1.Notoginsenoside R1 modulates mitophagy in human cardiomyocytes viathe Pink1/Parkin pathway after hypoxia/reoxygenation

ObjectiveTo investigate the mechanism by which Notoginsenoside R1 (NGR1) ameliorates hypoxia/reoxygenation (H/R)-induced injury in AC16 human cardiomyocyte cell lines through the regulation of mitophagy. MethodsCommon genes linked to hypoxia/reoxygenation injury and mitophagy were identified by intersecting data from GeneCards and MitoCarta databases. AC16 cell viability was assessed via CCK-8 assay under varying NGR1 concentrations (0, 6.25, 12.5, 25, 50, 100, 200, 300, 400, 500 μmol/L). AC16 cells were divided into the following groups: control group (Control), model group (H/R), and treatment groups (H/R + NGR1 at 100, 200 and 300 μmol/L). Mitochondrial membrane potential (ΔΨm) was measured using 5,5',6,6'-tetrachloro-1,1',3,3'-tetraethylbenzimidazolylcarbocyanine iodide (JC-1) staining. Transcriptional levels of mitophagy-related genes (Parkin, Pink1, P62) were quantified by reverse transcription-quantitative PCR (RT-qPCR). Protein expression of mitophagy-related markers (Parkin, Pink1, P62, and LC3BⅡ) was evaluated via Western blot analysis. Mitochondrial ultrastructure was visualized by transmission electron microscopy (TEM). ResultsCompared to the control group, cell viability in the H/R group significantly decreased (P<0.01). Treatment with NGR1 at concentrations above 100 μmol/L significantly enhanced the cell viability of AC16 cells compared to the H/R group (P<0.01). H/R induced a significant decrease in mitochondrial membrane potential (P<0.01), which was restored by NGR1 treatment (P<0.01). The mRNA levels of Parkin, Pink1, and P62 in the H/R group were upregulated compared to the control group (P<0.05), while NGR1 intervention downregulated their expression (P<0.05). Protein expression levels of Parkin, Pink1, and LC3BⅡ in the H/R group significantly increased, while P62 expression decreased compared to the control group (P<0.01). In contrast, different doses of NGR1 treatment significantly reduced the expression of Parkin, Pink1, and LC3BⅡ while increasing P62 expression (P<0.05). TEM revealed that the mitochondrial structure in the H/R group was severely disrupted, with fragmented and disorganized cristae, which was alleviated by NGR1. ConclusionNGR1 ameliorates H/R-induced AC16 cell injury, and its mechanism may be associated with modulating the Pink1/Parkin pathway to suppress excessive mitophagy.
2.Evaluation of public health governance capacity in Zhejiang Province

ObjectiveTo systematically assess the public health governance capacity in Zhejiang Province, to conduct an in-depth analysis of its strengths and weaknesses, so as to provide scientific basis and strategic recommendations for further enhancement. MethodsA systematic collection of policy documents, public information reports, and research literature related to public health governance capacity in Zhejiang Province from 2002 to 2023 was conducted (encompassing a total of 1 263 policy documents, 138 pieces of information reports and 631 research articles). Based on the evaluation criteria suitable for public health systems previously developed by the research team, the basic status and magnitude of change in public health governance capacity in Zhejiang Province was evaluated. Additionally, normative gap analyses were employed to identify the strengths and weaknesses. ResultsZhejiang Province ranked 4th nationwide in terms of public health governance capacity with a score of 733.4 points (1 000.0-point maximum). The province has effectively implemented the principle of health first (scoring 698.5 points in the assessment of health-first strategy implementation) and attached sufficient importance to health-related goals (scoring 658.2 points in the scientific rationality of goal setting). However, the implementation of inter-departmental coordination and incentive mechanisms only scored 178.7 points, the feasibility of management and monitoring mechanisms scored even lower at only 144.0 points, and the coverage of incentive mechanisms scored 286.0 points. ConclusionZhejiang Province has effectively implemented its health first strategy and attached great importance to health targets, but still needs to strengthen cross-departmental coordination mechanisms and health-oriented incentives.
3.Analysis of Risk Factors and Establishment of Prediction Model for Turbidity Toxicity Accumulation Syndrome in Patients with Chronic Atrophic Gastritis

