1.Trends analysis of pancreatic cancer mortality in Xuhui district, Shanghai from 1992 to 2021

Objective To study the death status of pancreatic cancer among residents in Xuhui district, Shanghai, from 1992 to 2021, and analyze its trends of change, so as to provide evidence for the prevention and treatment of pancreatic cancer. Methods Based on the database of Shanghai death registration system from 1992 to 2021, the crude mortality rate, standardized mortality rate, age-specific mortality rate and other indicators of pancreatic cancer among registered residents in Xuhui district were calculated. The Joinpoint software was used to analyze the trends of average annual percent change (AAPC) of pancreatic cancer mortality rate, and the age-period-cohort model was used to analyze the age effect, period effect and birth cohort effect pairs significant changes in pancreatic cancer mortality. Results In 2021, the mortality rate of pancreatic cancer in Xuhui district, Shanghai, ranked fourth among malignant tumors, and the winning rate and world standard rate of the whole population, males and females were 8.34/100 000 (8.81/100 000, 7.98/100 000) and 7.28/100 000 (7.69/100 000, 6.96/100 000), respectively, with males higher than females. AAPC of crude mortality rate and the standardized (6) mortality rate were higher in males than that in females. The age-specific mortality rate increased with the increase of age, and the highest mortality rate was found in 60-84 years old group. The age-period-cohort model showed that from 1992 to 2021, the annual net shift of pancreatic cancer mortality among the whole population, male and female residents in Xuhui district, Shanghai, was 1.22%, 1.58%, 1.15% (P=0.20, 0.19, 0.45) respectively, and the time trend was not significant. From the perspective of age effect, the risk of death from pancreatic cancer in the whole population and with age deviation in males had an obvious trend with increasing age (P<0.05), while the age effect in females had no obvious trend. From the perspective of period effect, no period deviation was significant in the whole population, males and females (P>0.05). In terms of cohort effects, there were significant differences in the whole population and the male cohort deviations(P<0.05). No significant cohort effect was observed in the female population. Conclusions The mortality rate of pancreatic cancer among registered residents in Xuhui district, Shanghai from 1992 to 2021, was on the rise, especially in the 60-84 years old group and male. The prevention and control of pancreatic cancer needs to develop effective epidemic prevention measures for corresponding populations.
2.Chemotherapy efficacy and plasma drug concentration changes in patient with limited-stage small cell lung cancer complicated with uremia:A case report and literature review

Limited-stage small cell lung cancer(SCLC)is a highly malignant and rapidly progressing neuroendocrine tumor,while uremia is a complication of the end-stage of chronic renal failure.The patients with SCLC complicated with uremia have poor treatment tolerance,limited options for anti-tumor treatment regimens,and great difficulty in diagnosis and treatment.This study analyzed one case of a 69-year-old male patient with limited-stage SCLC complicated with uremia(with a history of regular hemodialysis,3 times per week),to discuss his first-line treatment regimen,efficacy,and the impact of hemodialysis on the plasma concentrations of the anti-tumor drugs,and reviewed the relevant literature to provide a reference for the treatment of similar patients.The patient was admitted to the hospital due to"cough and hemoptysis for half a month"and was diagnosed with limited-stage SCLC stage ⅢA(T2aN2M0)by computed tomography(CT)and lung puncture biopsy.After discussion by the multi-disciplinary treatment(MDT)team,the patient received 6 cycles of Etoposide(VP-16)+carboplatin chemotherapy combined with adebrelimab immunotherapy,followed by sequential adebrelimab maintenance therapy.The efficacy was evaluated as partial response(PR)and the response is ongoing.During the treatment,level 4 hemoglobin decrease,level 3 neutropenia,and level 2 leukopenia occurred,which were alleviated after symptomatic treatment.The blood concentration monitoring results showed that the plasma concentrations of etoposide and carboplatin increased rapidly during drug infusion,and gradually decreased after the end of infusion.Hemodialysis could rapidly reduce the plasma concentration of carboplatin,but had no significant effect on the plasma concentration of etoposide.Therefore,the immunotherapy combined with reduced-dose chemotherapy regimen is safe and effective for this type of patient.Plasma drug concentration monitoring can be used to observe drug metabolism,but the optimal monitoring time points and clinical value need further study and validation.
3.Summary of best evidences for thirst management in patients with oral tracheal intubation

