1.Effect and mechanism of beta-caryophyllene in mice with osteoarthritis

BACKGROUND:β-Caryophyllene has a variety of pharmacological activities such as antioxidant,anti-inflammatory and anti-apoptotic,which may have a better therapeutic effect on osteoarthritis.OBJECTIVE:To investigate the effect and mechanism of β-caryophyllene on mouse osteoarthritis.METHODS:Forty C57BL/6J mice were randomly divided into sham group,model group,low-dose β-caryophyllene group and high-dose β-caryophyllene group,with 10 mice in each group.Hulth method was used to construct an osteoarthritis model in the latter three groups.Four weeks after modeling,70 and 140 mg/kg/d β-caryophyllene was intragastrically given in the low-and high-dose β-caryophyllene groups,respectively,and normal saline was given by gavage in the sham group and the model group,once a day,for 4 weeks.After administration,knee joint morphological changes were observed by hematoxylin-eosin staining,serum levels of inflammatory factors(tumor necrosis factor-α,interleukin-1β,interleukin-6,and interleukin-10)were detected by ELISA,and oxidative stress indexes(glutathione peroxidase,superoxide dismutase,and malondialdehyde)were detected by chemiluminescence.The expression levels of key proteins in the Sonic hedgehog(Shh)/glioma associated oncogene homolog 1(Gli1)signaling pathway were detected by immunohistochemistry and western blot.RESULTS AND CONCLUSION:(1)Compared with the sham group,a large number of inflammatory cells infiltrated in the knee joint of mice in the model group,cartilage tissue was seriously damaged,serum levels of tumor necrosis factor-α,interleukin-1β,interleukin-6,interleukin-10 and malondialdehyde were significantly increased(P<0.01),the activities of glutathione peroxidase and superoxide dismutase were significantly decreased(P<0.01),and the relative expression levels of Shh and Gli1 in the knee joint were significantly increased(P<0.01).(2)Compared with the model group,in the low-and high-dose β-caryophyllene groups,inflammatory cell infiltration in the mouse knee joint was decreased,cartilage tissue injury was alleviated,serum levels of tumor necrosis factor-α,interleukin-1 β,interleukin-6 and malondialdehyde were significantly decreased(P<0.05),the activities of glutathione peroxidase and superoxide dismutase were significantly increased(P<0.01),and the expression levels of Shh and Gli1 in the knee joint were significantly decreased(P<0.01).The above-mentioned improvements were more significant in the high-dose β-caryophyllene group than the low-dose β-caryophyllene group.To conclude,β-caryophyllene can improve osteoarthritis,and its mechanism may be related to reducing inflammation and oxidative stress damage by regulating the Shh/Gli1 signaling pathway.

3.Traditional Chinese Medicine Regulates Gut Microbiota to Intervene in Digestive System Malignant Tumors: A Review

Digestive system malignant tumors (DT) are one of the leading causes of death globally and carry a heavy economic burden. Gut microbiota plays a critical role in maintaining host health, including providing nutrition, defending against pathogens, and promoting immune development. In recent years, more and more studies have shown that dysbiosis of gut microbiota is closely associated with DT such as gastric cancer, liver cancer, and colon cancer. Therefore, targeted regulation of gut microbiota plays a potential role in inhibiting the growth and metastasis of DT, while its specific regulatory mechanism remains unclear. As the studies about the anti-tumor effects of traditional Chinese medicine (TCM), especially the basic and clinical studies on the regulation of gut microbiota by TCM in tumor treatment, have been growing, the therapeutic effects of TCM on DT have attracted much attention. This paper provides a systematic review of the relationship between gut microbiota and DT, as well as the related studies on the modulation of gut microbiota by TCM against DT, with the aim of providing a foundation and direction for future basic and clinical studies on DT. The literature review shows that gut microbiota influence the occurrence and development of DT through multiple pathways. These pathways include triggering chronic inflammation, producing oncogenic metabolites, inducing genomic instability, regulating the immune system, and altering the tumor microenvironment. TCM can exert anti-DT effects by regulating the composition of gut microbiota, modulating gut microbiota metabolites, repairing intestinal barrier function, and influencing immune functions. Therefore, understanding the relationship between gut microbiota and DT and the regulatory mechanisms of TCM may provide new strategies for future prevention and treatment of DT.

