1.Epidemiological characteristics of severe fever with thrombocytopenia syndrome cases in Xianju County, Taizhou City of Zhejiang Province from 2017 to 2024

ObjectiveTo analyze the epidemiological characteristics and clinical symptoms of severe fever with thrombocytopenia syndrome (SFTS) cases in Xianju County, Zhejiang Province from 2017 to 2024, and to provide a scientific basis for prevention and control measures to reduce the incidence and mortality. MethodsSFTS cases reported in Xianju County from 2017 to 2024 were collected from the China Information System for Disease Prevention and Control. Case-related data were obtained through epidemiological investigations and medical record reviews. Data were organized and analyzed using WPS 2019 and SPSS 22.0, spatial maps were generated with ArcGIS 10.2, and multivariate logistic regression analyses were employed to explore the factors associated with mortality in SFTS cases. ResultsFrom 2017 to 2024, a total of 36 SFTS cases were reported in Xianju County, with an incidence rate of 1.15/100 000 and a case fatality rate of 27.78% (10/36). The majority of cases occurred in individuals aged 60 years old and above (55.56%, 20/36), with farmers being the most affected occupational group (86.11%, 31/36). The highest number of reported cases occurred in June and July. The local tick density was relatively high (1.25 ticks per host, 340/271). Multivariate logistic regression analyses indicated that thrombocytopenia was a related factor for patient mortality. ConclusionXianju County is an area with high incidence and case fatality rate of SFTS. It is essential to strengthen public awareness campaigns, emphasizing personal protection during outdoor activities to reduce infections and lower the incidence rate. In addition, comprehensive control measures such as environmental cleanup and pesticide application should be taken to reduce tick density. Local medical institutions should enhance professional training to improve diagnostic and report sensitivity for SFTS. Early symptomatic treatment should be prioritized for elderly patients and severe thrombocytopenia cases to reduce the case fatality rate.
2.Causal Relationship Between Colorectal Cancer and Common Psychiatric Disorders: A Two-sample Mendelian Randomization Study

Objective To elucidate the causal relationships between colorectal cancer (CRC) and prevalent psychiatric disorders through a two-sample Mendelian randomization approach. Methods Utilizing publicly available genome-wide association study data, we explored the connections between CRC and various psychiatric disorders, including depression, anxiety, bipolar disorder, and schizophrenia. We applied three statistical analyses: inverse variance weighting, MR-Egger, and median weighting. Sensitivity analyses were conducted to ensure the reliability and validity of the results. Results Inverse variance weighting analysis showed no significant links between CRC and depression (P=0.090), anxiety (P=0.099), or schizophrenia (P=0.899). Conversely, a significant inverse relationship was found with bipolar disorder (P=0.010). Conclusion No causal connection exists between CRC and the psychiatric conditions of depression, anxiety, or schizophrenia. However, CRC may have a causal association with a reduced risk of bipolar disorder, further supporting the existence of the gut-brain axis.
3.Study on Common TCM Syndromes of Colorectal Cancer Based on Latent Structure Model Combined with Clustering Analysis

Objective To explore the distribution characteristics of common TCM syndromes in colorectal cancer;To provide a basis for standardized research on syndrome of colorectal cancer.Methods The literature related to TCM syndromes of colorectal cancer was retrieved from CNKI,Wanfang Data,VIP and SinoMed.Information on syndrome types and symptoms was extracted to establish a traditional Chinese medicine syndrome database for colorectal cancer.Frequency statistics were used to analyze distribution of colorectal cancer syndrome types and their symptom characteristics.Lantern 5.0 software was used to establish a latent structure model based on the LTM-EAS algorithm,and factor analysis and systematic clustering analysis were conducted using SPSS 27.0 software to infer potential patterns.Based on the results of comprehensive frequency statistics,systematic clustering and latent structure analysis,common TCM syndrome types and symptom characteristics of colorectal cancer were obtained.Results A total of 929 articles were included,totaling 2 465 syndrome items,involving 97 syndrome types,with high frequency of qi-blood deficiency syndrome,dampness and heat accumulation syndrome,spleen-kidney yang deficiency syndrome,etc.,and 281 symptoms,including abdominal distension,anorexia,abdominal pain and mental fatigue,etc.The latent structure analysis obtained 23 hidden variables,and 8 common syndromes were obtained according to professional knowledge.Factor analysis obtained 17 common factors,and further systematic cluster analysis inferred 9 potential syndrome types.Conclusion Common syndrome of colorectal cancer can be divided into 9 types,which are dampness and heat accumulation syndrome,blood stasis toxicity internal obstruction syndrome,qi stagnation and blood stasis syndrome,dampness and heat stasis toxicity syndrome,spleen deficiency and phlegm-dampness syndrome,spleen qi deficiency syndrome,qi and blood deficiency syndrome,liver-kidney yin deficiency syndrome,and spleen-kidney yang deficiency syndrome.The symptom characteristics of each syndrome are significant,which can provide reference for clinical differentiation and lay foundation for standardized research on syndrome.
4.Comparative Analysis of Posterior Nasal Nerve and Anterior Ethmoidal Nerve Ablation for Nasal Septal Deviation Complicated With Moderate to Severe Allergic Rhinitis

