1.Report of 4 cases of IgG4-related urinary diseases and literature review

[Objective] To explore the clinical features of IgG4-related urinary diseases so as to provide reference for the diagnosis and treatment of such diseases. [Methods] The clinical data of 4 cases of IgG4-related urinary system diseases diagnosed and treated in Xijing Hospital of Air Force Medical University during Aug.2019 and Dec.2023 were retrospectively collected.Here, we report on the diagnosis and treatment of these patients, analysing their symptoms, serology, imaging and pathology as well as their treatment and outcomes. [Results] The patients included 2 male and 2 female.The lesions were involved with the retroperitoneum and urinary system.Three patients had symptoms of lumbar pain.The imaging manifestations were complex, including retroperitoneal mass involving urinary system organs in 2 cases, tabdense shadow of the right kidney in 1 case, and simple cystic mass of kidney in 1 case.Serum IgG4 value was not detected before surgery.All patients underwent radical surgical treatment.Postoperative pathology showed fibrous tissue hyperplasia with a large number of plasma cells, lymphocytes, a few neutrophil infiltrates, and lymphoid follicles and obliterated vasculitis in some specimens.The number of IgG4+ plasma cells was more than 10 in all tissues under high power microscope.After surgery, 3 patients had symptoms improved, and serum IgG4 value was within the normal range; 1 patient (patem 3) had elevated IgG4 value during follow-up, received subsequent hormone therapy, and the serum IgG 4 level remained stable. [Conclusion] The symptoms of IgG4-related diseases involving the urinary system are non-specific, and the imaging findings are various, easily confused with other diseases.Early detection of serum IgG4 and biopsy pathology can help clinicians make correct diagnosis in the early stage.
2.Effects of fetal bovine serum on quality evaluation of human mesenchymal stem cells

Objective To explore and analyze the effects of different batches of fetal bovine serum(FBS) on the quality evaluation of human mesenchymal stem cells(hMSCs) products and provide reference for the development and quality control of hMSCs products.Methods Retrospective analysis was conducted on the immune-regulatory function data of standard cell line CCRC-hMSC-S1 cultured in medium supplemented with FBS from different batches, based on the quality review data of hMSCs products. The standard cell line CCRC-hMSC-S1 was co-cultured with allogeneic peripheral blood mononuclear cells(PBMCs) from 5-6 donors respectively, and five different batches of FBS(FBS-H, FBS-A, FBS-E1, FBS-E2 and FBS-E3)were added to the co-culture system to clarify the direct effects of different batches of FBS on the detection value of immuneregulatory function by CCRC-hMSC-S1. The hMSCs from eight different individuals were cultured respectively in five medium containing different batches of FBS for two weeks, and the immune-regulatory function and osteogenic differentiation capacity were evaluated to clarify the effects of long-term culture of different batches of FBS on the key quality attributes of hMSCs.Results In the co-culture system of hMSCs and PBMC, the detection value of regulatory T-cell(Treg) promotion rate in FBS-H culture system was significantly lower than that in FBS-A, FBS-E1, FBS-E2 and FBS-E3 culture systems(t = 3. 2,3. 0, 3. 2 and 2. 9, respectively, each P < 0. 05), and the detection value of T lymphocyte proliferation inhibition rate in FBSH culture system was significantly higher than that in FBS-A, FBS-E1, FBS-E2 and FBS-E3 culture systems(t = 3. 7, 3. 4,2. 8 and 3. 0, respectively, each P < 0. 05); the inhibitory ability of hMSCs long-term cultured in FBS-A on Th1 proliferation was significantly lower than that of hMSCs long-term cultured in FBS-H, FBS-E1, FBS-E2 and FBS-E3(t = 3. 0, 2. 4, 3. 7and 3. 3, respectively, each P < 0. 05); the suppressive effect of hMSCs long-term cultured in FBS-H on T lymphocyte proliferation was significantly higher than that of hMSCs long-term cultured in FBS-A, FBS-E1, FBS-E2 and FBS-E3(t = 4. 7,2. 6, 3. 4 and 3. 9, respectively, each P < 0. 05); the alizarin red S staining area of hMSCs long-term cultured in FBS-E3 after osteogenic differentiation was significantly lower than that of hMSCs long-term cultured in FBS-A, FBS-H and FBS-E1(t =4. 1, 5. 5 and 3. 9, respectively, each P < 0. 01).Conclusion In the research and development of hMSCs cell therapy products, it is necessary to pay close attention to the effects of FBS components on the quality evaluation of hMSCs, especially the immune-regulatory function and differentiation potential, and select suitable batches of FBS for the development and quality control of hMSCs products.
3.Influence mechanism of peer attachment on school adaptation of migrant children: the role of psychological resilience and sense of security

