1.Clinical Application of LEI's Massage Therapy in Treating Children's Diseases by Restraining Wood and Supporting Earth

[Objective]To summarize the clinical experience of LEI's massage therapy of restraining wood and supporting earth in treating cough variant asthma,infantile anorexia and tic disorder in children.[Methods]Through clinical follow-up study,based on three medical records of cough variant asthma,infantile anorexia and tic disorder treated with LEI's massage therapy of restraining wood and supporting earth,combining with learning from the clinic and author's realization on treatment of children's diseases by restraining wood and supporting earth,this paper analyzed and summarized the clinical experience of applying LEI's massage therapy in treating disorder of liver and spleen diseases in children by restraining wood and supporting earth.[Results]Children have the physiological characteristics of"spleen is often deficient,liver is often surplus".When the spleen is weak,the liver is strong,subjugation and counterrestriction relationship between liver and spleen causes the disorder of liver and spleen in children.The treatment should be based on the principle of restraining wood and supporting earth,added and subtracted herbs according to the characteristics of the disease,such as soothing the liver and strengthening the spleen,clearing the lung and resolving phlegm massage to treat liver depression and spleen deficiency,phlegm fire disturbing the lung type cough variant asthma;soothing the liver and strengthening the spleen,regulating the stomach and aiding in transport to treat liver depression and spleen deficiency,stomach disorders type of infantile anorexia;strengthening the spleen and eliminating phlegm,calming the liver and calming wind to treat tic disorder of liver depression and spleen deficiency,internal wind phlegm disturbance syndrome.[Conclusion]LEI's massage manipulation of restraining wood and supporting earth,which is mainly composed of tonifying the spleen channel and clearing the liver channel,can strengthen the spleen and stomach,soothe the liver Qi,invigorate Qi and blood,dredge the meridians and regulate Qi activity,as a result,children's Zang and Fu organs are harmonized,Qi and blood are abundant,Yin and Yang are balanced,and the disease is cured,which is worthy of clinical promotion and application.
2.Expression efficiency of three DNA plamids and their mRNAs expressing foot-and-mouth disease virus (FMDV) antigenic proteins.

Foot-and-mouth disease (FMD) is one of the major animal infectious diseases in the world. All cloven-hoofed animals are susceptible to FMD. Vaccination is still the first choice for the prevention and control of FMD. mRNA vaccines can be rapidly designed, synthesized, and produced on a large scale in vitro, and they can induce effective protective immune responses, demonstrating the advantages of rapid development, easy preparation, and low biosafety risks. The design of untranslated regions is a key to enhancing the expression and efficacy of mRNA vaccines. In order to generate an efficient FMD mRNA vaccine, we designed three FMD P12A3C expression vectors with different untranslated regions and synthesized corresponding mRNAs. By comparing expression efficiency of these vectors and their mRNAs at different time points and in different cell lines, we found that the mRNA P12A3C-UTR3 had the best expression and universality. This study laid a foundation for the development of mRNA vaccines against FMD and provided a theoretical basis for the optimal sequence design of efficient mRNA.

4.Rehabilitation effects of psychomotor therapy on young and middle-aged schizophrenic inpatients

BackgroundRehabilitation for schizophrenia typically relies on pharmacological interventions, yet their efficacy in improving social function and quality of life remains limited. In recent years, non-pharmacological approaches have shown promise in enhancing rehabilitation outcomes. However, research on the effectiveness of psychomotor therapy specifically for young and middle-aged schizophrenic inpatients is limited. ObjectiveTo explore the effects of psychomotor therapy on the rehabilitation of young and middle-aged schizophrenic inpatients, and to provide a reference for treatment strategies. MethodsA total of 104 young and middle-aged schizophrenic inpatients who met the International Classification of Diseases,tenth edition (ICD-10) diagnostic criteria and hospitalized in the Fourth People's Hospital of Wuhu from June 2021 to June 2022 were selected. Patients were randomly divided into two groups of 52 each using random number table method. Both groups received treatment with risperidone tablets (2~4 mg/d) , along with routine nursing care. Additionally, the research group received an extra 45~55 minutes of psychomotor therapy 2~3 times per week for 12 weeks. The control group received the same psychomotor therapy after the study. Positive and Negative Symptom Scale (PANSS), Scales of Social-skills for Psychiatric Inpatient (SSPI) and Insight and Treatment Attitude Questionnaire (ITAQ) were used to assess the patients before the intervention and at 4th, 8th and 12th week after the intervention. ResultsThe main effects of intervention at different time points for PANSS positive symptoms, negative symptoms and general psychopathology subscale scores, PANSS total score, SSPI score and ITAQ score were all statistically significant (F=33.989, 204.245, 82.817, 279.596, 26.144, 7.463, P<0.01). Furthermore, statistically significant between-group differences were observed in PANSS negative symptoms and general psychopathology subscale scores, PANSS total score, SSPI score and ITAQ score (F=30.053, 5.306, 33.417, 33.013, 18.608, P<0.05 or 0.01). Moreover, the interaction effect of time and group were statistically significant for PANSS positive symptoms, negative symptoms and general psychopathology subscale score, PANSS total score and SSPI score (F=3.472, 9.798, 3.843, 14.390, 20.661, P<0.05 or 0.01). After 12 weeks of intervention, the research group exhibited statistically significantly lower PANSS total score and subscale scores compared with baseline (P<0.01), while their SSPI total score was significantly higher than that of control group(P<0.01). Additionally, compared with the control group, the research group had statistically significantly lower PANSS total score and subscale scores , while their SSPI score was statistically significantly higher than those of control group(P<0.01). ConclusionPsychomotor therapy may contribute to the improvement of the psychiatric symptoms and social function in young and middle-aged inpatients with schizophrenia, enhancing their rehabilitation outcomes. 5.Mitochondrial-targeted and ROS-responsive nanocarrier via nose-to-brain pathway for ischemic stroke treatment.

