1.The efficacy of ganciclovir combined with spleen aminopeptide in treating infectious mononucleosis and its impact on the immune function of diseased children

Objective To investigate the efficacy of ganciclovir combined with spleen aminopeptide in treating infectious mononucleosis(IM)and its impact on the immune function of diseased children.Methods Totally 120 diseased children with IM accepted by our hospital from February 2022 to August 2024 were stochastically assigned into a treatment group and a monotherapy group.The monotherapy group received treatment with ganciclovir,while the treatment group received treatment with spleen aminopeptide in addition to the monotherapy group.The efficacy,symptom improvement time,serum inflammatory factors,immune indicators,and biochemical indicators before and after treatment were compared between the two groups.Results The efficacy of the treatment group was better than that of the monotherapy group,and the symptom relief time was shorter than that of the monotherapy group(P<0.05).After treatment,the serum amyloid A,TNF-α,IL-6,procalcitonin,CD3+T,CD8+T,lactate dehydrogenase,alanine aminotransferase,EBV-DNA,creatine kinase,and white blood cell count in both groups decreased,while CD4+T and CD4+T/CD8+T increased.The amplitude of the treatment group was greater than that of the monotherapy group(P<0.05).Conclusion The combination of ganciclovir and spleen aminopeptide in treating IM is beneficial for enhancing the immune function of T lymphocytes of diseased children,alleviating inflammatory reactions,improving biochemical indicators,and enhancing efficacy.
2.Adropin level and its correlation with collateral circulation status in elderly patients with acute myocardial infarction

Objective To explore the level of energy balance related protein antibody,adropin,and its correlation with collateral circulation status in elderly patients with AMI.Methods A total of 193 elderly AMI patients admitted to our department from February 2022 to February 2024 were enrolled,and based on their collateral circulation status,they were divided into a good circulation group(121 cases)and a poor circulation group(72 cases).The level of adropin was determined.Multivariate logistic regression analysis was employed to determine the influencing factors of poor collateral circulation in elderly AMI patients.Results The poor circulation group had significantly larger proportion of hypertension,higher fasting blood glucose(FBG),and larger red blood cell distribution width(RDW),but lower adropin level and mean platelet volume(MPV)when com-pared with the good circulation group(P<0.05,P<0.01).Multivariate logistic regression analy-sis showed that adropin,FBG,RDW,MPV,and hypertension were all influencing factors for poor collateral circulation in elderly AMI patients(P<0.05,P<0.01).The AUC value of adropin,FBG,RDW,MPV,and hypertension was 0.810,0.762,0.761,0.715 and 0.563,respectively in pre-dicting poor collateral circulation.Among them,adropin level had the highest predictive value(P<0.01).Conclusion The decrease in adropin level in elderly AMI patients is closely associated with poor collateral circulation,and it is a predictive factor for collateral circulation.

4.Effect of life reconstruction training on anxiety and depression among individuals with chronic spinal cord injury and their caregivers

Objective To explore the effect of life reconstruction training on anxiety and depression among individuals with chronic spinal cord injury(SCI)and their caregivers,and to analyze the influencing factors.Methods A total of 216 SCI patients and 79 caregivers who participated in life reconstruction training were recruited from the Hope House for SCI individuals in Shanghai Yangzhi Rehabilitation Hospital from May,2017 to Octo-ber,2019.They were assessed with Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)be-fore and after intervention.Results After intervention,the scores of SAS and SDS decreased(P<0.05)in both SCI patients and their caregivers.SAS scores were more likely to reduce in SCI patients who suffered anxiety at admission(OR=2.315,95%CI 1.328 to 4.033,P=0.003)and their caregivers(OR=3.632,95%CI 1.270 to 10.390,P=0.016),as well as non-agricultural-registered SCI patients(OR=1.908,95%CI 1.074 to 3.391,P=0.028).Being unmarried,having an injury duration≥21 years,injury due to disease,and having depression at admission significantly promoted the reduction in SDS scores among SCI patients(P<0.05).Caregivers with a junior-high-school education or less were more likely to experience reductions in scores of SAS and SDS compared with those with higher education(P<0.05).Caregivers with five to 15 years of caregiving experience achieved more anxiety reduction(OR=7.155,95%CI 1.806 to 28.342,P=0.005).Conclusion Life reconstruction training is effective on anxiety and depression among individuals with chronic SCI and their caregivers.It is recommended to prioritize anxiety interventions for patients with rural household registra-tion in China,along with depression management for the married/divorced/widowed individuals,with<21 years since injury,and trauma-induced lesions.For caregivers,it is needed to focus to anxiety and depression in better-educated individuals,as well as anxiety in those with either<5 or≥15 years of caregiving experience.

