Immune thrombocytopenia(ITP)is an autoimmune disease characterized primarily by bleeding and fatigue.ITP-related fatigue can substantially affect health-related quality of life and has been recognized as a treatment indication in the latest guidelines. However, its pathogenesis remains unclear, and targeted therapies are lacking. This paper explores the theoretical foundation of the"excessive fire consuming healthy qi"theory, linking two key conceptual pairs:"excessive fire"with inflammation and"healthy qi consumption"with mitochondrial dysfunction.Chronic inflammation leads to continuous"qi consumption"and damage to mitochondrial structure and function, ultimately resulting in symptoms of qi deficiency such as tiredness and fatigue. The imbalance between qi and fire, leading to imbalance of yin and yang,is the primary pathogenesis of ITP-related fatigue, with the disease mainly affecting the spleen.Considering the etiology and pathogenesis of ITP,and relative equilibrium of yin-yang,the therapeutic approach of invigorating spleen and benefiting qi to promote the production of blood serves as a core treatment strategy.Previous research has demonstrated the clinical efficacy of Jianpi Yiqi Shexue Formula, consisting of milkvetch root, tangshen,Indian bread, largehead atractylodes rhizome,India madder root, and ass hide glue in treating patients with ITP-related fatigue.Designed to invigorate the spleen and regulate qi and blood, this formula aligns with the pathogenesis of ITP-related fatigue characterized by deficiency of spleen qi and stomach qi.The formula also modulates the expression levels of key proteins and genes involved in mitochondrial energy metabolism and alleviates the immune dysfunction. These findings highlight the potential for further research into the mechanism of ITP-related fatigue, particularly by using inflammation-mediated mitochondrial energy metabolism as a promising entry point.

This study aims to investigate the mechanisms underlying the effects of the combined ac-tion of high-fat diet-induced obesity and the aflatoxin B1(AFB1)on hepatic oxidative stress and inflammatory responses.Thirty-six rats of similar weight and 4 weeks old were randomly divided into 4 groups, with 9 rats in each group:the blank control group(basal diet),the AFB1 group(0.4 mg/kg AFB1+basal diet),the HFD group(high-fat diet),and the HFD+AFB1 group(high-fat diet+0.4 mg/kg AFB1).Histological changes and lipid deposition were observed via hematox-ylin-eosin(HE)staining and Oil Red O staining.Levels of oxidative stress and inflammation-relat-ed factors were measured using commercial assay kits. The relative protein expression levels of factors involved in the Nrf2-Keap1 signaling pathway were assessed by Western blot analysis. The HE staining results showed that in the AFB1 group, the liver cells exhibited widespread watery de-generation,with shrunk and ruptured nuclei, inflammatory cells infiltration, and increased fibrosis. In the HFD group, liver cell fatty degeneration was observed, with cytoplasmic lipid droplet infil-tration.In the portal area, liver fibrosis was seen, with liver cell necrosis and inflammatory cell in-filtration in the fibrotic area, accompanied by lipofuscous granules. When HFD was applied to the AFB1 group, the abnormal state of liver interstitial and interstitial spaces was further aggravated, and a large number of lipid droplets appeared. The Oil Red O staining results showed that there were large numbers of dark red lipid droplets in the liver tissue of the HFD group, which were fused in strands. In the AFB1 group, lipid droplets could also be observed in the liver tissue of rats, but the number and degree were significantly less than those in the HFD group. The number and degree of red lipid droplets in the liver tissue of rats in the HFD+AFB1 group were higher than those in the AFB1 group and the HFD group.HFD exacerbated AFB1-induced oxidative stress by elevating ROS,MDA levels, and decreasing the expression of antioxidant stress factors such as CAT,SOD,Nrf2,HO-1,NQO1,and GCLC.Furthermore, the combined effect of HFD and AFB1 further significantly increased the levels of pro-inflammatory cytokines IL-2,IL-6,TNF-α,and IL-1β in the body.In summary,HFD treatment significantly exacerbated liver oxidative stress and in-flammatory responses in rats exposed to AFB1 through the Nrf2-keap1 signaling pathway.

AIM:This study aims to investigate the therapeutic effects of Xiaozhen Fang(XZF)on epidermal growth factor receptor(EGFR)inhibitor,erlotinib-induced skin toxicity in mice, with a focus on the underlying functional mechanisms.METHODS:A mouse model of skin toxicity was established and divided into three groups(n=5 per group):blank,erlotinib(150 mg/kg),and erlotinib(150 mg/kg)combined with XZF(45 g/kg)groups.Skin toxicity se-verity and body weight were evaluated.Western blot was performed to detect protein levels of gasdermin D(GSDMD),caspase-1,and interleukin-1β(IL-1β)in skin tissue.Immunofluorescence was employed to analyze gasdermin E,IL-1β,caspase-1,and nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3)expression in skin tissue. In vitro,HacaT cells were cultured and treated with erlotinib followed by different concentrations of XZF-containing se-rum.Cell viability was assessed by MTT assay.Cell ultrastructure was observed by transmission electron microscopy, and p53-binding protein 1(53BP1)expression was analyzed by immunofluorescence staining.RESULTS:The XZF signifi-cantly alleviated erlotinib-induced skin toxicity in mouse model,as evidenced by reduced rash incidence, alleviated limb swelling, and increased body weight(P<0.05).Expression of Gasdermins,IL-1β,caspase-1,and NLRP3 was distinctly down-regulated in dorsal skin tissue(P<0.05 or P<0.01).In vitro,XZF-containing serum markedly suppressed pyropto-sis in HacaT cells(P<0.01),preserved cell membrane integrity, and significantly reduced 53BP1 fluorescence intensity(P<0.01).CONCLUSION:The XZF mitigates EGFR inhibitor,erlotinib-induced skin toxicity in mice, potentially by regulating pyroptosis through the NLRP3/caspase-1/GSDMD signaling pathway. This mechanism exerts a cytoprotective ef-fect and alleviates erlotinib-induced skin toxicity.
