1.Prevalence of hypertension, blood pressure control and influencing factors in residents aged 35-75 years in Jiangsu Province

Objective:To understand the prevalence, blood pressure control, and influencing factors of hypertension in residents aged 35-75 years in Jiangsu Province, and provide data support and basis for hypertension prevention and treatment.Methods:A stratified cluster sampling method was adopted to conduct community population surveys in 22 districts and counties in 12 prefectures in Jiangsu from 2021 to 2022. A total of 123 531 permanent residents aged 35-75 years were included in the study. The questionnaire survey collected the information about the demographic characteristics, disease history, lifestyle, and control of hypertension of study subjects. Statistical software SPSS 23.0 was used for χ2 test and multivariate logistic regression analysis, and Joinpoint 4.9.1.0 software was used to evaluate the trend of age change. Results:The standardized prevalence of hypertension in study subjects was 46.60%, the prevalence was higher in men than in women, and in rural residents than in urban residents, and the standardized blood pressure control rate was 11.24%, and it was lower in men than in women and in rural residents than in urban residents. The hypertension prevalence and control rates showed increasing trends with age (both P<0.001). The multivariate analysis results showed that being man, older age, being rural resident, higher frequency of alcohol consumption, marital status of being separated and widowed, overweight and obese, abdominal obesity and stroke, diabetes mellitus and dyslipidaemic diseases histories, and family history of hypertension were risk factors for hypertension, and being man, being rural resident, living north area, higher frequency of alcohol consumption, and obesity were risk factors for blood pressure control. Conclusions:The prevalence of hypertension in residents aged 35-75 years in Jiangsu was high, and the rate of blood pressure control was low. It is suggested to take integrated intervention measures, especially in in rural residents and people with low levels of education to reduce the disease burden of the patients.
2.A novel TNKS/USP25 inhibitor blocks the Wnt pathway to overcome multi-drug resistance in TNKS-overexpressing colorectal cancer.

Modulating Tankyrases (TNKS), interactions with USP25 to promote TNKS degradation, rather than inhibiting their enzymatic activities, is emerging as an alternative/specific approach to inhibit the Wnt/β-catenin pathway. Here, we identified UAT-B, a novel neoantimycin analog isolated from Streptomyces conglobatus, as a small-molecule inhibitor of TNKS-USP25 protein-protein interaction (PPI) to overcome multi-drug resistance in colorectal cancer (CRC). The disruption of TNKS-USP25 complex formation by UAT-B led to a significant decrease in TNKS levels, triggering cell apoptosis through modulation of the Wnt/β-catenin pathway. Importantly, UAT-B successfully inhibited the CRC cells growth that harbored high TNKS levels, as demonstrated in various in vitro and in vivo studies utilizing cell line-based and patient-derived xenografts, as well as APCmin/+ spontaneous CRC models. Collectively, these findings suggest that targeting the TNKS-USP25 PPI using a small-molecule inhibitor represents a compelling therapeutic strategy for CRC treatment, and UAT-B emerges as a promising candidate for further preclinical and clinical investigations.
3.Correlation between anxiety and intimacy in high-risk pregnant women

Objective:To explore the correlation between anxiety and intimacy in high-risk pregnant women.Methods:From November 2023 to March 2024, convenience sampling was used to select 257 high-risk pregnant women admitted to Department of Obstetrics of four ClassⅢ Grade A hospitals in Beijing as participants. The survey was conducted using the General Information Questionnaire, Locke-Wollance Marital Adjustment Test (LWMAT), and Self-Rating Anxiety Scale (SAS). Hierarchical linear regression was used to analyze the correlation between anxiety and intimacy in high-risk pregnant women.Results:Among 257 high-risk pregnant women, the SAS score was 30.00 (26.00, 35.00), the anxiety incidence rate was 3.9% (10/257), the LWMAT score was 129.00 (113.50, 141.00), and 227 (88.3%) pregnant women perceived good intimacy. Hierarchical linear regression analysis showed that intimacy was a factor affecting the anxiety of high-risk pregnant women ( P<0.05) . Conclusions:Intimacy can negatively predict anxiety in high-risk pregnant women. Medical and nursing staff should pay attention to the intimacy between high-risk pregnant women and their spouses, and fully leverage the important role of intimacy in improving the negative emotions of high-risk pregnant women.
4.Clinicopathological factors and clinical significance of No.12b lymph node metastasis in gastric antrum cancer

