Enamel demineralization, the formation of white spot lesions, is a common issue in clinical orthodontic treatment. The appearance of white spot lesions not only affects the texture and health of dental hard tissues but also impacts the health and aesthetics of teeth after orthodontic treatment. The prevention, diagnosis, and treatment of white spot lesions that occur throughout the orthodontic treatment process involve multiple dental specialties. This expert consensus will focus on providing guiding opinions on the management and prevention of white spot lesions during orthodontic treatment, advocating for proactive prevention, early detection, timely treatment, scientific follow-up, and multidisciplinary management of white spot lesions throughout the orthodontic process, thereby maintaining the dental health of patients during orthodontic treatment.

Objective:To explore the characteristics of gray matter volume (GMV) and structural covariant network (SCN) in patients with cerebral small vessel disease (CSVD) related cognitive impairment. Methods:This was a cross-sectional study. Ninety-eight patients with CSVD who attended Wuxi People′s Hospital of Nanjing Medical University between October 2021 and December 2022 were prospectively included. The patients were evaluated using the cognitive status assessment scale and were categorized into 57 cases in the CSVD with cognitive impairment group and 41 cases in the CSVD without cognitive impairment group according to the presence or absence of cognitive impairment. 3D-T 1WI structural image data were collected, and GMV differences between the two groups were compared by SPM 12 toolbox and CAT12 toolkit. At the same time, Pearson correlation analysis was also performed to analyze the GMV of differences between the 2 groups and cognitive status assessment scale scores. The BCT software package based on MATLAB platform was used to construct the GMV-related structural covariant network (SCN), and the graph theory method was utilized for SCN analysis to calculate the area under the curve (AUC) of the global and local parameters within the set sparsity range, and the permutation test was used to compare the differences in the AUC of the 2 groups. Results:In the CSVD with cognitive impairment group, GMV in bilateral hippocampus, parahippocampal gyrus, fusiform gyrus, and left amygdala was significantly lower than that in the CSVD without cognitive impairment group (family wise error corrected P<0.05), and the GMV in these regions had correlation with cognitive status assessment scale ( P<0.05). At the global network level of the SCN, the area under the curve (AUC) of the characteristic path length was significantly higher in the CSVD with cognitive impairment group than in the CSVD without cognitive impairment group ( P=0.023), while the AUC of global efficiency was significantly lower in CSVD with cognitive impairment group than in the CSVD without cognitive impairment group ( P=0.005). At the local level, the nodal degree and nodal efficiency of the left putamen were significantly decreased in the CSVD with cognitive impairment group compared to the CSVD without cognitive impairment group (false discovery rate corrected P<0.05). Conclusions:GMV reduce in patients of CSVD with cognitive impairment in the bilateral hippocampus, parahippocampal gyrus, fusiform gyrus, and left amygdala. In the structural covariance network, characteristic path length increase while global efficiency reduce, and node degree and nodal efficiency of the left putamen reduce.

Active immunotherapy for cancer aims to treat disease by inducing effective cellular immunity and humoral immunity.Research on virus-like particles(VLPs)vaccines has made tremendous progress in recent years, dramatically reducing morbidity and mortality from some infectious diseases.VLPs are nanoparticles self-assembled from one or more structural proteins, with highly ordered repeat sequences and good immunogenicity, which can induce strong cellular immune and humoral immune responses.VLPs can overcome immunosuppressive state of tumor microenvironment, break self-tolerance,and trigger strong cytotoxic T lymphocyte activity, which is critical for both viral clearance and destruction of cancer cells. This article mainly reviews current research progress of VLPs-based cancer vaccines and potential defects of VLPs as vaccine carriers in development of cancer vaccines.

Virus-like particles(VLPs)are nanoparticles that are self-assembled from one or more structural proteins, which can be arranged in several layers or contain a lipid outer membrane.Due to the lack of genetic material,VLPs cannot infect host cells, but are highly immunogenic and can induce immune responses different from conventional inactivated vaccines.VLPs can be produced using a variety of systems including bacterial, yeast, plant, insect and mammalian cells.Compared with traditional vaccines,VLPs have incomparable advantages,so they are becoming more and more popular in the biomedical field. To date,a series of vaccine candi-dates based on VLPs have been developed for immunization and prevention of various infectious diseases. At the same time, the recent successful application of VLPs in targeted drug delivery and gene therapy has attracted attention. This paper mainly reviews the com-monly used expression systems of VLPs and the research progress of their applications.

In order to understand the infection and molecular genetic characteristics of goose astro-virus(GAstV)in Hotan,Xinjiang, visceral organs and swabs of dead goslings were collected asep-tically from three goose farms in Hotan,Yutian and Pashan counties, and GAstV was detected by RT-PCR.The positive samples were screened and identified in LMH cells, and the whole genome was sequenced, and the genetic characteristics of the isolates were analyzed. The results showed that the total positive rate of GAstV was 11.25%(65/578).Two strains of GAstV named as GAstV/XJHT-1 and GAstV/XJHT-2 were isolated and the lengths of their genome sequences were determined as 7 190 bp and 7 125 bp,respectively.Whole genome homology analysis showed that the homology of the two isolates with GAstV-1 and GAstV-2 was higher than 95%,and the homology with other sources(chicken, duck, and turkey)ranged from 54.1%to 61.5%.Genetic e-volution analysis showed that the genetic distance between GAstV isolates from Henan and Anhui was relatively close, suggesting that the isolated GAstV may be related to the introduction of gos-lings or goose eggs from these two places. The findings provide a basis for further development of vaccines or control products.

The lack of preoperative guidelines for the management of drugs for psychiatric disorders will affect the quality of surgical management of psychiatric patients and increase the probability of perioperative complications. To standardize the preoperative management of medications for the treatment of mental disorders, the Perioperative Assessment and Quality Improvement Society issued《Preoperative Management of Medications for Psychiatric Diseases:Society for Perioperative Assessment and Quality Improvement Consensus Statement》in February 2022 to provide clinicians with recommendations for the preoperative management of psychotropic medications. This article interpreted the consensus, summarized the perioperative interactions, special precautions, and auxiliary examination precautions, and provided corresponding preoperative suggestions on antidepressants, mood stabilizers, anxiety, antipsychotics, and attention deficit hyperactivity disorder drugs,to provide a reference for the standardized management of perioperative drugs.

Herald of Medicine 2024;43(9):1434-1439
Tumor necrosis factor alpha inhibitors(TNFαi)are a class of biological agents that suppress the immune system and lower inflammatory levels, and they are increasingly employed to treat autoimmune diseases. However,TNFαi theoretically can increase the risk of infection, tumor, and other complications. To increase the safety of perioperative medicine,it is necessary to balance the relative risks of increased complication rates due to continuous medication and symptom aggravation due to medication suspension for patients undergoing surgery.Currently, there is no comprehensive clinical guideline for the perioperative management of TNFαi in China. To provide a reference for the perioperative management of TNFαi,this article used the indications of rheumatoid arthritis, ankylosing spondylitis, inflammatory bowel disease, and psoriasis as its starting point to systematically sort out the recommendations and considerations for the use of TNFαi in the perioperative drug treatment of patients.
