1.Spatiotemporal clustering analysis and ARIMA-SVM model prediction of hand-foot-mouth disease in Zibo City in 2018- 2023

Objective To analyze the spatial and temporal distribution characteristics of hand-foot-mouth disease (HFMD) in Zibo City, to explore the key incidence areas, and to find a suitable prediction model, so as to provide reference for the prevention and control of HFMD. Methods The spatiotemporal clustering characteristics of HFMD in Zibo City from 2018 to 2023 were analyzed by using SaTScan 10.0.2 software and ArcGIS 10.7 software. A combination model of ARIMA and SVM was established, and the prediction results were verified and compared. Results Spatial clustering analysis showed that there was spatial clustering of the incidence of HFMD in various townships of Zibo City from 2020 to 2022. The high-high clustering areas and Getis-Ord hot spot areas were mainly concentrated in some main urban areas of Zhangdian District, Zichuan District, and Huantai County. A total of 2-5 aggregation areas were detected by spatiotemporal scanning analysis. The first-type aggregation areas were mainly concentrated in the towns of Zhangdian District, Huantai County, Linzi District, Zhoucun District and Gaoxin District. The aggregation months were July, August, September and November. The model prediction results showed that the ARIMA-SVM combined model was more accurate than the traditional ARIMA model. Conclusion There is a spatiotemporal clustering of hand-foot-mouth disease in Zibo City. The ARIMA-SVM combined model can be used to predict the incidence of hand-foot-mouth disease in Zibo City, and to strengthen health education and disease monitoring in high-risk areas and populations during the epidemic months.
2.Methodological correlation and efficacy analysis of the detection of hepatitis B virus surface antigen in clinical laboratory

Objective:To explore the evaluation and relationship of hepatitis B virus(HBV)surface antigen(HBsAg)in clinical laboratory in different detection systems,further scientifically and reasonably to explain the test results for serving clinical practices.Methods:During the period from June 2021 to July 2022,100 425 specimens of patients with screened,suspected and confirmed HBV infections were collected from the clinical departments(mainly infectious hepatology)of Nanyang Central Hospital.Detection methodology included quantitative(electrochemiluminescence and chemiluminescence),qualitative(gold standard),semi-quantita-tive(ELISA),and highly sensitive HBV-DNA(RT-PCR)methods,and then analyzed the strengths and weaknesses and closeness be-tween each methodology.The relationship between the two Roche HBsAg detection systems was analyzed by correlation analysis.The HBsAg efficacy analysis was validated using Cut/Off value setting and detection limit,which in turn analyzed the distribution of false-positive and false-negative reporting models.Results:Detection results for low-and medium-concentration HBsAg showed a correla-tion between the electrochemical luminescence semi-quantitative method and ELISA method.ELISA method still had advantages in terms of sensitivity and specificity when detecting HBsAg,and there were no significant differences compared to domestic and interna-tional HBsAg quantitative detection systems.Performance validation conducted in accordance with the CNAS-GL038 document showed that the minimum detection limit for HBsAg calculated using the ELISA method in this laboratory was 0.1 U/ml.When the ROC curve Cut/Off value was set to 0.105,the area under the curve was the largest(AUC=0.986).Based on Roche's semi-quantitative electroche-miluminescence detection and patient medical history,in the common reporting model for hepatitis B five-item detection using ELISA method,HBsAg false positives occurred most frequently when HBsAg was positive alone,and HBsAg occurred most frequently in the false positive range when the OD value was less than 0.5.In ELISA method for detecting HBsAg,as the OD value increased from 0.01 to 0.10,the number of false-negative results also increased.Roche Elecsys HBsAg Ⅱ Quant Ⅱ and Elecsys HBsAg Ⅱ testing systems exhibited good linearity under certain conditions,with a ratio of approximately 1/0.18.In Elecsys HBsAg Ⅱ Quant Ⅱ detection sys-tem,the test results of HBsAg samples diluted 400 times were highly consistent with the original test results with a coefficient of deter-mination R2=0.993 8.Conclusion:There was a certain relationship between various detection systems of HBsAg at a suitable concen-tration.The detection of HBsAg by ELISA can meet the needs of clinical detection.
3.Multi-center study on the difficulty and discrimination of the ICU Nurse Pressure Injury Prevention and Care Knowledge Questionnaire

