1.Influencing factors of work alienation among Chinese nurses: A meta-analysis

Background Work alienation is a subjectively negative psychological state characterized by detachment from one's job and its environment. Due to heavy workloads, high-intensity tasks, low compensation, as well as pressures related to promotion, nurses are particularly susceptible to work alienation. Objective To meta-analyze the primary factors affecting work alienation of nurses in China, providing an evidence-based foundation for improving nursing management and nurses' physical and mental health. Methods A systematic search was conducted across multiple databases, including PubMed, Embase, the Cochrane Library, Web of Science, CNKI, VIP, Wanfang Data, and DBM, for cross-sectional studies on work alienation and its influencing factors among nurses in China published from inception to December 2024. Two researchers independently conducted literature screening, data extraction, and risk of bias assessment for the included studies. Meta-analysis was performed using Stata 15.0 and RevMan 5.4 software. Pooled effect sizes were calculated using either fixed-effects or random-effects models based on the magnitude of heterogeneity. Results A total of 27 studies involving 13 028 nurses were included in the meta-analysis, from which 11 influencing factors of work alienation were identified. The pooled score for work alienation among nurses in China was 34.79 (95%CI: 32.56, 37.01). Subgroup analysis revealed that the emergency department reported the highest score (36.81, 95%CI: 31.64, 41.98), followed by the intensive care unit (34.99, 95%CI: 32.27, 37.72) and internal medicine department (33.90, 95%CI: 29.10, 38.71). Significant factors influencing work alienation included length of work experience [standardized mean difference (SMD)=0.32, 95%CI: 0.02, 0.61], professional title (SMD=0.30, 95%CI: 0.16, 0.44), monthly income (SMD=0.38, 95%CI: 0.14, 0.62), psychological capital (SMD=–0.41, 95%CI:–0.52, –0.29), work stress (SMD=0.54, 95%CI: 0.49, 0.58), attitude toward aggression and violence management (SMD=–0.38, 95%CI:–0.44, –0.32), and organizational climate (SMD=–0.47, 95%CI: –0.57, –0.36) (P <0.05). Sensitivity analysis confirmed the robustness of the meta-analysis results, and no significant publication bias was detected. Conclusion Nurses in China exhibit a high level of work alienation, particularly in high-acuity settings like emergency and intensive care unit departments compared to general departments. Shorter work experience (<5 years), lower professional title, lower monthly income (<5 000 CNY), and greater work stress are positively correlated with work alienation. In contrast, psychological capital, proactive attitudes toward managing aggression and violence, and a supportive organizational climate are negatively correlated. These findings indicate a need for nursing managers to develop targeted strategies based on these multifaceted factors to mitigate alienation among nursing staff.

and Relevance: Evidence from the literature suggests that, when integrated appropriately, simulation technologies, digital learning environments, and AI-assisted tools collectively enhance the key elements of veterinary training. These approaches can support safer clinical practice, help standardize skill development, and broaden access to quality training, particularly in resource-limited settings. Instead of hypothesizing new mechanisms, this review uses current evidence to highlight practical implications for curriculum design and clinical preparedness. Continued investment in faculty development, ethical use of AI, and infrastructure is essential for achieving sustainable, technology-enhanced veterinary education.
3.Predictive value of varying degrees of delirium on the occurrence of perioperative myocardial infarction following off-pump coronary artery bypass grafting

Objective To explore the predictive value of different degrees of delirium after off-pump coronary artery bypass grafting (OPCABG) for perioperative myocardial infarction (MI). Methods A retrospective analysis was conducted on the clinical data of patients who underwent OPCABG in the First Department of Cardiac Surgery at the First Hospital of Hebei Medical University between April 2018 and March 2024. Patients were divided into a mild delirium group, a moderate delirium group, and a severe delirium group based on the degree of delirium, and into a MI group and a non-MI group based on the occurrence of perioperative MI. Logistic regression analysis was used to investigate the predictive factors for secondary MI after OPCABG. The predictive value of different degrees of postoperative delirium for secondary perioperative MI was assessed using receiver operating characteristic (ROC) curves. Results A total of 436 patients were included, with 211 males and 225 females, and a median age of 51.00 (44.00, 57.75) years. Delirium occurred in 139 patients, with 52 in the mild delirium group, 29 in the moderate delirium group, and 58 in the severe delirium group. There were 101 patients in the MI group and 335 patients in the non-MI group. Logistic regression analysis showed that severe delirium was an independent predictor of secondary MI after OPCABG [OR=23.979, 95%CI (11.572, 49.691), P<0.001]. ROC curve analysis revealed that the area under the ROC curve for predicting perioperative MI by severe postoperative delirium was 0.709, with a sensitivity of 0.546 and a specificity of 0.964. Conclusion Severe postoperative delirium can be used as an indicator to predict secondary MI after OPCABG.

