhibited distinct morphological characteristics.Whole-genome analyses demonstrated high concordance among primary tumors, PDCs, and PDOs, confirming tumor representation across platforms. Multi-omics profiling identified platform-dependent molecular signatures; PDOs were enriched for extracellular matrix remodeling and stemness, whereas PDCs displayed proliferation- and immune-related signatures. Clinically relevant biomarkers, including HER2 and MET alterations, were concordant with primary tumors. Notably, drug responses differed between platforms and patients, indicating platform-dependent and patient-specific chemosensitivity.
Conclusions
Paired PDC and PDO models derived from the same patients preserved core patient-specific tumor characteristics while exhibiting distinct molecular and functional profiles. These findings underscore the culture platform as a critical determinant of experimental outcomes and therapeutic responses. Therefore, careful selection of an appropriate preclinical model is essential to accurately address biological questions and optimize precision oncology strategies.

3.Real-World Prevalence and Associated Factors of Nipple Eczema in Patients With Atopic Dermatitis

Background:
Nipple eczema (NE) manifests as pruritic or painful erythema, often accompanied by oozing, erosion, and lichenification. NE is observed in 6%–23% of patients with atopic dermatitis (AD); its prevalence is higher among adolescent and young adult women.
Objective:
To determine the prevalence and clinical correlates of NE in patients with AD.
Methods:
We conducted a retrospective cross-sectional study from July to December 2023, enrolling patients diagnosed with AD. Participants completed a questionnaire regarding their symptoms of AD and NE, as well as the impact of NE on their daily lives.
Results:
Of the 200 patients, 55 (27.5%) reported of having experienced NE. The mean age at NE onset was 22.75±9.77 years. The proportion of females with NE (70.9%) was significantly higher relative to those without NE (38.6%) (p<0.001). Factors significantly associated with the presence of NE in AD patients included female sex (odds ratio [OR], 4.90; 95% confidence interval [CI], 1.99–12.07; p=0.001), increased AD itch numerical rating scale (NRS) score (OR, 1.46; 95% CI, 1.11–1.91; p=0.007), increased AD pain NRS score (OR, 1.20; 95% CI, 1.01–1.44; p=0.041) and higher Investigator Global Assessment score (OR, 5.38; 95% CI, 2.07–13.99; p=0.001). Regarding nipple-specific health-related quality of life (HRQoL), 35.8% of patients reported severe impairment in HRQoL due to NE, and 45.3% experienced severe emotional distress.
Conclusion
Although the area affected by NE is small, dermatologists should be aware of its association with severe AD characteristics, and its significant impact on patient HRQoL.

Background:
Recently, the prevalence of atopic dermatitis (AD) in the elderly has increased and has been reported to be as high as 4%, emphasizing its clinical significance. Treating pruritus in elderly patients can be challenging due to the high prevalence of comorbidities and adverse events associated with conventional systemic medications.Dupilumab has demonstrated favorable safety and effectiveness across all age groups, making it a promising treatment option for the elderly population.
Objective:
To evaluate the efficacy and safety of dupilumab in elderly patients with AD or other pruritic conditions.
Methods:
A multicenter, retrospective, real-world study on the efficacy and safety of dupilumab was conducted in patients aged 65 years or older. Patients with AD and other pruritic conditions were assessed separately for dupilumab efficacy using objective and subjective scores. The patients who received regular injections were evaluated at 16 and 40 weeks after treatment.
Results:
Forty-seven patients participated in the study, of whom 37 were diagnosed with AD and 10 were classified as having other pruritic conditions. In patients with AD, there was a 78.6% reduction in the mean Eczema Area and Severity Index score at week 16 compared with baseline. All patients with other pruritic conditions demonstrated a decrease in the Investigator’s Global Assessment score of at least 2 points or a Numerical Rating Score reduction of at least 4 points. Adverse events occurred in five patients, but no serious adverse reactions were observed.
Conclusion
Dupilumab can be considered a safe and effective treatment for elderly patients with AD and other pruritic conditions.
5.N-acetylcysteine Restores Impaired Dentate Gyrus Neurogenesis in a Neonatal Maternal Separation Rat Model

