1.Investigation and disposal of a suspected outbreak of hospital-acquired carbapenem-resistant Acinetobacter baumannii infection in an ICU

OBJECTIVE To study the prevention and control strategies for carbapenem-resistant Acinetobacter bau-mannii(CRAB)infection through inves-tigating an outbreak of CRAB infection in an intensive care unit(ICU),and provide a scientific basis for the prevention and control of such hospital-acquired infections.METHODS Epide-miological investigations were conducted on patients with CRAB infection in the ICU of a hospital from Jul.7 to Jul.29,2023,and microbial sampling,identification and drug sensitivity testing were conducted on suspected con-taminated environments and items.Targeted prevention and control measures were taken to control the outbreak.RESULTS Within a short period,8 patients in this hospital developed CRAB hospital-acquired infection,among whom,the drug resistance profiles of CRAB isolated from the specimens of 7 patients in bed A4,A14,B18,B19,B20,B21 and B22 were consistent.Through environmental hygiene monitoring,CRAB isolated from patient clothing,isolation gowns and medical staff uniforms matched the drug resistance profiles of the seven patient iso-lates.After taking targeted measures,no new CRAB infection cases occurred in Oct.,and CRAB was no longer i-solated from the environment and medical fabrics.CONCLUSIONS The suspected outbreak of CRAB infection may be related to the inadequate management and contamination of medical fabrics.Therefore,in addition to strictly implementing the routine prevention and control measures for multidrug-resistant bacteria,it is also crucial to strengthen the standardized management of medical fabrics for the prevention and control of hospital-acquired in-fections and outbreaks.
2.Application effects of continuous and intermittent suturing of blood vessels under microscope in the repair of limb vascular injuries

Objective:To explore the application effects of continuous and intermittent suturing of blood vessels under microscope in the repair of limb vascular injuries.Methods:A total of 110 patients with limb vascular injuries admitted to Kaifeng 155 Hospital from Feb 2023 to May 2024 were selected as the research subjects.According to the different methods of vascular suturing,they were divided into two groups,55 patients who underwent intermittent suturing of blood vessels under microscope were selected as the control group,while 55 patients who underwent continuous suturing of blood vessels under microscope were selected as the observation group.The perioperative indicators(arterial and venous suture time,blood circulation recovery time,hospitalization time,total blood loss),blood electrolyte levels(serum potassium ion,sodium ion,and plasma carbon dioxide binding capacity),and coagulation function indicators[prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT)]before surgery,2 h and 1 d after surgery,and the incidence of complications were compared between the two groups.Results:The perioperative related indicators in the observation group were better than those in the control group(P<0.05).The serum potassium ion,sodium ion,plasma carbon dioxide binding capacity,PT,APTT and TT in the two groups at 2 h and 1 d after operation were significantly higher than those before operation(P<0.05).And these indicators in the observation group at 2 h after surgery were significantly higher than those in the control group(P<0.05).However,there was no statistically significant difference in the levels of these indicators between the two groups before surgery and at 1 d after surgery(P>0.05).The incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).Conclusion:In the repair of limb vascular injuries,continuous suturing of blood vessels under microscope can effectively shorten the time of vascular suture,reduce blood loss,improve coagulation function and blood electrolyte level,promote postoperative recovery,improve limb motor function,and reduce complications.

5.Exploring the Mechanism of Acanthopanax Senticosus in the Treatment of Alzheimer’s Disease Based on GEO Data Mining and Network Pharmacology

