1.Causal relationship between gut microbiota and idiopathic pulmonary fibrosis: A bi-directional two-sample Mendelian randomization study

Objective To investigate the causal relationship between gut microbiota and idiopathic pulmonary fibrosis (IPF). Methods Genome-wide association studies (GWAS) data of gut microbiota and IPF were obtained from MiBioGen and IEU OpenGWAS, respectively. Instrumental variables were screened by means of significance, linkage disequilibrium, weak instrumental variable screening, and removal of confounding factors (genetics, smoking, host characteristics). Inverse variance weighted (IVW) was used as the main Mendelian randomization (MR) analysis method, and the weighted median, simple mode, MR-Egger, and weighted mode were used to perform MR to reveal the causal effect of gut microbiota and IPF. The Cochrane's Q, leave-one-out, MR-Egger-intercept, and Mendelian randomization pleiotropy residual sum and outlier (MR-PRESSO) and Steiger tests were used to analyze the heterogeneity, horizontal pleiotropy, outliers, and directionality, respectively. Results IVW analysis results showed that Actinobacteria [OR=1.773, 95%CI (1.323, 2.377), P<0.001], Erysipelatoclostridium [OR=2.077, 95%CI (1.107, 3.896), P=0.023], and Streptococcus [OR=1.35, 95%CI (1.100, 1.657), P=0.004] could increase the risk of IPF. Bifidobacterium [OR=0.668, 95%CI (0.620, 0.720), P<0.001], Ruminococcus [OR=0.434, 95%CI (0.222, 0.848), P=0.015], and Tyzzerella [OR=0.479, 95%CI (0.304, 0.755), P=0.001] could reduce the risk of IPF. No significant heterogeneity, horizontal pleiotropy, outliers, and reverse causality were found. Conclusion Actinobacteria, Erysipelatoclostridium and Streptococcus may increase the risk of IPF, while Bifidobacterium, Ruminococcus and Tyzzerella may reduce the risk of IPF. Regulation of the above gut microbiota may become a new direction in the study of the pathogenesis of IPF.
2.Expression levels and clinical significance of microRNA-152-3p and sex determining region-box transcription factor 5 in gastric cancer tissues

Objective To investigate the expression levels of microRNA-152-3p(miR-152-3p)and sex determining region-box transcription factor 5(SOX5)in gastric cancer(GC)tissues and their clinical significance.Methods A total of 120 GC patients were selected as study subjects.Re-al-time quantitative polymerase chain reaction(qRT-PCR)was used to detect the expression levels of miR-152-3p and SOX5,and their relationships with the clinicopathological characteristics of GC pa-tients were analyzed.The Kaplan-Meier method was applied to draw survival curves,and the Cox re-gression model was used to screen the prognostic influencing factors of GC patients.Results The ex-pression level of miR-152-3p in GC tissues was lower than that in adjacent non-cancerous tissues,while the expression level of SOX5 was higher than that in adjacent non-cancerous tissues(P<0.05).The expressions of miR-152-3p and SOX5 in GC tissues were all associated with lymph node metastasis,degree of differentiation,and TNM stage(P<0.05).The expression level of miR-152-3p was negatively correlated with that of SOX5(r=-0.512,P<0.05),and there were targeted binding sites between miR-152-3p and SOX5.The 5-year specific survival rate of the low miR-152-3p expression group was lower than that of the high miR-152-3p expression group(60.53%versus 84.09%,x2=7.256,P<0.05).The 5-year specific survival rate of the high SOX5 expression group was 63.10%,which was lower than 83.33%of the low SOX5 expression group,with a statis-tically significant difference(x2=4.840,P<0.05).Multivariate Cox regression analysis showed that the expression levels of miR-152-3p and SOX5,as well as the TNM stage,were all independent prog-nostic influencing factors for GC patients(P<0.05).Conclusion Low expressions of miR-152-3p and high expression of SOX5 are observed in GC tissues,and their expression levels are closely re-lated to clinicopathological characteristics such as TNM stage and patients' prognosis.
3.Construction of perioperative clinical medicine pathway system based on work breakdown structure

In order to promote rational drug use in perioperative period, a perioperative clinical medication pathway system was constructed in the Affiliated Hospital of Qingdao University using the project management method of work breakdown structure (WBS). To establish this system, the following 7 tasks should be completed: requirement investigation of the pathway, formulation of drug usage standards, formulation of clinical medicine pathways, clinical communication and training, effect evaluation and supervision, informazation of medication supervision, and therapeutic drug monitoring, which were implemented by pharmacists of different specialties, respectively. After 4 years of effort, 6 general clinical medicine pathways were completed for antibiotics, analgesics, drugs in venous thromboembolism prophylaxis, nutritional support agents, airway management drugs, and proton pump inhibitors, respectively. These pathways had positive effects in improving the rational use of antibiotics, optimizing the postoperative pain management, and strengthening the risk assessment of thrombosis for patients in the surgical department. The personalized pathway constructed for the Cardiac Surgery Department and the multidimensional pharmaceutical intervention in the Anesthesiology Department also had remarkable effects. In conclusion, the construction of perioperative medication pathway system through WBS was helpful to refine the division of work tasks, reflect the value of pharmacists, and improve the quality of perioperative pharmaceutical services.
4.Design and Financial Release Estimation for the Individual Accounts Reform Plan of Basic Medical Insurance for Employees in the Context of Outpatient+Co-payment Reform