Background:
Recurrent gynecological clear cell carcinoma (rGCCC) has a low objective response rate (ORR) to chemotherapy. Previous preclinical and clinical data suggest a potential synergy between immune checkpoint inhibitors and bevacizumab in rGCCC.Dostarlimab, a humanized monoclonal antibody targeting programmed cell death protein 1 (PD-1), combined with the anti-angiogenic bevacizumab, presents a novel therapeutic approach. This study will investigate the efficacy of dostarlimab +/− bevacizumab in rGCCC.
Methods
DOVE is a global, multicenter, international, open-label, randomized phase 2 study of dostarlimab +/− bevacizumab with standard chemotherapy in rGCCC. We will enroll 198 patients with rGCCC and assign them to one of three groups in a 1:1:1 ratio: arm A (dostarlimab monotherapy), B (dostarlimab + bevacizumab), and C (investigator’s choice of chemotherapy [weekly paclitaxel, pegylated liposomal doxorubicin, doxorubicin, or gemcitabine]). Patients with disease progression in arm A or C will be allowed to cross over to arm B. Stratification factors include prior bevacizumab use, prior lines of therapy (1 vs. >1), and primary site (ovarian vs. non-ovarian). Key inclusion criteria are histologically proven recurrent or persistent clear cell carcinoma of the ovary, endometrium, cervix, vagina, or vulva; up to five prior lines of therapy; disease progression within 12 months after platinumbased chemotherapy; and measurable disease. Key exclusion criteria are prior treatment with an anti–PD-1, anti–programmed death-ligand 1, or anti–programmed death-ligand 2 agent.The primary endpoint is progression-free survival determined by investigators. Secondary endpoints are ORR, disease control rate, clinical benefit rate, progression-free survival 2, overall survival, and toxicity. Exploratory objectives include immune biomarkers.
5."Guangzhou Classification" of donor lung injury: a systematic evaluation and grading framework from pre-procurement to post-transplantation

Lung transplantation is a key therapeutic approach for patients with end-stage lung diseases. Although its clinical outcomes have significantly improved, multidimensional injuries sustained by donor lungs during procurement, preservation, and transplantation remain major challenges affecting graft survival and long-term prognosis. This article proposes the "Guangzhou Classification" for full-course management of donor lung injury, characterized by spatiotemporal dynamics. Based on the progression of disease stages, donor lung injuries are systematically divided into three types: primary injuries (including donor ICU-related lung injury, pathogen colonization, and cold ischemia injury), secondary injuries (such as ventilator-induced lung injury after transplantation, ischemia-reperfusion inflammatory storm, and early rejection), and accompanying injuries (organ toxicity caused by accumulation of postoperative sedatives, analgesics, and vasoactive drugs). Drawing on previous studies and the clinical experience of our center, this paper elaborates the temporal evolution, key risk factors, and prevention and treatment strategies of each injury category, and discusses future research directions. By targeting critical injury factors at each stage, this classification aims to optimize both short-term and long-term outcomes of lung transplantation.
6.Underlying target of bullatine A in treating rheumatoid arthritis based on LiP-SMap drug target proteomics

Aim To identify the underlying target of bullatine A(BA)against rheumatoid arthritis(RA)u-sing limited proteolysis-small molecule mapping(LiP-SMap)drug target proteomics and to provide a scientif-ic basis for clinical application of Aconiti brachypodi Radix in the treatment of RA.Methods LiP-SMap drug target proteomics was employed to perform bioin-formatics analysis for comparing and validating the dif-ferential protein expression after BA intervention.A collagen-induced arthritis(CIA)model was estab-lished in DBA/1 mice using bovine type Ⅱ collagen.The mice were then divided into the CIA model group,methotrexate-positive control group(MTX group),and BA groups(10 mg·kg-1 and 20 mg·kg-1)based on their clinical scores.After drug intervention,the thera-peutic efficacy against RA was assessed by joint index scores and foot thickness measurements.Histopatholog-ical changes in the arthritic joints of CIA mice were e-valuated using hematoxylin and eosin(HE)staining.Enzyme-linked immunosorbent assay(ELISA)was employed to detect inflammatory cytokines interleukin-17(IL-17)and total IgG and IgG3 anti-collagen-spe-cific antibodies levels from the serum of CIA mice.Flow cytometry was used to detect the expression levels of intracellular Th17 cells(IL-17+CD4+T cells)and Th1 cells(IFN-γ+CD4+T cells).Fluorescent quanti-tative PCR was performed to detect the expression of genes related to differential proteins.Results The proteomic analysis identified Serpinb1a as a protein with strong binding affinity to BA,and KEGG enrich-ment analysis indicated IL-17 signaling pathway was a crucial pathway of BA in against RA.BA treatment significantly reduced clinical scores and foot thickness,improved local arthritis symptoms in CIA mice,and al-leviated inflammatory cell infiltration into arthritic joints(P<0.05).Differential protein validation re-sults showed that BA had strong affinity with Serpinb1a(-5.92 kJ·mol-1)and downregulated the expres-sion of Serpinb1a mRNA.Furthermore,the administra-tion of BA markedly reduced serum IL-17 A levels from CIA mice,inhibited the expression of intracellular IL-17 A and IFN-γ cytokines in splenic CD4+T cells(P<0.05),and significantly downregulated the transcrip-tional expression of IL-17F(P<0.05).Conclusion BA exhibits therapeutic effects on collagen-induced arthritis,and its mechanism of action may involve the regulation of Serpinb1a and the IL-17 signaling path-way.
7.The application of competitive respiratory training in abdominal contrast-enhanced MRI

