Japanese spotted fever(JSF)is an infectious disease caused by Rickettsia japonica,with nonspecific clinical symptoms and a high risk of misdiagnosis.We reported a case of JSF,in which Rickettsia japonica was detected in blood cells by metagenomic next-generation sequencing.The patient recovered after treatment with doxycycline.This report provides a reference for the clinical diagnosis and treatment of JSF.

BACKGROUND: The association of systemic inflammatory response index (SIRI) with prognosis of coronary artery disease (CAD) patients has never been investigated in a large sample with long-term follow-up. This study aimed to explore the association of SIRI with all-cause and cause-specific mortality in a nationally representative sample of CAD patients from United States. METHODS: A total of 3386 participants with CAD from the National Health and Nutrition Examination Survey (NHANES) 1999-2018 were included in this study. Cox proportional hazards model, restricted cubic spline (RCS), and receiver operating characteristic curve (ROC) were performed to investigate the association of SIRI with all-cause and cause-specific mortality. Piece-wise linear regression and sensitivity analyses were also performed. RESULTS: During a median follow-up of 7.7 years, 1454 all-cause mortality occurred. After adjusting for confounding factors, higher lnSIRI was significantly associated with higher risk of all-cause (HR = 1.16, 95% CI: 1.09-1.23) and CVD mortality (HR = 1.17, 95% CI: 1.05-1.30) but not cancer mortality (HR = 1.17, 95% CI: 0.99-1.38). The associations of SIRI with all-cause and CVD mortality were detected as J-shaped with threshold values of 1.05935 and 1.122946 for SIRI, respectively. ROC curves showed that lnSIRI had robust predictive effect both in short and long terms. CONCLUSIONS SIRI was independently associated with all-cause and CVD mortality, and the dose-response relationship was J-shaped. SIRI might serve as a valid predictor for all-cause and CVD mortality both in the short and long terms.

BACKGROUND: Based on the China-VHD database, this study sought to develop and validate a Valvular Heart Disease- specific Age-adjusted Comorbidity Index (VHD-ACI) for predicting mortality risk in patients with VHD. METHODS & RESULTS: The China-VHD study was a nationwide, multi-centre multi-centre cohort study enrolling 13,917 patients with moderate or severe VHD across 46 medical centres in China between April-June 2018. After excluding cases with missing key variables, 11,459 patients were retained for final analysis. The primary endpoint was 2-year all-cause mortality, with 941 deaths (10.0%) observed during follow-up. The VHD-ACI was derived after identifying 13 independent mortality predictors: cardiomyopathy, myocardial infarction, chronic obstructive pulmonary disease, pulmonary artery hypertension, low body weight, anaemia, hypoalbuminaemia, renal insufficiency, moderate/severe hepatic dysfunction, heart failure, cancer, NYHA functional class and age. The index exhibited good discrimination (AUC, 0.79) and calibration (Brier score, 0.062) in the total cohort, outperforming both EuroSCORE II and ACCI (P < 0.001 for comparison). Internal validation through 100 bootstrap iterations yielded a C statistic of 0.694 (95% CI: 0.665-0.723) for 2-year mortality prediction. VHD-ACI scores, as a continuous variable (VHD-ACI score: adjusted HR (95% CI): 1.263 (1.245-1.282), P < 0.001) or categorized using thresholds determined by the Yoden index (VHD-ACI ≥ 9 vs. < 9, adjusted HR (95% CI): 6.216 (5.378-7.184), P < 0.001), were independently associated with mortality. The prognostic performance remained consistent across all VHD subtypes (aortic stenosis, aortic regurgitation, mitral stenosis, mitral regurgitation, tricuspid valve disease, mixed aortic/mitral valve disease and multiple VHD), and clinical subgroups stratified by therapeutic strategy, LVEF status (preserved vs. reduced), disease severity and etiology. CONCLUSION The VHD-ACI is a simple 13-comorbidity algorithm for the prediction of mortality in VHD patients and providing a simple and rapid tool for risk stratification.