BackgroundMigrant children face many challenges in the process of social change and adaptation to a new environment, especially in school adaptation. Studies have shown that peer attachment plays a vital role in the social adaptation of children and adolescents, while psychological resilience and sense of security, as important psychological resources, also play a moderating and mediating role in individuals' coping with environmental changes. However, there is a lack of systematic research on how peer attachment affects the school adaptation of migrant children through psychological resilience and whether this process is moderated by sense of security. ObjectiveTo explore the relationship between peer attachment and school adaptation of migrant children and to examine the path of psychological resilience and sense of security in it, so as to provide references for improving the school adaptation of migrant children. MethodsUsing cluster sampling method, 695 migrant children in grades 4 to 6 of a primary school in an urban-rural fringe area of Sichuan Province were selected from April 1 to 30, 2022. Assessments were conducted using Revised Inventory for Parent and Peer Attachment (IPPA-R), Resilience Scale for Chinese Adolescents (RSCA), Scale of Sense of Security of Children Left Behind (SSSCLB) and Scale of School Adjustment of Student (SSAS). Process 4.1 was used to examine the role of psychological resilience and sense of security. ResultsA total of 631 (90.79%) valid questionnaires were gathered. There were significant positive correlations among IPPA-R peer attachment subscale score, RSCA score, SSSCLB score and SSAS score (r=0.160~0.600, P<0.01). Peer attachment had a significant positive predictive effect on the school adaptation (β=0.178, P<0.01) and psychological resilience (β=0.518, P<0.01) of migrant children. Psychological resilience had positive predictive effect on the school adaptation (β=0.467, P<0.01). Psychological resilience played a partial mediating role in the relationship between peer attachment and school adaptation, with the mediating effect value was 0.242 (95% CI: 0.184~0.302), accounting for 57.62% of the total effect. Moreover, the interaction term between psychological resilience and sense of security had a significant predictive effect on school adaptation (β=0.103, P<0.01). ConclusionThe psychological resilience of migrant children plays a partial mediating role in the relationship between peer attachment and school adaptation, and the status of sense of security can moderate the relationship between psychological resilience and school adaptation of migrant children.
4.The impact of different surgical methods on the surgical outcomes and short-term prognosis of pig-to-pig kidney transplantation

Objective To investigate the impact of two different surgical methods, orthotopic kidney transplantation and abdominal heterotopic kidney transplantation, on the surgical outcomes of pig-to-pig kidney transplantation and the short-term survival of recipient pigs after surgery. Methods Twenty-four Bama miniature pigs were divided into two groups, with 12 pigs in each group, and underwent orthotopic kidney transplantation and abdominal heterotopic kidney transplantation, respectively. The perioperative indicators of the recipient pigs, renal blood perfusion, the overall incidence rate of complications and survival rate were compared between the two surgical methods. Results The total surgical time, renal artery anastomosis time, renal vein anastomosis time, cold ischemia time and total ischemia time were all shorter in the abdominal heterotopic kidney transplantation group than in the orthotopic kidney transplantation group, with statistically significant differences (all P<0.05). The number of satisfactory renal perfusion cases was higher in the abdominal heterotopic kidney transplantation group than in the orthotopic kidney transplantation group (83% vs. 75%), but the difference was not statistically significant (P>0.05). The total incidence of postoperative complications was 33% in the heterotopic kidney transplantation group, with a survival rate of 92%, and the cause of death was rupture of the vascular anastomosis. The total incidence of postoperative complications was 50% in the orthotopic kidney transplantation group, with a survival rate of 83%, and the causes of death were renal vein thrombosis and renal artery thrombosis. There were no statistically significant differences in the total incidence of postoperative complications and survival rates between the two groups (all P>0.05). Conclusions Compared with orthotopic kidney transplantation, abdominal heterotopic kidney transplantation showes better surgical outcomes in pig-to-pig kidney transplantation and is more beneficial for the short-term survival of recipient pigs after surgery. This provides experience for improving the stability of pig-to-non-human primate kidney xenotransplantation models in the future.