OBJECTIVE To explore the intervention mechanism of Bushen anzhi decoction (BSAZD) in rats with insomnia induced by kidney failing to store spirit. METHODS A total of 36 SD rats were randomly divided into normal group (normal saline), model group (normal saline), eszopiclone group (positive control group, 0.27 mg/kg), and low-, medium- and high-dose BSAZD groups (5.85, 11.70, 23.40 g/kg), with 6 rats in each group. Except for the normal group, rats in the remaining groups were subjected to combined administration of D-galactose and DL-4-chlorophenylalanine to establish the rat model of insomnia due to kidney failing to store spirit. After successful modeling, corresponding liquid medicine or normal saline was intragastrically administered once daily for 14 consecutive days. After the last administration, behavioral indicators and hippocampal histopathological morphology of rats were detected. Untargeted metabolomics was adopted to screen serum differential metabolites and enrich relevant signaling pathways. RESULTS Compared with the model group, the high-dose BSAZD group exhibited significantly increased open arm entries , open arm time and sleep time ( P <0.05), accompanied by markedly decreased closed arm entries, closed arm time and sleep latency ( P <0.05). Hippocampal neurons in the high-dose BSAZD group were arranged regularly, and the size, morphology and location of cell nuclei were nearly normal. Untargeted metabolomics analysis identified oxidized glutathione, glutamate and other differential metabolites in serum of rats in the high-dose BSAZD group; these differential metabolites were mainly enriched in GABAergic synapse, glutamatergic synapse, cAMP signaling pathway and 2-oxocarboxylic acid metabolism pathway. CONCLUSIONS BSAZD can improve sleep quality, relieve anxiety-like behaviors in rats with insomnia due to kidney failing to store spirit. Its therapeutic mechanism may be associated with regulating GABAergic and glutamatergic synaptic pathways to restore neurotransmitter balance and further alleviate oxidative stress injury, modulating the cAMP signaling pathway to facilitate neural repair, and intervening the 2-oxocarboxylic acid metabolism pathway to optimize energy supply.
5.Imaging Measurement of the Safe Distances of Acupuncture at Fengfu (GV 16) in Children Aged 4 to 8 Based on CT Three-Dimensional Reconstruction

ObjectiveTo measure the distance from the surface of Fengfu (GV 16) to the deep important bony structures in children aged 4 to 8 years based on CT three-dimensional reconstruction technology, thereby providing imaging reference data for clinical acupuncture safe operations in terms of different age groups and needle insertion angles. MethodsCervical CT image data were collected from healthy children aged 4 to 8 years with five age groups (4-, 5-, 6-, 7-, 8-year-old groups), 20 cases in each group. The images were imported into Mimics 21.0 software for three-dimensional reconstruction. The straight-line distances from the skin model surface of the Fengfu (GV 16) to the first underlying bony barrier (such as the lower edge of the occipital bone, the posterior arch of the atlas vertebra) were measured at intervals of 5° on both the transverse section (0° ± 35°) and the sagittal section (40° upward to 40° downward on the diagonal). The correlation between the measured distance, age, and angle was analyzed. ResultsIn the transverse and sagittal sections, the measured distance from the skin surface to the underlying bone of Fengfu (GV 16) at each angle increased with age. In the transverse section, the increase was relatively small within the 0° to 20° angle range, whereas a more pronounced increase was observed when the angle exceeded 20°; at measurement angles of 30° to 35°, the distance of the skin surface to the bone at Fengfu (GV 16) in the 7- and 8-year-old groups was significantly greater than that in the 4-year-old group (P<0.05). In the sagittal section, the measured distance from the skin surface to the bone at Fengfu (GV 16) was shortest and changed greatly at oblique upward angles of 15° to 40° within each age group, and increased markedly at near-vertical angles of 0° to 10°; it was longest at oblique downward angles of -40° to -5° and showed relatively gradual changes. ConclusionThis study obtained the imaging distance data from Fengfu (GV 16) to the surrounding bony structures in children aged 4 to 8 years through three-dimensional modeling, finding that this distance has clear age and angle dependence, which can provide quantitative evidence for developing individualized and precise safe acupuncture schemes at Fengfu (GV 16) in pediatric clinical practice.