Objective:To test the difficulty, discrimination, and reliability of the ICU Nurse Pressure Injury Prevention and Care Knowledge Questionnaire.Methods:Two researchers independently translated the ICU Nurse Pressure Injury Prevention and Care Knowledge Questionnaire and cross-checked it to form a Chinese version of the questionnaire. The Chinese version of the ICU Nurse Pressure Injury Prevention and Care Knowledge Questionnaire consists of 24 items, with correct answers scoring one point and incorrect answers scoring zero points, with a total score of 24 points. Convenience sampling was used to select ICU nurses from 14 GradeⅢ Class A hospitals in five provinces/autonomous regions and two municipalities in China for the survey between April and July 2023. The difficulty index, discrimination index, and Cronbach's α coefficient of the questionnaire were analyzed.Results:A total of 1 121 questionnaires were distributed, with 1 020 valid responses, yielding a valid response rate of 90.99%. The mean score of the 1 020 ICU nurses on the Chinese version of the ICU Nurse Pressure Injury Prevention and Care Knowledge Questionnaire was (16.10±5.58) , with a minimum score of 4.00 and a maximum score of 24.00. The Cronbach's α coefficient of the Chinese version of the ICU Nurse Pressure Injury Prevention and Care Knowledge Questionnaire was 0.91. The questionnaire's overall difficulty and discrimination indexes were 0.67 and 0.59, respectively.Conclusions:The Chinese version of the ICU Nurse Pressure Injury Prevention and Care Knowledge Questionnaire has appropriate difficulty, moderate discrimination, and strong reliability, making it a valuable tool for assessing ICU nurses' knowledge of pressure injury-related topics.
4.Impacts of dietary diary combined with path based eating management on gastrointestinal function and dietary health behavior in postoperative patients with laparoscopic distal gastric cancer

Objective To explore the impacts of dietary diary combined with path based eating management on gastrointestinal function and dietary health behavior in postoperative patients with laparoscopic distal gastric cancer.Methods From January 2021 to January 2023,150 patients who underwent laparoscopic distal gastric cancer surgery were randomly divided into two groups.The control group received routine dietary management intervention,while the observation group received dietary diary combined with path based eating management intervention.The gastrointestinal function and dietary health behavior of the two groups were compared.Results The excellent and good rate of gastrointestinal function recovery was 57.33%in the observation group,which was higher than 47.33%in the control group 3 d after operation(P<0.05);One month after surgery and three months after discharge,the scores of dietary health behaviors in the observation group were higher than those in the control group(P<0.05);One month after discharge and three months after discharge,the serum albumin level in observation group was(37.68±5.30)g/L,(38.25±4.82)g/L,and the hemoglobin level was(120.08±20.02)g/L,(124.62±24.23)g/L,which was higher than that in control group[1 month after discharge:(30.32±4.17)g/L,(32.99±4.41)g/L;3 months after discharge:(110.75±16.12)g/L,(115.34±18.44)g/L](P<0.05);The complication rate of the observation group was lower than that of the control group(P<0.05).Conclusion Dietary diary combined with path based eating management can promote the recovery of gastrointestinal function and improve dietary health behavior in postoperative patients with laparoscopic distal gastric cancer.
5.Research and application of a new deep learning based strategy for platelet histogram review

