1.Analysis of the Effect and Prognostic Factors of Deep Brain Electrical Stimulation Therapy on Parkinson's Disease Patients with Frozen Gait

Objective:To analyze the effect of deep brain electrical stimulation therapy on Parkinson's disease patients with frozen gait and explore its prognostic factors.Methods:A retrospective study was conducted on 86 Parkinson's disease patients with frozen gait admitted to our hospital from January 2020 to December 2022.All patients received deep brain electrical stimulation therapy.Conduct a 2-year outpatient follow-up for all patients,and analyze the scores of the Parkinson's Disease Rating Scale Part 2(UPDRS Ⅱ),Parkinson's Disease Rating Scale Part 3(UPDRS Ⅲ),Fugl Meyer Lower Limb Motor Function Assessment Scale(FMA),and Schwab&England Daily Activity Scale(S&E)before and after medication opening and closing,6 months after surgery,1 year after surgery,and 2 years after surgery.And based on the 2-year follow-up results of the patients,the prognosis level of the patients was evaluated according to the S&E score during drug closure.60 patients with drug closure S&E score>70 were divided into a good prognosis group,and 26 patients with drug closure S&E score≤70 were divided into a poor prognosis group.Logistic regression model was used to analyze the influencing factors of deep brain electrical stimulation therapy in Parkinson's disease gait freezing patients.Results:At 6 months,1 year,and 2 years after surgery,the UPDRS Ⅱ and UPDRS Ⅲ scores of all patients were lower than those before surgery(P<0.05).The UPDRS Ⅱ and UPDRS Ⅲ scores of patients before surgery,6 months,1 year,and 2 years after surgery were lower than those of patients before surgery(P<0.05);At 6 months,1 year,and 2 years after surgery,the FMA and S&E scores of all patients were higher than those before surgery(P<0.05).There was no significant difference in the FMA and S&E scores between 6 months,1 year,and 2 years after surgery(P>0.05).However,the FMA and S&E scores at 6 months,1 year,and 2 years after surgery were lower than those at the end of surgery(P<0.05);There was no significant difference between the gender,BMI,combined underlying disease,levodopa equivalent dose,preoperative UPDRSⅢ score before the on and off periods,and FMA scores before the on and off periods(P>0.05),Age,Parkinson's disease stage,preoperative UPDRS Ⅱ score between open and off period,and preoperative S&E score between the good prognosis and off prognosis groups(P<0.05);The UPDRS Ⅱ score(95%CI:1.353~5.782,OR value:2.462),S&E score(95%CI:1.658~4.687,OR value:2.789),and S&E score(95%CI:1.265~6.879,OR value:3.645)before opening surgery are independent influencing factors on deep brain electrical stimulation therapy in Parkinson's disease gait freezing patients(P<0.05).Conclusion:Deep brain electrical stimulation therapy can improve the condition and motor function of Parkinson's disease patients with frozen gait,but some patients may have poor prognosis due to the influence of UPDRS Ⅱ score before surgery and S&E scores during the opening and closing phases.
2.Methods and application effects of simultaneous correction of sunken upper eyelid during double eyelid plasty