8.Right ventricular-pulmonary artery connection for palliative treatment of pulmonary atresia with ventricular septal defect in children: A single-center retrospective study

Objective To compare the benefits and drawbacks of primary patch expansion versus pericardial tube right ventricular-pulmonary artery connection in patients diagnosed with pulmonary atresia with ventricular septal defect (PA/VSD). Methods A retrospective study was conducted on patients diagnosed with PA/VSD who underwent primary right ventricular-pulmonary artery connection surgery at our center between 2010 and 2020. Patients were categorized into two groups based on the type of right ventricular-pulmonary artery connection: a pericardial tube group and a patch expansion group. Clinical data and imaging findings were compared between the two groups. Results A total of 51 patients were included in the study, comprising 31 males and 20 females, with a median age of 12.57 (4.57, 49.67) months. The pericardial tube group included 19 patients with a median age of 17.17 (7.33, 49.67) months, while the patch expansion group consisted of 32 patients with a median age of 8.58 (3.57, 52.72) months. In both groups, the diameter of pulmonary artery, McGoon index, and Nakata index significantly increased after treatment (P<0.001). However, the pericardial tube group exhibited a longer extracorporeal circulation time (P<0.001). The reoperation rate was notably high, with 74.51% of patients requiring further surgical intervention, including 26 (81.25%) patients in the patch expansion group and 12 (63.16%) patients in the pericardial tube group. No statistical differences were observed in long-term cure rates or mortality between the two groups (P>0.005). Conclusion In patients with PA/VSD, both patch expansion and pericardial tube right ventricular-pulmonary artery connection serve as effective initial palliative treatment strategies that promote pulmonary vessel development and provide a favorable foundation for subsequent radical operations. However, compared to the pericardial tube approach, the patch expansion technique is simpler to perform and preserves some intrinsic potential for pulmonary artery development, making it the preferred procedure.
9.Construction of a refined management system for patients’self-carried medicine dispensing in PIVAS of our hospital

OBJECTIVE To standardize the management of patients’ self-carried medicines dispensed by Pharmacy Intravenous Admixture Services (PIVAS) of our hospital, improve the quality of medical care, and ensure the safety of patients’ medication. METHODS Based on evidence-based methodology, the research literature of domestic medical institutions on the management of patients’ self-carried medicines was systematically searched and analyzed, and the data on the use of patients’ self- carried medicines in our hospital from February to April 2023 were extracted based on our hospital’s information system. A fishbone diagram of the difficulties in constructing patients’ self-carried medicines within PIVAS of our hospital was drawn through brainstorming, thereby establishing a refined whole-process management system for patients’ self-carried medicines in PIVAS. The effectiveness of the system was evaluated through the indicators, including dispensing volume, error rate, and so on. RESULTS & CONCLUSIONS A total of 15 papers were included, covering descriptive studies on the reasons for the use of patients’ self- carried medicines, current status of their use, management system, management methods, management suggestions. There were 12 clinical departments in our hospital with a high proportion of patients using self-carried medicines, among which the pediatric neurology department had the most use of patients’ self-carried medicines, accounting for 10.65%. In terms of dosage form, injectable drugs accounted for 13.68%, all of anticancer drug were uniformly dispensed by our hospital’s PIVAS. PIVAS in our hospital had successfully constructed the management system for dispensing patients’ self-carried medicines, which included the processes of drug reception and preservation, medical document retention, doctor’s order review, drug placement, dispensing, review and transportation. After the implementation of the system, from May 2023 to October 2024, our hospital’s PIVAS dispensed 654 bags of patients’ self-carried medicines in total,with a monthly average of 36 bags. Zero error management 5);was realized for all patients’ self-carried medicines. The system operates stably and effectively, thereby ensuring the high-quality dispensing of patients’ self-carried medicines in PIVAS of our hospital.
10.Relationships among empathy,meaning in life and altruistic behavior in medical students

Objective:To investigate the relationship between empathy and meaning in life among medical students,and explore the mediating role of altruistic behavior.Methods:A total of 581 medical students were recrui-ted.They were assessed with the Interpersonal Reactivity Index-C(IRI-C),Altruistic Behavior Questionnaire of College Students(ABQ-CS),and Quadripartite Existential Meaning Scale(QEMS).The bootstrap method was used to test the hypothesized mediating effect.Results:The scores of IRI-C were positively correlated with the scores of ABQ-CS and QEMS(r=0.45,0.21,Ps<0.001).The scores of ABQ-CS were positively correlated with the QEMS scores(r=0.46,P<0.001).Altruism was a complete mediator between empathy and meaning in life,the indirect effect was 0.20(95%CI:0.15-0.26).Conclusion:This study reveals the underlying connections a-mong altruistic behavior,empathy,and meaning in life among medical students,suggesting that interventions targe-ting empathy and altruistic behavior may enhance their meaning in life.
