1.Construction and application of an intelligent insurance medication audit system based on pre-auditor system

The establishment of an intelligent audit system for hospital medical insurance is crucial for standardizing irrational practices and curbing the unwarranted escalation of medical insurance costs. A tertiary hospital has developed comprehensive prescription review rules in terms of indications, nutritional risk assessments, pain score evaluations, drug sensitivity test results, and target patient populations. When using medical insurance funds for settlement, both systematic and manual reviews are conducted, and three-level intervention measures such as " prompt, review, and interception" are implemented based on prescription issues. After the system was put into use, the hospital′s in-process information system supervision mode shifted from the previous focus on prompts to focus on audits. The medical insurance intelligent audit system provided timely prompts and interventions, and the number of unreasonable medical insurance prescriptions and deductions from medical insurance sampling prescriptions significantly decreased ( P<0.001), while the number of medical insurance medication prescription audits increased ( P<0.001). The application of the intelligent medical insurance medication audit system has improved the efficiency of prescription auditing and ensured the rationality of the use of medical insurance funds.
2.Comparison of Effect of Different Microbial Agents on Quality of Lycii Fructus Based on Multiple Index Components

ObjectiveTo reveal the effects of different microbial agents on quality of Lycii Fructus by comparing the differences in the contents of multiple types of chemical components in Lycii Fructus after the application of different microbial agents. MethodTaking Ningqi No. 7 as experimental material, four microbial agents, namely Peiyuan combined with Xinterui(TP group), Trichoderma harzianum combined with Bacillus subtilis(BW group), Genwuyou(MT group) and Junyiduo(JYD group), were applied, and no microbial agents was used as the blank group(CK group). Then the contents of total phenolics, total flavonoids, saccharides, amino acids, nucleosides and bases, betaine and other components in Lycii Fructus were determined by ultraviolet spectrophotometry(UV), high performance liquid chromatography(HPLC) and ultra-high performance liquid chromatography-mass spectrometry(UHPLC-MS/MS), and the methods such as multiple comparisons, principal component analysis(PCA) and partial least squares-discriminant analysis(PLS-DA) were used to analyze the effect of different microbial agents on the quality of Lycii Fructus. ResultMicrobial agents had different effects on chemical components of Lycii Fructus. The content of total phenolics was the highest in the TP group, and it varied significantly from the CK group(P<0.05). The total flavonoid content was the highest in the BW group, followed by the TP group. Both polysaccharide and alduronic acid contents were the highest in the JYD group. Betaine content in the TP and BW groups were significantly higher than that in the CK group(P<0.05). For the determined 23 kinds of amino acids, most of them were the lowest in the JYD group, and the highest in the MT group, while the nucleoside bases were higher in the MT and BW groups. It indicated that Lycii Fructus from different treatment groups could be distinguished clearly based on the determined 45 chemical components. The result of PLS-DA showed that the major differential components in each group were polysaccharides, glucose, fructose, betaine, alduronic acid, asparagine, sucrose, threonine, total flavonoids, alanine and total phenolics. The results of PCA composite scores based on the main differential components showed that composite scores of chemical components in each group were BW group>TP group>MT group>CK group>JYD group. ConclusionThe application of microbial agents of BW, TP and MT can promote the quality improvement of Lycii Fructus, and the application of JYD can promote the accumulation of polysaccharides and alduronic acid to a certain extent, but the overall effect on the quality of Lycii Fructus is not clear. This study lays the foundation for the green and healthy development of Lycii Fructus industry.
3.Dickkopf-1 inhibits the secretion of MUC5AC induced by Mycoplasma pneumoniae P1-C in mouse lung epithelial cells.