OBJECTIVE To screen the potential drug targets and signaling pathways of Acanthopanax senticosus for the treatment of Alzheimer’s disease(AD) by bioinformatics and network pharmacology-based approach, and to preliminarily validate its efficacy. METHODS The ingredients of Acanthopanax senticosus were obtained through literature, the ingredients were screened by Swiss ADME, and potential targets were predicted by Swiss Target Prediction. AD’s differentially expressed genes were screened from the GSE28146 dataset. The target of Acanthopanax senticosus and AD target were mapped to construct a “drug-ingredients-potential target-disease” network and protein-protein interaction network. The DAVID database was used for GO and KEGG enrichment analysis. Autodock software was used to verify the molecular docking between key active ingredients and core targets. AD mice model was induced by D-galactose combined with aluminum chloride. Morris water maze test was performed to examine the learning memory ability of each group of mice and to observe the pathological changes in the hippocampus of mice. RESULTS Screened to obtain 24 active components and 74 potential targets of Acanthopanax senticosus for the treatment of AD. “Drug-ingredients-potential target-disease” network indicated that quercetin and kaempferol were the main components of Acanthopanax senticosus for the treatment of AD, and the protein-protein interaction network indicated that STAT3, MAPK1 and PIK3CA were the key targets. Obtained 366 GO enrichment entries(P<0.01) and 109 KEGG enrichment pathways(P<0.01). It mainly involved PI3K-AKT, AGE-RAGE, TNF and other pathways. The molecular docking results showed that the main active ingredients of Acanthopanax senticosus were able to bind well to the main targets. The in vivo pharmacological results showed that Acanthopanax senticosus could significantly improve the learning and memory ability of mice, reduce hippocampal tissue damage, and decrease the content of TNF-α, IL-6, and IL-1β in hippocampal tissue. CONCLUSION Acanthopanax senticosus may exert anti-AD effects by inhibiting the expression of inflammatory factors and reducing inflammatory damage.
6.Effect and mechanism of curcumin and resveratrol on macrophages of mice with ulcerative colitis

Objective:To explore the effects and related mechanisms of curcumin and resveratrol on macrophages of colon tissues in ulcerative colitis (UC) mice.Methods:Sixty BALB/c mice were randomly and equally divided into CON group, DSS group, DSS+Cur group, DSS+Res group, Eve+Cur group and Eve+Res group. There were 10 mice in each group. CON group was the normal control group and received no treatment. UC model of mice was established by giving 3% dextran sodium sulfate (DSS) to drink freely for 7 days in other 5 groups. UC model of mice was simply established in DSS group. The gavage administrations of 0.4 ml curcumin and 0.4 ml resveratrol were performed every day in the mice of DSS+Cur group and DSS+Res group respectively after establishing the UC model. The gavage administration of 0.4 ml mTOR inhibitor Everolimus was performed in Eve+Cur group after 0.4 ml curcumin gavage administration and also was performed in Eve+Res group after 0.4 ml resveratrol gavage administration every day. The mice were sacrificed after 7 days of administration. The macrophages of colon tissues were isolated and cultured in each group. The general conditions of the mice were observed and the DAI score was evaluated. In order to examine the effects of curcumin and resveratrol on macrophages, CCK-8 method was used to detect the proliferation and flow cytometry was used to detect the apoptosis of macrophages. Fluorescence quantitative PCR and Western blot were used to detect the mRNA and protein expressions of the autophagy-related factors LC3, Beclin-1 and Atg12. ELISA was used to detect the level of the inflammatory factors IL-6 and TNF-α in culture supernatants of macrophages. In order to explore the associated mechanism, ELISA was used to detect the influence of mTOR inhibitor Everolimus on IL-6 and TNF-α, fluorescence quantitative PCR and Western blot were used to detect the expressions of SIRT1/mTOR pathway-related factors mTOR and SIRT1 mRNA and protein.Results:No death was observed in all of the mice. Compared with CON group, the proliferative activity of macrophages was significantly higher, and the apoptosis rate decreased significantly, the mRNA and protein expressions of Atg12, Beclin-1 and LC3 increased significantly in DSS Group, DSS+Cur group and DSS+Res group (all P<0.05) . Compared with DSS group, the proliferative activity was significantly lower, the apoptosis rate increased obviously, the mRNA and protein expressions of Atg12, Beclin-1 and LC3 decreased significantly in DSS+Cur group and DSS+Res group (all P<0.05) . Compared with CON group, the DAI scores increased, the mRNA and protein expressions of mTOR and SIRT1 decreased significantly, the levels of IL-6 and TNF-α increased significantly in DSS group, DSS+Cur group, DSS+Res group, Eve+Cur group and Eve+Res group (all P<0.05) . Compared with DSS group, the DAI scores decreased, the mRNA and protein expressions of mTOR and SIRT1 increased significantly, the levels of IL-6 and TNF-α decreased significantly in DSS+Cur group and DSS+Res group (all P<0.05) . 8.Improvement effects of Tongfengning Capsule on hyperuricemia and renal function in mice

