1.Association of Obesity Phenotypes With Cognitive Impairment and Genetic Stratification Analysis in Older Chinese Adults

Objective To evaluate the association of different obesity phenotypes and their components with the risk of cognitive impairment in older Chinese adults,and to assess the association between obesity and cognitive impairment in different cognition-related genetic backgrounds.Methods A cross-sectional study based on the West China Health and Aging Cohort was conducted.Logistic regression was applied to estimate the association of obesity phenotypes and components with cognitive impairment in older Chinese adults stratified by APOE gene and polygenic risk scores.Results A total of 7 316 participants were enrolled,of whom 1 820 had cognitive impairment.Weight gains were associated with a reduced risk of cognitive impairment(odds ratio[OR]=0.96,95%CI,0.95-0.97).Being overweight with a normal waist-to-hip ratio was a protective factor for cognition(OR=0.74,95%CI,0.61-0.90),whereas the coexistence of elevated waist-to-hip ratio and overweight did not increase the risk of cognitive impairment.Sarcopenia was associated with an elevated risk of cognitive impairment.This association was found in both overweight(OR=2.03,95%CI,1.71-2.41)and non-overweight older adults(OR=1.86,95%CI,1.58-2.20),and was significant across all polygenic risk score strata.Conclusion Increasing body mass may serve as a key protective factor against cognitive decline in older adults.Having sarcopenia and obesity is associated with an elevated risk of cognitive impairment,independent of genetic susceptibility.
2.Predictive value of T2*-mapping in early damage of medial meniscus posterior root in asymptomatic knee osteoarthritis

Objective To investigate the application of MRI T2*-mapping in the early damage of the medial meniscus posterior root(MMPR)in asymptomatic knee osteoarthritis(OA).Methods Eighty subjects were included in this study,35 were diagnosed with knee OA(OA group)and clinically confirmed MMPR injury,35 were asymptomatic OA group with gender and age matching,and 10 were normal control group.All subjects were examined by T2*-mapping.The T2*-mapping values at the bone attachment,middle part,and 1 cm bone attachment point of MMPR were measured in each group,and the consistency of T2*-mapping values between the knee OA group and the asymptomatic OA group was verified by the Kappa test.The T2*-mapping values of each measurement area were statistically compared,and the clinical diagnosis accuracy and other indicators of the T2*-mapping parameter values were statistically analyzed.Results The Kappa value of the knee OA group and the asymptomatic OA group analyzed by T2*-mapping was 0.787(P<0.01),Kappa statistical analysis showed that there was a good consistency between the two diagnostic results.The T2*-mapping values of the knee OA group,asymptomatic OA group,and normal control group at the bone attachment,middle part,and 1 cm bone attachment point of MMPR showed that the T2*-mapping values of each measurement area in the knee OA group and asymptomatic OA group were higher than those in the normal control group(P<0.05).The T2*-mapping values of the knee OA group were higher than those of the asymptomatic OA group,and the difference was statistically significant(P<0.05).While the T2*-mapping values were used in the asymptomatic OA group to diagnose the early damage of MMPR,the sensitivity,specificity,accuracy,negative predictive value,and positive predictive value were 89.6%,88.9%,91.1%,87.5%,and 88.3%respectively.Conclusion T2*-mapping value may be used as a reference index to predict the progression of knee OA,and has a certain value in the early diagnosis of asymptomatic OA MMPR injury.
3.Pharmaceutical service in a case of fat embolism syndrome following postoperative fracture

OBJECTIVE To analyze the pharmaceutical service process in a fracture patient complicated by fat embolism syndrome (FES) following postoperative fracture, aiming to provide a reference for clinical treatment and pharmaceutical service for similar patients. METHODS Clinical pharmacist participated in the entire treatment process of a patient with FES following postoperative fracture. Based on the patient’s clinical manifestations and test results, literature was reviewed to assist clinical physicians in formulating the therapeutic regimen of glucocorticoids. For the drug-related adverse reactions of renal function impairment and reduced platelet count that occurred during the treatment, suspicious drugs were analyzed and disposed of accordingly. RESULTS The clinical pharmacist recommended Hydrocortisone sodium succinate for injection (100 mg, q8 h, ivgtt, for about one week followed by a gradual dose reduction) for treating FES. The Vancomycin hydrochloride for injection used in this case was assessed as “very probably” associated with the adverse drug reactions of renal function impairment and thrombocytopenia. The clinical physician adopted the pharmacist’s medication recommendations, and the patient’s condition stabilized after treatment, with improvement in adverse reactions, and was discharged from the hospital. CONCLUSIONS The use of glucocorticoids in treating FES has a definite therapeutic efficacy. Clinical pharmacists should individualize the medication plan based on the patient’s pathological state and distinguish it from postoperative sepsis. Meanwhile, drug-induced adverse reactions in the kidney and blood system should be closely monitored.
4.Clinical application and efficacy analysis of castor stent graft in the treatment of aortic diseases

