1.Application value of TSE-DWI sequence in MRI examination of 3.0 T rectal cancer

Objective To explore the application value of turbo spin echo-diffusion weighted imaging(TSE-DWI)sequences in 3.0 T rectal cancer MRI examination by comparing the image quality difference be-tween TSE-DWI and conventional echo planar imaging-diffusion weighted imaging(EPI-DWI)sequences.Methods The medical records of forty-two patients with newly diagnosed rectal cancer confirmed by colonos-copy were prospectively collected.All patients simultaneously underwent both TSE-DWI and EPI-DWI se-quence scans.The image quality of the two groups performed the subjective evaluation(lesion contrast,mag-netic susceptibility artifacts,geometric distortion)and objective evaluation[signal-to-noise ratio(SNR),con-trast-to-noise ratio(CNR),contrast,etc].Results In the subjective evaluation,compared with the conven-tional EPI-DWI sequences,the proportions of subjective score 3-5 points for the lesion contrast,magnetic susceptibility artifacts and geometric distortion in the TSE-DWI sequences lesions were over 95%,which all were higher the conventional EPI-DWI sequences;among them,the differences in the subjective scoring results of magnetic susceptibility artifacts and geometric deformation were statistically significant(x2=12.464,5.610,P<0.05),while there was no statistical significance in lesion contrast(x2=2.363,P>0.05).In the objective evaluation,SNR and CNR in the TSE-DWI sequences all were higher than those in the conventional EPI-DWI sequences,in which the difference in CNR was statistically significant(Z=—5.645,P<0.05),while the difference in SNR was not statistically significant(Z=—1.057,P>0.05).The contrast in the TSE-DWI sequences was lower than that in the conventional EPI-DWI sequences,and the difference was statistical-ly significant(t=3.012,P<0.05).Conclusion The TSE-DWI sequence images have small geometric de-formation and few magnetic susceptibility artifacts,the image quality is superior to the EPI-DWI sequences.Especially in 3.0T rectal cancer MRI plain scan,the patients with gas accumulation in the intestinal lumen of rectal lesion could directly adopt the TSE-DWI sequences by replacing the conventional EPI-DWI sequences in order to increase the clinical application efficiency and image quality.
2.Summary of the best evidence for the prevention and management of perioperative gastrointestinal dysfunction in patients with colorectal cancer

Objective:To systematically retrieve, extract and summarize the best evidence regarding the prevention and management of perioperative gastrointestinal dysfunction in patients with colorectal cancer, and to provide an evidence-based basis for clinical nursing practice.Methods:All evidence on the prevention and management of perioperative gastrointestinal dysfunction in colorectal cancer patients in various domestic and international databases and websites such as UpToDate, China national knowledge infrastructure, etc. were searched according to the "6S" pyramid model, including clinical decisions, guidelines, expert consensus, systematic evaluations, and randomized controlled trials from the time of database construction to August 15, 2024, on a top-to-bottom basis. Literature screening, quality assessment, and evidence extraction were performed independently by 2 nursing researchers.Results:A total of 19 documents were finally included, included 4 guidelines, 3 expert consensus, 7 systematic evaluations, 1 Meta-analysis, and 4 randomized controlled trials, and 35 pieces of evidence were summarized in 6 areas, including screening and assessment, build a multidisciplinary team, preoperative preparation, intraoperative monitoring, postoperative care, and evaluation of outcomes.Conclusions:The evidence summarized in this study is both scientifically rigorous and practically applicable, and can be used by healthcare professionals to design care plans for colorectal cancer patients that accelerate the recovery of gastrointestinal function, thereby promoting evidence-based clinical nursing practice.

