1.Delayed Peripheral Facial Palsy after Acute Ischemic Stroke in the Territory of Anterior Inferior Cerebellar Artery

A 76-year-old male presented with dizziness and disequilibrium. Magnetic resonance imaging revealed an acute ischemic stroke in the left anterior inferior cerebellar artery (AICA) territory. Three days after admission the patient developed peripheral facial palsy with no radiological exacerbation of the infarction. He was managed with antiplatelet therapy and supportive care. Both the facial palsy and initial cerebellar symptoms resolved within 1 month. This case highlights delayed facial palsy as a rare presentation of AICA infarction.

Background:
Histone deacetylase (HDAC) inhibition offers potential anticancer effects across diverse cancers due to HDAC's significant role in cancer development and progression. Consequently, we demonstrated the therapeutic efficacy of the novel HDAC inhibitor, CG-745, in comparison with existing inhibitors such as suberoylanilide hydroxamic acid (SAHA) in non-small cell lung cancer (NSCLC) cells.
Methods:
CG-745's effect on apoptosis and reactive oxygen species (ROS)-dependent mitochondrial dysfunction was investigated using annexin V assay, MitoSoX, and Western blot in human A549 and H460 cells. Additionally, HDAC expression was analyzed through real-time polymerase chain reaction. We also evaluated the inhibitory effect of CG-745 on epithelial-mesenchymal transition (EMT) induced by transforming growth factor β1 (TGF-β1) via Western blot, scratch analysis, and matrigel invasion analysis.
Results:
Compared to SAHA, CG-745 inhibited cell viability and mRNA expression of HDACs such as HDAC1, HDAC2, HDAC3, and HDAC8. It also induced apoptosis, ROS, and mitochondrial dysfunction in a concentration-dependent manner. CG-745 reversed EMT triggered by TGF-β1 in A549 and H460 cells, and curtailed the migration and invasion enhanced by TGF-β1. CG-745 has demonstrably inhibited EMT and induced apoptosis in NSCLC cells.
Conclusion
CG-745 may represent a novel therapeutic strategy for NSCLC treatment.

7.The Value of Magnetic Resonance Imaging-Driven Apparent Diffusion Coefficient to Predict Pathologic Response of Neoadjuvant Chemotherapy in Muscle-Invasive Bladder Cancer: A Single-Center Retrospective Study

Purpose:
This study aimed to evaluate the predictive value of apparent diffusion coefficient (ADC) values from magnetic resonance imaging (MRI) in assessing pathologic response to neoadjuvant chemotherapy (NAC) in patients with muscle-invasive bladder cancer (MIBC) and the optimal cutoff value of ADC for predicting NAC response.
Materials and Methods:
Among 582 patients who underwent radical cystectomy between December 2011 and December 2024, we included 146 patients who received preoperative NAC and pretreatment MRI. Pathologic response was determined based on the surgical pathology results: patients with pathologic stage >pT2 and/or presence of nodal involvement (N+) were classified as nonresponders, while those with ≤pT2 and no nodal involvement (N0) were classified as responders. MRI-derived ADC values were compared between the 2 groups. The receiver operating characteristic (ROC) curve was used to determine the optimal cutoff value for predicting NAC response. Multivariate logistic regression analysis using preoperative parameter was performed and Kaplan-Meier survival analysis were utilized to assess overall survival (OS) and cancer-specific survival (CSS) according to NAC response.
Results:
Among the 146 patients, 90 (61.6%) were classified as responders, 56 (38.4%) were nonresponders. The mean ADC value was significantly higher in responders (1,100.35±249.12 mm2/sec) than in nonresponders (763.95±118.97 mm2/sec, p<0.001). ROC analysis demonstrated an area under the curve of 0.69, with an optimal cutoff value of 845.0 mm2/sec. Multivariate logistic regression identified ADC value as an independent predictor of NAC response (odds ratio, 1.025; 95% confidence interval, 1.006–1.043; p=0.008). During median follow-up 30.3 months, Kaplan-Meier analysis revealed that responders had significantly better 5-year OS (80.8% vs. 45.9%) and 5-year CSS (92.6% vs. 54.5%), with log-rank test p<0.001.
Conclusion
MRI-derived ADC values are a reliable imaging biomarker for predicting pathologic response to NAC in MIBC. Further validation in large-scale prospective studies is required to confirm these findings.
8.Cemented Bipolar Hemiarthroplasty with a Wiring Technique for Unstable Intertrochanteric Hip Fractures in Elderly Patients

