1.Association between uric acid and new-onset chronic kidney disease in middle-aged and elderly hypertensive patients

Objective:To explore the association between uric acid and new-onset chronic kidney disease (CKD) in middle-aged and elderly hypertensive patients.Methods:This was a retrospective cohort study. Middle-aged and elderly hypertensive patients who had attended at least two annual health examinations at Yongshun Community Health Service Center in Tongzhou District, Beijing, from June 2016 to December 2020 were enrolled. The time interval between the two physical examinations was three years. The first physical examination time served as the baseline, and the second as the end of follow-up. Based on the uric acid level at baseline, the participants were divided into the normal uric acid group and the hyperuricemia group. The relevant clinical data of the participants were collected. The endpoint of the study was new-onset CKD. A multivariate logistic regression model was used to analyze the association between uric acid and new-onset CKD in hypertensive patients.Results:A total of 2 472 middle-aged and elderly hypertensive patients with an average age of (62.43±7.02) years were included. Of these, 733(29.7%) were male. There were 710 patients with hyperuricemia (hyperuricemia group) and 1 762 patients with normal uric acid levels (normal uric acid group).After adjusting for age, sex, body mass index (BMI), systolic blood pressure, diabetes mellitus, estimated glomerular filtration rate (eGFR), and uric acid-lowering treatment, multivariate logistic regression analysis showed that combined with hyperuricemia was an independent risk factor for new-onset CKD in middle-aged and elderly hypertensive patients ( OR=3.00, 95% CI: 1.87-4.80, P<0.001). The results of multivariate logistic analysis showed that elevated uric acid level was an independent risk factor for new-onset CKD in both male and female middle-aged and elderly hypertensive patients (both P<0.05), and there was no sex interaction ( P for interactio n>0.05). The results of multivariate logistic analysis showed that the combination of asymptomatic hyperuricemia was an independent risk factor for new-onset CKD in middle-aged and elderly hypertensive patients ( OR=3.00, 95% CI: 1.87-4.80, P<0.001), and there was no gender interaction ( P for interactio n>0.05). Conclusions:Hyperuricemia is an independent risk factor for new-onset CKD in middle-aged and elderly hypertensive patients, and elevated uric acid levels increase the risk of new-onset CKD in both male and female patients. Moreover, asymptomatic hyperuricemia may increase the risk of new-onset CKD.

Outpatient humanistic care refered to providing a full process of caring medical services to outpatients. In order to standardize the human caring services for outpatients in medical institutions, promote the comprehensive service level of outpatient services, and improve the patient′s medical experience, Chinese Association for Life Care issued the group standard of Management Norms for Human caring of Outpatients in April 2023. This standard clarified the relevant terms and definitions of human caring for outpatients, specified the basic requirements for human caring, the humanistic quality and care responsibilities of outpatient staff, the outpatient care environment and facilities, the outpatient care process and measures, and quality management. It designed standardized and personalized full process care service norms, providing references for medical institutions at all levels to promote the development of human caring for outpatients.
4.Correlation Between Sagittal Orientation of Lumbar Pedicle Screw and Screw Loosening

Objective To analyze the correlation between the sagittal orientation of lumbar pedicle screws and screw loosening based on the craniocaudal cyclic load and artificial vertebra,and to provide a reference for the improvement of screw orientation during surgery,consequently reducing the incidence of screw loosening postoperatively.Methods Pedicle screws were inserted into artificial vertebrae in the cranial,parallel,and caudal orientations.After the artificial vertebra was installed in the material testing machine,the model was subjected to a 100 N preload and 166 N cyclic load for 10 000 cycles.During loading,displacements were recorded and cycle-displacement curves were drawn.After the test was completed and the screws were removed,the screw hole volume was measured and the screw hole volume ratio(the ratio of screw hole volume to the volume of the screw inserted into the vertebra)was calculated.Results The cyclic displacement curve showed that the cranial group had the largest displacement,followed by the parallel and caudal groups.The screw-hole volume ratio results showed that the cranial group had the highest ratio,followed by the parallel and caudal groups.Correlation analysis showed a negative relationship between screw sagittal orientation and terminal displacement(displacement corresponding to the 10 000th cycle)(ρ=-0.897,P=0.02)and screw hole volume ratio(ρ=-0.902,P=0.01).Conclusions There is a negative correlation between the sagittal orientation of the lumbar pedicle screw and screw loosening.Namely,when the screw sagittal orientation is adjusted from a cranial tilt to a parallel and even caudal tilt at a certain angle range,the incidence of screw loosening is reduced.
5.Dynamic changes and function of mitochondria in development of em-bryonic hematopoietic stem cells