MethodsClinical data of 180 patients with CAG who participated in the "clinical study of Xianglian Huazhuo Particles blocking CAG cancer transformation" of Hebei Sheng Zhong Yi Yuan from July 2021 to March 2022 were collected. After confounding factors were controlled by propensity score matching, patients were divided into a training set (namely dev) and a validation set (namely vad) in a seven to three ratio. The risk factors for CAG with turbidity toxin accumulation syndrome in the training set were investigated by using univariate Logistic regression analysis and least absolute shrinkage and selection operator (namely Lasso) regression algorithms. Subsequently, a model, named model 1se, was developed by using the training set data to predict the risk factors for CAG with turbidity toxin accumulation syndrome. The accuracy of the prediction model was assessed by using various methods, including the receiver operating characteristic (ROC) curve, Hosmer-Lemeshow test (H-L), calibration plot, and decision curve analysis (DCA). ResultsAge, body mass index (BMI), family history of cancer, job and life satisfaction, yellow and greasy fur with slippery pulse, and heavy body sensation were independent risk factors of the model. The prediction model showed excellent predictive value for both the training and validation sets. ConclusionThe established prediction model for CAG with turbidity toxin accumulation syndrome has high discrimination and excellent calibration, which could provide an excellent clinical basis for disease diagnosis and individualized treatment of patients.

5.Epidemiological characteristics of scrub typhus in Huai'an, Jiangsu Province in 2006 - 2024

Objective To describe and analyze the epidemiological characteristics of scrub typhus in Huai'an, Jiangsu Province from 2006 to 2024 and explore the long-term incidence trend and distribution of high-risk areas, and to formulate targeted prevention and control strategies. Methods The scrub typhus case report data of Huai'an from 2006 to 2024 in the Chinese Disease Prevention and Control Information System were extracted for descriptive analysis. Results A total of 898 cases of scrub typhus were reported in Huai'an, with an average annual incidence rate of 0.96 per 100 000 from 2006 to 2024. There was a turning point in the incidence trend of scrub typhus in 2011. From 2006 to 2011, the annual percentage change (APC) was 47.09% (95% CI: 7.53 - 859.39), and the upward trend was statistically significant (P < 0.05). From 2012 to 2024, the APC was -2.12% (95% CI: -29.09 - 3.75), and the downward trend was not statistically significant. October and November were the high-incidence months, and the total concentration from 2006 to 2024 was 0.93, indicating that scrub typhus had strict seasonality. The circular distribution method estimated that the peak period of the epidemic was from October 11th to November 25th, and the peak day of incidence was November 3rd. Jinhu County was a high-incidence area. The ratio of male to female cases was 1.03. The age group with the highest reported incidence was 40 to < 80 years old. The occupation with the highest proportion was farmers, accounting for 78.03%. Conclusion From 2006 to 2024, scrub typhus in Huai'an shows a peak every 3 - 4 years. Middle-aged and elderly farmers are the key population at risk, and Jinhu County is a key area. In the future, targeted health education should be carried out to effectively control the prevalence of scrub typhus.

Objective To explore the current situation and experience of the development of multi-hospital areas in foreign medical institutions,and to analyze its enlightenment to the construction of"one hospital with multiple campuses"in public hospitals in China.Methods Through the combing of relevant literature,it systematically analyzes the development status of multi-hospital construction of medical institutions in typical countries such as the United States,the United Kingdom,and Germany,summarizes the relevant experience of different countries,and analyzes the current situation of the construction of"one hospital with multiple campuses"in China's public hospitals.Results At present,the orderly development of multiple hospitals of foreign medical institutions mainly depends on the homogenization of medical care,the scientific management of human resources and the improvement of information construction.China can learn from its experience and technical means to build a development pattern of"one hospital with multiple campuses"suitable for China's national conditions.Conclusion In the future,the construction of"one hospital with multiple campuses"in China's public hospitals should focus on"rationalization of human resource allocation,homogenization of medical service quality,and intelligent information system construction",improve"human resource allocation",establish and improve"information sharing mechanism",differentiate the layout of"hospital functions",and strengthen"quality supervision and patient feedback",aiming to improve the construction effect of"one hospital with multiple campuses"in China's public hospitals.
7.Effects of Electroacupuncture at Acute Phase on Cognitive Dysfunction at Later Stage of MCAO Rats

