1.Determination of Volatile Organic Compounds in Medical Molecular Sieve Oxygen Concentrators by Thermal Desorption-Gas Chromatography-Mass Spectrometry.

A method for determining volatile organic compounds (VOCs) emitted from medical molecular sieve oxygen concentrators was developed using thermal desorption-gas chromatography-mass spectrometry (TD-GC-MS). The oxygen concentrator gas was sampled at a flow rate of 0.5 L/min through a branched sampling system onto Tenax GR/carbopack B adsorption tubes. The adsorbed compounds were desorbed and introduced using a programmed temperature vaporization inlet system, followed by chromatographic separation on an SH-I-624Sil MS column. Four VOCs (BHT-Q, PTBP, BHT-quinol, and EHB) were detected in the medical oxygen concentrator using this method. Calibration curves for these compounds exhibited excellent linearity ( R 2>0.99) within the range of 3~100 ng. With a sampling volume of 20 L, the detection limit of the four VOCs ranged from 0.003 9 to 0.022 2 μg/m 3. Spike recovery rates for the four VOCs were between 95% and 115%, with relative standard deviations (RSDs) below 5% ( n=6). The method is simple, rapid, highly sensitive, and accurate, making it suitable for VOCs detection in medical molecular sieve oxygen concentrators.

3.Study on the application of target oriented infusion with stroke volume variation guidance in elderly lumbar surgery

Objective To evaluate the effect of target oriented infusion guided by stroke volume variation(SVV)in elderly lumbar surgery.Method From January to December 2023,80 elderly patients undergoing elective general anesthesia for prone lumbar surgery were divided into two groups by random number table method:SVV guidance target-oriented infusion group(SVV group)and conventional infusion group(conventional group),with 40 cases in each group.The amount of infusion,blood loss,urine volume and use of vasoactive drugs in the two groups were recorded.MAP,HR,blood lactate concentration(Lac),base residual value(BE),and PaO2 were recorded at the time of entry(T1),before cuticle(T2),1 hour after prone position(T3),and after surgery(T4).Pulmonary ultrasound examinations were performed at 4,8,12 and 24 hours after surgery,and B-line scores were calculated,as well as the incidence of cardiac insufficiency,nausea,vomiting,delirium and hospital stay at 24 hours after surgery.Result Compared with normal group,in group SVV,intraoperative infusion volume[(1123+532)ml],vascular active drug utilization rate(15.0%),postoperative each point B line score[(1.55±1.32),(1.22±0.94),(1.01±0.93),(1.07±0.90)]were reduced(P<0.05).HR at T2~T4[(67±8,71±11,73±12)times/min]、PaO2 at T3,T4[(438±41,423±44)mmHg]were increased,Lac and BE at T2~T4 were(0.3±0.5,1.4±0.5,1.4±0.3)mmol/L.(0.3±0.6,0.3±1.0,0.3±1.1)was decreased(P<0.05).There were no significant differences between the two groups in cardiac dysfunction,nausea,vomiting,delirium and hospital stay after a 24-hour postoperative period(P>0.05).Conclusion SVV guided target oriented infusion in elderly lumbar surgery is beneficial to circulation stability and tissue perfusion,and reduces the production of lung water.
4.Changes in distribution of CD24 in immune microenvironment and its relation-ship with pathological status of axillary lymph nodes during neoadjuvant therapy for breast cancer