Objective To explore the effect of competitive respiratory training in patients undergoing abdominal contrast-enhanced magnetic resonance imaging(MRI).Methods A total of 115 patients who un-derwent abdominal contrast-enhanced MRI in the hospital from October 2021 to April 2022 were randomly as-signed to the control group(n=64)and the experimental group(n=51).The control group received routine one-on-one respiratory training,while the experimental group received a competitive respiratory training,wherein the first of the two individuals to achieve qualified breath-holding would undergo scanning first.The duration of respiratory training and scanning,as well as abdominal MRI image quality,were compared between the two groups.Image quality was assessed based on respiratory motion artifact scores,anatomical detail scores,and the percentage of sequences affected by respiratory motion artifacts.Results The average duration of respiratory training in the experimental group was significantly shorter than that in the control group[(39.90±14.26)s vs.(71.31±20.96)s,P=0.001].The difference in total scan time of the abdomen and up-per abdomen was not statistically significant(P>0.05).The respiratory motion artifact score was higher in the experimental group compared to the control group(P<0.05).The anatomical detail scores for both the FIESTA and LAVA sequences were higher in the experimental group than in the control group(P<0.05),while no significant difference was found for the T1 sequence score(P<0.05).The percentage of sequences affected by respiratory motion artifacts was significantly lower in the experimental group than in the control group(P=0.006).Conclusion Competitive respiratory training can shorten the duration of respiratory train-ing,improve breath-hold quality and persistence,reduce respiratory motion artifacts,and consequently enhance image quality in abdominal contrast-enhanced MRI.
8.Intestinal flora of personnel stationed on an island based on 16S rRNA high-throughput sequencing

Objective To analyze the characteristics of intestinal flora of personnel stationed on an island,so as to lay the foundation for maintaining the intestinal microecological balance of personnel stationed on island and provide accurate medical security.Methods Several subjects stationed on an island and several subjects from coastal areas were enrolled by random and sampling method,and their fecal samples were sequenced by 16S rRNA high-throughput sequencing.Diversity and composition of gut microbiota in 2 cohorts of personnel were compared.Results Alpha diversity analysis of intestinal flora showed that the abundance of intestinal flora in subjects stationed on the island was significantly higher than that of subjects from coastal areas.Beta diversity analysis indicated significant differences in the composition of intestinal microbial communities between the subjects stationed on the island and those from coastal areas(P=0.001).The abundance of the Bacteroidota in the intestinal tract of subjects stationed on the island was significantly lower than that of subjects from coastal areas(30.8%vs 48.3%,P<0.001),while the abundance of the Proteobacteria was significantly higher than that of subjects from coastal areas(28.3%vs 10.2%,P<0.001).After multiple hypothesis testing correction,it was found that the abundance of the Bacteroides,Roseburia,Alistipes,and Parabacteroides in the intestines of subjects stationed on the island decreased significantly,while the abundance of the Prevotella,Escherichia-Shigella,Citrobacter,and Eubacterium_coprostanoligenes increased significantly.Conclusion The special environment of islands affects the characteristics of intestinal flora of personnel,and the intestinal microecological health needs precise maintenance.
9.Simultaneous Determination of 21 Kinds of Aconitum Alkaloids in Biological Specimens and Herbal Wines Using Ultra-Performance Liquid Chromatography-Tandem Mass Spectrometry

