Objective:To understand the current situation of the specialized nursing alliance team after training, and to provide countermeasures for the further construction of the specialized nurse linkage team in medical institutions.Methods:From April to July in 2024, the descriptive qualitative research method was used, 25 community nursing professionals were interviewed by the objective sampling, and the contents were analyzed and refined by the Colaizzi 7-step analysis method.Results:A total of 25 community specialist nurses were all females, aged 31 - 47 years old. Four themes and 15 sub-themes were extracted: internal benefits after the linkage of the specialized nursing alliance team, external benefits after the linkage of the specialized nursing alliance team, existing difficulties in the construction of the specialized nursing alliance team, and future needs of the specialized nursing alliance team construction. Based on this analysis, the present situation of the team construction of specialized nursing alliance is made.Conclusions:Under the background of medical union, the team construction of specialized nursing alliance meets the needs of patients and policy trends, and has achieved phased results. In the future, it is still necessary to further improve the professional ability of community specialized nurses and strengthen multi-channel sustainable cooperation, including reshaping the structure of medical resources, strengthening capital investment, improving the utilization rate of information technology and strengthening the assessment mechanism to promote the improvement of the specialized nursing alliance team.

Objective:To establish an evaluation index system of home care quality for caregivers of children aged ≤3 years with burns.Methods:The combined approach of qualitative and quantitative research was used, and the fundamental principles of constructing an index system was followed. From January to February 2022, through literature analysis, conducting a semi-structured interview on 6 female nurses aged 31 to 51 years who worked for more than 10 years in the Department of Burns, Plastic Surgery, and Aesthetic Medicine of General Hospital of Ningxia Medical University, as well as 15 caregivers (11 females and 4 males) of hospitalized children with burns aged 28 to 54 years who received professional training from nursing staff, a preliminary evaluation index system of home care quality for caregivers of children ≤3 years old with burns was established using Maslow's hierarchy of needs as theoretical guidance. From March to June 2022, the Delphi method was applied to conduct inquiry by correspondence with 15 experts in relevant fields who met the inclusion criteria and were employed in general hospitals, specialized hospitals, and universities in the Ningxia Hui Autonomous Region (with their general information being recorded), and the evaluation index system was established and the weights of indicators at all levels were determined by the analytic hierarchy process. The effective recovery ratio of the inquiry form was calculated to determine the level of enthusiasm of experts, the average authority coefficient of all indicators was calculated to determine the level of expert authority, the average importance assignment and full score ratio of all indicators were calculated to determine the concentration of expert opinions, and the average Kendall's harmony coefficients and mean coefficient of variation for all indicators were calculated to determine the degree of coordination of expert opinions. The Kendall's harmony coefficients for the first level indicators, the total second level indicators, and the total third level indicators in each round of correspondence inquiry were statistically analyzed with chi-square test.Results:The preliminary evaluation index system of home care quality for caregivers of children ≤3 years old with burns included 5 first level indicators, 13 second level indicators, and 55 third level indicators. The majority of the 15 experts were women, aged 32-56 years, all with a bachelor's degree or above education background, intermediate or above professional titles, and worked for 14-31 years. The effective recovery ratios of the first and second rounds of inquiry forms were 1 (15/15) and 0.93 (14/15), respectively. The average authority coefficients of all indicators in the first and second rounds of correspondence inquiries were 0.87 and 0.90, respectively. The average importance assignment and average full score ratio of all indicators were 4.25 and 0.68 respectively in the first round of inquiry, and 4.39 and 0.70 respectively in the second round of inquiry. In the first and second rounds of inquiries, the average Kendall's harmony coefficients for all indicators were 0.50 and 0.62, respectively, and the average coefficients of variation for all indicators were 0.15 and 0.14, respectively. The Kendall's harmony coefficients of the five first level indicators (physiological needs, safety needs, needs for love and belonging, needs for respect, and needs for self-realization), the total second level indicators, and the total third level indicators in the first and second rounds of correspondence inquiries were statistically significant (with χ2 values of 22.18, 20.36, 23.42, 25.81, 21.44, 124.73, 524.05 and 24.27, 19.85, 22.39, 27.52, 20.83, 102.38, 503.27, respectively, P<0.05). The final evaluation index system of home care quality for caregivers of children ≤3 years old with burns included 5 first level indicators, 13 second level indicators, and 56 third level indicators. The weights of the first level indicators for physiological needs, safety needs, needs for love and belonging, needs for respect, and needs for self-realization were 0.343, 0.371, 0.091, 0.076, and 0.127, respectively. The weights of the second level indicators were 0.024 to 0.192, and the weights of the third level indicators were 0.001 to 0.089. Conclusions:The evaluation index system of home care quality for caregivers of children ≤3 years old with burns established on the basis of Maslow's hierarchy of needs theory is scientific and reliable, and can serve as a reference for home care of children with burns in this age group.