Oxidative stress injury and mitochondrial dysfunction are major obstacles to neurological functional recovery after ischemic stroke. The development of new approaches to simultaneously diminish oxidative stress and resist mitochondrial dysfunction is urgently needed. Inspired by the overproduced reactive oxygen species (ROS) at ischemic neuron mitochondria, multifunctional nanoparticles with ROS-responsiveness and mitochondrial-targeted (SPNPs) were engineered, achieving specific targeting delivery and controllable drug release at ischemic penumbra. Due to the nose-to-brain pathway, SPNPs which were encapsulated in a thermo-sensitive gel by intranasal administration were directly delivered to the ischemic penumbra bypassing the blood‒brain barrier (BBB) and enhancing delivery efficiency. The potential of SPNPs for ischemic stroke treatment was systematically evaluated in vitro and in rat models of middle cerebral artery occlusion (MCAO). Results demonstrated the mitochondrial-targeted and protective effects of SPNPs on H2O2-induced oxidative damage in SH-SY5Y cells. In vivo distribution analyzed by fluorescence imaging proved the rapid and enhanced active targeting of SPNPs to the ischemic area in MCAO rats. SPNPs by intranasal administration exhibited superior therapeutic efficacy by alleviating oxidative stress, diminishing inflammation, repairing mitochondrial function, and decreasing apoptosis. This strategy provided a multifunctional delivery system for the effective treatment of ischemic injury, which also implies a potential application prospect for other central nervous diseases.
6.Retrospective analysis of clinical characteristics of patients with metabolic-associated fatty liver disease at high and low altitude areas

Objective:To explore differences in clinical characteristics and hemoglobin levels between different age groups in patients with metabolic-associated fatty liver disease (MAFLD) at high and low altitude areas, so as to provide a basis for further research on the effect of chronic hypoxia on MAFLD.Methods:Liver function indexes, non-invasive fibrosis indexes, metabolic indexes, and routine blood test of 1 458 (Xining area of Qinghai province) and 1 633 cases (Huzhou area, Zhejiang province) with MAFLD who underwent physical examination were retrospectively analyzed. The total population of the two places were compared and analyzed with the hemoglobin reference limit of 180 g/L. The population of Xining was divided into high and low hemoglobin groups for comparative analysis. The population of the two places was divided into five groups according to age stratification (≤30 years old, 31-40 years old, 41-50 years old, 51-60 years old, ≥61 years old). After multivariate adjustment, the characteristics of high and low hemoglobin groups and MAFLD were compared between the two groups. Statistical analysis was performed with t-test or χ2 test. Results:The detected indexes values observed were higher in Xining than Huzhou area population [fibrosis indexes (FIB4, 1.08±0.02 vs. 1.19±0.02), erythrocyte (5.14±0.13 vs. 5.30±0.13), hemoglobin (156.82±0.37 vs. 164.19± 0.39), alanine aminotransferase (ALT, 33.70±0.66 vs. 43.68±0.70), aspartate aminotransferase (AST, 24.34±0.28 vs. 29.23±0.30), γ-glutamyltransferase (42.40±1.02 vs. 51.82±1.53), alkaline phosphatase (77.92±0.56 vs. 84.63±0.85), triglyceride (TG, 2.07±0.04 vs. 2.74±0.05), uric acid (UA, 383.42±2.15 vs. 406.44±2.36)]. The detected indexes values observed were higher in Huzhou than Xining area population [platelet count (220.54±1.32 vs. 181.62±1.40), total cholesterol (TC, 5.10±0.02 vs. 5.04±0.03), fasting blood glucose (FBG, 5.67±0.04 vs. 5.29±0.04)]. The differences were statistically significant ( P<0.01). Xining population UA and body mass index were increased in high hemoglobin group than low hemoglobin group, and the other parameters difference were not statistically significant. After the population in Xining was grouped by age, high and low FIB4, ALT, and AST and UA levels were detected in the age group of 31-40 and 51-60 years old, >61 years old, 31-40 years old, and the difference between hemoglobin groups were statistically significant ( P<0.01). Conclusion:Patients with MAFLD are more likely to develop fibrosis, liver function impairment, metabolic disorders and so on under high-altitude hypoxic environment. Additionally, there is certain correlation with disease severity and age changes, suggesting that chronic hypoxia can accelerate MAFLD progression.
7.The role and mechanism of miRNA in STZ-induced mouse diabetic hepatopathy