Objective To summarize the best evidences of thirst management in the patients with tra-cheal intubation to provide the evidence-based evidence for clinical practice.Methods According to the"6S"evidence pyramid model,the literatures on thirst management of the patients with tracheal intubation were systematically retrieved from the relevant guide websites,evidence-based databases,association websites and original literature databases at home and abroad.The databases included BMJ Best Practice,Up To Date,JBI Evidence-Based Health Care Center Database,Guidelines International Network(GIN),National Institute for Health and Clinical Excellence,National Institute for Health and Clinical Excellence(NICE),Scottish Inter-collegiate Guidelines Network(SIGN),Registered Nurses'Association of Ontario,Canada(RNAO),National Guideline Clearing House(NGC),Cochrane Library,American Society of Critical Care Medicine(SCCM),A-merican Association of Critical Care(AACN),European Society of Intensive Medicine(ESIM),Chinese Socie-ty of Critical Care Medicine,PubMed,Embase,CNKI,Wanfang Medical Network,VIP Database and Chinese Biomedical Literature Database(SinoMed).The retrieval limit was from the database establishment to August 31,2023.Two researchers trained by evidence-based nursing independently completed the literature quality e-valuation,and the literatures meeting the quality standards conducted the evidence extracted and summary.Results A total of 23 literatures were included,including 1 clinical decision,1 evidence summary,12 random-ized controlled trials,3 quasi-experimental studies,and 6 cross-sectional studies.Eighteen pieces of best evi-dence were formed,including six aspects:definition of thirst,classification of thirst degree,risk factors,assess-ment methods,intervention strategies and quality management.Conclusion Nurses should conduct the clini-cal transform and application of the best evidences by combining the clinical situation and specific department policies to alleviate the thirst symptoms in the patients with tracheal intubation.
4.L-Mimosine Modulates the ROS/JNK/c-Jun Signaling Axis to Trigger Mitochondrial Apoptosis in Melanoma Cells

OBJECTIVE To investigate the effects of L-mimosine on the proliferation and apoptosis of melanoma cells and its molecular mechanisms.METHODS The effects of different concentrations of L-mimosine on the viability of melanoma A375 and B16 cells were examined by the MTT assay.The ability of L-Mimosine to affect colony formation was assessed by the plate colony for-mation assay.The effects of L-mimosine on cell apoptosis,reactive oxygen species(ROS),and mitochondrial membrane potential were analyzed by flow cytometry.The changes in the expression levels of intracellular proteins such as Bcl-2,Bax,Cyt C,Cleaved Caspase-3,Cleaved Caspase-9,Bim-EL,Bad,p-JNK,p-c-Jun,p-MKK4,and p-ASK1 were detected by Western blot.The tumor-bearing mouse model was subjected to histological and TUNEL staining analysis of tumor tissues after treatment with normal saline,L-mimosine alone,and L-mimosine in combination with N-acetylcysteine(NAC).RESULTS L-mimosine significantly inhibited the prolifera-tion of A375 and B16 cells and significantly promoted cell apoptosis.Western blot analysis showed that L-mimosine could significantly increase the content of Apaf-1 and Cyt C in the cytoplasm,up-regulate the expression of Cleaved PARP,Cleaved Caspase-3,Cleaved Caspase-9,Bax,Bim-EL and Bad,and down-regulate the expression of anti-apoptotic protein Bcl-2.Further experiments showed that L-mimosine significantly induced the accumulation of ROS in A375 cells and activated the JNK/c-Jun signaling pathway,which was manifested by the significant increase of p-JNK,p-c-Jun,p-MKK4 and p-ASK1.Additionally,the antioxidant NAC could significantly antagonize the ROS accumulation induced by L-mimosine,thereby reversing the pro-apoptotic effects of L-mi-mosine on melanoma cells.In vivo experiments,NAC also significantly antagonized the tumor growth inhibition induced by L-mi-mosine,as evidenced by increased tumor volume and reversed expression levels of apoptosis-related proteins.CONCLUSION L-mi-mosine induces apoptosis in melanoma cells by increasing ROS levels and activating the JNK/c-Jun signaling pathway.
5.Construction and application evaluation of a core competency framework-based training program for cardiovascular nurse specialists