Objective:To investigate the method and clinical efficacy of simultaneous correction of sunken upper eyelid during double eyelid plasty.Methods:A retrospective analysis was conducted on patients with single eyelids and sunken upper eyelid treated at the Department of Plastic Surgery, the First People’s Hospital of Zhengzhou, from October 2022 to February 2024. All patients underwent incisional double eyelid plasty with individualized correction based on depression severity. For mild depression, a combination of pretarsal orbicularis oculi muscle flap folding and orbital septum fat flap transposition was performed for correction. For moderate depression, additional autologous stromal vascular fraction (SVF)-gel grafting would be performed if residual depression persisted after the aforementioned approach. For severe depression, if the orbital fat was sufficient, the orbicularis oculi muscle flap folding, orbital fat flap repositioning and autologous SVF-gel transplantation were used. If there was insufficient orbital fat, the correction involved the use of a orbicularis oculi muscle flap and SVF-gel transplantation. Postoperative follow-up was conducted regularly, and related complications were statistically analyzed. Six months after the surgery, two blinded plastic surgeons evaluated outcomes via visual analog scale (VAS), including five aspects: symmetry of palpebral folds(including width and curvature of the double eyelid fold), fold contour, depression improvement, scar quality, overall eyelid aesthetics. Each aspect was rated on a scale from 1 to 10, with higher scores indicating better outcomes. Patient satisfaction was scored separately (1-5 per item), with higher scores indicating greater satisfaction.Results:A total of 64 patients were enrolled, including 4 males and 60 females; the age ranged from 19 to 66 years[ (32.0±9.7) years]. The degree of sunken upper eyelid was mild in 33 cases, moderate in 19 cases and severe in 12 cases. All patients underwent double eyelid plasty and orbicularis oculi flap folding. Based on this, 45 cases (33 mild and 12 moderate cases) underwent orbital septum fat flap transposition, 11 severe cases underwent SVF-gel grafting, and 8 cases (7 moderate and 1 severe cases) required combined approaches to correct depression. During the procedure, 19 cases (38 sides) received (1.8±0.8) ml of SVF-gel injection. Among these cases, one case with severe sunken upper eyelid who did not receive orbital fat transfer was injected with high density fat (left side: 0.6 ml, right side: 0.8 ml) due to insufficient SVF-gel volume. Two cases with severe sunken upper eyelid who only had autologous SVF-gel transplantation did not fully correct the depression within 3 months post-surgery received a second SVF-gel injection[(1.0±0.1) ml]. Follow-up was conducted for 6 to 12 months after surgery. All patients experienced varying degrees of eyelid edema, which typically lasted 2 to 3 months. Ecchymosis (34 sides), conjunctival congestion (3 sides), and temporary ptosis (5 sides) all resolved within 2 weeks. Pigmentation (14 sides) and scar hyperplasia (3 sides) disappeared or stabilized within 6 months. No patients developed complications such as incision infection, hematoma, fat liquefaction, local skin unevenness, or induration. The surgeons’ VAS scores of the above five indicators were all > 8 points, and the satisfaction scores of patients for the five indicators were all > 4 points. In both scoring, the improvement of sunken upper eyelid scored the highest, which were (9.2 ± 0.9) points and (4.8 ± 0.6) points respectively.Conclusion:For patients with single eyelids and varying degrees of sunken upper eyelid, performing double eyelid plasty and orbicularis oculi muscle flap folding, individualized correction is achieved through autologous fat redistribution techniques, based on the severity of the sunken upper eyelid and the amount of orbital fat. This single procedure can restore upper eyelid volume and rejuvenate the appearance, resulting in a natural and aesthetically pleasing double eyelid with minimal complications and high patient satisfaction.
3.Predictive value of PLR,NLR and Fib expression in peripheral blood of stroke patients for deep vein thrombosis of lower limbs

Objective To explore the relationship between platelet-to-leukocyte ratio(PLR)and neutro-phil-to-lymphocyte ratio(NLR)and fibrinogen(Fib)levels in the peripheral blood of stroke patients and deep vein thrombosis(DVT)of the lower limbs,as well as their predictive value for wether DVT occurred in stroke patients.Methods A total of 106 stroke patients admitted to the hospital from January 1,2022 to January 30,2024 were retrospectively selected as the research subjects.According to the occurrence of DVT in the pa-tients,they were divided into the DVT group(n=38)and the non-DVT group(n=68),and the levels of PLR,NLR and Fib in the two groups were compared.Multivariate Logistic regression was used to analyze the influencing factors of DVT in stroke patients,and the receiver operating characteristic(ROC)curve was used to analyze the predictive value of PLR,NLR and Fib for DVT in stroke patients.Results The proportions of patients in the DVT group with NIHSS score>14 points,GCS score>8 points,and dehydrating drug use time>5 days were higher than those in the non-DVT group(P<0.05),and the levels of PLR,NLR and Fib were higher than those in the non-DVT group(P<0.05).The results of Logistic regression analysis showed that the upregulation of PLR,NLR and Fib levels were all risk factors for DVT in stroke patients(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of the combined prediction of DVT in stroke patients by PLR,NLR and Fib was 0.890,with a sensitivity of 81.62%and a specificity of 88.78%.The com-bined detection AUC was higher than that predicted separately by PLR,NLR and Fib(Z=2.768,3.424,3.657,all P<0.05).Conclusion Peripheral blood PLR,NLR and Fib levels are closely related to the occur-rence of DVT in stroke patients.]The combined application of PLR,NLR and Fib has a high predictive value in the occurrence of DVT in stroke patients and can be used as an effective biomarker to predict DVT occur-rence.
4.The Value of Angiography-derived Microcirculatory Resistance in Predicting Ventricular Remodeling in Patients With ST-segment Elevation Myocardial Infarction