Objective To monitor the susceptibility of clinical isolates to antimicrobial agents in tertiary hospitals in major regions of China in 2022.Methods Clinical isolates from 58 hospitals in China were tested for antimicrobial susceptibility using a unified protocol based on disc diffusion method or automated testing systems.Results were interpreted using the 2022 Clinical &Laboratory Standards Institute(CLSI)breakpoints.Results A total of 318 013 clinical isolates were collected from January 1,2022 to December 31,2022,of which 29.5%were gram-positive and 70.5%were gram-negative.The prevalence of methicillin-resistant strains in Staphylococcus aureus,Staphylococcus epidermidis and other coagulase-negative Staphylococcus species(excluding Staphylococcus pseudintermedius and Staphylococcus schleiferi)was 28.3%,76.7%and 77.9%,respectively.Overall,94.0%of MRSA strains were susceptible to trimethoprim-sulfamethoxazole and 90.8%of MRSE strains were susceptible to rifampicin.No vancomycin-resistant strains were found.Enterococcus faecalis showed significantly lower resistance rates to most antimicrobial agents tested than Enterococcus faecium.A few vancomycin-resistant strains were identified in both E.faecalis and E.faecium.The prevalence of penicillin-susceptible Streptococcus pneumoniae was 94.2%in the isolates from children and 95.7%in the isolates from adults.The resistance rate to carbapenems was lower than 13.1%in most Enterobacterales species except for Klebsiella,21.7%-23.1%of which were resistant to carbapenems.Most Enterobacterales isolates were highly susceptible to tigecycline,colistin and polymyxin B,with resistance rates ranging from 0.1%to 13.3%.The prevalence of meropenem-resistant strains decreased from 23.5%in 2019 to 18.0%in 2022 in Pseudomonas aeruginosa,and decreased from 79.0%in 2019 to 72.5%in 2022 in Acinetobacter baumannii.Conclusions The resistance of clinical isolates to the commonly used antimicrobial agents is still increasing in tertiary hospitals.However,the prevalence of important carbapenem-resistant organisms such as carbapenem-resistant K.pneumoniae,P.aeruginosa,and A.baumannii showed a downward trend in recent years.This finding suggests that the strategy of combining antimicrobial resistance surveillance with multidisciplinary concerted action works well in curbing the spread of resistant bacteria.
6.Construction and validation of a prediction model for prolonged hospitalization in patients with severe acute pancreatitis

Objective:To construction the risk factors associated with prolonged hospitalization in patients with severe acute pancreatitis (SAP) and develop a prediction model for assessing these risks.Methods:SAP patients admitted to the department of emergency of Hebei Province Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from January 2015 to December 2023 were retrospectively selected as the study subjects. The 75% of hospital stay was used as the cut-off point, and the patients were categorized into a normal group and an extended group. Clinical indicators of patients were collected, and independent risk factors for prolonged hospital stay in SAP patients were analyzed using multifactor Logistic regression. A prediction model was established, and a nomogram was created. The efficiency of the prediction model was evaluated using a receiver operator characteristic curve (ROC curve). The accuracy of the model was assessed using Hosmer-Lemeshow goodness-of-fit test. Decision curve analysis (DCA) was employed to evaluate the clinical applicability of the model. Finally, internal validation of the model was conducted using Bootstrap method.Results:A total of 510 patients with SAP were included, and the length of hospital stay was 18 (6, 44) days, including 400 cases in the normal group (<24 days) and 110 cases in the extended group (≥24 days). Multivariate Logistic regression analysis showed that abdominal effusion [odds ratio ( OR) = 4.163, 95% confidence interval (95% CI) was 2.105-8.234], acute physiology and chronic health evaluation Ⅱ (APACHEⅡ; OR = 1.320, 95% CI was 1.185-1.470), C-reactive protein (CRP; OR = 1.006, 95% CI was 1.002-1.011), modified CT severity index (MCTSI; OR = 1.461, 95% CI was 1.213-1.758), procalcitonin (PCT; OR = 1.303, 95% CI was 1.095-1.550) and albumin ( OR = 0.510, 95% CI was 0.419-0.622) were independent risk factors for prolonged hospital stay in SAP patients (all P < 0.01). ROC curve analysis showed that the area under the curve (AUC) of the model was 0.922 (95% CI was 0.896-0.947), the optimal cut-off value was 0.726, the sensitivity was 87.3%, and the specificity was 85.3%. Hosmer-Lemeshow test showed that χ 2 = 5.79, P = 0.671. It showed that the prediction model had good prediction efficiency and fit degree. The DCA curve showed that the prediction probability of the model could bring more clinical benefits to patients at 0.1 to 0.7. Bootstrap internal verification showed that the model had a high consistency (AUC = 0.916). Conclusions:Abdominal effusion, high APACHEⅡ score, high CRP, high MCTSI, high PCT and low albumin level are significantly associated with prolonged hospital stay in SAP patients. The prediction model can help clinicians make more scientific clinical decisions for SAP patients.

