1.The effect of different timing of polyethylene glycol electrolyte powder administration on intestinal cleansing efficacy

Objective To investigate the effects of a continuous-dose administration versus different dosage regimens of polyethylene glycol electrolyte solution(PEG)taken in two doses with a 12-hour interval on bowel cleansing efficacy,with the goal of optimizing bowel preparation protocols and improving patient tolerability.Methods 232 patients who underwent painless colonoscopy and used PEG as a bowel cleanser from June 2024 to September 2024 were selected as study subjects.Participants were divided into three groups:the control group(3.00 L PEG continuous dose),experimental group A(0.75 L+2.25 L PEG),and experimental group B(1.50 L+1.50 L PEG).All patients underwent painless colonoscopy within 4~6 h after completing PEG intake.The interval between the two doses of PEG in group A and group B was 12 h.The bowel cleansing efficacy was assessed by using the Boston bowel preparation scale(BBPS),and the rates of colon polyp detection,adverse reactions,sleep duration,and tolerability were recorded.Results There were no significant statistical differences in BBPS scores and colon polyp detection rates among the three groups(P>0.05).Experimental group B experienced the least adverse reactions,followed by experimental group A,while the control group reported the most significant adverse reactions(P<0.05).The timing of PEG administration did not have a significant impact on sleep duration among the three groups(P>0.05).Patients in experimental group B showed good tolerability to PEG and were willing to accept this bowel preparation regimen,followed by group A,while the control group exhibited the poorest tolerability,with significant statistical differences among the three groups(P<0.05).Conclusion The continuous administration and divided administration of PEG have no significant impact on the effectiveness of intestinal cleansing and the detection rate of colonic polyps.However,the divided PEG regimen with a 12 h interval results in fewer adverse reactions and better tolerance,especially the optimal regimen of taking 1.50 L PEG in two doses with a 12 h interval.
2.Effects of preoperative discussions on surgical knowledge acquisition and mastery among gastrointestinal surgery residents

Objective:To investigate the effects of preoperative discussions on the surgical acquisition and mastery of gastrointestinal surgery residents.Methods:A prospective randomized controlled trial was conducted in 90 residents and they were randomly divided into the conventional group,the simulation group,and the clinical group,with 30 residents in each group.Residents in each group received traditional review,simulated preoperative discussions training,and real preoperative discussions with patients'family members,respectively.Theoretical knowledge assessments were conducted at three stages.Communication skills were evaluated after stages Ⅱ and Ⅲ.The impact of different teaching methods on learning outcomes were compared.Results:There was no significant difference in the theoretical knowledge scores among the three groups at stage Ⅰ(P>0.05).At stage Ⅱ and Ⅲ,the theoretical knowledge scores in both the simulation and clinical groups were significantly higher than those in the conventional group(P<0.01).At stage Ⅲ,the theoretical knowledge score in the clinical group was higher than that in the simulation group(P<0.05).The communication skill in the simulation group at stage Ⅲ was higher than that at stage Ⅱ(P<0.05),and family satisfaction scores with the preoperative discussions at stageⅢ in the clinical group was higher than that at stage Ⅱ(P<0.05).Conclusions:Preoperative discussions,particularly direct interactions with patients'families,significantly enhances both the acquisition of surgical knowledge and the development of communication skills among gastrointestinal surgery residents.Simulated preoperative discussions also proves more effective than traditional teaching methods.

5.Prevention and Treatment of Schizophrenia by Traditional Chinese Medicine Targeting PI3K/Akt Signaling Pathway: A Review

As one of the most difficult-to-cure neuropsychiatric disorders in clinical practice, schizophrenia is mainly manifested by behavioral abnormalities and multidimensional cognitive dysfunction, and the recurrence rate and disability rate of the disease are increasing year by year, which seriously affects patients' social functioning and quality of life, and even threatens the physical and mental health of the surrounding population. At present, the treatment of schizophrenia is mainly based on antipsychotic drugs combined with psychotherapeutic techniques, which have limited long-term therapeutic effects and a high relapse rate. Traditional Chinese medicine (TCM) boasts the advantages of multi-targets, multi-pathways, multi-links, and multi-levels, and plays a crucial role in the prevention and treatment of schizophrenia and its prognosis. Phosphatidylinositol 3-kinase (PI3K) is widely present in cells and is involved in the regulation of protein synthesis and apoptosis, and the different isoforms of protein kinase B (Akt) are of great significance in cell growth, oxidative stress, neuronal development and other processes. In recent years, a large number of studies have found that the PI3K/Akt signaling pathway is closely related to schizophrenia. Through regulating the PI3K/Akt signaling pathway, TCM monomers and TCM compounds mainly affect key signaling molecules such as mammalian target of rapamycin (mTOR), glycogen synthase kinase (GSK), glucose transporter (GLUT) for glucose uptake and transport, and nuclear factor E2-associated factor 2 (Nrf2), which organize the intracellular network of centers and regulate the formation and plasticity of neuronal synapse, and they play an important role in mitigating schizophrenia by regulating the processes of cell proliferation, migration and apoptosis of neurons, and has the advantages of multi-targets, all-encompassing and low toxicity. This article analyzes and explains the mechanism of TCM intervention in the PI3K/Akt signaling pathway against schizophrenia, in order to provide a theoretical basis and reference for the prevention and treatment of schizophrenia by TCM.
6.Mental health status and influencing factors of China medical team members in Africa during COVID-19