Purpose:
Intertrochanteric hip fractures are commonly seen in the elderly population. Osteosynthesis is technically demanding and has a high rate of failure, especially in osteoporotic bones. Furthermore, delayed ambulation after surgery can be a risk factor for systemic complications. Active prevention of prolonged bed-ridden can improve the quality of life postoperatively. For early ambulation, the authors chose hemiarthroplasty as the surgical method. The purpose of this study was to assess the efficacy of cemented bipolar hemiarthroplasty with a wiring technique for unstable intertrochanteric hip fractures in the elderly.
Materials and Methods:
A retrospective review was conducted on the records of 41 patients with unstable intertrochanteric hip fractures treated with cemented bipolar hemiarthroplasty between January 2019 and December 2022. The mean patient age was 82 years, and cemented bipolar hemiarthroplasty with a wiring technique was performed in all cases. Clinical and radiologic outcomes were analyzed. The rate of complications and modified Harris hip score (HHS) at one-year follow-up were reviewed.
Results:
Early ambulation was initiated at a mean of 7.8 days postoperatively. Eight patients had systemic complications but recovered prior to discharge. There were no complications of loosening, dislocation, or infection indicated at the minimum one-year postsurgical follow-up. The mean modified HHS was 75.8.
Conclusion
Cemented bipolar hemiarthroplasty with a wiring technique showed positive clinical results in the treatment of unstable intertrochanteric hip fractures in elderly patients with osteoporosis. Furthermore, early ambulation could prevent recumbency-related complications. As a result, this technique is considered a good surgical alternative for an aging population.
9.The Influence of Femoral Internal Rotation on Patellar Tracking in Total Knee Arthroplasty Using Gap Technique

Femoral internal rotation in total knee arthroplasty (TKA) is well known as one of the main causes of patellar maltracking. Although femoral internal rotation in TKA is considered unacceptable due to the risk of patellar maltracking, it is sometimes required for ligament balancing. We evaluated the influence of femoral internal rotation on patellar tracking in TKA performed using the gap technique.
Methods:
From April 2008 to May 2018, 1,612 cases of TKA were done. Among them, 245 cases of TKA for osteoarthritis were followed up for at least 1 year and included in this study. We compared patellar tracking in two groups; group I consisted of 99 cases whose femoral rotation was less than 0° and group II consisted of 146 cases whose femoral rotation was 3°–5° external rotation. Preoperative femoral rotation was measured with the condylar twist angle (CTA) by using computed tomography. The patella was replaced in all cases. Patellar tracking was evaluated with patellar tilt angle (lateral tilt [+] and medial tilt [–]) in the merchant radiograph. Statistical analysis was done using Mann-Whitney U-test. Clinical assessment was performed using the Knee Society clinical rating system.
Results:
The preoperative CTA was 5.3° ± 1.6° in group I and 5.4° ± 1.6° in group II, showing no statistically significant difference between groups (p = 0.455). Intraoperative femoral rotation was –0.5° ± 0.8° in group I and 3.9° ± 0.8° in group II when the gap technique was used (p < 0.001). The postoperative patellar tilt angle was –0.4° ± 3.6° in group I and 0.1° ± 4.1° in group II with no statistically significant difference (p = 0.251).
Conclusions
Compared with femoral external rotation, femoral internal rotation with ligament balance in TKA was not more associated with patellar maltracking. Therefore, patellar tracking might be related with ligament balance in flexion regardless of the anatomic femoral rotational alignment.