Objective To explore the effects and potential mechanism of electroacupuncture at acute phase on cognitive dysfunction at later stage in rats with middle cerebral artery occlusion(MCAO).Methods Totally 39 SD rats were randomly divided into sham-operation group,model group and electroacupuncture group,with 13 rats in each group.MCAO model was established by Zea Longa thread embolism method in model group and electroacupuncture group.The sham-operation group only separated blood vessels.Electroacupuncture group was treated with electroacupuncture at"Baihui"and"Shenting"acupoints at 5 min and 6 h after model establishment respectively,for 30 min.The body mass,modified neuropathy severity scale(mNSS),and percentage of stiff immobility time of rats were monitored weekly,Nissl staining was used to observe the morphology of hippocampal tissue on the 28 d,immunofluorescence staining was used to detect the expression of Ki67,dual cortin(DCX)and neuronal differentiation factor 1(NeuroD1)in dentate gyrus of the hippocampus,the expression of synaptic vesicle protein 2A(SV2A)protein in dentate gyrus of hippocampus was detected by Western blot.Results Compared with the sham-operation group,on the first day after operation,the body mass of the model group rats decreased,then gradually increased,and on the 28th day,the body mass was still lower than that of the sham-operation group(P<0.05);on the first day after operation,the mNSS score significantly increased,gradually decreased thereafter,and remained higher than the sham-operation group until the 28th day(P<0.01);the percentage of stiff immobility time significantly decreased on the first day after operation,gradually stabilized,and remained lower than the sham-operation group on the 28th day(P<0.05);the hippocampal tissue structure was significantly damaged,and the density of Ki67+DCX+NeuroD1 positive cells in the dentate gyrus of hippocampus was significantly increased(P<0.05),the expression of SV2A protein in the dentate gyrus of hippocampal was significantly decreased(P<0.05).Compared with the model group,the body mass of rats in the electroacupuncture group showed an increasing trend(P<0.05),the mNSS score decreased(P<0.01),and the percentage of stiff immobility time increased(P<0.05);the ischemic injury area was significantly reduced,the structure was clearer,and the disordered and irregularly cells arrangement were reduced,the density of Ki67+DCX+NeuroD1 positive cells in the dentate gyrus of hippocampus significantly increased(P<0.05),and the expression of SV2A protein in the dentate gyrus of hippocampal significantly increased(P<0.05).Conclusion Electroacupuncture at acute phase at"Baihui"and"Shenting"can promote the recovery of cognitive dysfunction in MCAO rats in the later stage,and its mechanism may be related to enhancing the proliferation and differentiation of neural precursor cells in the dentate gyrus of hippocampus,promoting neurogenesis.
8.Characteristic Distribution of Traditional Chinese Medicine Syndrome Types in Gouty Arthritis Complicated with Bone Erosion and Analysis of the Associated Factors

Objective To investigate the characteristic distribution of traditional Chinese medicine(TCM)syndrome types in gouty arthritis patients complicated with bone erosion,and to analyze the associated factors.Methods A total of 318 gouty arthritis patients hospitalized in the Rheumatology and Immunology Department of Foshan Hospital of Traditional Chinese Medicine between November 2021 and November 2023 were included.The patients were allocated to a bone erosion group(142 cases)and a non-bone erosion group(176 cases)according to the findings of musculoskeletal ultrasound.The clinical data of gender,age,body height,body mass,body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP),course of gout,findings of musculoskeletal ultrasound,complete blood count[white blood cell count(WBC),platelet count(PLT),hemoglobin(Hb),and red blood cell count(RBC)],blood uric acid(UA),creatinine(Cr),erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),total cholesterol(TC),and low-density lipoprotein cholesterol(LDL-C)of the patients during hospitalization were collected.Differences in general information,laboratory parameters,and distribution of TCM syndrome types were compared between the two groups,and then the potential influencing factors of bone erosion in gouty arthritis patients were explored.Results(1)Analysis of general information showed that the bone erosion group had longer course of gout and slightly lower BMI than that the non-bone erosion group,and the differences were statistically significant(P<0.05 or P<0.01).However,there were no statistically significant differences in age,body height,body mass,SBP and DBP among the patients of the two groups(P>0.05).(2)Analysis of laboratory indicators showed that the levels of Cr,UA,ESR and TC in the bone erosion group were significantly higher than those in the non-bone erosion group,and the differences were statistically significant(P<0.05 or P<0.01).However,no statistically significant differences in the levels of CRP,WBC,RBC,PLT and LDL-C were presented between the two groups(P>0.05).(3)Analysis of the distribution of TCM syndromes showed that in the bone erosion group,stagnant-heat obstruction syndrome accounted for the largest proportion(33.10%),followed by phlegm-turbidity obstruction syndrome(29.58%),damp-heat accumulation syndrome(22.54%),and liver-kidney yin deficiency syndrome(14.79%);in the non-bone erosion group,damp-heat accumulation syndrome accounted for the largest proportion(38.07%),followed by liver-kidney yin deficiency syndrome(32.95%),phlegm-turbidity obstruction(14.77%),and stagnant-heat obstruction(14.20%).There was statistically significant difference in the distribution of TCM syndromes between the two groups(P<0.01).(4)The results of logistic regression analysis suggested that the higher the levels of ESR,UA,and TC,the longer the course of gout.And the TCM syndromes of stagnant-heat obstruction and phlegm-heat obstruction were the risk factors for bone erosion in gouty arthritis(P<0.05 or P<0.01).Conclusion Gouty arthritis patients with bone erosion exhibit higher UA,ESR,Cr,and TC levels,longer course of disease,and higher incidence of stagnant-heat obstruction syndrome and phlegm-turbidity obstruction syndrome.Bone erosion in gouty arthritis is closely associated with elevated UA,ESR,TC,and some specific TCM syndrome patterns.

