ObjectiveTo explore the current situation and influencing factors of humanistic care experience among patients visiting traditional Chinese medicine (TCM) outpatient clinics in China, and to provide a basis for optimizing TCM-characterized services in both TCM and Western medicine hospitals. MethodsA multi-center cross-sectional study was conducted using convenience sampling to select 35 hospitals across 13 provinces in China (including 3 TCM hospitals and 32 TCM outpatient clinics in general hospitals). A total of 3,430 patients were surveyed using the general information questionnaire and the Outpatient Humanistic Care Experience Questionnaire, with data collected via Questionnaire Star. Univariate analysis and multiple linear regression were employed to examine the impacts of patient characteristics, visit characteristics, hospital type (TCM hospital/general hospital), and geographic region (eastern/central/western) on humanistic care experience. ResultsThe total score of humanistic care experience was 194 (188, 233). Univariate analysis showed that gender, educational level, current residence, per capita monthly household income, location attribute of medical institutions, number of previous visits to this hospital, payment method of medical expenses, previous hospitalization history in this hospital, frequency of outpatient visits within the past 12 months, self-rated disease severity, familiarity with the outpatient procedures, implementation of the follow-up service provided by the hospital, satisfaction with follow-up services, the grade of the hospital visited, geographical region of the hospital visited, and the department visited had an impact on the humanistic care experience during outpatient visits (P<0.05). Multivariate analysis demonstrated that educational level (β=0.609, P=0.011), self-rated disease severity (β=-0.646, P=0.047), familiarity with outpatient procedures (β=4.784, P<0.001), satisfaction with follow-up services (β=6.365, P<0.001), and the grade of the hospital visited (β=-5.487, P<0.001) affected the humanistic care experience in outpatient medical treatment, explaining 24.4% of the total variation. ConclusionHumanistic care experience in TCM outpatient clinics is influenced by multiple factors. It is recommended to optimize the medical treatment process, strengthen doctor-patient communication training, and establish a precise follow-up mechanism, with a focus on improving care perceptions among patients with lower education levels and those attending primary-level hospitals, to refine the TCM-characterized service system.

Tinnitus is a refractory condition in otology. It is widely accepted that patients with tinnitus can only adapt to it,for there is no universally recognized effective pharmacotherapy till now.Through the interpretation of classical Chinese medical texts and after decades of clinical practice,Professor Liu Peng proposes that the primary pathogenesis of tinnitus is due to spleen-stomach dysfunction, largely caused by unhealthy life styles.Based on the holistic perspective of traditional Chinese medicine(TCM),he developed the Yishi therapy for tinnitus management.Yishi therapy emphasizes the integration of medical treatment and self-care via prioritizing mentorship and assisting with medical techniques. The mentorship is embodied by identifying and correcting patients' unhealthy living habits, guiding the patients to maintain dietary moderation, regular living, and emotional regulation, thereby activating their self-recuperative powers and promoting tinnitus.Medical techniques refer to the utilization of oral use of herbal medicine and external therapies on the basis of adjustment of lifestyle, aiming to restore the spleen-stomach function of ascending of lucidity and descending of turbidity, thereby alleviating tinnitus.Oral use of herbal medicine is performed with Guzhong Decoction(mainly composed of Astragali Radix,Codonopsis Radix,Cinnamomi Ramulus,Zingiberis Rhizoma,Atractylodis Macrocephalae Rhizoma,Pinelliae Rhizoma Praeparatum,Amomi Fructus,Acori Tatarinowii Rhizoma, and Glycyrrhizae Radix et Rhizoma Praeparata cum Melle)as the basic formula, through modified use based on syndrome differentiation.External treatment mainly refers to auricular acupoint pressing and occipital-knocking therapy.By combining the medical treatment and self-care,Yishi therapy exerts reliable efficacy for the treatment of tinnitus through regulating spleen-stomach function and activating the self-recuperative powers, and will provide a novel therapeutic approach to tinnitus.