Objective:To investigate the clinicopathological factors and clinical significance of (micro)metastasis in No.12b lymph node in patients with gastric antrum cancer.Methods:This was a retrospective cohort study of data of 242 patients with gastric adenocarcinoma without distant metastasis, complete follow-up data, and no preoperative anti-tumor therapy or history of other malignancies. All study patients had undergone radical gastrectomy (at least D2 radical range) + No.12b lymph node dissection in the Department of Gastric Surgery of Liaoning Cancer Hospital from January 2007 to December 2012. Immunohistochemical staining with antibody CK8/18 was used to detect micrometastasis to lymph nodes. Patients with positive findings on hematoxylin and eosin stained specimens and/or CK8/18 positivity in No.12b lymph node were diagnosed as having No.12b (micro)metastasis and included in the No.12b positive group. All other patients were classified as 12b negative. We investigated the impact of No.12b (micro)metastasis by comparing the clinicopathological characteristics and recurrence free survival (RFS) of these two groups of patients and subjecting possible risk factors to statistical analysis.Results:Traditional hematoxylin-eosin staining showed that 15/242 patients were positive for No.12b lymph nodes and 227 were negative. A total of 241 negative No. 12b lymph nodes were detected. Immunohistochemical testing revealed that seven of these 241 No.12b lymph nodes (2.9%) were positive for micrometastasis. A further seven positive nodes were identified among the 227 nodes (3.1%) that had been evaluated as negative on hematoxylin–eosin-stained sections. Thus, 22 /242 patients' (9.1%) No.12b nodes were positive for micrometastases, the remaining 220 (90.9%) being negative. Factor analysis showed that No.12b lymph node (micro) metastasis is associated with more severe invasion of the gastric serosa (HR=3.873, 95%CI: 1.676-21.643, P=0.006), T3 stage (HR=1.615, 95%CI: 1.113-1.867, P=0.045), higher N stage (HR=1.768, 95%CI: 1.187-5.654, P=0.019), phase III of TNM stage (HR=2.129, 95%CI: 1.102-3.475, P=0.046), and lymph node metastasis in the No.1/No.8a/No.12a groups (HR=0.451, 95%CI: 0.121-0.552, P=0.035; HR=0.645, 95%CI:0.071-0.886, P=0.032; HR=1.512, 95%CI: 1.381-2.100, P=0.029, respectively). Survival analysis showed that the 5-year RFS of patients in the No.12b positive group was worse than that of those in the No.12b negative group (18.2% vs. 34.5%, P<0.001). Independent predictors of RFS were poorer differentiation of the primary tumor (HR=0.528, 95%CI:0.288-0.969, P=0.039), more severe serous invasion (HR=1.262, 95%CI:1.039-1.534, P=0.019), higher T/N/TNM stage (HR=4.880, 95%CI: 1.909-12.476, P<0.001; HR=2.332, 95%CI: 1.640-3.317, P<0.001; HR=0.139, 95%CI: 0.027-0.713, P=0.018, respectively), and lymph node metastasis in the No.12a/No.12b group(HR=0.698, 95%CI:0.518-0.941, P=0.018; HR=0.341, 95%CI:0.154-0.758, P=0.008, respectively). Conclusion:Detection of micrometastasis can improve the rate of positive lymph nodes. In patients with gastric antrum cancer, dissection of group No.12b lymph nodes may improve the prognosis of those with intraoperative evidence of tumor invasion into the serosa, more than two lymph node metastases, and suspicious lymph nodes in groups No.1 / No.8a / 12a.