Mycoplasma pneumoniae is the most common pathogen of respiratory tract infection in children and adults. Clinical observation shows that M. pneumoniae infection can cause massive mucus secretion in the respiratory tract, which makes the breathing of patients difficult. Studies have shown that M. pneumoniae infection can cause massive secretion of mucin 5AC (MUC5AC). Adhesin P1 plays an important role in the pathogenesis of M. pneumoniae infection by mediating the adhesion of pathogens to host cells, and the C-terminal residues of P1 (P1-C) are immunogenic. This study investigated the molecular mechanism of Wnt/β-catenin signaling pathway inhibitor Dickkopf-1 (DKK1) in the secretion of MUC5AC in mouse airway epithelial cells (MAECs) induced by P1-C. Scanning electron microscope and hematoxylin-eosin staining were used to observe the effect of P1-C on mucus secretion of MAECs. Protein chip was used to detect the secretion of cytokines and analyse the enrichment of related signaling pathways induced by P1-C in MAECs. Periodic acid schiff stain (PAS) staining, Tunel staining and Masson staining were used to detect the damage of the lungs of mouse exposed to P1-C. Immunohistochemistry was used to detect the secretion of MUC5AC expression, and Western blotting was used to reveal the molecular mechanism of DKK1-regulated secretion of MUC5AC induced by P1-C protein in MACES. The results showed that P1-C induced the massive secretion of mucus and inflammatory factors in MAECs. During P1-C infection, DKK1 down-regulated janus kinase 2 (JAK2), phosphorylation signaling and transcription activator 1 (p-STAT1) and phosphorylation signaling and activator of transcription 3 (p-STAT3) expression. Overexpression of DKK1 significantly up-regulated the expression of MUC5AC repressor transcription factor fork-head box protein A2 (FOXA2). At the same time, the expression of MUC5AC induced by P1-C was inhibited significantly. It is speculated that DKK1 can effectively reduce the secretion of MUC5AC in MAECs induced by P1-C by inhibiting the JAK/STAT1-STAT3 signaling pathway and up-regulating the expression of FOXA2.

4.Mitochondrial Function Reveals Connotation of "Seven-Seven" Theory of Huangdi's Internal Classic and Mechanism of Herbal Intervention for Elderly Women Undergoing IVF-ET

ObjectiveTo study the changes of mitochondrial function of ovarian granulosa cells in women of different ages and the effect of Erzhi-Tiangui prescription on in vitro fertilization-embryo transfer (IVF-ET) outcomes for elderly women, so as to verify the connotation of the "Seven-Seven" theory in the Huangdi's Internal Classic (《黄帝内经》). MethodA total of 150 infertility patients undergoing IVF-ET at the Reproductive and Genetic Center of Integrative Medicine, Affiliated Hospital of Shandong University of Traditional Chinese Medicine were recruited and assigned into "hree-Seven/Four-Seven (30 cases), Five-Seven (60 cases), and Six-Seven (60 cases) groups according to the "Seven-Seven" theory. The Five-Seven and Six-Seven groups were further assigned into control and Chinese medicine subgroups using the random number plus envelope method, and the Chinese medicine group was administrated with Erzhi Tiangui prescription from the start day of controlled ovulation stimulation cycle to the trigger day. The IVF outcome was observed, and Western blot was employed to determine the levels of mitofusin 1 (MFN1), mitofusin 2 (MFN2), and dynamin-related protein 1 (Drp1) in the ovarian granulosa cells. ResultCompared with the Three-Seven/Four-Seven group, the control subgroups of the Five-Seven and Six-Seven groups showed decreased retrieved oocytes, two pronuclear (2PN) embryos, available embryos, high-quality embryos, clinical pregnancy rate, and live birth rate (P<0.05). Moreover, the control subgroup of the Six-Seven group showed decreased fresh embryo transfer rate(P<0.05). Compared with the control subgroup of the Five-Seven group, that of the Six-Seven group showed reduced retrieved oocytes, 2PN embryos, available embryos, high-quality embryos, and clinical pregnancy rate (P<0.05). The Chinese medicine subgroup had more retrieved oocytes, 2PN oocytes, and available embryos than the control subgroup in the Five-Seven groups (P<0.05). The Chinese medicine subgroup had more retrieved oocytes, than the control subgroup in the Six-Seven groups (P<0.05). The control subgroup of the Six-Seven group showed lower expression levels of Mfn1 and Mfn2 and higher level of Drp1 than the control subgroup of the Five-Seven group (P<0.05), which indicated that the levels of Mfn1 and Mfn2 in ovarian granulosa cells were down-regulated while the expression of Drp1 was up-regulated with aging (P<0.05). The Chinese medicine subgroup had higher Mfn2 level and lower Drp1 level than the control subgroup in the Five-Seven group (P<0.05), and the Chinese medicine subgroup had higher Mfn1 and Mfn2 levels and lower Drp1 level than then control subgroup in the Six-Seven group (P<0.05). ConclusionsThe prognosis of IVF in women after "Five-Seven" became worse with aging, and the mitochondria in ovarian granulosa cells showed decreased fusion ability and increased fission, which verified the connotation of the "Seven-Seven" theory from the mitochondrial function. Erzhi Tiangui prescription can regulate the mitochondrial function of ovarian granulosa cells in elderly women, up-regulate the expression levels of Mfn1 and Mfn2 to promote mitochondrial fusion, and down-regulate the expression of Drp1 to reduce mitochondrial fission, thus alleviating the ovarian hypofunction caused by aging, improve the development potential of oocytes, and improve the IVF outcomes of elderly women. However, this prescription has limited efficacy for the elderly women in the age range of "Six-Seven".
5.Construction of tunnel-type PICC catheterization procedure guided by ultrasound and intracavitary electrocardiogram positioning

