BACKGROUND/OBJECTIVES: Postpartum bone metabolism undergoes substantial alterations based on breastfeeding status. Research on maternal calcium intake and bone recovery, particularly among Asian women, remains limited. This study investigated postpartum change in bone mineral density (BMD) and its association with calcium intake during late pregnancy and post-delivery according to breastfeeding status. SUBJECTS/METHODS: Thirty-six women (22 breastfeeding, 14 non-breastfeeding) underwent dual-energy X-ray absorptiometry 3 times: 1) within 3 days of delivery, 2) at 6 mon postpartum for non-breastfeeding women or within 1 mon of weaning for breastfeeding women, and 3) at 12 mon postpartum. Dietary calcium intake was evaluated using a food frequency questionnaire and dietary supplement survey at all visits. Association between calcium intake with percentage change in BMD (%ΔBMD) was assessed by linear regression. RESULTS: The BMD at the lumbar spine, total hip, femoral neck, and trochanter significantly decreased during breastfeeding, whereas non-breastfeeding women displayed increased BMD at the femoral neck at 6 mon postpartum. In non-breastfeeding women, postpartum calcium intake was positively associated with %ΔBMD at the total hip, femoral neck, and intertrochanter (P < 0.05). Few significant associations were observed between %ΔBMD and late pregnancy intakes across all women, regardless of breastfeeding status. At 12 mon, BMD in breastfeeding women recovered to delivery levels, while non-breastfeeding women maintained their BMD observed at 6 mon postpartum. CONCLUSION Postpartum calcium intake is positively associated with bone recovery in non-breastfeeding women, suggesting that its effects on bone recovery potentially vary with lactation status.

Methods: Using the Population, Intervention, Comparison, Outcomes (PICO) framework, we developed key clinical questions and conducted systematic literature reviews. A multidisciplinary panel of experts, organized by the Korean Dementia Association, evaluated randomized controlled trials and observational studies. Recommendations were graded for evidence quality and strength using Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) methodology. Results: Three main recommendations are presented: (1) For AD, cholinesterase inhibitors (donepezil, rivastigmine, galantamine) are strongly recommended for improving cognition and daily function based on moderate evidence; (2) Cholinesterase inhibitors are conditionally recommended for vascular dementia and Parkinson’s disease dementia, with a strong recommendation for Lewy body dementia; (3) For moderate to severe AD, NMDA receptor antagonist (memantine) is strongly recommended, demonstrating significant cognitive and functional improvements. Both drug classes showed favorable safety profiles with manageable side effects. Conclusions This guideline offers standardized, evidence-based pharmacologic recommendations for dementia management, with specific guidance on cholinesterase inhibitors and NMDA receptor antagonists. It aims to support clinical decision-making and improve patient outcomes in dementia care. Further updates will address emerging treatments, including amyloid-targeting therapies, to reflect advances in dementia management.

Objectives: Nutrient intake during pregnancy and lactation is crucial for the health of both mother and offspring. Diet and nutrient metabolism potentially vary according to ethnicity and fetal number; nevertheless, recent studies validating this are inadequate. Furthermore, few studies have tracked changes in intake after delivery. We compared the food and nutrient intakes between pregnant women in Korea carrying singletons and multiples during late pregnancy and assessed their changes through postpartum. Methods: Ninety-eight pregnant women were recruited from Chonnam National University Hospital between January 2019 and December 2023, and 48 responded to follow-up. Third trimester and postpartum intake were assessed via food frequency questionnaires and supplement questionnaires. Student’s t-test, Mann–Whitney U test, chi-square test, paired t-test or Wilcoxon signed-rank test was performed and adjustments were made for covariates. Results: Nutrient intake was generally adequate relative to the Dietary Reference Intakes for Koreans, with no differences between singleton- and multiple-pregnancy women. Sixty-six of 98 (67%) pregnant women consumed meat, fish, vegetables, and fruit daily. Dairy intake was low, while the mean iron intake during pregnancy reached 54.2 ± 34.0 mg/d, exceeding the tolerable upper intake level, mainly owing to supplements. Postpartum fruit and vitamin C intake decreased, with no significant differences between breastfeeding and non-breastfeeding women. Conclusion Dietary intake did not significantly differ between Korean singleton- and multiple-pregnancy women. Dairy intake was low and iron intake was excessive. Fruit intake decreased after delivery; however, difference in dietary intake according to breastfeeding status was minimal. Nutritional education may be necessary to promote a balanced diet in pregnant and postpartum women. Trial Registration: Clinical Research Information Service Identifier KCT0005118.

Objective: Hyperuricemia has been described commonly in preeclamptic pregnancies, often prece ding the diagnosis of preeclampsia and historically was used as a diagnostic marker of preeclampsia.The aim of this study was to determine the usefulness of gestation corrected hyperuricemia (GCH) to predict the recurrence of preeclampsia on subsequent pregnancy. Methods: The retrospective study of 64 women who had previous preeclampsia and checked serum uric acid was analyzed. GCH was defined as being one standard deviation above the gestation-specific mean. And we used uric acid z-scores ([serum uric acid value-gestation specific mean]/standard deviation of the population) to account for gestation-specific alterations in uric acid and tested this as a continuous variable. The relationship between GCH and recurrence of preeclampsia on subsequent pregnancy was analyzed. Obstetric outcomes were reviewed according to absence or presence of GCH. P<0.05 was considered as significant. Results: Of 64 women, seventeen had the development of recurrent preeclampsia (26.6%). The absence or presence of GCH was not associated with the recurrence of preeclampsia on subsequent pregnancy (P=0.267). And gestation-specific uric acid z-score as a continuous variable did not show any association with the prediction of preeclampsia on subsequent pregnancy (P=0.427). GCH was associated with the small for gestational age (P=0.010). Conclusion GCH does not predict the recurrence of preeclampsia on subsequent pregnancy.