A method for simultaneous determination of 21 kinds of Aconitum alkaloids(ATS)in biological specimens and infused liquor using ultra-performance liquid chromatography-tandem mass spectrometry(UPLC-MS/MS)was developed.The biological samples were pretreated with methanol-acetonitrile(1∶2,V/V)for protein precipitation,while infused liquors were diluted 100-fold with acetonitrile,followed by centrifugation,and filtration by a 0.22-μm membrane.Chromatographic separation was carried out on an EC-C18 column using gradient elution with the mixture of 10 mmol/L ammonium acetate and 0.2%formic acid as mobile phase A and acetonitrile as mobile phase B.With this method,all the analytes were separated within 9.5 min.The samples were detected in positive ESI mode with dynamic multiple reaction monitoring(MRM)and quantified via external standard calibration.The results showed that the concentrations of the analytes in the range of 2-1000 ng/mL had excellent linearity(R2>0.9992)with the peak area.The developed method was successfully used for detection of 21 kinds of aconitum alkaloids,with limits of detection of 0.5-2 ng/mL,quantification limits of 2-6 ng/mL,intra/inter-day precision≤6.0%,spiked recoveries of 89.4%-100.9%,extraction recoveries of 74.2%-104.4%,and matrix effects ranging from-11.1%to 9.2%in blood/urine.The method was applied to detection of 12 samples from 4 fatal aconite poisoning cases,and all 21 kinds of ATS with total alkaloid concentrations of 0.04-4.18 μg/mL in blood and 154.96-422.83 μg/mL in medicinal liquors were detected.Tissue distribution revealed that the order of concentrations from highest to lowest is as follows:urine(157.22 μg/mL)>gastric contents(51.37 μg/mL)>kidney(21.6 μg/g)>whole blood(4.18 μg/mL)>liver(0.03 μg/g).This method showed many advantages such as simple pretreatment,low detection limits,accurate quantification,broad analyte coverage,and superior anti-interference capability in complex matrices,proving ideal for forensic and toxicological analysis of aconitum alkaloids.
10.Accuracy of ΔVpeak-CA combined with tidal volume challenge test in predicting fluid responsiveness of patients receiving lung-protective ventilation

Objective:To evaluate the accuracy of respiratory variation in carotid artery blood flow peak velocity (ΔVpeak-CA) combined with tidal volume challenge (TVC) test in predicting fluid responsiveness of patients receiving lung-protective ventilation.Methods:Eighty American Society of Anesthesiologists Physical Status classification Ⅰ-Ⅲ patients of either sex, aged 18-80 yr, with a body mass index of 18-30 kg/m 2, undergoing elective open abdominal surgery with general anesthesia, were selected. Lung-protective strategies were used during surgery: FiO 2 40%, tidal volume (V T) 6 ml/kg (ideal body weight), personalized positive end-expiratory pressure. TVC was conducted at 5 min after hemodynamic stabilization following intubation (T 1): V T was adjusted from 6 ml/kg to 8 ml/kg, lasting for 1 min (T 2), and then V T was decreased to 6 ml/kg. Ultrasound was used to measure ΔVpeak before and after TVC test, and ΔVpeak-CA was calculated. Volume expansion was conducted at 5 min after TVC test, and an increase in cardiac index (△CI≥15%) after volume expansion was considered as positive fluid responsiveness. The patients were divided into fluid responsiveness group (R group) and non-responsiveness group (NR group). The receiver operating characteristic curve was plotted and the area under the receiver operating characteristic curve (AUC) was calculated to evaluate the efficiency of ΔVpeak-CA at T 1 (ΔVpeak-CA T 1), ΔVpeak-CA at T 2 (ΔVpeak-CA T 2) and the difference in ΔVpeak-CA between T 1 and T 2 (ΔVpeak-CA T 2-T 1) in predicting fluid responsiveness. Results:Seventy-three patients were finally included in this study, with 45 in R group and 28 in NR group. The AUC of ΔVpeak-CA T 2 in predicting fluid responsiveness was 0.880, with the sensitivity of 0.778 and the specificity of 0.857, and the optimum cut-off value was 14.5%. The AUC of ΔVpeak-CA T 2-T 1 in predicting fluid responsiveness was 0.876, with the sensitivity of 0.667 and the specificity of 0.964, and the optimum cut-off value was 5.5%, and the AUC of ΔVpeak-CA T 1 in predicting fluid responsiveness was only 0.646. Conclusions:ΔVpeak-CA combined with TVC test can accurately predict fluid responsiveness of patients receiving lung-protective ventilation.