Objective:To construct the tunnel-type peripherally inserted central catheter (PICC) catheterization procedure guided by ultrasound and intracavitary electrocardiogram positioning, so as to provide guidance for clinical practice.Methods:From May to July 2021, the first draft of the tunnel-type PICC catheterization procedure guided by ultrasound and intracavitary electrocardiogram positioning was formed through literature review and expert meeting. The convenience sampling method was used to select 18 experts from 8 provinces/municipalities across the country to conduct Delphi expert consultation, solicit expert opinions, and then discuss in groups to determine the final draft of the tunnel-type PICC catheterization procedure. The degree of enthusiasm, authority, and coordination of opinions of experts were expressed by the effective recovery rate of the questionnaire, the ratio of experts who put forward opinions, the expert authority coefficient, Kendall coordination coefficient, and the coefficient of variation.Results:A total of two rounds of expert consultations were carried out. The effective recovery rates of the questionnaires were 90.00% and 100.00%, respectively. The ratio of experts who proposed revisions were 83.33% and 5.56%, respectively, and the expert authority coefficients were both 0.90, Kendall coordination coefficients were from 0.112 to 0.170 ( P<0.05) , and the coefficient of variation was 0 to 0.122. In the second round, the mean score of importance of each level of operation steps was 4.64 to 5.00. The final constructed tunnel-type PICC catheterization procedure guided by ultrasound and intracavitary electrocardiogram positioning includes 4 first-level operation steps, 19 second-level operation steps and 39 third-level operation steps. Conclusions:The tunnel-type PICC catheterization procedure guided by ultrasound and intracavitary electrocardiogram positioning is reliable and of high quality, which has important guiding significance for clinical catheterization practice.
6.Eating behaviors related to brucellosis infection in Mongolian middle school students in Abaga Banner, Inner Mongolia

Objective:
To understand current situation and associated factors of brucellosis infection among students in Mongolian middle school in Abaga Banner, and to provide basis for further development of prevention and treatment strategies.
Methods:
Serological examinations were performed on pupils and middle school students in a Mongolian middle school in Abaga Banner. Questionnaire surveys were used to investigate students knowledge of eating behaviors related to brucellosis infection.
Results:
A total of 500 primary and middle school students were investigated, and 6 cases of brucellosis were confirmed, and the infection rate was 1.20%. There were 4 cases of male infection and 2 cases of female infection. The infection rate in boys (1.61%) was higher than that of girls (0.79%). Higher proportion was found in 14-year-old group (n=3), accounting for 17.40%. The residence period of 10-15 years includes 6 cases of all infected persons. The number of infections in Narenbaolige Town was 4, accounting for 23.40%. Univariate logistic regression analysis shows that frequent consumption of unsterilized dairy products, milking, processing lambs or slugs and infected cloth The disease was positively correlated (OR=11.80, 139.14, 8.02, P<0.05). Multivariate Logistic regression analysis showed that milking was positively correlated with brucellosis infection(OR=20.19, 95%CI=2.01-202.74, P=0.01).
Conclusion
The brucellosis infection of Mongolian primary and middle school students in Abaga Banner is related to a variety of related dietary factors, and its prevention and treatment should be strengthened.
7.Relationship between serum folate and CIN1 prognosis and its interaction with HR-HPV infection