6.The clinical application value of semiconductor laser combined with flat-panel CT in guiding foramen ovale puncture

Objective To explore the clinical application value of semiconductor laser combined with flat-panel CT guidance in foramen ovale puncture for trigeminal neuralgia(TN).Methods A total of 86 patients with primary TN were selected and randomly divided into control group(n=43)and observation group(n=43).The control group used empirical manual puncture,and the observation group used semiconductor laser combined with flat-panel CT-guided radiofrequency ablation.The first puncture success rate,puncture success time,total operation time,and intraoperative flat-panel CT scan times were carefully evaluated.Results The puncture success time,total operation time,and intraoperative flat-panel CT scan times in the observation group were significantly lower than those in the control group(P<0.05).In addition,the first puncture success rate and surgical efficiency 12 months after surgery were significantly improved(P<0.05).The incidence of facial congestion in the observation group was significantly lower than that in the control group(P<0.05),but there was no statistically significant difference in the incidence of chewing weakness,cerebrospinal fluid leakage,and intracranial infection between the two groups(P>0.05).Conclusion Minimally invasive foramen ovale puncture under the guidance of semiconductor laser combined with flat-panel CT for the treatment of TN has the advantages of simple operation,accurate positioning,low risk,and few complications,with important clinical application value.
7.Analysis on the Group Characteristics of Highly Cited Chinese Researchers in the Medical Field

Purpose/Significance To probe into the characteristics of high-level talents in the medical field in recent years,and to provide references for formulating talent cultivation policies and effectively playing the role of scientific and technological talents in China.Method/Process Based on Elsevier's 2017-2021"Highly Cited Chinese Researchers"list,the distribution pattern and development trend of candidates in the medical field for 5 consecutive years are analyzed.Result/Conclusion The study shows that the candidates are mainly from universities and research institutes,which indicates that there is a positive relationship between scientific research achieve-ments and scientific research resource agglomeration.The number of people involved in clinical medicine and pharmacy has a significant advantage.In the past 5 years,most of the candidates have maintained high scientific research output and research activity.Based on this research,suggestions for the development of top talents in the medical field are proposed.
8.Effect of lysophosphatidic acid on hepatoma cells and related mechanism

ObjectiveTo investigate the expression of lysophosphatidic acid (LPA) in patients with liver cancer, as well as its influence on malignant biological behavior of liver cancer and related regulatory mechanism. MethodsFrom January 2016 to December 2022, 26 patients with liver cancer, 28 patients with liver cirrhosis, and 28 individuals undergoing physical examination were enrolled. ELISIA was used to measure the content of LPA in plasma and peritoneal effusion of the patients with liver cancer or liver cirrhosis accompanied by peritoneal effusion, and the content of LPA was measured in plasma of the normal population at the same time, so as to clarify the difference in the expression of LPA in different populations, such as the patients with liver cancer and those with liver cirrhosis. MTT cell proliferation assay and cell migration assay were used to observe the influence of LPA and its inhibitor pertussis toxin (PTX) on the proliferation, migration, and invasion of SMMC7721 cells. In order to investigate the effect of LPA on the expression of RhoA and its upstream and downstream molecules FAK and P53 after binding to its receptor, qPCR and Western blot were used to observe the effect of LPA on the mRNA and protein expression levels of P53, FAK, and RhoA in SMMC7721 cells. A one-way analysis of variance was used for comparison of the means of continuous data between multiple groups, and the SNK-q test was used for comparison between two groups. ResultsCompared with the patients with liver cirrhosis, the patients with liver cancer had a significantly higher concentration of LPA in plasma (4.99±0.55 μmol/L vs 2.63±0.43 μmol/L, P<0.05) and peritoneal effusion (5.19±0.63 μmol/L vs 2.91±0.46 μmol/L, P<0.05), and the patients with liver cancer also had a significantly higher level of plasma LPA than the normal population (4.99±0.55 μmol/L vs 1.61±0.39 μmol/L, P<0.05). The cell proliferation assay showed that LPA significantly promoted the proliferation of SMMC7721 cells, and cell proliferation rate increased with the increase in dose and time; in particular, the middle-and high-dose groups had a significantly higher proliferation rate than the control group (P<0.05). PTX inhibited the proliferative capacity of SMMC7721 cells in a time-dependent manner, and there was a significant difference between the groups (P<0.05). The proliferation rate of the 72-hour high-dose LPA group was 3.6 times that of the control group, while the proliferation rate of the PTX group was 0.6 times that of the control group; the proliferation rate of the 72-hour high-dose LPA+PTX group was 1.2 times that of the control group. In addition, LPA increased the migration ability of hepatoma cells, while PTX inhibited their migration, in a time-dependent manner, and there was a significant difference between the groups (P<0.05). The migration rate of the 72-hour high-dose LPA group was 3.09 times that of the control group, while the migration rate of the PTX group was 0.4 times that of the control group; the migration rate of the 72-hour high-dose LPA+PTX group was 0.99 times that of the control group. qPCR and Western blot showed that there were significant reductions in the mRNA and protein expression levels of P53 in SMMC7721 cells after LPA treatment, while there were significant increases in the mRNA and protein expression levels of FAK and RhoA; there was a significant difference between the LPA group and the control group (P<0.05). ConclusionThere is an abnormal increase in the expression of LPA in patients with liver cancer, and LPA can promote the proliferation of liver cancer cells and increase their migration ability. At the same time, LPA changes the expression levels of P53, FAK, and RhoA, which may be associated with the promotion of tumor development and progression by LPA.
9.Application value of CT and MRI radiomics based on machine-learning method in diagnosing pancreatic cancer