The comprehensive detection and identification of active ingredients in complex matrices is a crucial challenge.Liquid chromatography coupled with high-resolution mass spectrometry(LC-HRMS)is the most prominent analytical platform for the exploration of novel active compounds from complex matrices.However,the LC-HRMS-based analysis workflow suffers from several bottleneck issues,such as trace content of target compounds,limited acquisition for fragment information,and uncertainty in interpreting relevant MS2 spectra.Lycibarbarspermidines are vital antioxidant active ingredients in Lycii Fructus,while the reported structures are merely focused on dicaffeoylspermidines due to their low content.To comprehensively detect the new structures of lycibarbarspermidine derivatives,a"depict"strategy was developed in this study.First,potential new lycibarbarspermidine derivatives were designed according to the biosynthetic pathway,and a comprehensive database was established,which enlarged the coverage of lycibarbarspermidine derivatives.Second,the polarity-oriented sample prep-aration of potential new compounds increased the concentration of the target compounds.Third,the construction of the molecular network based on the fragmentation pathway of lycibarbarspermidine derivatives broadened the comprehensiveness of identification.Finally,the weak response signals were captured by data-dependent scanning(DDA)followed by parallel reaction monitoring(PRM),and the efficiency of acquiring MS2 fragment ions of target compounds was significantly improved.Based on the integrated strategy above,210 lycibarbarspermidine derivatives were detected and identified from Lycii Fructus,and in particular,170 potential new compounds were structurally characterized.The integrated strategy improved the sensitivity of detection and the coverage of low-response components,and it is expected to be a promising pipeline for discovering new compounds.
8.Clinical Application of Microwave Ablation in Potentially Resectable Colorectal Cancer With Simultaneously Multiple Liver Metastases

Objective To analyze the clinical efficacy of microwave ablation in the colorectal cancer with simultaneously multiple liver metastases that was initially evaluated as potentially resectable.Methods The patients with potentially resectable colorectal cancer with simultaneous multiple liver metastases treated in the De-partment of General Surgery of the First Affiliated Hospital of Hebei North University,the Center of Minimally Invasive Therapy in Oncology of Traditional Chinese and Western Medicine in Dongzhimen Hospital of Beijing U-niversity of Chinese Medicine,and the Second Department of General Surgery in the Fourth Hospital of Hebei Medical University from October 1,2018 to October 1,2020 were selected in this study.The general data,pathological features,treatment methods,and clinical efficacy of the patients were collected.According to the treatment methods,the patients were assigned into a surgical resection group(conversion therapy + laparoscopic primary resection +hepatectomy)and a microwave ablation group(conversion therapy +laparoscopic primary re-section +microwave ablation).The surgical indicators(operation duration,time to first postoperative anal ex-haust,hospital stay,etc.)and postoperative complications(anastomotic stenosis,anastomotic hemorrhage,incision infection,etc.)were compared between the two groups.The survival period was followed up,including the overall survival period and disease-free survival period,and the survival curves were drawn to analyze the clinical efficacy of the two treatment regimens.Results A total of 198 patients with potentially resectable color-ectal cancer with simultaneous multiple liver metastases were included in this study.Sixty-six patients were cured by neoadjuvant chemotherapy(FOLFOX or FOLFIRI),including 30 patients in the surgical resection group and 36 patients in the microwave ablation group(with 57 tumors ablated).After the first ablation,54(94.74%)tumors achieved complete ablation,and all of them reached no evidence of disease status after re-ablation.The microwave ablation group had shorter operation duration,less intraoperative blood loss,shorter time to first post-operative anal exhaust,shorter time of taking a liquid diet,shorter hospital stay,and lower hospitalization cost than the surgical resection group(all P<0.001).In addition,the microwave ablation group had lower visual analogue scale score(P<0.001)than the surgical resection group.The incidences of complications such as inci-sion infection(P =0.740),anastomotic fistula(P =1.000),and anastomotic stenosis(P =1.000),the over-all survival period(P =0.191),and the disease-free survival period(P =0.934)showed no significant differ-ences between the two groups.Conclusions For patients with colorectal cancer with simultaneous multiple liver metastases initially assessed as potentially resectable,laparoscopic primary resection +surgical resection/microwave ablation after conversion therapy was safe,effective,and had similar survival outcomes.Microwave ablation outper-formed surgical resection in postoperative recovery,economy,and tolerability,being worthy of clinical promotion.
9.Relationship between the expression of low-level placental growth factor in the second trimester of pregnancy and pathological obstetrics