【Objective】 To analyze the mental health status and influencing factors of China medical team (CMT) members in Africa during COVID-19 pandemic. 【Methods】 From July 2021 to August 2021, 72 members of the 8th CMT in Malawi, the 36th CMT in Sudan and the 22nd CMT in Zambia were tested by 12-item General Health Questionnaire (GHQ-12), Generalized Anxiety Disorder-7 (GAD-7), and Patient Health Questionnaire-9(PHQ-9), general information form and influencing factors form. 【Results】 The results of GHQ-12 were positive for 33.3% (24/72) of the CMT members. 51.4% (37/72) of the CMT members showed different levels of anxiety: 40.3% (29/72) of them had mild anxiety, 8.3% (6/72) of them had moderate anxiety, and 2.8% (2/72) of them had severe anxiety. 52.8% (38/72) of the CMT members had different degrees of depression: 34.7% (25/72) of them had mild depression, 11.1% (8/72) of them had moderate depression, 4.2% (3/72) of them had moderate-severe depression, and 2.8% (2/72) of them had severe depression. The CMT members who contacted with COVID-19 patients got significantly high scores of GHQ-12, GAD-7 and PHQ-9 (P<0.05) compared to those who did not have contact with COVID-19 patients. And CMT members who did not adapt to the local social life got significantly higher scores than those who adapted to the local social life (P<0.05). These factors were the main factors affecting the mental health of the CMT members. 【Conclusion】 During COVID-19, the psychological pressure of CMT members was increased significantly, and both the incidence and severity of anxiety and depression were increased. Paying attention to and improving CMT members’ mental health status can ensure the smooth development of medical aid to Africa.

The fall armyworm (FAW), Spodoptera frugiperda, is a destructive pest native to America and has recently become an invasive insect pest in China. Because of its rapid spread and great risks in China, understanding of FAW genetic background and pesticide resistance is urgent and essential to develop effective management strategies. Here, we assembled a chromosome-level genome of a male FAW (SFynMstLFR) and compared re-sequencing results of the populations from America, Africa, and China. Strain identification of 163 individuals collected from America, Africa and China showed that both C and R strains were found in the American populations, while only C strain was found in the Chinese and African populations. Moreover, population genomics analysis showed that populations from Africa and China have close relationship with significantly genetic differentiation from American populations. Taken together, FAWs invaded into China were most likely originated from Africa. Comparative genomics analysis displayed that the cytochrome p450 gene family is extremely expanded to 425 members in FAW, of which 283 genes are specific to FAW. Treatments of Chinese populations with twenty-three pesticides showed the variant patterns of transcriptome profiles, and several detoxification genes such as AOX, UGT and GST specially responded to the pesticides. These findings will be useful in developing effective strategies for management of FAW in China and other invaded areas.

8.Modified transobturator tape for female stress urinary incontinence

Objective To explore the efficacy and safety of modified transobturator tape (TOT) for female stress urinary incontinence (SUI).Methods From June 2015 to June 2017,a total of 87 SUI patients,including 35 patients underwent standard TOT operation (standard TOT group) and 52 patients underwent modified TOT operation (modified TOT group),were retrospectively reviewed.There was no statistical difference of age [(59.7 ± 10.3) yea~ vs.(56.3 ± 9.1) years],BMI [(24.I ± 9.7) kg/m2 vs.(24.6 ± 9.3) kg/m2],diabetes history [31.4% (11/35) vs.26.9% (14/52)],mixed urinary incontinence [45.7% (16/35) vs.48.1% (25/52)] and the daily amount of urine pad [(4.3 ±2.7) vs.(3.9 ± 2.1)] between the two groups (P > 0.05).The operative time,intraoperative complications,and postoperative complications were collecteded in two groups.Patients were followed up at 3 months,6 months,and 1 year after surgery.Results There was no significant difference in operation time [(21.1 ± 4.3) min vs.(20.5 ± 5.7) min],intraoperative hemorrhage [(18.3 ± 9.1) ml vs.(25.7 ± 8.3) ml] and postoperative incidence of urinary retention [2.9% (1/35) vs.3.8% (2/52)] between the two groups (P > 0.05).The incidence of postoperative leg pain was significantly lower in modified TOT group than in TOT group[1.9% (1/52) vs.20.0% (7/35),P < 0.05].There was no significant difference in subjective cure rate and objective cure rate between the two groups at 3 months,6 months and 1 year after surgery (P > 0.05).Conclusions Compared with the standard TOT,the modified TOT of modified puncture port has a similar cure rate and efficiency.However,the use of modified TOT can significantly reduce the incidence of postoperative short-term leg pain,but the long-term efficacy still needs to be further followed-up.

Objective:
To explore the difference of liver inflammation and fibrosis in patients with chronic hepatitis B virus (HBV) infection and chronic hepatitis C virus (HCV) infection, and to investigate the relationship between hepatic pathology and alanine aminotransferase (ALT).
Methods:
57 patients with chronic HCV infection and 346 patients with chronic HBV infection who were hospitalized at Shengjing Hospital of China Medical University from January 2012 to September 2016 were enrolled. In chronic HBV infection, including 88 cases whose ALT were more than two times of upper limited of normal (ALT≥2×ULN) and 258 cases whose ALT were less than two times of upper limited of normal (ALT < 2×ULN).All the patients were underwent liver biopsy. Chronic HBV infection (ALT≥2×ULN and ALT < 2×ULN) and chronic HCV infection were compared respectively. Statistical analyses were performed using a Univariate
