ObjectiveThis study focused on the marketed Chinese patent medicines for the treatment of Functional Diarrhea (FDr) in China and their prescription characteristics, in order to provide support for the clinical application and research and development of anti-FDr Chinese patent medicines. MethodsCollect the information of Chinese patent medicines that have been marketed to treat FDr, use Microsoft Excel 2021 software to conduct preliminary data collation and statistical analysis, and use the ancient and modern medical record cloud platform (V2.3.9) to analyze the standardized Chinese patent medicine prescriptions from the aspects of drug nature and taste, medication characteristics and prescription rules. Results147 kinds of FDr Chinese patent medicines were included in this study. There are a total of 40 varieties of FDr Chinese patent medicines suitable for children; The distribution of dosage forms is mainly pills, tablets, and capsules. 110 prescriptions were screened, among which the proportion of Chinese patent medicines for the treatment of spleen deficiency syndrome was the highest; The top three drug use frequency were licorice, Atractylodes macrocephala, and Poria cocos; The medicinal properties are mainly warm and flat, and the medicinal taste is mostly pungent, sweet and bitter, and most of them belong to the two meridians of the spleen and stomach; The association rules analysis obtains 20 strong association pairing sets; Three drug combinations were obtained by cluster analysis. ConclusionFDr Chinese patent medicine shows unique value in clinical application, especially in the field of children. However, there are still problems such as strong professionalism in the indication expression of drug instructions, limited coverage of the medical insurance catalog, and lack of high-level evidence-based medicine and pharmacoeconomic evidence. To this end, in the future, efforts should be made to build a multi-level evidence-based evidence system, improve medication compliance, and deepen research on syndrome-based medication laws, so as to enhance the clinical application value and scientific connotation of FDr Chinese patent medicines.

Allergen-specific immunotherapy(AIT) remains the only therapeutic approach with the potential to modify the natural course of allergic rhinitis(AR). Nevertheless, considerable inter-individual variability exists in patients'responses to AIT. To facilitate more reliable assessment of treatment efficacy, the China Rhinopathy Research Cooperation Group(CRRCG) convened young and middle-aged nasal experts in China to formulate the present consensus. The recommended subjective outcome measures for AIT comprise symptom scores, medication scores, combined symptom and medication scores, quality-of-life assessments, evaluation of disease control, and assessment of comorbidities. Objective indicators may supplement these measures. Currently available objective approaches include skin prick testing, nasal provocation testing, and allergen exposure chambers. However, these methods remain constrained by practical limitations and are not yet appropriate for routine implementation in clinical efficacy evaluation. In addition, several biomarkers, including sIgE and the sIgE/tIgE ratio, sIgG4, serum IgE-blocking activity, IgA, cytokines and chemokines, as well as immune cell surface molecules and their functional activity, have been shown to have associations with AIT outcomes. While these biomarkers may complement subjective assessments, they are subject to significant limitations. Consequently, large-scale multicenter trials and real-world evidence are required to strengthen the evidence base. The present consensus underscores the necessity of integrating patients'subjective experiences with objective testing throughout the treatment process, thereby providing a more comprehensive and accurate framework for efficacy evaluation. Looking forward, future investigations should prioritize the incorporation of multi-omics data and artificial intelligence methodologies, which hold promise for overcoming current limitations in assessment strategies and for advancing both the standardization and personalization of AIT.

Azvudine and nirmatrelvir/ritonavir (Paxlovid) are recommended for COVID-19 treatment in China, but their safety and efficacy in the elderly population are not fully known. In this multicenter, retrospective, cohort study, we identified 5131 elderly hospitalized COVID-19 patients from 32,864 COVID-19 patients admitted to nine hospitals in Henan Province, China, from December 5, 2022, to January 31, 2023. The primary outcome was all-cause death, and the secondary outcome was composite disease progression. Propensity score matching (PSM) was performed to control for confounding factors, including demographics, vaccination status, comorbidities, and laboratory tests. After 2:1 PSM, 1786 elderly patients receiving azvudine and 893 elderly patients receiving Paxlovid were included. Kaplan-Meier and Cox regression analyses revealed that compared with Paxlovid group, azvudine could significantly reduce the risk of all-cause death (log-rank P = 0.002; HR: 0.71, 95% CI: 0.573-0.883, P = 0.002), but there was no difference in composite disease progression (log-rank P = 0.52; HR: 1.05, 95% CI: 0.877-1.260, P = 0.588). Four sensitivity analyses verified the robustness of above results. Subgroup analysis suggested that a greater benefit of azvudine over Paxlovid was observed in elderly patients with primary malignant tumors (P for interaction = 0.005, HR: 0.32, 95% CI: 0.18-0.57) compared to patients without primary malignant tumors. Safety analysis revealed that azvudine treatment had a lower incidence of adverse events and higher lymphocyte levels than Paxlovid treatment. In conclusion, azvudine treatment is not inferior to Paxlovid treatment in terms of all-cause death, composite disease progression and adverse events in elderly hospitalized COVID-19 patients.