Objective:To construct and evaluate a training program for cardiovascular nurse specialists based on a core competency framework.Methods:Among 61 trainees participating in the first training class for cardiovascular nurse specialists organized by a provincial nursing society, a training program focusing on the nine core competencies for cardiovascular nurse specialists was implemented, which consisted of 4-week theoretical and 4-week practical training. The effectiveness of the training program was evaluated using the Kirkpatrick model. Data analyses were performed by using SPSS 26.0. Categorical data were presented as the number of cases; continuous variables in normal distribution were expressed as mean±standard deviation; and continuous variables in non-normal distribution were presented as median (interquartile range) and compared using non-parametric tests.Results:At the reaction level, the satisfaction rates of trainees with the theoretical and practical sections of the training program were 98.36% (60/61) and 95.08% (58/61), respectively. At the learning level, the comprehensive assessment score of the trainees was (83.01±3.39) points, and all of them successfully obtained their completion certificates, with a pass rate of 100.00% (61/61). At the behavior level, the core competencies for cardiovascular nurse specialists were significantly improved after training [the total score increased from 291.00 (254.00, 334.25) to 410.50 (354.50, 433.00), P<0.001]. At the results level, at six months after training, there were significant increases in the number of participants engaging in cardiovascular care practices, clinical nursing education and guidance, leadership roles, and research projects within their units as well as the number of individuals achieving career advancement (all P<0.05). Conclusions:The trainees are highly satisfied with the core competency-focused cardiovascular nurse training program, which can improve core competencies and cardiovascular nursing capabilities, expand the scope of cardiovascular nursing services, and foster the sustained advancement of the participants.
6.Characteristics of multimodal fundus imaging in patients with idiopathic retinal vasculitis, aneurysms and neuroretinitis syndrome

Objective:To observe and analyze the multimodal imaging characteristics of fundus in patients with idiopathic retinal vasculitis, aneurysms and neuroretinitis (IRVAN) syndrome.Methods:A retrospective study. From June 2015 to March 2024, 6 patients (11 eyes) diagnosed with IRVAN syndrome in Shaanxi Eye Hospital were included in the study. All patients underwent examinations including best-corrected visual acuity (BCVA), color fundus photography, fluorescein fundus angiography (FFA), optical coherence tomography (OCT) and OCT angiography (OCTA). At the same time, FFA combined with indocyanine green angiography (ICGA) was performed in 6 eyes. Follow-up ranged from 2 to 23 months. Multimodal imaging features were analyzed retrospectively. The number of retinal aneurysms detected by FFA, ICGA, and OCTA was compared by using the Wilcoxon signed-rank test.Results:In 11 eyes of 6 cases, a total of 1 male (2 eyes) and 5 females (9 eyes) with the mean age of (31.67±12.91) years were included in this cohort. Color fundus photography showed clear optic disc boundaries in 5 eyes, optic disc aneurysms in 8 eyes, retinal aneurysms in 4 eyes; exudation in 9 eyes, localized around aneurysms. On OCT, vitreous high reflective dots and epiretinal membrane on optic disc in all 11 eyes, and macular epiretinal membrane in 3 eyes were revealed. FFA showed optic disc aneurysms and retinal aneurysms in 9 eyes, late optic disc hyperfluorescence in 11 eyes; local arterial leakage in 3 eyes, local venous leakage in 8 eyes, non-perfusion area in all 11 eyes, and retina neovascularization in 3 eyes. Optic disc aneurysms and retinal aneurysms in 5 eyes (total 18 aneurysms) on ICGA were shown compared with optic disc aneurysms in 4 eyes and retinal aneurysms in 5 eyes (total 13 aneurysms) on simultaneous FFA. OCTA revealed neovascularization on the optic disc in 2 eyes, optic disc aneurysmsin 8 eyes, retinal aneurysms in 1 eye (total 2 aneurysms); while on simultaneous FFA, optic disc aneurysms in 8 eyes and retinal aneurysms in 3 eyes (total 5 aneurysms) with no optic disc were displayed. During OCTA follow-up, new aneurysms appeared at the bifurcation of arteries with an increasing angle between them and non-perfusion area enlargement on FFA. Compared with FFA and ICGA, OCTA in detecting the number of aneurysms had no statistics significance ( Z=-1.342, -1.342; P>0.05). Conclusion:Multimodal imaging can demonstrate characteristics of IRVAN syndrome, ICGA provides superior visualization of optic disc and retinal aneurysms, while OCTA confirms optic disc neovascularization and enlargement of artery angles at arterial bifurcations.
7.Comparative analysis of etiological characteristics of influenza-like illness and severe acute respiratory infection in Guangdong Province from August to December 2023