Objective:To summarize the use of Castor stent graft in aortic diseases and to analyze their efficacy.Methods:The clinical data of patients with aortic diseases treated with Castor stent graft from November 2017 to August 2022 in Fujian Provincial Hospital were collected and divided into branched stent group and branched stent co-operative group according to the operation method, and the clinical data of both groups were summarized.Results:A total of 75 cases of aortic disease were treated with Castor stents, and finally 53 cases were classified as branched stent group and 22 cases as branched stent co-operative group. The operations in both groups were successful. The median operative time in the branched stent group was 120(100, 160)min, and the median postoperative hospital stay was 7.0(5.5, 10.5)days.There was no postoperative ischemic stroke, no spinal cord ischemia. One case of new aortic dissection occurred. During follow-up, there was one lost case and two deaths, and the rest did not have endoleak, branch stent stenosis, ischemic stroke or re-intervention. In the branched stent co-operative group, there was one postoperative ischemic stroke, one case of slight stenosis of the left subclavian artery stent during follow-up, the remaining cases had satisfactory postoperative outcomes.Conclusion:Castor stent graft is a safe and effective procedure in the treatment of aortic diseases. And Castor stent graft can be used in combination with other endovascular repair techniques in the treatment of complex aortic diseases, with safe and reliable postoperative outcomes.
5.Quantitative MRI analysis of anterior cruciate ligament sprain and chronic injury of knee joint and comparison study with arthroscopy

Objective To study the application value of 3.0T MRI T2 mapping quantitative technology in the diagnosis of anterior cruciate ligament sprain and chronic injury of knee joint.Methods A total of 82 subjects were studied,and the experimental group 72 cases was divided into grade Ⅰ injury group(25 cases),grade Ⅱ injury group(25 cases),chronic injury group(22 cases),and control group 10 cases.The experimental group met the criteria of arthroscopy.The proximal,middle,and distal segments of the anterior cruciate ligament were selected as the region of interest(ROI),and T2 mapping values were measured.The differences in T2 mapping values of each area were compared between and within the groups,while compared with arthroscopy.Results The T2 mapping values in grade Ⅰ,Ⅱ,and chronic injury groups were higher than those in control group(P<0.05).Comparison within the experimental group:the T2 mapping values of each area in grade Ⅱ injury group were higher than those in grade Ⅰ injury group and chronic injury group(P<0.05).The T2 mapping values of each area in grade Ⅰ injury group were higher than those in chronic injury group(P<0.05).The specificity,sensitivity,positive predictive value,negative predictive value and accuracy of T2 mapping in diagnosing anterior cruciate ligament grade Ⅰ injury were 94.7%,95.5%,89.7%,96.6%,and 90.2%respectively.The specificity,sensitivity,positive predictive value,negative predictive value,and accuracy of grade Ⅱ injury were 89.4%,87.9%,92.1%,93.4%,and 93.8%respectively.The specificity,sensitivity,positive predictive value,negative predictive value,and accuracy of chronic injury were 92.2%,95.4%,90.3%,87.6%,and 91.5%respectively.Kappa test showed a good con-sistency between T2 mapping results and arthroscopic results,with a Kappa value of 0.763(P<0.01).Conclusion The value of MRI T2 mapping can provide a reference for the clinical diagnosis of anterior cruciate ligament sprain and chronic injury of knee joint,and the results are in good agreement with the control of arthroscopy.
6.Establishment of Specific Chromatogram and TLC Identification for Qingxin Lianziyin