With the accelerating process of aging, the burden of elderly care in China is increasing dramatically. However, the current care models were difficult to meet the multi-level and diversified needs of the elderly. Time banking mutual care model is proposed as an important auxiliary model. Since the elderly in urban communities are the major participants of time banking mutual care model, understanding their willingness to participate is very important for the effective implementation and sustainable development of time banking mutual care model. This paper summarizes the research progress and the major influencing factors of the willingness of the elderly to participate in time banking mutual care model, in order to provide theoretical support for alleviating the increasingly severe burden of elderly care in China and realizing active aging.

Objective To explore the application of Duff in treatment for patients with early stage threat ened abortion.Method 1000 patients with early stage threatened abortion admitted from Jan.2015 to Jun.2016 were selected,and were divided into two groups.All patients were treated by conventional therapy and Duff therapy.Treatment effects of the two groups were observed and compared.Results The differences of clinical characteristics between the two groups had no statistical significance (all P＞0.05).No serious adverse reactions oc curred to either group.There was no significant difference in remission and disappearance time of clinical symptoms between the two groups (t=0.334,0.367,all P＞0.05).The total treatment efficiency was higher in the observation group than in the control group,but the differences had no statistical significance (x2=0.058,/P＞0.05).Conclusion There is a certain application value of Duff treatment for early stage threatened abortion,which is safe,efficient,and convenient,worthy of promotion.

Objective To study the living quality and mental health status in losing-single-child family couples .Methods A total of 841 losing-single-child family couples and 674 family couples with children alive conforming to the entering group condition were random sampled from the three districts of Chongqing main city .The on-site survey was performed by using the general condi-tion questionnaire ,90 Symptom Checklist (SCL-90) ,Self-rating Anxiety Scale (SAS ) ,Self- rating Depression Scale (SDS ) ,Social Support Rating Scale (SSRS) ,Memorial University of Newfoundland Scale of Happiness (MUNSH) and WHO Quality of Life Scale (WHOQOL-BREF) Chinese version .Results There were no differences in gender ,age and culture between the losing-single-child family couples group and family couples with children alive group (P>0 .05);the SCL-90 total score and each factor score , SAS and SDS score in the losing-single-child family couples group were significantly higher than those in family couples with chil-dren alive group(P<0 .05) ,the SSRS score ,MUNSH score and WHOQOL-BREF Chinese version evaluation score were signifi-cantly lower than those in the family couples with children alive group(P<0 .05);the multivariate regression analysis showed that the four regression equations in the social relation field ,environment field ,depression factors and total happiness had statistical sig-nificance (P<0 .05) .Conclusion The living quality ,mental health status and subjective well-being in the losing-single-child family couples are significantly affected ,timely psychological crisis intervention and persistent psychological support and mental health ed-ucation should be give them to improve their psychological health status ,increase the living quality and subjective well-being .

