1.Effect of transcutaneous auricular vagus nerve stimulation on patients with prolonged disorders of consciousness

Objective To investigate the effect of transcutaneous auricular vagus nerve stimulation(ta-VNS)on the recovery of con-sciousness in patients with prolonged disorders of consciousness(pDoC).Methods From January,2023 to August,2024,50 patients with pDoC in Zhejiang Provincial People's Hospital were ran-domly divided into control group(n=25)and observation group(n=25).Both groups received conventional wakefulness-promoting treatment and Xingnao Kaiqiao acupuncture,and the observation group added ta-VNS treatment on the basis of the control group,for four weeks.Each group was further divided into unresponsive wakefulness syndrome/vegetative state(UWS/VS)and minimally consciousness state(MCS)types based on their initial the score of Coma Recovery Scale-Revised(CRS-R).They were assessed with Glasgow Coma Scale(GCS),CRS-R,EEG grading and upper limb somatosensory-evoked potentials(SSEP)before and four weeks af-ter treatment.Results Three in the observation group and five in the control group dropped down for transferring to other hospitals.There was no significant difference in all the indexes between two groups before treatment(P>0.05).After treat-ment,the GCS score(|t|>16.000,P<0.001),CRS-R score(|t|>14.318,P<0.001)and N20 amplitude of SSEP(|t|>5.247,P<0.05)improved,the EEG grading was reduced(Z>2.264,P<0.05),and the N20 latency of SSEP was shortened(|t|>2.884,P<0.05)in both UWS/VS and MCS type patients in the observation group;and they were better in the observation group than in the control group for the same type(|t|>1.883,|Z|>2.244,P<0.05).Conclusion ta-VNS can significantly promote the awakening of patients with pDoC from UWS/VS and MCS,and im-prove the functional status of electroencephalogram activities and sensorimotor conduction pathways.
2.Application of transcranial magnetic stimulation in rehabilitation field:a bibliometric analysis from 2014 to 2024

Objective To analyze the research hotspots and frontier trends of transcranial magnetic stimulation(TMS)in the field of rehabilitation in the last decade.Methods The literatures of TMS in the field of rehabilitation from January,2014 to September,2024 were searched in the database of Web of Science core collection,and the visual analysis was carried out by using CiteSpace 6.4.R1 software.Results A total of 1 065 articles were included,showing an increasing trend in annual publication volume.China was the leading country in publication volume,Harvard University was the institution with the largest number of pub-lished articles,and Simone Rossi was the most frequently cited author.Keywords with high co-occurrence fre-quency included stroke,noninvasive brain stimulation,plasticity,cortical excitability,θ burst stimulation and up-per limb.Brain-computer interface was the bursting keyword in the past two years.Conclusion In the past decade,the research interest in TMS in the field of rehabilitation has been on the rise.It focused on the application of multimodal combined therapies,including TMS,to improve the upper limb function and aphasia of stroke patients and to explore the plasticity of brain nerves.Future research should focus on the deep integration of TMS and brain-computer interface technology to realize the individualization and accuracy of TMS treatment.
3.Clinical Observation on Modified Danggui Yinzi Combined with Thumb-Tack Needling for Subcutaneous Embedding in the Treatment of Chronic Spontaneous Urticaria of Wind Dryness Due to Blood Deficiency Type

Objective To observe the clinical efficacy of modified Danggui Yinzi combined with thumb-tack needling for subcutaneous embedding in the treatment of chronic spontaneous urticaria(CSU)of wind dryness due to blood deficiency type.Methods A total of 68 cases of patients admitted to the dermatology wards and outpatient clinics of The Seventh Clinical School of Guangzhou University of Chinese Medicine with a definitive diagnosis of CSU of wind dryness due to blood deficiency type were selected as the study subjects from July 2023 to February 2024.The patients were randomly divided into the observation group and the control group according to the random number table method,with 34 cases in each group.The control group was treated with Cetirizine Dihydrochloride Tablets,and the observation group was treated with modified Danggui Yinzi combined with thumb-tack needling for subcutaneous embedding based on the treatment of the control group,the course of treatment covered four weeks.After one month of treatment,the clinical efficacy of the two groups was evaluated,and the changes in Urticaria Activity Score(UAS)and Dermatology Life Quality Index(DLQI)scores and traditional Chinese medicine(TCM)syndrome scores before and after treatment were observed in the two groups.The changes of serum total immunoglobulin E(total IgE)levels before and after treatment in the two groups were compared.The safety and recurrence cases of the two groups were evaluated.Results(1)The total effective rate was 91.18%(31/34)in the observation group and 70.59%(24/34)in the control group.The efficacy of the observation group was superior to that of the control group,the difference being statistically significant(P<0.05).(2)After two,three,and four weeks of treatment,the UAS scores of the patients in the two groups were improved significantly(P<0.05),and the improvement in the observation group was significantly superior to that in the control group,the difference being statistically significant(P<0.01).(3)After treatment,the DLQI scores and TCM syndrome scores of the patients in the two groups were improved significantly(P<0.05),and the improvement in the observation group was significantly superior to that in the control group,the difference being statistically significant(P<0.01).(4)After treatment,the total IgE levels of patients in the two groups were improved significantly(P<0.05),and the improvement in the observation group was significantly superior to that in the control group,the difference being statistically significant(P<0.01).(5)The comparison of the incidence of adverse reactions between the two groups showed that the difference being not statistically significant(P>0.05).(6)In the follow-up after the completion of treatment,the recurrence rate was 5.88%(2/34)in the observation group and 23.53%(8/34)in the control group,and the recurrence rate of the observation group was significantly lower than that of the control group,the difference being statistically significant(P<0.05).Conclusion Modified Danggui Yinzi combined with thumb-tack needling for subcutaneous embedding in the treatment of CSU of wind dryness due to blood deficiency type can significantly alleviate the clinical symptoms of the patients,improve their quality of life,and reduce the recurrence rate.

6.Prediction of short-term outcome after subacute ischemic stroke using multiple layer perceptron neural network

Objective To establish a predictive model using multiple layer perceptron (MLP) for short-term outcome after subacute ischemic stroke.Methods From January, 2019 to September, 2021, 60 readmission-inpatients in Department of Rehabilitation, Ruijin Hospital, Shanghai Jiaotong University School of Medicine were collected the clinical features of first admission (less than 30 days after attack), and the outcomes were assessed with modified Rankin Scale (MRS) three months after the first admission. The risk factors were screened with single factor analysis, and the short-term outcome predictive models were established with multi-factor Logistic regression and MLP. The predictive accuracy of both models was calculated, and the predictive effects were compared with Receiver Operating Characteristic (ROC) curve.Results For multi-factor Logistic regression, the predictive accuracy was 73.3%, and the area under ROC curve was 0.851. For MLP, the predictive accuracy was 88.9%, and the area under the ROC curve was 0.930.Conclusion The prediction of short-term outcome after subacute ischemic stroke can be done with MLP model.
7.Advance in Hoffa's Disease and Substance P Positive Nerve Fibers (review)

Hoffa's disease is the clinical condition characterized by anterior knee joint pain secondary to inflammation and/or impingement of the infrapatellar fat pad. The substance P positive nerve fibers rich in the infrapatellar fat pad were stimulated by the noxious stimuli,which may associated with the pain of Hoffa's disease. The relationship between Hoffa's disease and substance P positive nerve fibers in the infrapatellar fat pad was reviewed in this paper.