7.The pathogenesis of dry eye disease was explored based on the Wnt/β-catenin pathway

Abstract
Dry eye disease is a chronic ocular surface inflammatory disease caused by abnormal tear quality or quantity and decreased tear film stability due to various reasons, and often accompanied by ocular discomfort such as itching, dryness, foreign body sensation, and visual dysfunction, which can seriously affect the patient′s quality of life and visual quality if not intervened in time. With the change of social lifestyles, the increase of environmental pollution and the trend of population aging, dry eye disease has become the most common ocular surface disease besides refractive error. Currently, dry eye disease is widely recognized as a non-infectious immune-related inflammatory disease, but the signaling pathways involved in dry eye disease are poorly understood. Whether dry eye disease is caused by excessive tear evaporation, insufficient tear production, or mucin deficiency, the ocular surface tissues(cornea/conjunctiva) inevitably undergo pathological processes such as aberrant proliferation, squamous epithelial hyperplasia, initiation of corneal damage repair mechanisms, and reduction in the number of conjunctival goblet cells, whereas the Wnt/β-catenin pathway is known to have a wide range of biological functions and plays an important role in cell proliferation, differentiation, and stemness maintenance. Therefore, this review describes the pathogenesis and potential experimental therapeutic options of the Wnt/β-catenin signaling pathway in dry eye disease from this perspective, aiming to provide new targets for the treatment of dry eye disease and achieve the goal of controlling the disease progression from the root.
8.Epidemiological and spatial-temporal clustering characteristics of pertussis in Hebei Province from 2013 to 2022

Objective:To analyze the spatial-temporal epidemiological characteristics of pertussis from 2013 to 2022 in Hebei Province and to provide a reference for improving prevention and control measures.Methods:Based on the data of pertussis reported in Hebei Province during 2013-2022 to analyze the popular characteristic, the ArcGIS 10.8 software was used to construct a ring map and to perform spatial autocorrelation analysis; the SaTScan 10.1 software was used for spatial-temporal scan statistics.Results:There were 6 715 cases of the cumulative report in Hebei Province from 2013 to 2022 without death. The annual report incidence was 0.90/100 000. The overall incidence rate showed an upward trend from 2013 to 2019, and during 2020-2021, it showed a sharp decline, but in 2022, it showed a sharp increase. Summer and autumn are the peak seasons of the epidemic. The incidence was highest in age group <1 year (48.67%), and the lowest age group in age group ≥15 years (0.45%) and mainly scattered children (78.03%); the incidence about men is higher than women. Spatial autocorrelation analysis showed that the onset of pertussis has spatial clustering, and high-high clusters were found in Langfang, Baoding, and Cangzhou, the top three countries with reported incidence. The area covered by a low-low cluster was consistent with the distribution of the corresponding low-incidence areas in this study. Space-time scan detects five statistically significant areas, and three zones were concentrated in 2022.Conclusions:The incidence of pertussis in Hebei had obvious season, population, and area-specific differences. There was obvious spatiotemporal and clustering, so the control of key areas should target the characteristics of time and space.
9.The effect of glucose-6-phosphate dehydrogenase deficiency on allogeneic hematopoietic stem cell transplantation in patients with hematological disorders