Objectives:To explore the correlation between angiography-derived microcirculatory resistance(AMR)and microvascular obstruction(MVO)extent,and AMR's predictive value for ventricular remodeling in patients with ST-segment elevation myocardial infarction(STEMI)post-primary percutaneous coronary intervention(PCI).Methods:This retrospective study included STEMI patients who underwent PCI at Beijing Anzhen Hospital from April 2019 to January 2023.Patients underwent acute and follow-up cardiac magnetic resonance(CMR)examinations 3-7 days and 3 months post-PCI.Ventricular remodeling was defined as a 20%or more increase in left ventricular end-diastolic volume at follow-up.Patients were divided into ventricular remodeling and non-ventricular remodeling groups.Baseline clinical characteristics,AMR values and CMR indices of both groups were compared.Pearson's correlation coefficient was used to explore the correlation between AMR and MVO extent on CMR.Logistic regression and receiver operating characteristic curve analysis were employed to evaluate the predictive performance of AMR for ventricular remodeling.Results:A total of 168 STEMI patients([56.4±11.4]years,139[82.7%]males)were included,with 49(29.2%)in the ventricular remodeling group and 119(70.8%)in the non-ventricular remodeling group.AMR was positively correlated with MVO extent on CMR(r=0.42,P<0.01).Compared to the non-ventricular remodeling group,patients in the ventricular remodeling group exhibited a higher AMR value(3.00[2.56,3.52]mmHg?s/cm vs.2.48[2.20,2.74]mmHg?s/cm,1 mmHg=0.133 kPa,P<0.01).Multivariate logistic analysis showed that AMR was independently associated with ventricular remodeling post-PCI in STEMI patients.For every 0.2 mmHg·s/cm increase in AMR,the risk for ventricular remodeling increased 45.1%(adjusted OR=1.451,95%CI:1.228-1.714,P<0.01).Area under the curve of AMR for predicting ventricular remodeling was 0.769.Conclusions:AMR is positively correlated with MVO extent and is an independent predictor for ventricular remodeling in STEMI patients post-PCI.
5.Establishment and validation of a predictive model for increased drainage volume after open transforaminal lumbar interbody fusion

Objective To analyze the risk factors for increased drainage volume after open transforaminal lumbar interbody fusion(TLIF),and to establish a predictive model and then validate it.Methods The clinical data of 680 patients who underwent open TLIF at the General Hospital of Northern Theater Command from January 2016 to December 2019 were collected and the patients were randomly divided into the training group(n=476)and the validation group(n=204).Taking the predictive factors screened out by LASSO regression analysis as independent variables,a multivariate Logistic regression predictive model was constructed.The model was internally validated through the receiver operating characteristic(ROC)curve,Hosmer-Lemeshow goodness-of-fit test,and calibration curve,and its clinical utility was assessed via decision curve analysis(DCA).Results LASSO regression analysis screened out four predictive variables:age,number of surgical segments,operative duration,and intraoperative blood loss.The multivariate Logistic regression predictive model demonstrated that age≥60 years,number of surgical segments≥4,operative duration≥2 hours,and intraoperative blood loss≥200 mL were independent influencing factors for the increased postoperative drainage volume in patients undergoing TLIF(P<0.05).ROC curve analysis revealed an area under the curve(AUC)of 0.816(95%CI:0.798 to 0.867)in the training group and 0.783(95%CI:0.685 to 0.823)in the validation group,indicating that the predictive model had good discriminatory ability.Additionally,the Hosmer-Lemeshow goodness-of-fit test and calibration curve indicated that the predictive model had a good degree of fit,and the predicted probability was basically consistent with the actual probability,demonstrating a good calibration.The DCA results confirmed that this predictive model could be applied in clinical practice.Conclusion The risk factors for increased drainage volume after open TLIF include age,number of surgical segments,operative duration,and intraoperative blood loss.The predictive model established based on these factors demonstrates good performance,and it can be applied in clinical guidance for the selection of drainage tube removal time after TLIF.
6.Clinical significance of postoperative recurrence pattern and prognosis of pancreatic ductal adenocarcinoma