Objective:To evaluate the relationship between serum folate and the prognosis of cervical intraepithelial neoplasia grade I (CIN1) and the interaction between folate and high risk human papillomavirus (HR-HPV) infection.Methods:From a community-based married women cohort established in Jiexiu and Yangqu County of Shanxi Province from June to December 2014, a total of 564 eligible women with CIN1 by pathologically diagnosed were recruited. The pathological examination was performed again 12 months later. According to the prognosis of CIN1, participants were divided into CIN1 regression group, persistence and progression group, respectively. Nested case-control study was used to explore the relationship between serum folate and CIN1 prognosis, and additive model was used to analyze the interaction between serum folate and HR-HPV infection.Results:Among 564 CIN1 patients, 479 cases underwent pathological examination again, 331 were divided in CIN1 regression group and other 148 in persistence and progression group. The levels of serum folate in CIN1 regression group and persistence and progression group were (18.890±8.360) and (15.640±5.550) nmol/L, respectively, and the difference was statistically significant ( Z=-6.937, P<0.001). HPV infection was detected in 154 patients, including 148 cases of HR-HPV infection and 6 cases of low risk human papillomavirus (LR-HPV) infection. Univariate analysis showed that there were significant differences in the age, passive smoking, frequency of pudendal cleaning, frequency of cleaning after sex, frequency of changing underwear, serum folate and HR-HPV infection between regression group and persistence and progression group ( P<0.05). Multivariate logistic regression analysis showed that the frequency of pudendal cleaning ( OR=0.422, 95% CI: 0.238-0.750), frequency of changing underwear ( OR=0.574, 95% CI: 0.355-0.928), serum folate (13.06-16.78nmol/L: OR=4.806, 95% CI: 2.355-9.810; ≤13.05nmol/L: OR=8.378, 95% CI: 4.024-17.445), HR-HPV infection ( OR=1.852, 95% CI: 1.170-2.933) were the independent influencing factors of CIN1 prognosis. Interaction analysis showed that the relative excess risk of low serum folate level and HR-HPV infection for the CIN1 persistence and progression was 4.992 (95% CI: 0.189-9.796), attributable proportion due to interaction was 0.552 (95% CI: 0.279-0.824), synergy index was 2.632 (95% CI: 1.239-5.588), a OR of serum folate≤16.78 nmol/L and HR-HPV infection positive was 9.055 (95% CI: 4.878-16.807). Conclusion:Low serum folate level could increase the risk of CIN1 persistence and progression, and might enhance the risk when combined with HR-HPV infection.
8.Effect of dietary water-soluble vitamins on the poor prognosis of low-grade cervical intraepithelial neoplasia-a prospective cohort study

Objective:To explore the effect of dietary water-soluble vitamins on the poor prognosis of low-grade cervical intraepithelial neoplasia (CINⅠ).Methods:A total of 564 participants with CINⅠ were selected from a community-based married women cohort established in 2014. The general baseline information was collected, and HPV genotyping was detected. A food frequency questionnaire was used to investigate dietary water-soluble vitamins' consumption and determine their exposure levels. The follow-up program was carried out for 12 months, and the regression, persistence, and progression of CINⅠ was determined according to histopathological results at 12-month.Results:The low-levels of dietary water-soluble vitamins could promote CINⅠ's persistence and progression, showing the increasing tendency of CINⅠ poor prognosis with the decrease of water-soluble vitamin levels. Especially folate, VB1, VB2, VB6, niacin, biotin, and VC ( RR values were 15.22, 1.86, 1.92, 2.11, 2.98, 2.14, 2.19, respectively) played an important role. Considering the status of high-risk human papillomavirus (HR-HPV) infection, our results showed that the low levels of folate, niacin, biotin could promote the poor prognosis of CINⅠ. Conclusions:The low-level of dietary water-soluble vitamins could facilitate the poor prognosis of CINⅠ. In addition, folate, niacin, biotin were more effective on HR-HPV positive women.
9.Prognosis of high-risk HPV infection and its influences by vaginal micro-environmental factors