Objectives:To determine the effect of glucose-6-phosphate-dehydrogenase (G6PD) deficiency on patients’ complications and prognosis following allogeneic stem cell hematopoietic transplantation (allo-HSCT) .Methods:7 patients with G6PD deficiency (study group) who underwent allo-HSCT at Peking University People's Hospital from March 2015 to January 2021 were selected as the study group, and thirty-five patients who underwent allo-HSCT during the same period but did not have G6PD deficiency were randomly selected as the control group in a 1∶5 ratio. Gender, age, underlying diseases, and donors were balanced between the two groups. Collect clinical data from two patient groups and perform a retrospective nested case-control study.Results:The study group consisted of six male patients and one female patient, with a median age of 37 (range, 2-45) years old. The underlying hematologic diseases included acute myeloid leukemia ( n=3), acute lymphocytic leukemia ( n=2), and severe aplastic anemia ( n=2). All 7 G6PD deficiency patients achieved engraftment of neutrophils within 28 days of allo-HSCT, while the engraftment rate of neutrophils was 94.5% in the control group. The median days of platelet engraftment were 21 (6–64) d and 14 (7–70) d ( P=0.113). The incidence rates of secondary poor graft function in the study group and control group were 42.9% (3/7) and 8.6% (3/35), respectively ( P=0.036). The CMV infection rates were 71.4% (5/7) and 31.4% (11/35), respectively ( P=0.049). The incidence rates of hemorrhagic cystitis were 57.1% (4/7) and 8.6% (3/35), respectively ( P=0.005), while the bacterial infection rates were 100% (7/7) and 77.1% (27/35), respectively ( P=0.070). The infection rates of EBV were 14.3% (1/7) and 14.3% (5/35), respectively ( P=1.000), while the incidence of fungal infection was 14.3% (1/7) and 25.7% (9/35), respectively ( P=0.497). The rates of post-transplant lymphoproliferative disease (PTLD) were 0% and 5.7%, respectively ( P=0.387) . Conclusions:The findings of this study indicate that blood disease patients with G6PD deficiency can tolerate conventional allo-HSCT pretreatment regimens, and granulocytes and platelets can be implanted successfully. However, after transplantation, patients should exercise caution to avoid viral infection, complications of hemorrhagic cystitis, and secondary poor graft function.
10.Analysis of gene characteristics of Streptococcus pneumoniae based on whole-genome sequencing

Objective:To analyze 80 strains of Streptococcus pneumoniae by using whole-genome sequencing technology, including serotype characteristics, drug resistance genes, carriage of virulence genes, multilocus sequence typing (MLST), and relatedness between strains. Methods:Whole-genome sequencing was performed on Streptococcus pneumoniae. Serotypes, resistance genes, virulence genes, and sequence typing (ST) were analyzed in conjunction with the CARD database, the NCBI database, the VFDB database, and the PubMLST website. The strains were compared by using KSNP4 based on the analysis of single-nucleotide polymorphisms (SNPs). An evolutionary tree was constructed with the IQ-TREE software and beautified by using EvolView software. Results:Of all 80 Streptococcus pneumoniae strains, serotypes 19F, 19A, 14 and 23F were predominant, 30 strains were from cases of invasive pneumococcal disease (IPD) and 50 strains were from cases of non-invasive pneumococcal disease (NIPD). The virulence genes psaA, nanB, lytA, lytC, and ply were detected in all cases, while cps4 A, cps4 B, and hysA were detected in 98.75% of cases, and nanA was detected in 90.00%, rrgA and rrgB showed statistical differences between IPD and NIPD( P<0.05). The resistance genes patA, patB, tetM, pmrA, PBP1 b, RlmA( II), and ermB were detected in all cases, while PBP1 a was detected in 98.75% of cases. There was a statistically significant difference between IPD and NIPD for mefA and msrD( P<0.05). MLST analysis of 80 strains of bacteria showed that the ST types were mainly ST271, ST320, ST876, ST81, in which seven new ST types were emerged; the phylogenetic tree could be divided into three branches, The dominant ST types of the strains with serotypes 14, 23F, 19F, and 19A were ST876, ST81, ST271, and ST320, respectively. Conclusions:The study finds that the main serotypes of the 80 strains are 19F, 19A, 14 and 23F. The strains have more varieties of virulence genes, resistance genes and high carriage of some genes; rrgA, rrgB and mefA, msrD differ between IPD and NIPD. ST-types are characterised by diversity and new ST-types are constantly emerged. A correlation exists between serotypes and systemic evolution in the phylogenetic analyses, with each serotype contains its own dominant ST-types.