In recent years,the application of long non-coding RNAs(lncRNAs)in the treatment of dis-eases has been widely concerned.LncRNAs have been proved to play a crucial role in tumor therapy.They usually act as oncogene or suppressor genes to regulate the occurrence and development of tumors.Previously,our group discovered a new lncRNA 606938 that can be used as a target in colorectal cancer through transcriptome analysis.However,its molecular mechanism in colorectal cancer is still unclear.In this study,RT-qPCR test shows that lncRNA 606938 is low expressed in colorectal cancer cell lines com-pared with normal colon epithelial cells.The clonogenicity was decreased by 97％and 54.5％in lncRNA 606938 overexpressed DLD-1 and SW620 cells,respectively.The results of flow cytometry assay showed that overexpression of lncRNA 606938 caused cell cycle arrest at G1 phase(They were prolonged by 50.5％and 63.9％,respectively)and promoted apoptosis of colorectal cancer cells,which was increased by 1.9％and 3.3％,respectively.Western blot results showed that overexpression of lncRNA 606938 down-regulated the expression of related oncogenes(β-catenin,c-Myc,cyclin D1,cyclin D2,cyclin D3,CDK 4,CDK 6)in colorectal cancer cells,and the expression of tumor suppressor genes(TRIM33,caspase 2,and actived caspase 3)was up-regulated.The above results were reversed after knockdown of lncRNA 606938.Mechanistically,it has been confirmed through RNA pulldown assay,RIP assay,and Rescue assay that lncRNA 606938 can target and promote TRIM33 expression,thereby promoting the degradation of β-catenin and blocking the expression of β-catenin and its downstream oncogenes such as c-Myc and cycline D1 to inhibit the progression of colorectal cancer.This study elucidates the molecular mechanism by which the lncRNA 606938 targets TRIM33/β-catenin signaling pathway,inhibits the pro-liferation and promotes the apoptosis of colorectal cancer cells.This study reveals the potential of lncRNA 606938 as a tumor suppressor gene,providing new target and strategies for the clinical treatment of color-ectal cancer.

Objective To study the effects of Jinshui Liujun decoction(JLD)on airway inflammation and interferon-γ(IFN-γ)/interleukin-4(IL-4)imbalance in asthma.Methods The ovalbumin(OVA)was used to replicate the asthmatic mouse by injection and nebulization inhalation.Asthmatic mice were randomly divided into model group,positive control group(1.0 mg·kg-1 dexamethasone)and experimental-L,-M,-H groups(4.1,8.2 and 16.4 g·kg-1 JLD).The blank group and the model group were given purified water by gavage for 2 consecutive weeks.The lung function[tidal volume(TV)and respiratory rate(RR)]of mice was measured by lung function test system after administering JLD by gavage.IFN-γ,IL-4 in bronchoalveolar lavage fluid(BALF)and their genes in lung tissue were determined through enzyme-linked immunosorbent assay and quantitative real-time polymerase chain reaction respectively.Results The TV levels in blank group,model group,positive control group and experimental-L,-M,-H groups were(118.90±24.20),(87.10±13.36),(112.70±18.92),(105.20±15.24),(107.60±16.47)and(110.10±17.41)μL;RR levels were(126.60±17.57),(178.80±31.06),(133.00±19.45),(146.00±25.82),(141.30±24.78)and(137.00±22.81)bpm;IFN-γ levels were(110.95±9.26),(60.16±2.75),(98.09±8.21)、(79.58±6.49)、(88.16±7.58)and(97.26±7.23)pg·mL-1;IL-4 levels were(118.06±8.47)、(416.08±22.21)、(272.17±13.14)、(319.34±15.27)、(298.77±14.83)and(278.71±14.24)pg·mL-1;the IFN-γ mRNA levels were 2.70±0.08,1.00±0.00,2.65±0.06,1.89±0.04,2.24±0.05 and 2.58±0.06;IL-4 mRNA levels were 0.25±0.02,1.00±0.00,0.26±0.02,0.61±0.05,0.50±0.03 and 0.33±0.03,respectively.There were statistically significant differences between the above indicators of model group and blank group,and experimental-L,-M,-H groups and model group(P＜0.05,P＜0.01).Conclusion JLD has a certain antiasthmatic effect and one of mechanisms is to alleviate the IFN-γ/IL-4 immune imbalance and inhibit airway inflammation.

Pulmonary blast injury has become the main type of trauma in modern warfare, characterized by externally mild injuries but internally severe injuries, rapid disease progression, and a high rate of early death. The injury is complicated in clinical practice, often with multiple and compound injuries. Currently, there is a lack of effective protective materials, accurate injury detection instrument and portable monitoring and transportation equipment, standardized clinical treatment guidelines in various medical centers, and evidence-based guidelines at home and abroad, resulting in a high mortality in clinlcal practice. Therefore, the Trauma Branch of Chinese Medical Association and the Editorial Committee of Chinese Journal of Trauma organized military and civilian experts in related fields such as thoracic surgery and traumatic surgery to jointly develop the Clinical treatment guideline for pulmonary blast injury ( version 2023) by combining evidence for effectiveness and clinical first-line treatment experience. This guideline provided 16 recommended opinions surrounding definition, characteristics, pre-hospital diagnosis and treatment, and in-hospital treatment of pulmonary blast injury, hoping to provide a basis for the clinical treatment in hospitals at different levels.