Objective To investigate clinical therapeutic value of combined posterior nasal nerve and anterior ethmoidal nerve ablation in patients with nasal septal deviation and moderate to severe persistent allergic rhinitis.Methods A total of 47 patients diagnosed with nasal septal deviation and moderate to severe persistent allergic rhinitis from April to December 2024 were divided into two groups.The control group(n=22)underwent septoplasty and bilateral inferior turbinate out-fracture,with postoperative symptom control managed by budesonide nasal spray.The experimental group(n=25)received the same septoplasty and turbinate surgery,supplemented by bilateral posterior nasal nerve and anterior ethmoidal nerve ablation.Symptom improvement was compared between groups by using the Visual Analogue Scale(VAS)and Rhinoconjunctivitis Quality of Life Questionnaire(RQLQ)scores preoperatively and3 months postoperatively.Results The operation time of the control group was(36.3±5.9)min,significantly shorter than that of the experimental group[(59.4±6.6)min,t=12.496,P=0.000].Both groups of patients showed significant improvement in VAS and PQLQ scores for nasal symptoms such as nasal congestion,runny nose,sneezing,and itching at 3 months after surgery compared to preoperative levels(all P=0.000).The mean VAS score of the four symptoms in the control group was(4.6±0.9)points at3 months after surgery,which was significantly lower than that before surgery[(6.3±1.1)points,t=9.796,P=0.000].The mean VAS score of the four symptoms in the experimental group was1.0(0.3-4.3)points at3 months after surgery,which was significantly lower than that before surgery[7.0(4.5-9.0)points,Z=-4.376,P=0.000].The improvement rate of the VAS score in the experimental group was(82.4±14.2)%,significantly higher than that in the control group[(26.9±11.7)%,t=14.510,P=0.000].At3 months after surgery,the RQLQ score of the control group[(2.3±0.8)points]was significantly lower than that before surgery[(3.3±0.8)points,t=10.055,P=0.000].The RQLQ score of the experimental group after surgery was 1.4(0.8-3.5)points,which was significantly lower than the preoperative score[3.6(1.5-6.1)points,Z=-4.373,P=0.000].The improvement rate of RQLQ score in the experimental group was(53.0±14.6)%,significantly higher than that in the control group[(30.2±13.4)%,t=5.555,P=0.000].Conclusion Postnasal nerve and anterior ethmoidal nerve ablation combined with nasal septal deviation correction can significantly improve nasal symptoms and quality of life in patients with nasal septal deviation complicated with moderate to severe allergic rhinitis compared with simple nasal septal deviation correction,but the operation time is prolonged.
5.DiaSphere embolized microsphere TACE for treating primary hepatocellular carcinoma:A prospective multicenter randomized controlled study

Objective To observe the effectiveness and safety of DiaSphere embolized microsphere TACE for treating primary hepatocellular carcinoma(HCC).Methods Totally 188 patients with HCC were prospectively enrolled and randomly assigned to research group(n=93)and control group(n=95),who underwent TACE with DiaSphere embolized microspheres and Embosphere embolized microspheres,respectively.The incidence of TACE-related adverse events were recorded.The therapeutic efficacy 1 month after the first TACE,also 1 and 3 months after the last TACE,and liver functions 1 month after the first and last TACE were compared between groups.Results In research group,there were 69 cases underwent 1 time TACE,22 cases underwent 2 times and 2 cases underwent 3 times TACE,while in control group,there were 82 cases underwent 1 time and 13 cases underwent 2 times TACE,respectively.No statistical difference of the incidence of adverse events was found between groups(77.42%[72/93]vs.76.84%[73/95],P=1.000).One month after the first TACE,7 cases in research group and 11 cases in control group were lost to follow-up,respectively.One month after the last TACE,12 cases were lost to follow-up in both groups,and 3 months after the last TACE,28 cases were lost to follow-up in both groups.No significant difference of objective response rate nor disease control rate was found between groups at the above time points(all P>0.05).One month after the first and last TACE,liver function indicators were not different between groups(all P>0.05).Conclusion Both the short-term efficacy and safety of TACE with DiaSphere embolized microspheres for treating HCC were good.
6.Research Progress on the Antitumor Effects and Mechanisms of Qi-Regulating and Detoxifying Tradi-tional Chinese Medicine