Rheumatic diseases are chronic inflammatory autoimmune diseases that can affect multiple organs and systems. In clinical practice, most patients with rheumatic diseases present with asymptomatic liver function abnormalities during the course of the disease, and the etiology of such diseases may be associated with the rheumatic disease itself, medications, metabolism, viruses, or the presence of other chronic liver diseases. Immune-mediated inflammatory responses play a significant role in liver involvement (including hepatocyte injury, intrahepatic vascular lesions, and hepatic fibrosis) in rheumatic diseases. This article discusses the clinical features and management of liver involvement secondary to rheumatic diseases, in order to enhance the understanding of this condition among specialists in related fields.

ObjectiveTo investigate the therapeutic effect of sequential administration of Dihuang Baoyuan granules (DHBY, the prescription for consolidating body resistance) and Fuling Yunhua granules (FLYH, the prescription for treating symptoms) on spontaneous type 2 diabetes mellitus (T2DM) in mice. MethodsAccording to the fasting blood glucose (FBG) level, 12-week-old db/db mice were randomized into six groups: model, DHBY (18.02 g·kg-1), FLYH (14.80 g·kg-1), sequential administration 1 (SEQ-1, DHBY 18.02 g·kg-1+FLYH 14.80 g·kg-1), sequential administration 2 (SEQ-2, FLYH 14.80 g·kg-1+DHBY 18.02 g·kg-1), and dapagliflozin (Dapa, 1.3 mg·kg-1). The m/m mice in the same litter were selected as the normal group. The mice were administrated with corresponding drugs by gavage for 8 consecutive weeks. During the 8 weeks of drug administration and 2 weeks after withdrawal, the retinal thickness, FBG, hemoglobin A1c (HbA1c), and insulin were determined, and histopathological changes of the pancreas, liver, kidney, and retina were observed by hematoxylin-eosin (HE) staining. ResultsCompared with the model group, SEQ-1 for 4 weeks lowered the FBG level (P<0.05), raised the insulin level, decreased the triglyceride (TG) level (P<0.05), increased the number of optic ganglion cells and diminished vacuolar degeneration of pancreatic islet and liver. SEQ-2 lowered FBG and HbA1c levels (P<0.05), rose the insulin level, increased the retinal thickness and the number of optic ganglion cells (P<0.05), and alleviated vacuolar degeneration of pancreatic islet and liver. Two weeks after drug withdrawal, Dapa tended to increase FBG and HbA1c compared with those at the time of drug withdrawal. However, the levels of FBG and HbA1c in the SEQ-2 group remained decreasing (P<0.05). ConclusionSEQ-1 and SEQ-2 can lower the blood glucose level and ameliorate diabetic retinopathy, and SEQ-2 outperformed DHBY and FLYH in lowering the blood glucose level. Moreover, SEQ-2 can maintain the blood glucose-lowering effect after drug withdrawal.