Objective:To develop an artificial intelligence (AI)-based platelet review strategy to identify abnormal platelet histograms with no significant difference between initial impedance platelet count (PLT-I) and PLT-F results.Methods:This study included 5 119 routine blood analysis in Nanfang Hospital of Southern Medical University and its Ganzhou branch from July 2023 and March 2024. Specimens exhibiting abnormal platelet histograms and an initial platelet count >40×10?/L underwent review using the fluorescent platelet count (PLT-F) channel. Consistency of the results was defined as a difference between impedance platelet count (PLT-I) and PLT-F less than ±20% of the PLT-F results. A deep learning model was developed using platelet and red blood cell histogram data from a training set of 3 807 specimens. The model′s diagnostic performance was evaluated on an independent external validation set ( n=805) using receiver operating characteristic (ROC) curve analysis. Changes in the number of reviewed samples and sample turnaround time were analyzed to assess its clinical utility. Results:The deep learning model based on platelet and red blood cell histograms achieved an area under the ROC curve (AUC) of 0.854 in the training set. At a cutoff value of 0.1, the sensitivity was 0.954 and specificity was 0.358. The model could reduce review by 16.80% (190/1 131). In the validation set, the AUC was 0.805, with a sensitivity of 0.955 and specificity of 0.307, corresponding to a reduction of 17.41% (47/270) in reviewed specimens.Conclusion:The platelet review prediction model developed based on deep learning technology can efficiently identify samples with consistent results before and after review, reducing unnecessary reviews and shortening specimen testing time, thereby improving the efficiency of platelet test.
6.Clinical laboratory testing and molecular epidemiological characterization of influenza A virus in a hospital in Henan Province from 2016-2024

Objective:To investigate the methodological differences in the detection, the inflammatory markers and the pathogenic epidemiological characteristics of influenza A virus in clinical laboratories, in order to provide more diagnostic and epidemiological data for diagnosis and prevention for children with influenza A.Methods:A retrospective cross-sectional study was conducted to collect 96 731 patients with suspected influenza A from January 2016 to October 2024 in Nanyang City Center Hospital from the Clinical Laboratory Testing Information System, including 5 731 patients with confirmed influenza A, aged 5.2 (2.8, 43.7) years old. We analyzed the distribution of influenza A patients from age and mixed infections, the relationship between patient age and positive detection rate by restricted cubic spline (RCS), analyzed differences in testing methods used Kappa consistency testing and receiver operating characteristic (ROC) curves, established a model of inflammatory markers by logistic regression, as well as developed a prediction model and also the mutation of the hemagglutinin (HA) sequence of the influenza A subtype H3N2 virus using evolutionary tree analysis.Results:RCS analysis showed an inverted 'S' shaped non-linear relationship between the positive detection rate of influenza A and the age groups of the patients. Among the mixed infections, 1.43%(1 352/94 867) of the cases were combined with Mycoplasma pneumoniae infection. The Kappa values of reverse transcription PCR (RT-PCR) and serological indirect immunofluorescence assay (IFA) for detecting influenza A in nasopharyngeal swabs and alveolar lavage fluid in clinical laboratories were 0.632 and 0.809, respectively, and those of magnetic particle chemiluminescence assay were 0.614 and 0.668, respectively, and the area under curves in ROC curve of IFA and RT-PCR were 0.869 and 0.792, respectively. The inflammatory indexes were usually elevated in severe children compared with mild children. By binary logistic regression model analysis, neutrophil-to-lymphocyte ratio, D-dimer/fibrinogen and prognosis nutrition index were the risk factors and serum amyloid A/C reactive protein ratio was the protective factor for severe children with influenza A, and the OR values of the above factors were 1.760, 7.076, 1.045, and 0.719, respectively, and P<0.01. By the Bayesian Interdiction Criterion, the optimal seasonal autoregressive moving average mixed model for influenza A epidemics was ARIMA (1, 1, 1) (2, 1, 2) 12 with the highest prediction accuracy of 98.63%. The seven strains of H3N2 all belonged to the same isoforms, with nucleotide similarity of the HA gene ranging from 99.5% to 99.9%, and the glycosylation site, receptor-binding site, and the conserved amino acid residue Glycosylation sites, receptor binding sites and conserved amino acid residues remained unchanged. HA sequence analysis showed that the prevalent strains in Nanyang had undergone mutation to different degree compared with the vaccine strains. Conclusion:Scientific and rational testing and characteristic inflammatory markers in the clinical laboratory are of great clinical value in the diagnosis of children with severe influenza A. At the same time, the epidemiological monitoring of influenza A variants should be strengthened.
7.Application of ultra micro angiography in assessment of activity of Crohn's disease in pediatric patients