Objective To compare the complications and outcomes of urgent?start peritoneal dialysis (PD) and hemodialysis (HD) in end?stage renal disease (ESRD) patients, and explore the safety and effectiveness of PD which was as an urgent?start dialysis modality in ESRD patients. Methods All patients for urgent?start dialysis, who initiated dialysis without a long?term dialysis access or had the long?term dialysis access under 30 days in Renji Hospital from January 1st 2013 to December 31st 2014, were enrolled. According to the dialysis modalities, patients were divided into PD group and HD group. Participants were followed up until death, transferred to other centers, lost of follow up or January 1st 2016. Dialysis?related complications within 30 days of implantation, complications of reimplantation and the occurrence of bacteremia between two groups were compared, and their survival rates were tested by Kaplan?Meier curves. Results Among 178 patients in this study, there were 96 (53.9%) patients in PD group and 82 (46.1%) patients in HD group. Compared with those of HD group, patients of PD group presented more cardiovascular disease [21(21.9%) vs 8(9.8%), P=0.029], higher serum potassium [(4.5±0.8) mmol/L vs (4.3±0.8) mmol/L, P=0.038], but less heart failure (NYHA Ⅲ?Ⅳ) [26(30.2%) vs 40 (48.8%), P=0.014], lower brain natriuretic peptide (BNP) [328.5 (129.5, 776.8) ng/L vs 503.5(206.0, 1430.0) ng/L, P=0.008], higher hemoglobin [(81.5 ± 17.7) g/L vs (75.3 ± 22.5) g/L, P=0.039], higher serum albumin (33.5±5.7) g/L vs (31.3±6.7) g/L, P=0.022] and higher serum pre?albumin (304.5±78.0) mg/L vs (257.0 ± 86.1) mg/L, P<0.001]. PD group presented less dialysis?related complications [5 (5.2%) vs 20(24.4%), P<0.001], less dialysis?related complications requiring reimplantation [1(1.0%) vs 20(24.4%), P<0.001] and less bacteraemia [3(3.1%) vs 11(13.4%), P=0.011]. The 3?, 6?and 12?month patient survival rates of PD and HD group were 97.9% vs 98.4%, 97.9% vs 98.4%, and 92.1%vs 93.0% respectively, and no significant difference was found (Log ? rank=0.004, P=0.947). Conclusions Patients with urgent?start PD have less complications within 30 days of implantation and occurrence of bacteremia than patients with urgent?start HD, and the same survival rates. PD may be a feasible and safe urgent?start dialysis modality for ESRD patients.

Objective To investigate the health-related quality of life (HRQoL) and its influencing factors in patients with gastric remnant cancer (GRC).Methods A total of 130 patients received gastrectomy more than 10 years ago because of early gastric cancer.According to the gastric remnant canceration after the surgery,patients were divided into GRC group (80 cases) and gastric remnant group (50 cases).Both the patients of these two groups accepted questionnaires of cancer patients' quality of life questionnaire (QLQ) C30 and gastric cancer specific questionnaire QLQ-STO22,the HRQoL were evaluated.The comparison between two groups with normal distribution of data was aralyzed by t test.Wilcoxon rank sum test was applied for the comparison between two groups with non-normal distribution of data.Optimal scaling regression analysis was used for screening HRQoL related clinical influencing factors.Results All the research objectives finished the questionnaires survey.Compared with health control group,the scores of GRC group were lower,which included quality of life evaluation,physical function,role function,emotional function,cognitive function and social function,and the differences were statistically significant (t =-6.678,-7.111,-10.605,-5.748,-4.765 and-21.170,all P＜0.01).The scores of fatigue,pain,diarrhea,nausea and vomiting,loss of appetite,economic difficulties,dysphagia,abdominal pain,reflux symptoms,diet restricted,anxiety,physical appearance and hair loss were higher,and the differences were significant (t=6.925,4.218,4.728 and Z=-5.236,-7.890,-7.698,-10.058,-3.612,-6.914,-9.711,9.940,-7.987 and-4.966,all P＜0.01).Compared with gastric remnant group,the scores of GRC group were lower,which included quality of life evaluation,physical function,role function,emotional function,cognitive function and social function,and the differences were statistically significant (t=-5.861,-5.821,-7.077,-7.999,-2.808and-5.710,all P＜0.01).However the scores of fatigue,pain,diarrhea,nausea and vomiting,loss of appetite,economic difficulties,dysphagia,diet restricted,anxiety,physical appearance and hair loss were higher,and the differences were significant (t=9.363,6.842 and Z=-2.654,-6.256,-3.266,-4.132,-2.854,-7.996,-4.258 and-2.005,all P＜0.01).Compared with health control group,the score of social function of gastric remnant group was lower and the difference was significant (t =-9.820,P＜ 0.01); the scores of diarrhea,nausea and vomiting,economic difficulties,dysphagia,reflux symptoms,diet restricted,physical appearance and hair loss were higher,the differences were statistical significant (t=3.020 and Z=-1.981,-3.775,-6.505,-6.098,-8.032,-3.369 and-3.147,all P＜0.05) ; the symptom scores of fatigue and pain were lower,the differences were significant (t=-2.890 and-2.439,all P＜0.05).HRQoL of patients was positively correlated with education degree,marital status,family income and the disease course of gastric remnant.Conclusions HRQoL of GRC patients decreased to certain degree.Education degree,marital status,family income and the disease course of gastric remnant were the influential factors of HRQoL in GRC patients.