Early-life stress (ELS) is a major contributor to neurodevelopmental vulnerability, particularly within the dentate gyrus (DG), where oxidative burden and microglial activation disrupt adult neurogenesis. Here, we examined whether N-acetylcysteine (NAC), a cysteine prodrug and glutathione precursor, could counteract impaired neurogenesis induced by neonatal maternal separation (NMS). Adolescent NAC administration restored the number of Ki67 + proliferating progenitors and DCX + immature neurons in the DG of NMS rats, accompanied by reduced reactive oxygen species, suppressed iNOS induction, and attenuated microglial activation. NAC also normalized EAAC1 expression, indicating enhanced neuronal antioxidant capacity. Notably, NAC rescued diminished neurogenesis in EAAC1 knockout mice, demonstrating its efficacy under both stress-induced and transporter-deficient redox imbalance. These findings identify NAC as a potent modulator of hippocampal neuroplasticity, acting through the restoration of redox and inflammatory homeostasis, and support its potential as an early therapeutic strategy to mitigate long-lasting neurodevelopmental consequences of ELS.
6.Guidelines for Packaging, Transport, and Storage of Source Cells for Organoids

This report presents guidelines for the systematic management of packaging, storage, transportation, and traceability of source cells used for organoid research. Given the important role of source cells in organoid studies, it is important to ensure the preservation of their quality and integrity throughout transportation and distribution processes. The proposed guidelines, therefore, call for a cohesive strategy through these stages to minimize the risks of contamination, deterioration, and loss–threats that significantly compromise the safety, efficacy, and efficiency of source cells. Central to these guidelines is the quality control measures that include roles and responsibilities across the entire supply chain, with recommendations specific to packaging materials, transportation facilities, and storage management. Furthermore, the need for an integrated management system is emphasized, spanning from source cell collection to the final application. This system is crucial for maintaining the traceability and accountability of source cells, facilitating the sharing, distribution, and utilization on a global scale, and supporting to advance organoid research and development.

Background:
During the coronavirus disease 2019 (COVID-19) pandemic, patients with myasthenia gravis (MG) were more susceptible to poor outcomes owing to respiratory muscle weakness and immunotherapy. Several studies conducted in the early stages of the COVID-19 pandemic reported higher mortality in patients with MG compared to the general population. This study aimed to investigate the clinical course and prognosis of COVID-19 in patients with MG and to compare these parameters between vaccinated and unvaccinated patients in South Korea.
Methods:
This multicenter, retrospective study, which was conducted at 14 tertiary hospitals in South Korea, reviewed the medical records and identified MG patients who contracted COVID-19 between February 2022 and April 2022. The demographic and clinical characteristics associated with MG and vaccination status were collected. The clinical outcomes of COVID-19 infection and MG were investigated and compared between the vaccinated and unvaccinated patients.
Results:
Ninety-two patients with MG contracted COVID-19 during the study. Nine (9.8%) patients required hospitalization, 4 (4.3%) of whom were admitted to the intensive care unit. Seventy-five of 92 patients were vaccinated before contracting COVID-19 infection, and 17 were not. During the COVID-19 infection, 6 of 17 (35.3%) unvaccinated patients were hospitalized, whereas 3 of 75 (4.0%) vaccinated patients were hospitalized (P < 0.001). The frequencies of ICU admission and mechanical ventilation were significantly lower in the vaccinated patients than in the unvaccinated patients (P = 0.019 and P = 0.032, respectively). The rate of MG deterioration was significantly lower in the vaccinated patients than in the unvaccinated patients (P = 0.041). Logistic regression after weighting revealed that the risk of hospitalization and MG deterioration after COVID-19 infection was significantly lower in the vaccinated patients than in the unvaccinated patients.
Conclusion
This study suggests that the clinical course and prognosis of patients with MG who contracted COVID-19 during the dominance of the omicron variant of COVID-19 may be milder than those at the early phase of the COVID-19 pandemic when vaccination was unavailable. Vaccination may reduce the morbidity of COVID-19 in patients with MG and effectively prevent MG deterioration induced by COVID-19 infection.