The top 5 bacterial species were Escherichia coli(50.0%), Klebsiella pneumoniae(26.1%), Pseudomonas aeruginosa(5.1%), Acinetobacter baumannii complex(5.0%)and Enterobacter cloacae complex(4.1%). The ESBL-producing rates in Escherichia coli, Klebsiella pneumoniae and Proteus mirablilis were 46.8%(2 685/5 741),18.3%(549/2 999)and 44.0%(77/175),respectively. The prevalence of carbapenem-resistant Escherichia coli(CREC)and carbapenem-resistant Klebsiella pneumoniae(CRKP)were 1.3%(76/5 741)and 15.0%(450/2 999);32.9%(25/76)and 78.0%(351/450)of CREC and CRKP were sensitive to ceftazidime/avibactam combination, respectively. 94.7%(72/76)and 90.2%(406/450)of CREC and CRKP were sensitive to aztreonam/avibactam combination. Furthermore,57.9%(44/76)and 79.1%(356/450)were sensitive to imipenem/relebactam combination. The prevalence of carbapenem-resistant Acinetobacter baumannii(CRAB)complex was 64.6%(370/573),while more than 80.0% of CRAB complex was sensitive to tigecycline, eravacycline and polymyxin B. The prevalence of carbapenem-resistant Pseudomonas aeruginosa(CRPA)was 17.0%(99/581). There were differences in the composition ratio of Gram-negative bacteria in bloodstream infections and the prevalence of important Gram-negative bacteria resistance among different regions in China, with statistically significant differences in the prevalence of CREC,CRKP,CRPA and CRAB complex( χ2=10.6,28.6,10.8 and 19.3, P<0.05). The prevalence of ESBL-producing Escherichia coli, CREC,CRAB complex and CRKP were higher in provincial hospitals than those in municipal hospitals( χ2=12.5,9.8,12.7 and 57.8,all P<0.01). Conclusions:Gram-negative bacteria are the main pathogens causing bloodstream infections in China, and Escherichia coli is ranked in the top, while the trend of Klebsiella pneumoniae increases continuously with time. CRKP infection shows a slow upward trend,CREC infecton maintains a low prevalence level, and CRAB complex infection continues to exhibit a high prevalence rate. The composition and resistance patterns of pathogens causing bloodstream infections vary to some extent across different regions and levels of hospitals in China.

Objective:To systematically evaluate predictive models for stress urinary incontinence (SUI) in pregnant and postpartum women, providing a reference for model development, application, and promotion. Methods:A comprehensive literature search was conducted in PubMed, Embase, Web of Science, Cochrane Library, China National Knowledge Infrastructure, Wanfang Database, and China Biology Medicine disc for studies on SUI predictive models in pregnant and postpartum women. The search period was from database inception to September 30, 2024. Two researchers independently screened the literature and extracted data according to inclusion and exclusion criteria. The risk of bias in the predictive models was assessed using the prediction model risk of bias assessment tool. Results:A total of 23 studies were included, covering 31 predictive models for SUI, with a combined sample size of 14 473 women. Among them, six models focused on predicting SUI in pregnant women, while 25 models were developed for postpartum SUI. The predictive factors identified in these models were categorized into nine groups, including: general information for pregnant and postpartum women, delivery data, neonatal data, past history, abortion history, lifestyle data, pelvic floor muscle screening results, 2D and 3D ultrasound data, and serological indicators. Among these, age, mode of delivery, parity, body mass index, history of SUI, and neonatal weight were widely recognized as key predictive factors. External validation was performed in five studies. Five studies showed good applicability and low bias risk, except for one study that had limitations in both bias risk and applicability, and the remaining studies exhibited a high risk of bias but demonstrated good applicability. Conclusions:The methodological quality of SUI predictive models for pregnant and postpartum women needs further improvement. External validation remains insufficient. Future model development should be based on large-sample, prospective studies, incorporating appropriate predictive factors and stratifying SUI risk in different populations to enhance clinical applicability.