【Objective】 To reveal possible mechanisms of miRNA in diabetic hepatopathy through bioinformatics method. 【Methods】 Subset data of miRNA and their matched mRNAs in the liver of STZ-induced diabetic mice and the normal liver tissues of congenial mice by detecting on microarrays were collected from GEO database; information from the database and bioinformatics analysis were applied to mine a batch of miRNAs in diabetic hepatopathy and targeted mRNAs regulated. Then qRT-PCR was used to verify the expressions of miRNAs in diabetic liver from 20 STZ-treated Kunming mice and 10 normal homologous mice. 【Results】 Via detection and analysis, miRNAs differentially expressed (including 96 up-regulated and 77 down-regulated) were significantly obtained. Groups of miRNAs and their effectors (mRNAs) that may be related to the pathological process of diabetic liver disease in mice were screened by GO and KEGG enrichment analyses, combined with relevant protein annotations in the databases and references. The expressions of miR-200a-3p, miR-200b-3p and miR-222-3p in the mice’s liver tissue detected by qRT-PCR were significantly down-regulated. In addition, the expressions of related effector genes CERS6, MYBL1, SCD2, SLCO1A4 and PLK2 were up-regulated, while the expressions of ACSS2, BCL6 and SLC10A2 were down-regulated. 【Conclusion】 The variation trend of those candidate miRNAs in mouse diabetic liver compared with that in control livers was consistent with that of the previous studies and prediction, which revealed their potential molecular regulation in this disease process.

Objective To explore the effect of comprehensive nursing intervention on ICGR15, psychological ability and self-care ability of patients with hepatocellular carcinoma treated by transcatheter hepatic arterial chemoembolization.Methods A total of 140 patients received transcatheter hepatic arterial chemoembolization in our hospital were divided into control group (n=70) and experimental group (n=70) according to the different nursing methods.The control group received routine nursing care and the experimental group received comprehensive nursing intervention.The ICGR15 level, psychological ability and self-care ability of two groups were observed and compared.The postoperative complications were recorded.Results After nursing, the ICGR15 level in the experimental group was significantly lower than that in the control group(P<0.01).The psychological ability and self-care ability of the experimental group were better than that of the control group(P<0.01).The total complication rate of the experimental group was 12.86% (9/70), which was significantly lower than 34.29%(24/70) of the control group, and the differences were statistically significant (P<0.01).Conclusion Comprehensive nursing intervention can effectively reduce the damage of liver reserve function and the incidence of complications, and improve the patient''s psychological and self-care ability.So it is worthy of clinical promotion.

10.Value of C-reactive protein level on transplantation day in predicting early post-transplant infections and outcomes of allogeneic hematopoietic stem cell transplantation.

OBJECTIVETo investigate the value of C-reactive protein (CRP) on transplantation day in predicting early post-transplant infections and outcomes of allogeneic hematopoietic stem cell transplantation (allo-HSCT).
METHODSWe retrospectively analyzed the clinical data of 78 recipients undergoing allo-HSCT. The clinical reference value of CRP on transplantation day was determined, and its sensitivity and specificity for diagnosing bacteremia was analyzed using receiver-operating characteristic curve (ROC). The incidence of transplant-related complications, overall survival, and relapse rate of the patients were analyzed with respect to the CRP level.
RESULTSThe clinical reference value of CRP for diagnosing bacteremia was 23.3 mg/L (AUC=0.735 [95% CI: 0.623-0.848], P=0.001), which had a diagnostic sensitivity and specificity of 0.793 and 0.592, respectively. Compared with the patients with low CRP levels, the patients with high CRP levels tended to have delayed neutrophil reconstitution and platelet engraftment by 0.71 days (P=0.237) and 4.09 days (P=0.048), respectively, and had a significantly higher incidence of bacteremia (17.1% vs 53.5%, P=0.001) and CMV viremia (37.1% vs 72.1%, P=0.003) within 100 days following the transplantation; the incidences of EBV viremia, pulmonary invasive fungal infection, or acute graft versus host disease (aGVHD) showed no significant difference between the two groups (41.9% vs 22.9%, P=0.094; 14.0% vs 5.7%, P=0.285; 51.2% vs 45.7, P=0.656, respectively). During the follow-up for a median of 318 (7-773) days in high-CRP group and for 299 (78-747) days in low-CRP group, the high-CRP group showed a significantly lower 2-year overall survival than the low-CRP group (42.5% vs 78.4%, P=0.022), and tended to have a higher 2-year cumulative relapse rate (52.3% vs 19.8%, P=0.235). Logistic multivariate analysis identified a high CRP level on transplantation day as the independent risk factor for post-transplant bacteremia within 100 days (OR=5.090 [95% CI: 1.115 -23.229], P=0.036).
CONCLUSIONA high CRP level on transplantation day can be indicative of a high risk of early post-transplant bacteremia and CMV viremia and also a poor prognosis following allo-HSCT.