Chinese Journal of Tissue Engineering Research 2025;29(4):692-699
BACKGROUND:Quercetin has anti-inflammatory,anti-cancer,anti-oxidation,anti-aging,anti-depression and other pharmacological effects, and has high medicinal value.Quercetin can promote wound healing in normal rats, but few drugs with quercetin as the main component have been developed, which limits its wide application in clinical practice. OBJECTIVE:To investigate the application effect of quercetin-carboxymethyl chitosan hydrogel on diabetic wound in rats by loading quercetin with hydrogel material. METHODS:(1)Carboxymethyl chitosan hydrogels and quercetin-carboxymethyl chitosan hydrogels were prepared respectively, and the micromorphology and in vitro drug release properties of the hydrogels were characterized.(2)Cell experiment:Mouse L929 fibroblasts were cultured in four groups. The blank control group was cultured conventionally.Carboxymethyl chitosan hydrogel, quercetin solution and quercetin-carboxymethyl chitosan hydrogel were added to pure hydrogel group, drug group, and drug-loaded hydrogel group, respectively,to detect cell proliferation and migration ability.(3)Animal experiments:Diabetic models were established in 80 SD rats. After successful modeling,full-layer skin defect wounds with a diameter of 2 cm were made on the back of rats, and these models were randomly divided into four groups.Normal saline, carboxymethyl chitosan hydrogel, quercetin solution, and quercetin-carboxymethyl chitosan hydrogel were injected into the wounds of blank control group, pure hydrogel group, drug group, and drug-loaded hydrogel group, respectively. Each group contained 20 animals, which were bound with sterile gauze.Wound healing, pathological morphology, expression of inflammatory factors, and angiogenesis were observed after operation. RESULTS AND CONCLUSION:(1)Under scanning electron microscope, the microstructures of the two hydrogels were similar, both showed loose porous network structure, and quercetin-carboxymethyl chitosan hydrogels had good drug release performance in vitro.(2)Compared with blank control group, the cell proliferation and mobility of the other three groups were increased(P<0.05).The cell proliferation and mobility of drug-loaded hydrogel group were higher than those of pure hydrogel group and drug group(P<0.05).(3)The wound healing rate of the drug-loaded hydrogel group was higher than that of the other three groups at 5 and 11 days after operation. The wound healing rate of rats in the hydrogel group, pure hydrogel group, and drug group reached 100%18 days after operation, which was higher than that in the blank control group(P<0.05).Hematoxylin-eosin staining showed that intact skin and skin appendages were visible on the wounds of rats in the drug-loaded hydrogel group 18 days after surgery, while intact skin and skin appendages were visible on the wounds of rats in the blank control group, pure hydrogel group, and drug group 25 days after surgery. The levels of tumor necrosis factor α and interleukin-6 in the drug-loaded hydrogel group were lower than those in the blank control group at 5,11,and 18 days after surgery(P<0.05),and the levels of transforming growth factor β1 at 11 and 18 days after surgery were lower than those in the blank control group(P<0.05).The mRNA expressions of CD31 and vascular endothelial growth factor α in the drug-loaded hydrogel group were higher than those in the other three groups at 11 and 18 days after operation(P<0.05).(4)These findings indicate that quercetin in quercetin carboxymethyl chitosan hydrogel can regulate inflammatory response, accelerate angiogenesis, promote the proliferation and migration of fibroblasts, and enhance the healing of diabetic wounds in rats.

Chinese Journal of Radiation Oncology 2025;34(12):1264-1268
Radiation therapy is a major treatment modality for many malignant tumors. However, patients exhibit marked individual differences in radiosensitivity. Radiation resistance is persistently strengthened through the antioxidant pathway mediated by sulfiredoxin-1 (SRXN1), forming a key bottleneck in improving therapeutic efficacy. Studies have shown that SRXN1 displays dual regulatory roles in the response to radiation. On one hand, SRXN1 enhances cystine uptake and glutathione synthesis via the Kelch-like ECH-associated protein 1 (KEAP1)-nuclear factor erythroid 2-related factor 2 (NRF2)/solute carrier family 7 member 11 (SLC7A11) axis, thereby suppressing lipid peroxidation-induced ferroptosis and promoting radioresistance. On the other hand, excessive activation of the SRXN1-regulated thioredoxin system depletes reduced nicotinamide adenine dinucleotide phosphate, triggering disulfidptosis through actin cytoskeleton depolymerization. This dynamic equilibrium mechanism reveals for the first time that SRXN1 determines radiosensitivity through time-dependent regulation of two interconnected cell death pathways. This article systematically elucidates how SRXN1 dynamically modulates the radiosensitivity of malignant tumors via the ferroptosis-disulfidptosis interaction network, and reviews the underlying molecular mechanisms, providing a theoretical foundation for developing dual-death-inducing nanomedicines.

Chinese Journal of General Surgery 2025;34(5):945-952
Background and Aims:Implant-based breast reconstruction is one of the most common reconstructive approaches after mastectomy for breast cancer. However, the incidence of postoperative complications remains significant, and the associated risk factors have not been fully elucidated. This study aimed to analyze the incidence of surgical complications following implant-based breast reconstruction in patients undergoing mastectomy at our center and to identify potential risk factors. Methods:A retrospective analysis was conducted on patients who underwent mastectomy and implant-based breast reconstruction at Sun Yat-sen Memorial Hospital between May 2004 and August 2022.Patients were grouped according to the presence or absence of postoperative surgical complications, and multivariate Logistic regression was used to identify independent risk factors. Results:A total of 545 patients with 602 reconstructed breasts were included.Surgical complications occurred in 13.6%(82/602)of the cases, including infection, wound dehiscence/poor healing, flap or nipple-areola necrosis, and implant leakage/rupture.Multivariate analysis revealed that nipple-areola complex resection(OR=1.934,95%CI=1.056-3.542,P=0.033),postoperative radiotherapy(OR=2.483,95%CI=1.527-4.036,P<0.001),implant volume≥300 mL(OR=1.663,95%CI=1.025-2.696,P=0.039),and surgeon experience with fewer than 10 cases(OR=1.804,95%CI=1.092-2.979,P=0.021)were all independent risk factors for complications. Conclusion:NAC resection, radiotherapy, large implant volume, and limited surgical experience are important independent risk factors for postoperative surgical complications following implant-based breast reconstruction.Thorough preoperative evaluation and appropriate surgical planning are essential to minimize risks.
