Objective To investigate the effect of different decompression methods on the efficacy of the unilateral biportal endoscopy(UBE)in the treatment of severe lumbar spinal stenosis(LSS).Methods A total of 203 elderly patients with LSS who underwent UBE treatment in our hospital from September 2021 to June 2024 were selected and divided into the unilateral group(102 cases)and the bilateral group(101 cases)according to different decompression methods. The unilateral group underwent unilateral decompression during operation, while the bilateral group underwent bilateral decompression(with bilateral decompression through the same incision and unilateral approach)during operation. The perioperative conditions, anatomical image parameters, visual analogue scale(VAS)scores,Oswestry disability index(ODI),and complications were compared between the two groups. The rehabilitation progress and complications of patients with different Charlson comorbidity index(CCI)values were compared. Results The operation time of patients in the bilateral group was longer than that in the unilateral group(P<0.05).The dural sac area and lumbar spinal canal area of patients in the bilateral group were larger than those in the unilateral group immediately after operation and 3 months after operation(P<0.05).The VAS scores and ODI 7 days and 3 months after operation of patients in the bilateral group were lower than those in the unilateral group(P<0.05).There was no statistically significant difference in the total incidence of complications between the two groups of patients(P>0.05).The first time of getting out of bed after operation and hospital stay in patients with CCI value>2 scores were longer than those in patients with CCI value≤2 scores(P<0.05).Conclusion Compared with unilateral decompression, bilateral decompression for elderly patients with severe LSS takes a longer surgical time, but it can better restore lumbar shape, alleviate pain, improve lumbar function, and has higher safety, and the milder the patient's condition, the faster the postoperative recovery.

Endometriosis(EMs)is a common estrogen-depend-ent clinical disease with the pathological characteristics of malig-nant tumors, which has great impact on women's physical and mental health. In recent years, experimental exploration has re-vealed that ectopic foci are in a hypoxic environment outside the uterine cavity, and mitochondria,as the"functional factories"of the cells, play an important role in the process of planting and in-vasion,and the mitochondrial quality control system, which in-cludes mitochondrial oxidative stress,kinetics,autophagy,bio-genesis and calcium homeostasis,is a key mechanism for the e-quilibrium of the mitochondrial function. The mitochondrial quality control system, including mitochondrial oxidative stress kinetics, autophagy, biogenesis and calcium homeostasis,is a key mechanism for mitochondrial functional balance. Therefore,to clarify the role of the mitochondrial quality control system in the development of EMs with the help of rational and rigorous experi-mental and clinical studies can not only help to clarify the patho-genesis of the disease, but also explore the key targets in the prevention and treatment of the disease. Therefore, this article summarizes the research progress of mitochondrial quality control system in endometriosis, with a view to providing reference and theoretical basis for the etiology, pathogenesis and prevention strategies of EMs.

Drug clinical trial is a method of experimental epidemiology to evaluate the effectiveness and safety of medicines. This article introduced the types and design thought of innovation in drug clinical trial design, and provided methodological reference for related researches.Adaptive design is a complex and innovative clinical trial design, which can be divided into group sequential design, sample size re-estimation,seamless trial, enrichment design and master protocol design(basket trial, umbrella trial, platform trial, etc.)according to the purpose of adaptability. The adaptive design has greater adjustment flexibility, which overcomes the shortcomings of conventional clinical trials to a certain extent, then improves the validity of the trial results and the strength of the evidence. The design innovation and remodeling of drug clinical trials will provide more powerful evidence-based evidence for the realization of precision medicine.

Aim To investigate the intestinal protection and possible mechanism of magnolol(MG)in newborn rats with necrotizing enterocolitis(NEC).Methods The rats were randomly divided into control group(Ctrl group),model group(NEC group)and treatment group(MG group). The NEC model was induced by hypoxia, cold stimulation, deep formula milk and LPS intragastric administration in 7-day-old rats for four days. They were killed after five days of treatment with MG(20 mg·kg-1).HE staining was used to observe the intestinal pathological injury.Western blot was used to detect the expressions of IL-1 β,TNF-α,NL-RP3,ASC,caspase-1 and tight junction protein in the distal ileum of rats.Colon contents were collected for 16S rDNA sequencing to understand the gut microbio-ta.Results MG improved the body mass and intesti-nal injury of NEC neonatal rats. The expressions of in-testinal IL-1β,TNF-α,NLRP3,ASC and caspase-1 proteins were down-regulated,and the expressions of Claudin,Occludin and ZO-1 proteins were up-regula-ted.16S rDNA showed that MG increased the diversity of intestinal flora, and at the phylum level,MG in-creased the abundance of firmicutes and bacteroides in NEC model, and decreased the abundance of pro-teobacteria.At the genus level,MG treatment in-creased the abundance of Lactobacillus,unclassified_Muribaculaceae,Racteroides,but decreased the abun-dance of Escherichia_Shigella,Rodentibacter and Fuso-bacterium.Conclusion MG intervention can protect the intestinal tract of NEC rats by potentially improving barrier function, and regulating the intestinal microbiota through the NLRP3/ASC/caspase-1 signaling pathway.

Background:
and Purpose Symptomatic intracranial hemorrhage (sICH) following endovascular thrombectomy (EVT) is a severe complication associated with adverse functional outcomes and increased mortality rates. Currently, a reliable predictive model for sICH risk after EVT is lacking.
Methods:
This study used data from patients aged ≥20 years who underwent EVT for anterior circulation stroke from the nationwide Taiwan Registry of Endovascular Thrombectomy for Acute Ischemic Stroke (TREAT-AIS). A predictive model including factors associated with an increased risk of sICH after EVT was developed to differentiate between patients with and without sICH. This model was compared existing predictive models using nationwide registry data to evaluate its relative performance.
Results:
Of the 2,507 identified patients, 158 developed sICH after EVT. Factors such as diastolic blood pressure, Alberta Stroke Program Early CT Score, platelet count, glucose level, collateral score, and successful reperfusion were associated with the risk of sICH after EVT. The TREAT-AIS score demonstrated acceptable predictive accuracy (area under the curve [AUC]=0.694), with higher scores being associated with an increased risk of sICH (odds ratio=2.01 per score increase, 95% confidence interval=1.64–2.45, P<0.001). The discriminatory capacity of the score was similar in patients with symptom onset beyond 6 hours (AUC=0.705). Compared to existing models, the TREAT-AIS score consistently exhibited superior predictive accuracy, although this difference was marginal.
Conclusions
The TREAT-AIS score outperformed existing models, and demonstrated an acceptable discriminatory capacity for distinguishing patients according to sICH risk levels. However, the differences between models were only marginal. Further research incorporating periprocedural and postprocedural factors is required to improve the predictive accuracy.