Objective:This study aimed was to analyze the disease progression of postoperative recurrence/me-tastasis of pancreatic ductal adenocarcinoma(PDAC)and provide clinical evidence for evaluating the difference in prog-nosis between site and pattern of recurrence/metastasis.Methods:The clinical data of 167 patients with PDAC who underwent pancreatectomy in Panjin Liao-Oil Gem Flower Hospital from January 2011 to June 2019 were retrospec-tively analyzed.Kaplan-Meier curves and Cox-regression analyses were applied to assess survival outcomes.Results:In this study,96 patients(57.5%)recurred within 12 months after surgery,and the median recurrence time was 10.9 months.Compared to patients without recurrence,patients with recurrence/metastasis had higher baseline CA19-9>100 IU/mL and were treated more often with neoadjuvant chemotherapy(P<0.05).In the no recurrence group,patients had histopathological analyses yielded lower less poorly differentiated tumors,less positive margin,less pT3+4 stages and pN1+2 stages(P<0.05).In a multivariate Cox regression analysis,receipt of neoadjuvant chemotherapy,poor tumor differ-entiation and positive margin were independent predictors for recurrence/metastasis within 12 months after surgery(P<0.05).Neoadjuvant treated patients had less pT1+2 stages(P=0.015),a lower lymph node ratio,less perineural invasion,less positive margin,and were less likely to be treated with adjuvant chemotherapy compared to upfront surgery treated patients(P<0.05).Recurrence/metastasis rates were higher in the neoadjuvant treated cohort versus the upfront surgery cohort(P=0.014).There was no statistical difference in the recurrence/metastasis rate distribution between neoadjuvant therapy and upfront surgery,and neoadjuvant therapy exhibited no significant impact on recurrence/metastasis patterns in PDAC(P>0.05).The overall survival period of patients without recurrence/metastasis within 12 months was signifi-cantly longer than that of patients with recurrence/metastasis after PDAC resection(P<0.001).Conclusion:The varying site recurrence/metastasis rates and time to recurrence/metastasis,underline the inherent heterogeneity of PDAC and the specific interactions in the tumor microenvironments at the recurrence/metastasis sites.Further research is essential to establish optimal personalized treatment strategies for patients with recurrent PDAC.

If there was insufficient orbital fat, the correction involved the use of a orbicularis oculi muscle flap and SVF-gel transplantation. Postoperative follow-up was conducted regularly, and related complications were statistically analyzed. Six months after the surgery, two blinded plastic surgeons evaluated outcomes via visual analog scale (VAS), including five aspects: symmetry of palpebral folds(including width and curvature of the double eyelid fold), fold contour, depression improvement, scar quality, overall eyelid aesthetics. Each aspect was rated on a scale from 1 to 10, with higher scores indicating better outcomes. Patient satisfaction was scored separately (1-5 per item), with higher scores indicating greater satisfaction.Results:A total of 64 patients were enrolled, including 4 males and 60 females; the age ranged from 19 to 66 years[ (32.0±9.7) years]. The degree of sunken upper eyelid was mild in 33 cases, moderate in 19 cases and severe in 12 cases. All patients underwent double eyelid plasty and orbicularis oculi flap folding. Based on this, 45 cases (33 mild and 12 moderate cases) underwent orbital septum fat flap transposition, 11 severe cases underwent SVF-gel grafting, and 8 cases (7 moderate and 1 severe cases) required combined approaches to correct depression. During the procedure, 19 cases (38 sides) received (1.8±0.8) ml of SVF-gel injection. Among these cases, one case with severe sunken upper eyelid who did not receive orbital fat transfer was injected with high density fat (left side: 0.6 ml, right side: 0.8 ml) due to insufficient SVF-gel volume. Two cases with severe sunken upper eyelid who only had autologous SVF-gel transplantation did not fully correct the depression within 3 months post-surgery received a second SVF-gel injection[(1.0±0.1) ml]. Follow-up was conducted for 6 to 12 months after surgery. All patients experienced varying degrees of eyelid edema, which typically lasted 2 to 3 months. Ecchymosis (34 sides), conjunctival congestion (3 sides), and temporary ptosis (5 sides) all resolved within 2 weeks. Pigmentation (14 sides) and scar hyperplasia (3 sides) disappeared or stabilized within 6 months. No patients developed complications such as incision infection, hematoma, fat liquefaction, local skin unevenness, or induration. The surgeons’ VAS scores of the above five indicators were all > 8 points, and the satisfaction scores of patients for the five indicators were all > 4 points. In both scoring, the improvement of sunken upper eyelid scored the highest, which were (9.2 ± 0.9) points and (4.8 ± 0.6) points respectively.Conclusion:For patients with single eyelids and varying degrees of sunken upper eyelid, performing double eyelid plasty and orbicularis oculi muscle flap folding, individualized correction is achieved through autologous fat redistribution techniques, based on the severity of the sunken upper eyelid and the amount of orbital fat. This single procedure can restore upper eyelid volume and rejuvenate the appearance, resulting in a natural and aesthetically pleasing double eyelid with minimal complications and high patient satisfaction.
