1.Influence of family empowerment-based supervised follow-up nursing on patients with inflammatory bowel disease

Objective:To explore the impact of family empowerment-based supervised follow-up nursing on the self-care ability and quality of life of patients with inflammatory bowel disease (IBD), and to provide references for improving the treatment attitudes and compliance behaviors of patients.Methods:A randomized controlled study was conducted. Convenience sampling was used to select 100 IBD patients admitted to the Department of Gastroenterology at Lishui Central Hospital from January 2021 to June 2023. The patients were randomly divided into a study group (receiving family empowerment-based supervised follow-up nursing) and a control group (receiving routine continuing care). Before the intervention and three months after the intervention, the self-care ability, medication compliance, disease control effect, and quality of life were compared between the two groups.Results:A total of 99 IBD patients were included, with 50 in the study group, 27 males and 23 females, aged (39.55 ± 5.21) years old, and 49 in the control group, 28 males and 21 females, aged (40.24 ± 7.16) years old. Before the intervention, there were no statistically significant differences in self-care ability, medication compliance, disease control effect, and quality of life between the two groups (all P>0.05). After three months of intervention, the self-care ability, medication compliance and quality of life scores in the study group were (141.27 ± 13.48), (91.02 ± 4.69), (185.21 ± 9.51) points, which were higher than the control group (120.17 ± 11.08), (85.69 ± 4.49), (173.58 ± 9.27) points, respectively. The differences were statistically significant ( t =-8.50, -5.77, -6.16, all P<0.05). The disease control effect score in the study group was (100.77 ± 9.42) points, lower than the control group (125.99 ± 7.87) points, the differences was statistically significant ( t =14.44, P<0.05). Conclusions:Family empowerment-based supervised follow-up nursing can improve the self-care ability, medication compliance, disease control effect, and quality of life in IBD patients.
2.Therapeutic effect and mechanism of individualized treatment for hypertension patients with coronary heart disease under non-invasive cardiac output measurement

Objective To investigate the efficacy and mechanism of individualized treatment for hypertension patients with coronary heart disease under non-invasive cardiac output measurement.Methods A total of 94 hypertension patients with coronary heart disease in the hospital from January 2020 to August 2022 were selected and randomly divided into two groups,with 47 cases in each group.Control group was treated with conventional treatment,while observation group was treated with individualized treatment under non-invasive cardiac output measurement,and both group were continuously treated for 3 months.Clinical efficacy,the blood pressure control[diastolic blood pres-sure(DBP),systolic blood pressure(SBP)],cardiac ultrasound parameters[left ventricular end-systolic diameter(LVESD),left ventricular mass index(LVMI),interventricular septal thickness(IVS),left ventricular end-diastolic diameter(LVEDD)],heart rate variability[the standard devia-tion of the average RR interval(SDANN),the standard deviation of the RR interval of all sinus beats(SDNN),and the proportion of adjacent NN>50 ms in the number of sinus beats(PNN50)],indi-cators of myocardial injury[α-hydroxybutyrate dehydrogenase(α-HBDH),lactate dehydrogenase(LDH),creatine kinase(CK)],transforming growth factor-β1(TGF-β1)/pancreatic cancer-deficiency factor(Smads)signaling pathway,and incidence of major adverse cardiovascular events(MACE)were compared between the two groups.Results The total effective rate of the observation group was 95.74%,which showed no significant difference compared with 85.11%of the control group(P>0.05).After 1 and 3 months of treatment,the levels of DBP and SBP in both groups were de-creased significantly compared with those before treatment(P<0.05),and the LVMI,IVS,LVEDD and LVESD in the observation group were significantly lower than those in the control group(P<0.05).After 3 months of treatment,the SDANN,PNN50 and SDNN in observation group were significantly higher than those in control group(P<0.05),while CK,α-HBDH and LDH as well as the expression levels of TGF-β1R Ⅰ,TGF-β1R Ⅱ,Smad1 and Smad2 were significantly lower than those in the control group(P<0.05).After 3 months of follow-up,the incidence rate of MACE in observation group was 6.38%,which showed no significant difference compared with 14.89%in control group(P>0.05).Conclusion Compared with conventional treatment regi-men,individualized treatment regimen with non-invasive cardiac output measurement can signifi-cantly alleviate myocardial injury,reverse ventricular hypertrophy and improve heart rate variability,and its mechanism may be related to the inhibition of the TGF-β1/Smads signaling pathway.

4.Application of Duff in treatment for early stage threatened abortion

Objective To explore the application of Duff in treatment for patients with early stage threat ened abortion.Method 1000 patients with early stage threatened abortion admitted from Jan.2015 to Jun.2016 were selected,and were divided into two groups.All patients were treated by conventional therapy and Duff therapy.Treatment effects of the two groups were observed and compared.Results The differences of clinical characteristics between the two groups had no statistical significance (all P>0.05).No serious adverse reactions oc curred to either group.There was no significant difference in remission and disappearance time of clinical symptoms between the two groups (t=0.334,0.367,all P>0.05).The total treatment efficiency was higher in the observation group than in the control group,but the differences had no statistical significance (x2=0.058,/P>0.05).Conclusion There is a certain application value of Duff treatment for early stage threatened abortion,which is safe,efficient,and convenient,worthy of promotion.
5.The correlation of notch-1 expression and comprehensive analysis with prognosis in small cell lung cancer

Objective To investigate the clinical significance of Notch-1 expression in small cell lung cancer(SCLC) and non-small cell lung cancer(NSCLC),and analyze the role of it in prognosis.Methods SP immunohistochemistry was used to detect the expression of Notch-1 antibody in 43 of SCLC and 40 of NSCLC tissues.Further analysis was carried out to interpret the association of Notch-1 antibody expression with clinicopathological features,lymph node metastasis and prognosis in SCLC.Results The positive rate of Notch-1 expression was 20.93% ( 9/43 ) in SCLC,while 65.00% ( 26/40 ) in NSCLC.The expression of Notch-1 antibody was associated with clinical stage and lymph node metastasis ( x2 =5.42,P < 0.05 ; x2 =4.88,P < 0.05respectively),but was not associated with age,sex,tumor location,tumor size ( Ps > 0.05 ).Compared with NSCLC,the expression rates of Notch-1 antibody were significantly higher in SCLC ( x2 =16.50,P < 0.05 ).Kaplan-Meier survival analysis indicated that the survival time of patients with positive Notch-1 expression was significantly longer than that of patients with negative staining( x2 =19.87,P < 0.05 ).Cox regression analysis showed that Notch-1 antibody could significantly reduce the risk of death in patients with SCLC.Conclusion The positive expressions of Notch-1 were significantly different in SCLC and NSCLC,which linked to the clinicalstage,lymph node metastasis and poor prognosis.Accordingly,the expression of Notch-1 may have good value in diagnosis and prognosis.