ObjectiveTo explore the data standard system of electronic medical records in the field of rehabilitation, focusing on the terminology and coding standards, data structure, and key content categories of rehabilitation electronic medical records. MethodsBased on the Administrative Norms for the Application of Electronic Medical Records issued by the National Health Commission of China, the electronic medical record standard architecture issued by the International Organization for Standardization and Health Level Seven (HL7), the framework of the World Health Organization Family of International Classifications (WHO-FICs), Basic Architecture and Data Standards of Electronic Medical Records, Basic Data Set of Electronic Medical Records, and Specifications for Sharing Documents of Electronic Medical Records, the study constructed and organized the data structure, content, and data standards of rehabilitation electronic medical records. ResultsThe data structure of rehabilitation electronic medical records should strictly follow the structure of electronic medical records, including four levels (clinical document, document section, data set and data element) and four major content areas (basic information, diagnostic information, intervention information and cost information). Rehabilitation electronic medical records further integrated information related to rehabilitation needs and characteristics, emphasizing rehabilitation treatment, into clinical information. By fully applying the WHO-FICs reference classifications, rehabilitation electronic medical records could establish a standardized framework, diagnostic criteria, functional description tools, coding tools and terminology index tools for the coding, indexing, functional description, and analysis and interpretation of diseases and health problems. The study elaborated on the data structure and content categories of rehabilitation electronic medical records in four major categories, refined the granularity of reporting rehabilitation content in electronic medical records, and provided detailed data reporting guidance for rehabilitation electronic medical records. ConclusionThe standardization of rehabilitation electronic medical records is significant for improving the quality of rehabilitation medical services and promoting the rehabilitation process of patients. The development of rehabilitation electronic medical records must be based on the national and international standards. Under the general electronic medical records data structure and standards, a rehabilitation electronic medical records data system should be constructed which incorporates core data such as disease diagnosis, functional description and assessment, and rehabilitation interventions. The standardized rehabilitation electronic medical records scheme constructed in this study can support the improvement of standardization of rehabilitation electronic medical records data information.

ObjectiveTo analyze the data structure and standards of rehabilitation outpatient medical records, to provide data support for improving the quality of rehabilitation outpatient care and developing medical insurance payment policies. MethodsBased on the normative documents issued by the National Health Commission, Basic Standards for Medical Record Writing and Standards for Electronic Medical Record Sharing Documents, in accordance with the Quality Management Regulations for Outpatient (Emergency) Diagnosis and Treatment Information Pages (Trial), reference to the framework of the World Health Organization Family of International Classifications (WHO-FICs), the data framework and content of rehabilitation outpatient medical records were determined, and the data standards were discussed. ResultsThis study constructed a data framework for rehabilitation outpatient medical records, including four main components: patient basic information, visit process information, diagnosis and treatment information, and cost information. Three major reference classifications of WHO-FICs, International Classification of Diseases, International Classification of Functioning, Disability and Health, and International Classification of Health Interventions,were used to establish diagnostic standards and standardized terminology, as well as coding disease diagnosis, functional description, functional assessment, and rehabilitation interventions, to improve the quality of data reporting, and level of quality control in rehabilitation. ConclusionThe structuring and standardization of rehabilitation outpatient medical records are the foundation for sharing of rehabilitation data. The using of the three major classifications of WHO-FICs is valuable for the terminology and coding of disease diagnosis, functional description and assessment, and intervention in rehabilitation outpatient medical records, which is significant for sharing and interconnectivity of rehabilitation outpatient data, as well as for optimizing the quality and safety of rehabilitation medical services.