AIM:To describe the dynamic characteristics of mitochondria in the development of mouse embryonic hematopoietic stem cells,and to explore the function of mitochondria in this process.METHODS:Single-cell transcrip-tomic data of continuous developmental hematopoietic stem cell-related populations were analyzed to describe the dynamic changes of genes related to mitochondrial synthesis and energy metabolism.To explore the dynamic changes in the number and activity of mitochondria during the development of hematopoietic stem cells,we detected the mitochondrial number and mitochondrial membrane potential of the cells in each stage of hematopoietic stem cell development by fluorescent probe staining combined with flow cytometry.We added small molecule inhibitors of mitochondrial synthesis and energy metabolism and used hematopoietic cell colony formation assay to detect the effect of mitochondrial function inhibition on the induction of hematopoietic products in vitro.RESULTS:(1)Single-cell transcriptome analysis showed that genes in-volved in mitochondrial synthesis and oxidative phosphorylation were significantly up-regulated in endothelial cell and type Ⅰ pre-hematopoietic stem cell compared with those involved in glycolysis,and these genes could significantly distinguish continuous dynamic populations.(2)The results of fluorescence staining and flow cytometry analysis showed that mito-chondrial number and mitochondrial membrane potential had an increasing trend during the continuous development of he-matopoietic stem cell,reaching the highest level in the precursor stage of type 2 pre-hematopoietic stem cell,and decreasing in the mature hematopoietic stem cell of fetal liver.(3)Compared with control group,inhibition of mitochondrial respirato-ry chain Ⅰ and Ⅴ significantly reduced the number of hematopoietic colonies(P<0.05).CONCLUSION:(1)Genes re-lated to mitochondrial synthesis and oxidative phosphorylation are highly expressed in hemogenic endothelial cells and type Ⅰpre-hematopoietic stem cells,and can be used to distinguish continuous developing populations.(2)The mitochondrial number and mitochondrial membrane potential increased continuously during hematopoietic stem cell development and reached the highest level in type 2 pre-hematopoietic stem cells.(3)Inhibition of mitochondrial respiratory chain Ⅰ and Ⅴ significantly reduced the production of hematopoietic products in vitro.
6.Comparison of scope of nurse practice in different countries

Objective:To compare the policies and literature on scope of nurse practice in various countries, so as to provide reference for optimizing the expansion of scope of nurse practice in China.Methods:Literature on nurse practice was systematically searched on PubMed, Web of Science, China National Knowledge Infrastructure, WanFang Data, and nursing policy organization websites in various countries. The search period was from database establishment to June 2023.Results:A total of 19 articles from six countries were included. There were elements such as registration types and requirements, practice methods, prescription authority, supervision departments, and relevant laws. The inspiration for the hierarchical management of nurses in China mainly included four aspects: improving the hierarchical registration and management system, clarifying the job responsibilities of nurses; guiding the reasonable flow of nursing talents, encouraging multi-site practice in nurses; cultivating nurses' prescription ability and granting them prescription authority; improving supervision and legal protection to ensure that nurses practiced in accordance with the law.Conclusions:It is urgent to expand the practice scope of the nursing team in our country, supplement grassroots medical and health talents, undertake the functions of case management for chronic patients, improve the quality of life of patients, and meet the important task of community and family nursing needs.

