Background and Aims:Papillary thyroid carcinoma(PTC)is the most common endocrine malignancy in China, with cervical lymph node metastasis being a frequent and critical clinical feature that directly affects patient prognosis and recurrence risk.In recent years, with the rapid increase in the prevalence of overweight and obesity in China, the role of body mass index(BMI)in various tumors has attracted growing attention. This study aimed to investigate the association between overweight and cervical LNM in PTC,analyze sex-specific differences and influencing factors, and provide evidence for precise clinical management. Methods:A retrospective analysis was conducted on the clinicopathologic data of 1 445 patients with classical PTC treated at Xiangya Hospital of Central South University between August 2021 and June 2022.Patients were divided into groups based on the presence or absence of lymph node metastasis.Restricted cubic spline analysis explored the nonlinear relationship between BMI and lymph node metastasis risk.Univariate and multivariate Logistic regression analyses were applied to identify independent risk factors.Furthermore,sex-stratified analysis was performed among overweight patients(BMI≥24 kg/m2)to determine sex-specific risk factors for lymph node metastasis. Results:Among all patients,716(49.6%)had lymph node metastasis.Univariate analysis showed that BMI,sex, age, tumor diameter, multifocality, and extrathyroidal extension were significantly associated with cervical lymph node metastasis in PTC patients(all P<0.05).A nonlinear positive correlation was observed between BMI and lymph node metastasis risk, which was more pronounced in male patients.Additionally,BMI was positively correlated with triglyceride levels and negatively correlated with high-density lipoprotein cholesterol.Sex-stratified analysis revealed that in overweight male patients, younger age(OR=0.954),larger tumor diameter(OR=1.085),and multifocality(OR=2.776)were independent risk factors for LNM;in overweight female patients, younger age(OR=0.943)and larger tumor diameter(OR=1.074)were the main influencing factors. Conclusion:Overweight is closely associated with cervical lymph node metastasis in PTC,and the high-risk factors for LNM differ between male and female overweight patients.Young age, larger tumor size, and multifocality in overweight males, and young age and larger tumors in overweight females indicate a higher risk of metastasis. It is recommended that high-risk populations receive enhanced preoperative evaluation and individualized lymph node dissection strategies to achieve precise treatment and improved risk control.

Objective To predict the radiation impact of discharging wastewater containing uranium within the specified limit generated during the normal operation of a new production line at a nuclear fuel plant on the receiving water body and its downstream, and to provide a reference for the management of radioactive liquid effluent discharge from nuclear facilities. Methods Based on the technical guidelines for environmental impact assessment, literature on radiation environmental impact assessment, and data collected from on-site investigations, appropriate hydrological parameters and prediction models were selected to analyze and predict the variation pattern of radioactive nuclide uranium along the receiving water body and the radiation exposure of nearby residents. Results The maximum increase in uranium concentration in the receiving water body and its downstream caused by the discharge of uranium-containing wastewater was 1.14 μg/L. The maximum predicted concentration was 2.75 μg/L after adding the background data of the water body. The resulting maximum individual annual effective dose for the public was 1.49 × 10−4 mSv/a. Conclusion The maximum predicted uranium concentration in the receiving water body and its downstream is lower than the uranium concentration limit of 30 μg/L specified in the Standards for Drinking Water Quality (GB 5749-2022). The maximum individual annual effective dose for the public is much lower than the control value of 0.2 mSv/a specified in the Radiation Protection Regulations for Uranium Processing and Fuel and Fuel Manufacturing Facilities (EJ 1056-2018). The radiation impact is acceptable.

Journal of Clinical Medicine in Practice 2025;29(1):77-82
Objective To investigate the impact of dexmedetomidine on epidural fibrosis after spinal surgery in rats through the transforming growth factor-β1(TGF-β1)signaling pathway.Meth-ods A total of 40 rats were selected, with 10 rats assigned to control group. The remaining 30 rats underwent spinal surgery modeling and were randomly divided into model group, dexmedetomidine group, and dexmedetomidine+TGF-β1 agonist group, with 10 rats in each.Rats with unsuccessful modeling were excluded, resulting in 9 rats in each group for subsequent analysis.Pathological char-acteristics of epidural scar tissue, fibroblast count,PCNA-positive cell expression rate, levels of in-flammatory markers[interleukin(IL)-6,IL-1β,IL-8],stress response indicators[malondialdehyde(MDA),superoxide dismutase(SOD)],and expression levels of mRNAs and proteins related to the TGF-β1 signaling pathway were observed and compared among the groups. Results Compared with the control group, the other three groups showed increased fibroblast count, elevated PCNA-positive cell expression rate, higher levels of IL-6,IL-1β,IL-8,and MDA,lower SOD levels, and increased expression levels of TGF-β1 mRNA,Smad3 mRNA,and TGF-β1 and Smad3 proteins, with statistically significant differences(P<0.05).Compared with the model group, both the dexmedetomidine group and the dexmedetomidine+TGF-β1 agonist group exhibited decreased fibroblast count,re-duced PCNA-positive cell expression rate, lower levels of IL-6,IL-1 β,IL-8 and MDA,higher SOD levels, and decreased expression levels of TGF-β1 mRNA,Smad3 mRNA,and TGF-β1 and Smad3 proteins, with statistically significant differences(P<0.05).Compared with the dexmedetomidine group, the dexmedetomidine+TGF-β1 agonist group showed increased fibroblast count, elevated PCNA-positive cell expression rate, higher levels of IL-6,IL-1 β,IL-8 and MDA,lower SOD lev-els,and increased expression levels of TGF-β1 mRNA,Smad3 mRNA,and TGF-β1 and Smad3 proteins, with statistically significant differences(P<0.05).Conclusion Dexmedetomidine can significantly alleviate the inflammatory response and stress response after spinal surgery in rats and reduce postoperative epidural fibrosis by inhibiting the TGF-β1 signaling pathway.

This study introduces the development of a 96-channel genetic analyzer and its application in forensic medicine. The instrument integrates a fluorescence excitation system,a fluorescence spectral imaging system, and a data acquisition and analysis system, enabling the simultaneous detection of 96 DNA samples.Performance validation in forensic science demonstrated that the analyzer can resolve DNA fragments differing by 1 bp,with accurate DNA typing and a detection sensitivity of 0.125 ng,while maintaining good repeatability. The system can effectively process various forensic samples, including blood cards, saliva cards, and shed cell swabs. In practical applications, the analyzer has significantly improved detection efficiency and shown excellent performance.Looking forward,by broadening the detection range of the nine-color fluorescence spectrum and enhancing the level of data intelligence, the instrument is expected to further strengthen its high-throughput multi-locus detection capability and efficiency, providing more powerful technical support for forensic DNA analysis.

Metering unit is important component of medical service items and also serve as significant bases for scientifically estimating the costs of medical services.National Technical Specification of Medical Service Items(2023 Edition)has set up elements such as"metering unit"and"adjustment coefficient for resource consumption in special circumstances",which clearly stipulate the metering units of each medical service item and the multiples for discretionary increases or decreases in the overall resource consumption when special circumstances occur. These two elements are important references for the cost accounting in medical institutions. The project team has elaborated on the definitions, establishment principles and special circumstances of these two types of elements, namely"metering units"and"adjustment coefficient for resource consumption in special circumstances",providing references for the application of relevant governments and healthcare institutions.