ObjectiveTo establish the specific chromatogram and thin layer chromatography(TLC) of Qingxin Lianziyin(QXLZY) benchmark samples, in order to clarify the key quality attributes and provide a reference for the quality evaluation of QXLZY. MethodHigh performance liquid chromatography(HPLC) specific chromatogram of QXLZY benchmark samples was developed by using a YMC Hydrosphere C18 column(4.6 mm×250 mm, 5 μm) with the mobile phase of acetonitrile(A)-0.2% formic acid aqueous solution(B) for gradient elution(0-10 min, 5%-20%A; 10-20 min, 20%A; 20-25 min, 20%-24%A; 25-40 min, 24%-30%A; 40-55 min, 30%-50%A; 55-65 min, 50%-100%A; 65-75 min, 100%A; 75-75.1 min, 100%-5%A; 75.1-90 min, 5%A), and the detection wavelength was 360 nm. Ultra-high performance liquid chromatography-linear ion trap/orbitrap mass spectrometry(UHPLC-LTQ-Orbitrap MS) with electrospray ionization(ESI) was used to identify the components of QXLZY benchmark samples by accurate relative molecular weight and multilevel MS fragment ion information, the detection conditions were positive and negative ion modes and data dependency scanning mode. TLC identification methods for Ophiopogonis Radix, Lycii Cortex, Nelumbinis Semen, Poria, Astragali Radix and Ginseng Radix et Rhizoma in QXLZY were established. ResultA total of 15 characteristic peaks were identified from Glycyrrhizae Radix et Rhizoma, Plantaginis Semen and Scutellariae Radix, and the relative standard deviations of the retention times of 15 characteristic peaks in 15 batches of QXLZY benchmark samples were≤3% with peak 8(baicalin) as the reference peak. A total of 100 compounds, including flavonoids, organic acids, saponins, amino acids and others, were identified in the benchmark samples by UHPLC-LTQ-Orbitrap MS. The established TLC had good separation and was suitable for the identification of Ophiopogonis Radix, Lycii Cortex, Nelumbinis Semen, Poria, Astragali Radix and Ginseng Radix et Rhizoma in QXLZY. ConclusionThe material basis of QXLZY benchmark samples is basically determined by MS designation and source attribution. The established specific chromatogram and TLC of QXLZY are simple, stable and reproducible, which can provide a reference for the development and quality control of QXLZY.
7.Analysis on the Selection of Acupoints and the Application of Manipulation in the Treatment of Diabetes Peripheral Neuropathy with Tuina

Objective To explore the rules of acupoint selection and manipulation application of Tuina in the treatment of diabetes peripheral neuropathy(DPN)with data mining technology.Methods The clinical research literature of Tuina for DPN from October 2022 was selected by searching four Chinese databases that CNKI,WanFang,VIP and Chinese Biomedical Literature Database.Using the traditional Chinese medicine inheritance assistance platform software,analyze and summarize the rules of selecting acupoints and applying manipulations in the treatment of DPN with Tuina.Results A total of 110 articles were included,including 65 acupoints and 33 manipulations.The acupoints with the highest frequency are Zusanli(ST 36),Sanyinjiao(SP 6),Taixi(KI 3),Yongquan(KI 1),Taichong(LR 3),Yanglingquan(GB 34),etc.The meridians mainly include Taiyin Spleen Meridian of Foot,Yangming Stomach Meridian of Foot,Taiyang Bladder Meridian of Foot and Shaoyin Kidney Meridian of Foot.Five-shu Points,Lower He-sea Points and Yuan-primary Points are commonly used specific points.The acupoints are mostly distributed in the lower limbs.The categories of Tuina manipulations mainly include squeezing-pressing manipulations,pushing rolling manipulations and composite manipulations.The Tuina manipulations mainly include kneading manipulation,pressing manipulation,point-pressing manipulation,pressing-kneading manipulation and twisting manipulation with both palms.Conclusion The acupoint selection and manipulation application of Tuina in treating DPN have certain rules,and the main treatment principles are to regulate the organs and dredge collaterals,and to replenish qi and promote blood circulation,which can provide objective basis for clinical treatment.
8.Application of 3.0T MR T2 mapping in the diagnosis of early temporomandibular joint dysfunction