Objective:To study distribution of CD24 in immune microenvironment and its relationship between pathological status of axillary lymph nodes during neoadjuvant therapy(NAT)for breast cancer.Methods:A total of 200 breast cancer patients admitted to the First People's Hospital of Chenzhou City from February 2021 to February 2024 were selected as research objects.According to CD24 expression,patients were divided into high expression group(n=126)and low expression group(n=74),and divided into pCR group(n=91)and non-pCR group(n=109)according to clinical treatment effect.Hierarchical regression model was used to analyze influence of different clinicopathological features on CD24 expression,generalized linear mixed model was constructed to judge relationship between CD24 and pCR,and dose-response relationship between CD24 and pCR was analyzed by restricted cubic spline method,GMDR model was used to analyze interaction of CD24 and immune microenvironment factors on pCR.Results:CD24 expression was correlated with TNM stage,vascular invasion,number of lymph nodes and tumor markers levels.CD24 mRNA level in patients after NAT showed a gradually decreasing trend.Compared with before treatment,CD24 mRNA expression at 1 month,3 months and 6 months after treatment had statistical significance(P<0.05),immune microenvironment factors levels in patients were signifi-cantly changed,and CD3+T,CD4+T,CD8+T,FOXP3,IL-10,IgG and IgM levels were increased,while CD10+T,IL-6 and IL-17 levels were decreased.TNM stage,vascular invasion,number of lymph nodes,CA125 and CEA levels were significant positive related with CD24 level(P<0.05).Tumor aspect ratio,TNM stage,vascular invasion,number of lymph nodes,HER-2,Ki-67,CD24 expres-sions and CD24 mRNA level were correlated with post-NAT axillary lymph node pCR in breast cancer patients.CD24 mRNA expres-sion was a risk factor for post-NAT axillary lymph node pCR,and probability of pCR in patients decreased significantly with increase of CD24 mRNA expression.CD24 interacted with CD3+T,CD4+T,CD8+T,CD10+T,FOXP3,IL-6,IL-10,IL-17,IgG and IgM on pCR.Conclusion:CD24 level in patients after NAT showes a gradually decreasing trend.CD24 mRNA expression is a risk factor for axillary lymph node pCR after NAT.With increase of CD24 mRNA expression,probability of pCR in patients decreases significantly.

7.Comparison of artificial reconstruction and reconstruction using artificial intelligence in coronary artery volume rendering

Objective To explore artificial reconstruction vs artificial intelligence reconstruction in coronary artery volume rendering(VR).Methods A total of 110 coronary heart disease(CHD)patients were enrolled in the study,and underwent computed tomography arteriography.Artificial reconstruction(artificial group)and artificial intelligence reconstruction(intelligent group)were conducted in coronary artery VR.The image quality of VR and curved planar reconstruction(CPR),the diagnosis coincidence rate of arterial stenosis,and vascular reconstruction score were compared between two groups.Results The VR image quality of intelligent group was significantly higher than that of artificial group,with excellence rate of 90.91%vs 80.00%;and intelligent group scored higher than artificial group in CPR image quality(P<0.05).The differences in the diagnostic coincidence rates of arterial vessels(RCA,LAD,LCX)with no stenosis,slight stenosis,moderate stenosis,and vascular occlusion between two groups were trivial(P>0.05),while the two groups differed significantly in the diagnosis coincidence rates of mild stenosis and severe stenosis(P<0.05).The mean total score,the median value of the 3 main branches+posterior descending branch+intermediate branch+diagonal branch+blunt margin branch,and the median value of the other vascular branches were all significantly increased in intelligent group as compared with artificial group(P<0.05).Conclusion Artificial intelligence reconstruction has higher application value than artificial reconstruction in coronary artery VR.
8.Effect of Wenweiyang decoction on mast cell activation and SCF/c-Kit signaling pathway in rats with functional dyspepsia

AIM:To investigate the mechanism of action of Wenweiyang decoction(WWYD)in treating func-tional dyspepsia in rats based on mast cell activation and stem cell factor(SCF)/receptor tyrosine kinase c-Kit signaling pathway.METHODS:Sixty SD rats were randomly divided into control group,model group,ranitidine hydrochloride capsule group,and low-,medium-and high-dose WWYD groups,with 10 rats in each group.The rat model of functional dyspepsia was established by tail clamping and irregular feeding compound senna method.After modeling,the rats in con-trol group and model group were given normal saline,while those in low-,medium-and high-dose(0.743 g/mL,1.485 g/mL and 2.970 g/mL)WWYD groups and ranitidine hydrochloride capsule(3 g/L)group were treated with corresponding drugs by intragastric administration.After treatment,the propulsion rate of the small intestine was measured by the carbon ink propulsion method.Rat duodenal mast cells were observed and counted by toluidine blue staining.ELISA was used for determination of mast cell tryptase(MCT)and histamine(HA)content in rat duodenum.The mRNA levels of SCF and c-Kit in duodenum were detected by RT-qPCR.Western blot and immunohistochemistry were employed to determine the ex-pression levels of SCF and c-Kit in the duodenum.RESULTS:Compared with model group,WWYD treatment signifi-cantly increased the propulsion rate of the small intestine in rats(P<0.05).ELISA results showed that WWYD reduced the number of mast cells and the content of MCT and HA in the duodenal mucosa tissue of rats(P<0.05).Western blot and immunohistochemistry results suggested that WWYD up-regulated the protein expression levels of c-Kit and SCF in the duodenal tissue of rats(P<0.05),and increased the numbers of SCF and c-Kit positive cells.RT-qPCR results indicated that WWYD up-regulated the mRNA expression of c-Kit and SCF in the duodenum of rats(P<0.05).Moreover,the small intestinal propulsion rate was negatively correlated with MCT and HA content,and positively correlated with the expres-sion of SCF and c-Kit.CONCLUSION:Wenweiyang decoction promotes rat duodenal motility,and its mechanism may be related to the inhibition of rat duodenal MCT and HA production and activation of SCF/c-Kit signaling pathway.
9.Clinical and genetic characteristics of 17α-hydroxylase/17,20-lyase deficiency in childhood