Objective:To discuss the influence of Tongfengning Capsule (TFN) in the levels of uricacid (UC),creatinine (Cr) and urea nitrogen (BUN) in mouse serum and the activities of the xanthine oxidase (XOD),adenosine deaminase (ADA) activity in the liver homogenate of the mice with hyperuricemia,and to observe the improvement effect of TFN on the pathological changes of liver tissue and to clarify its mechanisms.Methods:The models of mouse hyperuricemia were induced by yeast extract with potassium oxonate.Seventy mice were divided into blank control group,model group,low (200 mg · kg 1),medium (400 mg · kg-1) and high (800 mg · kg-1) doses of TFN groups,allopurinol positive drug control group (50 mg · kg-1),Tongfengshu (TFS,600 mg · kg-1) positive drug control group (n=10).The levels of UC,Cr,BUN in serum and the activities of XOD,ADA in homoggenate were detected and the histopathological changes of the kidney tissue of the mice were measured with HE staining.Results:Compared with blank control group,the levels of serum UC,Cr and BUN ofthe mice in model group were significantlyincreased (P<0.01),and the activities of XOD and ADA in liver tissue were also increased (P<0.01).Compared with model group,the levels of serum UC,Cr and BUN of the mice in positive drug control groups and different doses of TFN groups were decreased (P<0.01),and the activities of XOD and ADA in liver tissue were also decreased (P<0.05),especially in high dose of TFN group.Compared with model group,the pathologic changes such as renal glomerulus atrophy,renal interstitial fibrosis and expansion of renal tubule of the mice in positive drug control groups and high dose of TFN group were improved to a certain extent.Conclusion:TFN has improvement effcet on the hyperuricemia in the mice and its mechanism is related to the inhibition of uricogenesis and the promotion of UC excretion.
9.Laparoscopic breast-conserving surgery combined with radiofrequency ablation to treat the early-stage breast cancer

Objective To analyze the clinical efficacy of the laparoscopic breast-conserving surgery combined with radiofrequency ablation to treat the early-stage breast cancer.Methods We collected 55 patients diagnosed early-stage breast cancer in retrospect,which started from January 2014 to December 2016.Twenty-seven of them were performed the laparoscopic breast-conserving surgery combined with radiofrequency ablation while others went through laparoscopic breast-conserving surgery without radiofrequency ablation.Meanwhile,we adopted the student t-test and the chi-square test to compare results of two groups.More specific,the main indexes of this study are including the post-operative local recurrence,the incidence of fat liquefaction or the incision-infection,operation time,post-operative hospital stay and the hospitalization expense.Results The laparoscopic breast-conserving surgery combined with radiofrequency ablation group had low local-recurrence than the laparoscopic breastconserving surgery group (0 and 7.69%).Additionally,there were no statistical differences between two groups in the incidence of fat liquefaction.However,The laparoscopic breast-conserving surgery combined with radiofrequency ablation group had more hospitalization expense than the laparoscopic breast-conserving surgery group [(4.1 ± 0.7) ten thousand yuan and (2.3 ± 0.6) ten thousand yuan,P < 0.05].Conclusions Although the laparoscopic breast-conserving surgery combined with radiofrequency ablation group remarkably increased the hospitalization expense because of the utility of the radiofrequency ablation related apparatus,it may provide the probability of shaving more residual tumor cell and may low down the recurrence,especially not rising up the incidence of the post-operative fat liquefaction.Therefore,this surgery method might be one of the potential developments in the minimal-invasive of early stage breast cancer.