Objective To quantitatively study the pathological characteristics of the joint disc in the early stage of temporomandibular joint dysfunction based on 3.0T MR T2 mapping technology.Methods There were 90 patients in the study,including 30 in the control group and 60 in the experimental group.All patients were clinically diagnosed with early temporomandibular joint dysfunction.The 90 patients underwent conventional 3.0T MR and T2 mapping scanning in open and closed mouth positions,the T2 mapping pseudo-color images of the temporomandibular joint disc was observed,and the T2 mapping values at the anterior extension,anterior,middle,posterior bands of the temporomandibular joint disc,as well as the upper and lower plates of the double plate area were quantitatively measured.The T2 mapping values of the above sites were compared between and within the two groups.Results The T2 mapping values in the anterior extension,anterior band,middle band,posterior band,upper plate and lower plate of the bilateral temporomandibular joint disc in the open and closed positions of the experimental group were significantly higher than those in the control group(P<0.05).The T2 mapping values in the open and closed positions of the experimental group were higher than those in other areas(P<0.05).There was no significant difference in T2 mapping values between the control group and the experimental group in the observation areas of the bilateral tem-poromandibular joint disc at the open and closed positions(P>0.05).Conclusion The 3.0T MR T2 mapping can be used to quantitatively assess the joint disc injury of temporomandibular joint dysfunction in the early stage,and the value of T2 mapping in the middle band of the disc in the experimental group is higher than that in other observation areas.
9.Effects of ginkgolide B on Caspase-3/PTEN/Akt pathway and cell proliferation and apoptosis in hypoxia/reoxygenation cardiomyocytes

Objective To investigate the effect of ginkgolide B (GB) on cysteinyl aspartate specific proteinase-3 (Caspase-3)/chromosome 10 deletion phosphatase-tension protein homologue (PTEN)/protein kinase B (Akt) pathway and cell proliferation and apoptosis in hypoxia/reoxygenation cardiomyocytes. Methods H9C2 cells were cultured in vitro. A control group was cultured in serum-free DMEM high glucose medium at 37°C and 5% CO2 for 28 hours. The remaining groups were prepared with hypoxia/reoxygenation models. A GB low-dose group and a GB high-dose group were treated with GB pretreatment with final concentration of 50 μmol/L and 200 μmol/L respectively at 1 h before hypoxia/reoxygenation. A carvedilol group was treated with carvedilol of a final concentration of 10 μmol/L at 1 h before hypoxia/reoxygenation. The proliferation and apoptosis of H9C2 cells were detected, and the levels of lactate dehydrogenase (LDH), malondialdehyde (MDA), reactive oxygen species (ROS), PTEN, Akt, phosphorylated Akt (p-Akt) and Caspase-3 in H9C2 cells were also detected. Results Compared with the control group, the proliferation rate of H9C2 cell, and the levels of PTEN, Akt and p-Akt in other groups decreased, and the apoptosis rate, and the levels of LDH, MDA, ROS and Caspase-3 increased (P<0.05). Compared with the hypoxia/reoxygenation group, the proliferation rate of H9C2 cell, and the levels of PTEN, Akt and p-Akt in all GB dose groups and the carvedilol group increased; the apoptosis rate, and the levels of LDH, MDA, ROS and Caspase-3 decreased, and the effect of GB was in a dose dependent manner; however, the effect of GB was not as strong as carvedilol (P<0.05). Conclusion GB can inhibit H9C2 cell apoptosis and promote H9C2 cell proliferation by activating Caspase-3/PTEN/Akt pathway.
10.Establishment of a Method for Determination of Amygdalin in Quality Control System of Qingfei Paidu Granules

ObjectiveAlmond, which is bitter in taste, contains traces of toxic substances. For the sake of the safety of prescriptions containing this medicinal material, the processing method of "soaking in boiling water" was selected. Moreover, through literature research and network pharmacology, the characteristic index of this medicinal material was determined. On this basis, a method was established for the determination of amygdalin in Qingfei Paidu Granules (QFPD) and the transfer rate of it in the processing of this prescription was monitored, aiming at improving the quality control system of QFPD. MethodThe high performance liquid chromatography conditions are as follows: YMC Triart C18 column (4.6 mm × 150 mm, 5 µm), mobile phase of methanol-water with flow of 1.0 mL·min-1, column temperature of 35 ℃, and detection wavelength of 210 nm. ResultThe linear curve fitted well and the average recovery of amygdalin was 97.74% with RSD of 4.3%. The transfer rates of amygdalin from the medicinal material to the extract, from extract to concentrate, and from concentrate to granules were investigated with this method. The result showed that the average transfer rate from the medicinal material to the granules was (60±3.91)%. The comparison of transfer rate between the processes suggesting that the extraction of the medicinal material might be the key part influencing the prescription preparation. ConclusionThe method is simple, sensitive, reproducible, stable, and accurate, and the index is reasonable. Thus, the method can be used for the quality control of QFPD and determination of transfer rate of components in the preparation of QFPD. This study further improves the quality control standard of almond in QFPD, which can serve as a reference for the clinical application of QFPD.