Objective To explore the clinical and genetic characteristics of 17α-hydroxylase/17,20-lyase deficiency(17OHD)in childhood.Methods The clinical features,laboratory and imaging examination results,gene mutation characteristics of 4 children diagnosed with 17OHD in Children's Hospital Affiliated to Zhengzhou University from January 2016 to December 2022 were retrospectively analyzed,and the literature was reviewed and summarized.Results At the time of diagnosis,the age of 4 children ranged from 11 months and 21 days to 10 years and 6 months.All patients had karyotypes of 46,XY.Social gender:1 male and 3 females.The chief complaints were 1 case of short penis,1 case of inguinal mass,and 2 cases of grade 2 hypertension,and 3 cases of testes were found in scrotum,groin and inguinal annulus,respectively.The levels of cortisol,testosterone,and androstenedione decreased at 8 o'clock in 4 children,while the levels of adrenocorticotropic hormone,progesterone,luteinizing hormone,and follicle stimulating hormone increased.17 hydroxyprogesterone was normal.Mild decrease in blood potassium levels(3.44-3.48 mmol/L)was found in 3 cases.One case of CYP17A1 homozygous mutation and three cases of compound heterozygous mutation were found,among which c.563 A>G and c.436+1G>T were new mutation sites that had not been reported in the past,and 3 cases had c.985_987delinsAA mutation.All 4 cases received oral hydrocortisone treatment.Conclusions Abnormal external genitalia,inguinal/labial mass and hypertension are the main features of 46,XY type 17OHD in childhood.Early hydrocortisone replacement therapy can effectively prevent the complications of 17OHD.The CYP17A1 c.985_987delinsAA mutation may be a hot topic mutation in children with 17OHD in China.
10.Application of vacuum drainage technology in the renovation of radioactive wastewater treatment system in a hospital

Objective:To study the application value of vacuum drainage technology in radioactive wastewater discharge system of hospitals, and provide theoretical basis and reference for relevant institutes to design or renovate their radioactive wastewater discharge systems.Methods:Based on the composition of vacuum drainage system and gravity drainage system, the differences in water consumption and other factors of different systems were theoretically evaluated. To take the radioactive wastewater discharge system in a hospital as a research example, the storable periods of the fixed decay pool were calculated by using the two different drainage techniques.Results:Compared with the traditional gravity drainage technology, the vacuum drainage technology could save a lot of water and significantly reduce wastewater discharge. The water consumption was 16% of that of ordinary toilets with gravity drainage. By using the vacuum drainage technology to reform the radioactive wastewater discharge system of the hospital under the same amount of diagnosis and treatment workload, the storable period of radioactive wastewater could be extended by nearly three times, from 92.5 d before the renovation to 297 d after the renovation, without increasing the capacity of the decay pool. After the transformation, the total activity and volumetric activity concentration of 131I when discharging wastewater from each decay tank could be reduced to 4 096 Bq and 0.17 Bq/L, respectively, and the volumetric activity concentration was much lower than the national emission requirements. Conclusions:Vacuum drainage can significantly reduce wastewater production, and is worth promoting and applying in nuclear medicine departments.