Objective:To investigate the value of ultra micro angiography(UMA)imaging in evaluating the inflammation activity of pediatric Crohn's disease(CD)patients.Methods:In this prospective study,71 pediatric CD patients confirmed by clinical symptoms,laboratory and ileocolonoscopical results who planed to accept infliximab injection treatments in Xinhua Hospital Affiliated to Shanghai Jiaotong University School of Medicine from April to December 2024 were included. The CD patients were included and divided into 3 groups according to the pediatric Crohn's disease activity index(PCDAI)score:inactive CD group( n=44),mild CD group( n=17),and moderate-to-severe CD group( n=10). Before treatment,pediatric CD patients underwent B mode ultrasound(BMUS),the parameters such as bowel wall thickness(BWT),bowel wall stratification,mesenteric lymph nodes(MLNs),and existence of mesenteric fat hypertrophy(MFH)were recorded. Color Doppler flow imaging(CDFI)and UMA were used to evaluate the blood flow in the most thickened diseased bowel wall using Limberg scores. The diffierences of the above parameters among the 3 groups were compared. The diagnostic performance of UMA and CDFI in assessing the inflammatory activity of pediatric CD was evaluated. Results:There were statistically significant differences in BWT among the 3 groups(all P<0.05). However,there were no statistically significant differences in MLNs among the 3 groups(all P>0.05). Compared with the inactive CD,there were statistically significant differences in bowel wall stratification and MFH in both the mild and moderate-to-severe CD groups(all P<0.05). When assessing bowel wall blood flow signals using CDFI,there was no statistically significant difference in blood flow signals in the bowel wall of the mild CD compared with the inactive CD( P>0.05). However,the blood flow signals in the bowel wall of the moderate-to-severe CD was significantly increased( P<0.05). There was also statistically significant difference in blood flow signals in the bowel wall between the mild and moderate-to-severe CD groups( P<0.05). When assessing bowel wall blood flow using UMA,there were statistically significant differences in blood flow signals in the bowel wall among all 3 groups(all P<0.05). As the degree of inflammation increased,the blood flow signals in the bowel wall also increased significantly. ROC analysis revealed that diagnostic performance of the UMA-BMUS model was significantly better than that of the CDFI-BMUS model(AUC 0.945 vs. 0.873,sensitivity 88.9% vs. 74.1%,specificity 97.7% vs. 95.5%;all P<0.05). Conclusions:By sensitively depicting the low-velocity and tiny blood flow signals of the inflamed bowel wall,UMA is an effective noninvasive imaging method to make semi-quantitative evaluation of the activity of pediatric Crohn's disease.

10.Mitophagy in cardiovascular diseases

Cardiovascular diseases(CVD)remain a leading cause of mortality worldwide,with complex pathogenic mechanisms.Mitochondrial dysfunction,a key hallmark in CVD pathology,disrupts cellular homeostasis through oxidative stress,calcium imbalance,and reduced ATP production,ultimately damaging cardiomyocytes.Mitopha-gy,a selective autophagy process,plays a crucial protective role by degrading dysfunctional mitochondria,thus maintain-ing mitochondrial quality and quantity balance in cells.Recent studies have highlighted the significant regulatory role of mitophagy in various CVD pathological processes,including heart failure,myocardial infarction,ischemia/reperfusion in-jury,hypertension,and atherosclerosis.Notably,a variety of drugs have been shown good potential in the targeted regula-tion of mitophagy activity,which provides a new idea for the treatment of CVD.This review systematically elucidates the latest advances in understanding the molecular mechanisms,pathophysiological functions,and potential therapeutic appli-cations of mitophagy in CVD.It provides a theoretical foundation for deepening our comprehension of CVD pathogenesis and developing innovative treatment strategies,offering new insights into cardiovascular disease management.
