1.Steroids combined with anticoagulant in acute/subacute severe cerebral venous thrombosis.

BACKGROUND:
Inflammation plays a critical role in severe cerebral venous thrombosis (CVT) pathogenesis, but the benefits of anti-inflammatory therapies remain unclear. This study aimed to investigate the association between steroid therapy combined with anticoagulation and the prognosis of acute/subacute severe CVT patients.
METHODS:
A prospective cohort study enrolled patients with acute/subacute severe CVT at Xuanwu Hospital (July 2020-January 2024). Patients were allocated into steroid and non-steroid groups based on the treatment they received. Functional outcomes (modified Rankin scale [mRS]) were evaluated at admission, discharge, and 6 months after discharge. Serum high-sensitivity C-reactive protein (hs-CRP), interleukin-6 (IL-6), cerebrospinal fluid (CSF) IL-6, and intracranial pressure were measured at admission and discharge in the steroid group. Fundoscopic Frisén grades were assessed at admission and 6 months after discharge. Univariate and multivariate logistic regression were used to evaluat associations between steroid use and favorable outcomes (mRS ≤2) at the 6-month follow-up. Paired tests assessed changes in hs-CRP and other variables before and after treatment, and Spearman's correlations were used to analyze relationships between these changes and functional improvements.
RESULTS:
A total of 107 and 58 patients in the steroid and non-steroid groups, respectively, were included in the analysis. Compared with the non-steroid group, the steroid group had a higher likelihood of achieving an mRS score of 0-2 (93.5% vs . 82.5%, odds ratio [OR] = 2.98, P = 0.037) at the 6-month follow-up. After adjusting for confounding factors, the result remained consistent. Pulsed steroid therapy did not increase mortality during hospitalization or follow-up, nor did it lead to severe steroid-related complications (all P >0.05). Patients in the steroid group showed a significant reduction in serum hs-CRP, IL-6, CSF IL-6, and intracranial pressure at discharge compared to at admission, as well as a significant reduction in the fundoscopic Frisén grade at the 6-month follow-up compare to at admission (all P <0.001). A reduction in serum inflammatory marker levels during hospitalization positively correlated with improvements in functional outcomes ( P <0.05).
CONCLUSION:
Short-term steroid use may be an effective and safe adjuvant therapy for acute/subacute severe CVT when used alongside standard anticoagulant treatments, which are likely due to suppression of the inflammatory response. However, these findings require further validation in randomized controlled trials.

2.Meta-synthesis of qualitative researches on the caring experience of caregivers of lung transplantation patients

Objective:To systematically review and integrate the caring experience of caregivers of lung transplant patients.Methods:Qualitative studies on the caregiving experience of caregivers of lung transplant patients were searched by computer from PubMed, Web of Science, Embase, Cochrane Library, China Biology Medicine disc, China National Knowledge Infrastructure and Wanfang data, and the search period was from establishment of the databases to April 30, 2023. The qualitative research quality evaluation criteria (2016 edition) of the Joanna Briggs Institute Evidence Based Health Care Center in Australia were used to evaluate the quality of the included literature, and the Meta-synthesis was used to integrate the literature results.Results:A total of ten articles were included, and 33 clear research results were extracted, which were summarized into eight new categories, and finally summarized into four integrated results, such as heavy burden experience, strong demand, positive experience and satisfaction with the medical service system.Conclusions:Medical workers should attach importance to and pay attention to the burden and needs of caregivers of lung transplant patients, provide professional and emotional support to caregivers, improve their caring ability and quality, and ultimately improve the quality of life of lung transplant patients.

Gastrointestinal mucositis is one of the most debilitating side effects of the chemotherapeutic agent irinotecan (CPT-11). Andrographolide, a natural bicyclic diterpenoid lactone, has been reported to possess anti-colitis activity. In this study, andrographolide treatment was found to significantly relieve CPT-11-induced colitis in tumor-bearing mice without decreasing the tumor suppression effect of CPT-11. CPT-11 causes DNA damage and the release of double-stranded DNA (dsDNA) from the intestine, leading to cyclic-GMP-AMP synthase (cGAS)‒stimulator of interferon genes (STING)-mediated colitis, which was significantly decreased by andrographolide both in vivo and in vitro. Mechanistic studies revealed that andrographolide could promote homologous recombination (HR) repair and downregulate dsDNA‒cGAS‒STING signaling and contribute to the improvement of CPT-11-induced gastrointestinal mucositis. These results suggest that andrographolide may be a novel agent to relieve gastrointestinal mucositis caused by CPT-11.
5.Meta-analysis for the association between TP53 Arg72Pro polymorphism and human thyroid cancer risk