Objective:To investigate the current situation and influencing factors of patients′ satisfaction with nursing humanistic care, and to provide reference for improving the quality of such care provided by hospitals.Methods:From July to August 2022, outpatients and inpatients in 30 provinces were selected by multi-stage stratified sampling as the survey objects. A cross-sectional survey was conducted on an online platform, using the general information questionnaire and Chinese version of methodist health care system nurse caring instrument revised by the research group. The latter instrument consists of 12 dimensions. namely care coordination, competence, teaching/learning, emotional support, respect for individuality, physical comfort, availability, helping/trusting relationship, patient/family engagement, physical environment, spiritual environment and outcomes. Descriptive analysis was performed on the data collected by the questionnaires, and independent sample t-test and one-way ANOVA were used to analyze the influencing factors of patient satisfaction. Results:A total of 107 hospitals were selected for questionnaire survey, including 86 tertiary hospitals and 21 secondary hospitals, and 29 108 valid questionnaires were recovered. The patient satisfaction with nursing humanistic care scored (5.40±0.86); the top three dimensions were competence (5.50±0.89), emotional support (5.47±0.88) and helping/trusting relationship (5.46±0.86); the lowest scoring dimensions were teaching/learning (5.38±1.01), spiritual environment (5.36±1.04) and patient/family engagement (5.11±1.28). Differences with gender, age, marital status, child status, educational level, occupation, place of residence, economic region, per capita monthly income of the family, type of medical insurance, medical department visited and surgery or not presented significant differences on the patient satisfaction with nursing humanistic care scores ( P<0.05). Conclusions:The satisfaction of patients with hospital′s nursing humanistic care in China was at the middle to upper level. In the future, health education for patients should be strengthened, and a mode of family-engaged nursing humanistic care should be constructed in line with the Chinese cultural background. In the process of nursing services, the particularity of patient groups should be considered to better meet their needs.
9.Acceptance and influencing factors of home maintenance of family members of elderly discharged patients with PICC catheter

Objective:To explore home maintenance acceptance of family members of elderly discharged patients with peripherally inserted central catheter (PICC) catheter and to analyze its influencing factors.Methods:From January to November 2019, a total of 143 family members of elderly patients who were given PICC in three ClassⅢ Grade A hospitals in Wenzhou and discharged with catheter were selected as research objects. A questionnaire survey was conducted on 143 family members of elderly discharged patients with PICC catheter using a general information questionnaire, Catheter Maintenance Related Cognitive Questionnaire and Barthel Index Rating Scale (BI) . A total of 143 questionnaires were distributed in this study and a total of 143 valid questionnaires were recovered. The effective recovery rate was 100%.Results:The acceptance rate of home maintenance of 143 family members of elderly discharged patients with PICC catheter was 60.14% (86/143) . Single factor analysis showed that home maintenance acceptance score of family members of elderly discharged patients with PICC catheter had statistically significant differences in working status of family members, ADL score of patients, whether they stayed in bed, whether the catheter maintenance in the residence was convenient, whether they were confident in catheter observation and treatment and whether they were worried about the effect of home maintenance ( P<0.05) . Multivariate logistic regression analysis showed that the convenience of catheter maintenance in the residence, self-care ability of patients and whether they were worried about the maintenance effect were the influencing factors of the home maintenance acceptance of family members of elderly discharged patients with PICC catheter ( P<0.05) . Conclusions:The family maintenance acceptance of elderly discharged patients with PICC catheter is at a medium level. It is necessary to strengthen the maintenance knowledge education and operation skills training of family members with inconvenient catheter maintenance in residence and poor self-care ability of patients and help them to improve the maintenance effect so as to better carry out home maintenance.
10. Evaluation of carotid plaque vulnerability by contrast-enhanced ultrasound and superb microvascular imaging

Objective:
To observe the plaque surface morphology and neo-vascularization within the plaque using superb microvascular imaging(SMI)and contrast-enhanced ultrasound(CEUS), and to compare the correlation and consistency of the two technologies based on making clinical diagnosis of symptomatic carotid artery plaque as the standard.
Methods:
A total of 92 patients undergoing carotid ultrasonography in Beijing Tiantan Hospital from August 2016 to October 2017 were recruited, including 61 males and 31 females, aged(63.5 ± 7.7)years(range, 42-80 years.A total of 105 plaques were found.According to whether or not to have plaque-induced ipsilateral ischemic symptoms, patients were divided double-blindly into the symptom group and the non-symptom group.The features of plaque(including surface morphology and neo-vascularization)were recorded.The ultrasonographic findings were chi-squarely tested.
Results:
There were statistically significant differences in the surface morphology and enhancement degree of CEUS between carotid artery plaque-induced ischemic symptom group and the non-symptom group(