Objective: Our goal was to review outcomes of patients with heterotopic pregnancies who under went surgical treatment. Methods: From January 1, 2005 to December 31, 2021, 27 women with heterotopic pregnancies who had undergone surgical treatment were enrolled. We investigated pregnancy outcomes based on clinical records and follow-up telephone interviews. Three of the 27 patients were excluded because we were unable to contact them. Results: Of the 24 patients included in the study, 17 underwent salpingectomies for tubal pregnan cies, 5 underwent wedge resections for cornual pregnancies, and 1 underwent a unilateral salpingooophorectomy for ovarian pregnancy. One patient with a suspected heterotopic cervical pregnancy had undergone extraction with forceps and curettage under abdominal ultrasound guidance. Three patients had spontaneous abortions after treatment for heterotopic pregnancies (2 tubal pregnancies, one cervical pregnancy). Twenty-one of 24 patients had heterotopic pregnancies that occurred after assisted reproductive technology. Three cases were heterotopic pregnancies that occurred without reproductive interventions. Among 24 patients who underwent surgical treatment, 18 patients (75.0%) had full-term deliveries. Conclusion There were no hospitalizations due to preterm labor, and 21 deliveries (87.5%) were either late preterm or full-term. There were no fetal abnormalities or maternal complications. Although the development of assisted reproductive technologies has increased the incidence of heterotopic pregnancy, the outcomes of heterotopic pregnancies are generally good.

Purpose: The purpose of this study was to determine the effects of ankle dorsiflexion, neutral, and plantarflexion on lower extremity muscle activity and break point angle (BPA) during Nordic hamstring exercises. Methods: Twenty-four members of a college soccer club (age, 21.68±2.39 years; height, 175.63±4.76 cm; weight, 71.88±6.29 kg) were recruited to participate in the experiment, and all subjects were measured three times in triplicate for Nordic hamstrings at three different ankle angles with all subjects in one group. Surface electromyography equipment (miniDTS, Noraxon Inc.) was used to acquire 3 seconds of muscle activity data at the starting point (90°) during exercise, and motion analysis software (Kinovea version 0.9.5, Kinovea) was used to collect kinematic data at the point where knee strike angular velocity exceeded 30°/sec for BPA data acquisition. Results: The results of this study, lower extremity muscle activity was not significantly different in the three variants of Nordic hamstring exercises, but BPA was significantly lower in the dorsiflexion position (60.28°±6.35°) compared to the neutral position (65.32°±6.35°) and plantarflexion position (63.82°±7.01°) (p＜ 0.001). Conclusion These results suggest that the dorsi flexion position of the ankle during Nordic hamstring exercises allows the body to maintain the position for a longer period of time against eccentric forces in situations where the body is moving forward. This suggests that the ankle dorsi flexion position can be used as a position for effective Nordic hamstring exercises in amateur soccer players.

OBJECTIVES: In this study, we sought to evaluate the association between smoking status and subclinical coronary atherosclerosis, as detected by coronary computed tomography angiography (CCTA), in asymptomatic individuals. METHODS: We retrospectively analyzed 9,285 asymptomatic participants (mean age, 53.7±8.0 years; n=6,017, 64.8% male) with no history of coronary artery disease (CAD) who had undergone self-referred CCTA. Of these participants, 4,333 (46.7%) were considered never smokers, 2,885 (31.1%) former smokers, and 2,067 (22.3%) current smokers. We assessed the degree and characteristics of subclinical coronary atherosclerosis using CCTA, with obstructive CAD defined as a diameter stenosis of at least 50%. RESULTS: Compared with never-smokers, former smokers exhibited no significant differences in the probabilities of obstructive CAD, any coronary plaque, calcified plaque, or mixed plaque, as determined using adjusted odds ratios (aORs; p>0.05 for all). However, the risk of non-calcified plaque was significantly higher in former smokers (aOR, 1.34; 95% confidence interval [CI], 1.00 to 1.78; p=0.048). Current smokers had significantly higher rates of obstructive CAD (aOR, 1.46; 95% CI, 1.10 to 1.96; p=0.010), any coronary plaque (aOR, 1.41; 95% CI, 1.20 to 1.65; p<0.001), calcified plaque (aOR, 1.32; 95% CI, 1.13 to 1.55; p=0.001), non-calcified plaque (aOR, 1.72; 95% CI, 1.28 to 2.32; p<0.001), and mixed plaque (aOR, 2.00; 95% CI, 1.39 to 2.86; p<0.001) compared to never smokers. CONCLUSIONS This cross-sectional study revealed a significant association between current smoking and subclinical coronary atherosclerosis, as detected on CCTA. Additionally, former smoking demonstrated an association with non-calcified plaque, indicating elevated cardiovascular risk.