Objective To explore the association of single nucleotide polymorphism ( SNP ) in TP53 Arg72Pro (rs1042522) locus with thyroid cancer risk in human. Methods Articles involved in the association between SNP in TP53 Arg72Pro ( rs1042522) locus and thyroid cancer risk were retrieved from PubMed, Embase, and Web of Science databases, and studies which met the inclusion criteria were included. The meta-analysis, sensitivity analysis, subgroup analysis, and the assessment of publication-bias were performed by Stata 14. 0 software. The odds ratio ( OR) and their corresponding 95% Confidence Intervals ( CI) were used to determine the strength of association between SNP in TP53 Arg72Pro locus and thyroid cancer risk. Results Thirteen case-control studies were eligible for this meta-analysis, including 2112 thyroid cancer cases and 4000 control subjects. Overall, mutated homozygous genotype ( Pro/Pro) in TP53 Arg72Pro ( rs1042522) locus was associated with significantly increased thyroid cancer risk(Recessive model, OR=1.78, 95%CI 1.24-2.56, P=0.002), showing a significantly higher Pro mutation frequency among thyroid cancer patients ( Allelic model, OR=1. 35, 95% CI 1. 12-1. 63, P=0.002). In the stratified analysis, mutated homozygous genotype (Pro/Pro) in TP53 Arg72Pro (rs1042522) locus was only asscociated with significantly increased thyroid cancer risk among Asians, but not among Europeans and South Americans;mutated homozygous genotype ( Pro/Pro) in TP53 Arg72Pro ( rs1042522) locus was asscociated with significantly increased risk of papillary thyroid carcinomas ( PTC) among total population, but not medullary thyroid carcinomas. Conclusion There is a significant association between TP53 Arg72Pro polymorphism in TP53 and thyroid cancer risk, and the mutated homozygous genotype ( Pro/Pro) in this locus of TP53 maybe a risk factor for thyroid carcinoma among Asians.
6.Qualitative research of medication compliance on corticosteroids in patients with interstitial lung disease

Objective To investigate the influencing factors of medication compliance of glucocorticoid for patients with interstitial lung disease (ILD). Method The outline in-depth interviews with 15 patients with ILD was done using content analysis method, and the influence factors of patients medication compliance were analyzed. Result The influence factors of medication compliance in ILD patients can be summarized as the following three themes: patients′individual factors, factors related to medical and absence of social and family support. Conclusion Nurses should master the influencing factors of hormone medication adherence in ILD patients, ready to take appropriate intervention measures, improve the medication compliance in patients with ILD hormone and promote their rehabilitation.
7.Dermatopathological changes in a Bama miniature pig model of steroid-induced dermatitis after treatment with intense pulsed light

ObjectiveTo assess the impact of intense pulsed light (IPL) on the dermatopathological manifestation in a Bama miniature pig model of steroid-induced dermatitis.MethodsFive female Bama miniature pigs aged two months were selected.The white skin areas with white hair at both sides of the neck served as the target area.Halometasone(0.05%) cream was applied to the right target area twice daily for 60 days to establish a model of steroid-induced dermatitis.Then,3 pigs were randomly selected and irradiated with IPL of 25 J/cm2 at the model area with an interval of 3 weeks for 9 weeks,the remaining 2 pigs receiving no treatment served as the natural recovery group.Finally,skin tissues were obtained from the left and right target areas and subjected to haematoxylin and eosin staining for the observation of histopathological changes.ResultsA significant increase was observed in the layer number of keratinocytes and thickness of dermal collagen fiber in the IPL-treated pigs compared with the pigs in natural recovery group (6.27 ± 1.26 vs.2.98 ±0.92,t =3.27,P< 0.01; 1.88 ± 0.19 mm vs.0.84 ± 0.15 mm,t =4.25,P< 0.01).Moreover,IPL irradiation resulted in the regression of telangiectasis in the dermis.ConclusionIPL may increase skin thickness,relieve flushing and improve skin elasticity efficiently.
8.Effects of different treatments on hypertensive cerebral hemorrhage

Objective To compare and analyze the surgical and non-surgical treatment results of hypertensive cerebrat hemorrhage.Methods 29 vases of hypertensive cerebral hemorrhage patients are randomly divided into two groups:13 cases were treated by surgical treatment,and 16 cases were treated by medicine treatment.Results In surgical group,dead 3 cases,plant survival 5 eases,death disablity rate is 62%;and in non-surgical group:dead 1 case,plant survival 5 eases,death disablity rate is 44%.Conclusion There is no significant difierence between surgical and non-surgical treatment of hypertensive cerebral hemorrhage.It should be thought more to use surgical treatment on hypertensive cerebral hemorrhage.