Objective:To investigate the application value of CT and MRI imageomics based on machine learning method in the diagnosis of pancreatic cancer.Methods:The clinical data of 62 patients with surgically resected and pathologically confirmed pancreatic cancer, who underwent enhanced CT scan, MRI plain or enhanced scan in Shanghai General Hospital between January 2014 and December 2021 were collected. According to the chronological order of surgery, 49 patients from January 2014 to December 2020 were enrolled in the training set and 13 patients from January 2021 to December 2021 were enrolled in the validation set. 3D-slicer 4.8.1 software was used to draw the region of interest in each layer of CT and MRI images for cancerous and paracancerous tissue segment. Image features were extracted by Python and the optimal feature set from the training set data was obtained by using Lasso regression model. The machine learning decision tree model was constructed. The receiver operating characteristic curve(ROC) curve was drawn, and the area under the curve (AUC) was calculated to evaluate the value of these three kinds of imageomics models in the diagnosis of pancreatic cancer.Results:The 1 767 CT features and 1 674 MRI features were obtained from enhanced CT scan, MRI plain scan and enhanced MRI scan, respectively. For the differential diagnosis model of cancerous tissue and paracancerous tissue, the enhanced CT scan data model obtained the optimal feature set involving 6 features, the MRI plain scan model obtained the optimal feature set involving 16 features, and the enhanced MRI scan model obtained the optimal feature set involving 15 features. The diagnostic model based on enhanced CT scan had an AUC of 0.98 in the training set and 1 in the verification group. The AUC of the MRI plain scan and enhanced MRI scan models in both the training set and the validation set was 1. The specificity and sensitivity of machine learning decision tree model based on the three kinds of imageomics models in the diagnosis of cancerous tissue and paracancerous tissue were 100%. For the differential diagnosis model of splenic artery wrapping, the enhanced CT scan model didn′t obtain the optimal features and had no diagnostic efficacy. The MRI plain scan model and enhanced MRI scan model obtained the optimal feature set involving 5 and 4 features, respectively. The AUC of the MRI plain scan model in the training set and the validation set were 0.862 and 0.750, respectively, with diagnostic sensitivity of 93.8% and 50.0%, and specificity of 78.6% and 100%, respectively. The AUC of the enhanced MRI scan model in the training set and the validation set were 0.950 and 0.861, respectively, with diagnostic sensitivity of 90.0% and 93.6%, and specificity of 100% and 78.6%, respectively.Conclusions:Based on the radiomics of CT enhanced, MRI plain scan and enhanced MRI scan, the machine learning diagnostic model has an accuracy of more than 90% in differentiating pancreatic cancer from paracancerous tissue. For the differentiation of splenic artery wrapping in pancreatic cancer, the diagnostic model based on enhanced MRI scan haS the best diagnostic efficiency.
10.Treatment strategy and mechanical exploration of early lumbar spondylolysis

Lumbar spondylolysis is one of the common diseases of low back pain caused by spinal surgery. Its treatment options vary depending on different conditions, from early conservative ones to late surgical ones. There are still disputes over various conservative treatments, choice of surgical methods and the biomechanics of different internal fixation techniques to repair spondylolysis. Therefore, this review summarizes the clinical outcomes of previous clinical treatments of lumbar spondylolysis and the biomechanical characteristics of various techniques to find the mechanical and evidence-based clinical data that may facilitate the treatment of lumbar spondylolysis.