4.Clinical Study on Huangjing Jiangya Decoction in the Treatment of Patients with Hypertension of Qi-Deficiency Type Accom-panied by Insomnia

OBJECTIVE To observe the effect of Huangjing Jiangya Decoction on blood pressure and sleep in patients with hy-pertension of qi-deficiency type accompanied by insomnia.METHODS 73 patients with hypertension of qi-deficiency type accompa-nied by insomnia who met the inclusion criteria were selected and randomly divided into an observation group of 36 cases and a control group of 37 cases.The control group was treated with amlodipine besylate tablets,and the observation group was given Huangjing Jian-gya Decoction oral treatment on the basis of the control group.Both groups were treated continuously for 8 weeks.The changes in TCM syndrome scores,office blood pressure monitoring(OBPM),home blood pressure monitoring(HBPM),24-hour ambulatory blood pressure monitoring(ABPM),Pittsburgh Sleep Quality Index(PSQI)scores and clinical efficacy of the two groups of patients before and after treatment were observed.RESULTS After treatment,the TCM syndrome scores in the observation group were significantly decreased(P<0.05,P<0.01),which were better than the control group(P<0.01);OBPM and HBMP in both groups were signifi-cantly reduced(P<0.05,P<0.01),the observation group was better than the control group(P<0.05,P<0.01);the ABPM of the observation group was significantly reduced(P<0.01),which was better than the control group(P<0.05,P<0.01);the sleep quali-ty,sleep latency,sleep duration,daytime dysfunction score and PSQI total score of the observation group were significantly decreased(P<0.01),which were better than those in the control group(P<0.05,P<0.01);the clinical efficacy of hypertension and insomnia in the observation group was both better than the control group(P<0.01).CONCLUSION Huangjing Jiangya Decoction combined with amlodipine can improve the symptoms of patients with hypertension of qi-deficiency type accompanied by insomnia,lower blood pressure,improve sleep quality,shorten sleep latency,alleviate daytime dysfunction,and has good clinical efficacy.
5.Mechanisms of cholesterol metabolism imbalance in a PA-induced non-alcoholic fatty liver disease cell model

Liver cholesterol metabolism disorder plays an important role in the development of non-alcoholic fatty liver disease (NAFLD).In order to reveal the molecular mechanism of cholesterol homeostasis imbalance induced by saturated fatty acids, HepG2 cells were stimulated with palmitic acid (PA).Lipids accumulation was analyzed by Oil Red O staining, intracellular triglyceride and cholesterol quantification.The level of genes and proteins related to cholesterol homeostasis was measured by RT-qPCR and western blotting.Additionally, intracellular bile acids and mitochondrial oxysterols were detected by LC-MS/MS.The results demonstrated that intracellular lipids such as TG and TC were significantly increased in the model with PA stimulation.Although no significant difference was detected in genes related to cholesterol synthesis and uptake, the protein expression of ABCG5 and LXRα were significantly down-regulated, indicating a decrease in cholesterol efflux.Meanwhile, the gene expression of STARD1 and CYP7B1, which are responsible for bile acid alternative synthesis, were markedly enhanced, along with a significant increase of cholesterol and 27-OHC in mitochondria and CDCA in cells.These results suggested that PA overload may disrupt cholesterol homeostasis by inhibiting cholesterol efflux and promoting bile acids synthesis.
6.Application value of incision with no compression bandage combined with early function rehabilitation for patients with breast cancer after operation

Objective To explore value of incision with no compression bandage combined with early function rehabilitation for patients with? breast cancer? after operation.Methods A total of 150 breast cancer patients underwent unilateral modified radical mastectomy were randomly divided into experimental group and the control group,with 75 cases per group.The control group used conventional nursing,and the experimental group adopted postoperative fast-track surgery nursing,and the patients were followed up for one year after surgery.Drainage tube removal time,wound healing,hospitalization days,cost of treatment,limb shoulder joint activity and limb lymphedema incidence were compared between groups.Results The incidences of skin flap necrosis,subcutaneous effusion,and upper limb lymphedema of experimental group were not higher than the control group.The function recovery of upper limb exercise was obviously better than the control group,the difference was statistically significant (P < 0.05).No significant difference was seen in extubation time,hospitalization days and cost of treatment in two groups (P > 0.05).Conclusion The incision with no compression bandage combined with functional exercise can promote the recovery for postoperative patients with breast cancer,and it has important clinical value.
