BACKGROUND:At present, the surgical treatment of atlantoaxial dislocation mainly adopts the posterior atlantoaxial screw-rod internal fixation system for lifting and reduction.During the operation, the curvature of the connecting rod is often increased to increase the drop between the atlantoaxial vertebrae to improve the reduction effect, but it increases the difficulty and risk of surgery. The axis pivot screw directly increases the reduction drop between the atlantoaxial vertebrae, but the extent to which it increases the reduction capacity is unclear.OBJECTIVE:To test the reduction ability of axis pivot screw and compare it with ordinary screw.METHODS:Six fresh human craniocervical specimens were used in study. The joint capsules of two lateral mass joints and atlanto-odontoid joint and transverse ligament were removed to make an atlantoaxial instability model.Three kinds of internal fixation were performed successively on both sides of the axis of each specimen:uniaxial axis pivot screws(group A),multi-axial axis pivot screws(group B)and ordinary screws(group C).Flexible ultra-thin film pressure sensors were placed in the anterior atlanto-odontoid space. Two connecting rods with the same curvature were placed to simulate the lifting and reduction, and the pressure of the anterior atlanto-odontoid space was obtained.Comparative analysis was conducted among the three groups.RESULTS AND CONCLUSION:(1)The anterior atlanto-odontoid space pressure of groups A-C was(97.59±8.58),(60.43±5.09),and(22.74±0.81)N,respectively.There were significant differences among the three groups(F=251.603,P=0.000).The pairwise comparison among the three groups showed significant differences(P=0.000).(2)The axis pivot screw applied to the posterior atlantoaxial screw-rod internal fixation system can improve the reduction capacity compared with the common cervical posterior screw, and the uniaxial axis pivot screw has more reduction capacity than the multi-axis uniaxial axis pivot screw to improve the posterior atlantoaxial screw-rod internal fixation system.

Objective To identify biomarkers that characterize its bitter-cold properties of Scutellaria Radix on the basis of evaluating its cold and hot properties,as well as possible metabolic pathways and related targets; To explore its molecular mechanism. Methods Totally 40 mice were randomly divided into a control group and a treatment group, and were orally administered with normal saline and Scutellaria Radix decoction, respectively, for 4 consecutive days. The cold and hot plate differential method was used to evaluate the cold and hot tendencies of the mice;UPLC-MS/MS technology was used to analyze mouse blood samples, differential metabolites were screened using principal component analysis, partial least squares discriminant analysis and orthogonal partial least squares discriminant analysis methods, and metabolic pathway analysis was performed;network modular analysis of differential metabolites was performed using Cytoscape 3.9.0 software to identify potential molecular targets. Results On the second day of administration, the anal temperature of mice in the treatment group decreased significantly compared to the control group(P<0.01);in the cold and hot tendency test, the mice in the treatment group showed an overall increase in high-temperature tendency and a higher proportion of high-temperature zone retention. There was a statistically significant difference(P<0.01,P<0.05)between the treatment group and the control group on the 2nd and 3rd day of treatment;the pattern recognition analysis of serum metabolome data showed that the serum samples of the treatment group and the control group could be completely separated, and a total of 14 differential metabolites were screened out;metabolic pathway analysis identified 16 related pathways, including unsaturated fatty acid biosynthesis, linoleic acid metabolism, citric acid cycle(TCA cycle),arachidonic acid metabolism, glycine, serine and threonine metabolism, steroid hormone biosynthesis, etc.;a total of 16 modules were obtained through network modular analysis, among which the arachidonic acid metabolism pathway and linoleic acid metabolism pathway modules were larger;the nodal degree values of arachidonic acid and linoleic acid were greater than the mean, involving arachidonic acid metabolism and linoleic acid metabolism pathways;by screening 26 genes associated with the cytochrome P450 enzyme system were obtained. Conclusion Scutellaria Radix may regulate the body's energy metabolism, achieve its biological effects, and characterize its medicinal properties by intervening in metabolic pathways such as arachidonic acid and linoleic acid.

Objective To evaluate a model of management for chronic pain disease coordinated community health services and district hospital. Methods The epidemiology of chronic pain disease was surveyed in Haotao community of Zhongshan, before and after the establishment of the model of management. Results The prevalence rate of chronic pain disease was 51.83%, in which it was 47.06% in men, and 57.32% in women. After the establishment of the model, the efficacy of good to excellent improved from 30.22% to 67.08%, the cost for medicine reduced 36.16%, time for rest reduced from 32.42 d to 15.25 d (P<0.05). Conclusion The model of management that coordinated community health services and district hospital, including demonstration diagnosis and treatment through internet, tele-consultations and health education, is effective on chronic pain disease.

Objective To discuss the treatment and clinical results of acetabular fracture. Method Clinical data of 78 cases of acetabular fracture was analyzed retrospectively on treatment methods and clinical effects from August 1988 to December 2007, 41 cases were performed operation and 37 cases were performed non-operative management. Result The treatment effects were excellent in 43 cases, good in 22 cases, fair in 10 cases, poor in 2 cases and dead in 1 cases, with a good rate of 83.3%, and 20 cases had complications: 1 posterior dislocations of hip, 13 posttraumatic arthritis, 1 incisional infection, 5 ectopic ossi-fication. Conclusions It is very important to choose appropriate treatment methods by judging the real state of acetabular fractureexactly. Acetabular reconstruction plate is convenient, safe, and reliable in treat-ment of posterior wall and superior wall fractures.