Objective:To describe the characteristics of high-risk human papillomavirus (HR-HPV) infection and its influences by vaginal micro-environmental factors.Methods:A total of 421 women with HR-HPV infection and normal cervix diagnosed by pathology were selected from a community cohort established earlier by our research group for a 24-month follow-up. The baseline data were collected at enrollment. The vaginal micro-environmental factors were detected by using the combined detection kit of aerobic vaginitis and bacterial vaginosis. And the HPV was detected by using the flow-through hybridization technology. According to the HR-HPV infection status in baseline and 24 months of follow-up, with the prognosis characteristics of HR-HPV infection described, the impact of vaginal micro-environmental factors on the prognosis of HR-HPV infection was explored as well.Results:Among 390 HR-HPV infected women who completed 24 months of follow-up, the top five types of persistent HR-HPV infection rate appeared as HPV16 (24.1%), HPV58 (22.2%), HPV53 (21.7%), HPV52 (20.0%), and HPV39 (11.8%), respectively. The results showed that women with abnormal vaginal pH (a OR=1.74, 95% CI: 1.08-2.80), abnormal neuraminidase (a OR=2.70, 95% CI: 1.52-4.83), or abnormal leucocyte esterase (a OR=3.41, 95% CI: 2.13-5.44), the risk of HR-HPV persistent infection increased. The abnormalities of neuraminidase and leukocyte esterase could increase the risk of persistent infection of homotype and heterotypic HR-HPV. Conclusions:HPV16 was prone to persistent infection. Abnormalities of vaginal pH, neuraminidase, and leukocyte esterase might increase the risk of HR-HPV persistent infection. Both the abnormalities of neuraminidase and leukocyte esterase play an essential role in the HR-HPV persistent infection.
10.The optimal model of diagnosis to low-grade cervical intraepithelial neoplasia by combined detecting vaginal micro-environmental factors, based on the high-risk HPV infection

Objective:To investigate the diagnostic value of different vaginal micro-environmental factors in low-grade cervical intraepithelial neoplasia (CIN Ⅰ) and determine the optimal model in high-risk human papillomavirus (HR-HPV) infection.Methods:A total of 926 women, including 623 with normal cervical (NC) condition and 303 CINⅠ patients, had undergone pathological examinations, and were enrolled in the study. All the women were from a community previously established cohort. Vaginal cleanliness, pH, H 2O 2, β-glucuronidase, coagulase, sialidase, and leukocyte esterase (LE) were detected by the combined detection method aerobic vaginitis/bacterial vaginosis in vaginal secretions. HPV genotyping was performed by using the flow-through hybridization technology. The data were analyzed by SAS 9.2 and SPSS 23.0. Results:The vaginal cleanliness, pH, sialidase, and LE were determined as the representative vaginal micro-environment factors by principal component analysis. Based on logistic regression theory to analyze the ROC curve, the results showed that the highest sensitivity was with pH value (76.2%), and the highest specificity was with sialidase (90.9%). The area under ROC curve were higher in combination detection modes of sialidase+LE (0.714), pH+sialidase+LE (0.719), vaginal cleanness+sialidase+LE (0.713) and pH+vaginal cleanness+sialidase+LE (0.709). According to HR-HPV infection status, the TOPSIS method was used to analyze the combined detection optimal model. Specifically, we found that the best diagnostic model was pH+sialidase +LE ( C i=0.585) in the HR-HPV positive group and vaginal cleanness+sialidase+LE ( C i=0.641) in the negative group. Conclusions:The combined detection of vaginal microenvironment factors could be used for auxiliary diagnosis for CINⅠ. It would be more effective when detecting pH, sialidase, and LE in HR-HPV positive women while vaginal cleanness, sialidase, and LE in HR-HPV negative women at the same time.