Objective:To compare the etiological characteristics between influenza-like illness (ILI) cases in outpatient and emergency departments and those with severe acute respiratory infection (SARI) in Guangdong Province, hoping to provide scientific evidence for the treatment, prevention, and control of respiratory infectious diseases.Methods:Laboratory testing for multiple respiratory pathogens was conducted on 6 090 specimens collected from ILI and SARI cases in Guangdong Province from August to December 2023. Chi-square test was used to analyze the differences in positive rates. Results:The overall positive rate of respiratory pathogens was 49.5% (3 016/6 090). The positive rate was 54.5% (2 260/4 145) in ILI cases and 38.9% (756/1 945) in SARI cases. The overall positive rate was higher in ILI cases than in SARI cases across all genders and age groups, and in most cities of the province, with statistically significant differences ( P<0.05). No significant difference was found in the overall positive rate between different genders. However, the difference between different age groups was statistically significant( P<0.001), with the highest rate identified in children aged 5-14 years (57.2%, 957/1 673). The main pathogens detected in ILI cases were influenza virus, human rhinovirus/enterovirus, and Streptococcus pneumoniae, while in SARI cases they were Mycoplasma pneumoniae, human rhinovirus/enterovirus, and influenza virus. The positive rates of adenovirus, human parainfluenza virus, SARS-CoV-2, human coronavirus, influenza virus, and Streptococcus pneumoniae were significantly higher in ILI cases than in SARI cases, while the positive rate of Mycoplasma pneumoniae was significantly higher in SARI cases ( P<0.05). Moreover, ILI cases were characterized by a higher risk of coinfection compared with SARI cases, especially in males or those aged 25-59 years. Conclusions:There are differences in the detection rates and spectrum of respiratory pathogens between ILI and SARI cases in Guangdong Province. Case features should be considered when developing strategies for preventing and treating respiratory infections.
8.Investigation for research competency of undergraduate interns in rehabilitation therapy using WHO rehabilita-tion competency framework

Objective To investigate the research competency among undergraduate interns of rehabilitation therapy.Methods In July,2024,a total of 81 undergraduate rehabilitation therapy interns from Department of Rehabilitation Medicine,Guangdong Provincial People's Hospital were self-rated the research competency,using research com-petency and activities,level 1 of World Health Organization rehabilitation competency framework.Semi-struc-tured interviews were conducted to investigate the willingness and difficulties to participate in research.Results The self-rated competencies were the lowest in behavior of C2.3 Assists in assessing rehabilitation service user needs and priorities for research,and Activity of A2 Disseminating evidence.For interviews,72 students would participate in researches,primarily motivated by career advancement and postgraduate admission competition.The main difficulties included insufficient faculty mentorship(n=48),limitation in knowledge(n=43)and diffi-culties in formulating scientific questions(n=43).Conclusion Most rehabilitation therapy undergraduates would like to participate in researches,but are suboptimal in re-search competency due to personal and environmental constraints.
9.A review on research trends of American ginseng: A bibliometric analysis

American ginseng (Panax quinquefolius L.), a widely used herbal medicine and dietary supplement, has attracted increasing attention from both academia and industry in recent years. To better understand the research frontiers and hotspots, we conducted a bibliometric analysis of American ginseng studies indexed in the Web of Science Core Collection from 1985 to 2024, using CiteSpace and VOSviewer. A total of 1169 publications were identified, with a marked increase in output since 2011. Hotspot analysis revealed growing interest in pharmacological effects, ginsenoside analysis, polysaccharide studies, and quality control. Furthermore, we assessed future research trends, suggesting that quality control and the modulation of gut microbiota will remain central topics. This study provides a clearer understanding of the evolving research landscape on American ginseng and offers guidance for future investigations.
10.Anti-angiogenic therapy as a beacon of hope in the battle against pulmonary NUT midline carcinoma.

Primary pulmonary nuclear protein of the testis (NUT) midline carcinoma (NMC) is a rare and highly aggressive thoracic malignancy that poses significant diagnostic and therapeutic challenges in clinical practice. This tumor is characterized by its heterogeneous clinical presentations and poor prognosis, often evading accurate initial diagnosis. In this study, we present two cases of primary pulmonary NMC treated with an integrated therapeutic approach combining anti-angiogenic agents, platinum-based chemotherapy, and radiotherapy. This multimodal strategy achieved survival durations of 32 and 13 months, respectively, surpassing the currently reported median survival of advanced NMC. Through a systematic literature review of reported cases, we have summarized the currently used diagnostic methods and treatment modalities for NMC. Our findings suggest that multimodal therapy incorporating anti-angiogenic treatment may offer superior clinical outcomes compared to conventional monotherapy regimens, particularly for patients who are not eligible for surgery. This comprehensive investigation enhances our understanding of NMC management by elucidating diagnostic pitfalls through histopathological correlation and proposing an effective therapeutic combination that demonstrates improved survival outcomes. By providing valuable insights into the diagnosis and treatment of primary pulmonary NMC, we hope to contribute to the development of more effective strategies for managing this rare and aggressive malignancy.