Malignant tumors represent a significant contributor to the increasing global burden of disease.Contemporary medical therapies often face challenges such as high recurrence rates and considerable adverse effects,making the exploration of more efficient and safer treatment strategies for cancer patients a current research priority.Traditional Chinese Medicine(TCM)has demonstrated unique value in oncology,exhibiting comprehensive advantages including multi-target effects,diverse active components,and low drug resistance,particularly in enhancing efficacy,reducing toxicity,improving prognosis,and elevating patients'quality of life.Through a literature review and based on the Cancer Toxin pathogenesis theory,this article systematically elaborates on the antitumor effects and underlying mechanisms of Qi-regulating and detoxifying Chinese medicines,aiming to provide new insights and scientific evidence for the TCM-based diagnosis and treatment of malignant tumors.
7.Ten new lignans with anti-inflammatory activities from the leaves of Illicium dunnianum.

The anti-inflammatory phytochemical investigation of the leaves of Illicium dunnianum (I. dunnianum) resulted in the isolation of five pairs of new lignans (1-5), and 7 known analogs (6-12). The separation of enantiomer mixtures 1-5 to 1a/1b-5a/5b was achieved using a chiral column with acetonitrile-water mixtures as eluents. The planar structures of 1-2 were previously undescribed, and the chiral separation and absolute configurations of 3-5 were reported for the first time. Their structures were determined through comprehensive spectroscopic data analysis [nuclear magnetic resonance (NMR), high-resolution electrospray ionization mass (HR-ESI-MS), infrared (IR), and ultraviolet (UV)] and quantum chemistry calculations (ECD). The new isolates were evaluated by measuring their inhibitory effect on NO in lipopolysaccharide (LPS)-stimulated BV-2 cells. Compounds 1a, 3a, 3b, and 5a demonstrated partial inhibition of NO production in a concentration-dependent manner. Western blot and real-time polymerase chain reaction (PCR) assays revealed that 1a down-regulated the messenger ribonucleic acid (mRNA) levels of tumor necrosis factor α (TNF-α), interleukin-6 (IL-6), COX-2, and iNOS and the protein expressions of COX-2 and iNOS. This research provides guidance and evidence for the further development and utilization of I. dunnianum.

10.Air pollution and adult hospital admissions for ischemic stroke: a time-series analysis in Inner Mongolia, China.

BACKGROUND:
Previous studies have demonstrated that short-term exposure to ambient particulate matter elevates the risk of ischemic stroke in major urban areas of various countries. However, there is a notable gap in research focusing on remote areas inhabited by ethnic minorities and the cumulative effects of air pollutants. Our study conducted in the area aims to explore the potential association between ischemic stroke and air pollutants and contribute to improving health outcomes among the community.
METHODS:
This retrospective observational study was conducted at the Xing'an League People's Hospital in Inner Mongolia. The medical records of 4,288 patients admitted for IS between November 1, 2019, and October 31, 2020, were reviewed. Data on demographics (age and sex), air pollutants (PM10, PM2.5, NO2, NO, CO, and O3), and meteorological factors (daily average temperature, daily average wind speed, and daily average atmosphere pressure) were collected and analyzed. The statistical analysis included descriptive statistics, Poisson distribution analysis to evaluate the adverse effects of atmospheric pollutants on daily hospitalizations, and subgroup analysis to determine whether gender and age could modify the impact on hospitalizations.
RESULTS:
A substantial correlation was revealed in single-day lags model. The peak delayed effects of PM10, PM2.5, SO2, and NO2 were observed at lag8 (PM10 (OR = 1.016, 95%CI 1.002, 1.030), PM2.5 (OR = 1.027, 95%CI 1.007, 1.048), SO2 (OR = 1.153, 95%CI 1.040, 279) and NO2 (OR = 1.054, 95%CI 1.005, 1.105)) while males exhibited a consistent trend from lag0 to lag8 (PM10 (OR = 1.035, 95%CI 1.018, 1.053), PM2.5 (OR = 1.056, 95%CI 1.030, 1.082), SO2 (OR = 1.220, 95%CI 1.072, 1.389), NO2 (OR = 1.126, 95%CI 1.061, 1.120), CO (OR = 10.059, 95%CI 1.697, 59.638) and O3 (OR = 0.972, 95%CI 0.946, 0.999)). When gender and age were considered, a positive impact was also observed after three days cumulative effect in males.
CONCLUSIONS
There is a significant cumulative effect of exposure to air pollution on IS hospital admissions, especially the males and patients under the age of 65. Our results also suggested that a notable association between CO and NO2 in two-pollutant models.