Objective:To study the relationship between the low expression of placenta growth factor (PLGF) in the second trimester of pregnancy and obstetrics pathology.Methods:A total of 177 cases of pregnant women in the second-trimester who received regular prenatal checkups in the Department of Obstetrics, Beijing Friendship Hospital, Capital Medical University from March 2023 to May 2024 were selected as research subjects. Among them, 87 cases expressed low level of PLGF; 90 cases with normal PLGF were randomly selected as the control group. The measurement data of normal distribution were expressed as xˉ± s, the comparison of mean between two groups was performed by independent sample t-test, the percentage was calculated by counting data, and the comparison of rates between two groups was performed by χ2 test. P≤0.05 was statistically significant. Results The incidence of preeclampsia was 13.79% (12/87) in the low PLGF group and 2.22% (2/90) in the control group. There was significant difference between the two groups ( χ2=8.13, P=0.004). In 87 cases with low level of PLGF, there were 10 cases of fetal growth restriction and 11 cases of premature delivery during pregnancy. Compared with normal pregnancy group, the rate of fetal growth restriction and premature delivery in low level of PLGF group was significantly higher, there was significant difference between the two groups ( χ2=4.33, P=0.037; χ2=5.26, P=0.022). The incidence of fetal distress in the PLGF low-level group was 10.34% (9/87), while the incidence of fetal distress in the PLGF normal group was 4.44% (4/90). There was no statistically significant difference in the incidence of fetal distress between the two groups ( χ2=2.26, P=0.132). In this study, 3 cases (3.45%, 3/87) of placental abruption occurred in the PLGF low-level group, while only 1 case (1.11%, 1/90) occurred in the PLGF normal group. Although the incidence of placental abruption in the PLGF group was higher than that in the PLGF normal group, the difference between the groups was not statistically significant ( χ2=0.29, P=0.589). Conclusion:The decrease of PLGF in the second trimester of pregnancy is closely related to preeclampsia, fetal growth restriction and premature delivery. For pregnant women with low level of PLGF.
10.Expert consensus on perioperative basic prevention for lower extremity deep venous thrombosis in elderly patients with hip fracture (version 2024)

Hip fracture in the elderly is characterized by high incidence, high disability rate, and high mortality and has been recognized as a public health issue threatening their health. Surgery is the preferred choice for the treatment of elderly patients with hip fracture. However, lower extremity deep venous thrombosis (DVT) has an extremely high incidence rate during the perioperative period, and may significantly increase the risk of patients′ death once it progresses to pulmonary embolism. In response to this issue, the clinical guidelines and expert consensuses all emphasize active application of comprehensive preventive measures, including basic prevention, physical prevention, and pharmacological prevention. In this prevention system, basic prevention is the basis of physical and pharmacological prevention. However,there is a lack of unified and definite recommendations for basic preventive measures in clinical practice. To this end, the Orthopedic Nursing Professional Committee of the Chinese Nursing Association and Nursing Department of the Orthopedic Branch of the China International Exchange and Promotive Association for Medical and Health Care organized relevant nursing experts to formulate Expert consensus on perioperative basic prevention for lower extremity deep venous thrombosis in elderly patients with hip fracture ( version 2024) . A total of 10 recommendations were proposed, aiming to standardize the basic preventive measures for lower extremity DVT in elderly patients with hip fractures during the perioperative period and promote their subsequent rehabilitation.