Based on the national policy direction of improving the individual account crediting method for employees'basic medical insurance,it designed three individual account reform scenarios,considering the individual account reform and fund sustainability considerations.Based on the 2022 data of the basic medical insurance for employees in Shandong Province,simulations were conducted to predict the overall scale of individual account reform funds released and individual subtractions under each scenario after the full implementation of the individual account reform in 2024,as well as to conduct an early warning analysis of public opinion risks for key populations.The results indicated that Plan I had the largest total amount of released funds from individual accounts(16.55 billion yuan),but the highest per capita monthly reduction in credits(52.97 yuan),thereby increasing the risk of generating social public sentiment.Plan III minimized the resistance to the implementation of the individual account reform but has the smallest total amount of released funds from individual accounts(13.484 billion yuan).Plan II achieved areas such as the provincial government and oil fields have the highest public sentiment risk due to the highest reduction ratio for retirees(>90%);11 coordinated areas have a percentage of retiree subtraction of more than 80%.It provided empirical evidence for policymakers on different plans for the reform of individual accounts in the employee basic medical insurance and their impacts,and suggested that high-risk areas should be given special attention,adopting refined and differentiated policy measures to effectively alleviate social public sentiment pressure and ensure the smooth progress of the reform.

7.Risk factors and risk prediction model for coronary atherosclerotic heart disease in Xining Area

Objective To screen the risk factors and risk prediction model for coronary atherosclerotic heart disease in Xining area. Methods Five hundred and eighteen patients with coronary atherosclerotic heart disease who attended the Qinghai Specialized Hospital for Cardiovascular and Cerebrovascular Diseases and Cerebrovascular Diseases from May 2018 to September 2022 were selected as the observation group, and another 421 patients with non-coronary heart disease were set as control group. The general information of patients were collected. The risk factors affecting the development of coronary heart disease were screened using Logistic regression analysis, and a risk prediction model was constructed, then receiver operating characteristic (ROC) curve was plotted to validate the predictive value of risk model. Results The gender ratio and smoking history yielded no statistical difference between two groups (P>0.05), but statistical difference was found in age, body mass index (BMI), diabetes history, hypertension history, smoking history and family history between two groups (P<0.05). Serum levels of total cholesterol (TC), low-density lipoprotein (LDL) and uric acid (UA) were all higher than the control group, and serum high-density lipoprotein (HDL) level was lower than the control group, with statistical difference (all P<0.05). Multivariate Logistic regression analysis denoted that age, BMI, diabetes history, hypertension history, smoking history, family history, TC, LDL, and UA were risk factors for the development of coronary heart disease (P<0.05). ROC curve analysis yielded an AUC of 0.890 for the risk model, with a sensitivity and specificity of 72.03% and 91.14%, respectively. Conclusion Patient's age, BMI, and the presence of diabetes mellitus, hypertension and smoking, family history, abnormal blood lipid profiles, and abnormal blood uric acid are all risk factors for the development of coronary heart disease, and the risk model constructed on the basis of the above risk factors has a high degree of sensitivity and specificity, which is of great value in accurately evaluating the risk of coronary heart disease.
8.Therapeutic Effect of Gandou Decoction on Wilson's Disease with Dampness Heat Accumulation Accompanied by Rapid Eye Movement Sleep Behavior Disorder

ObjectiveTo explore the clinical efficacy of Gandou decoction in treating Wilson's disease (WD) with dampness heat accumulation accompanied by rapid eye movement (REM) sleep behavior disorder (RBD). MethodFrom April 2019 to August 2023,62 patients with dampness heat accumulation type WD accompanied by RBD who met the inclusion criteria were selected from the Department of Encephalopathy at the First Affiliated Hospital of Anhui University of Chinese Medicine. They were randomly divided into a control group and an observation group with 31 cases each using a computer distributor. The control group received routine copper removal treatment,while the observation group received additional treatment with Gandou decoction on the basis of the control group. Eight days was one course of treatment,totaling three courses. The scores of traditional Chinese medicine syndromes,RBD screening questionnaire (RBDSQ) scores,RBD questionnaire-Hong Kong (RBDQ-HK) scores,polysomnography (PSG) parameters,24-hour urine copper (24 h U-Cu) levels,and non-ceruloplasmin-bound copper (NCC) levels between the two groups before and after treatment were compared,and adverse reactions were observed. ResultSixty trial cases were ultimately completed,with 30 cases in each group. Before treatment,there was no statistically significant difference in various indicators between the two groups, and thus they were comparable. Compared with those before treatment,the traditional Chinese medicine syndrome scores,RBDSQ scores and RBDQ-HK scores of the two groups were significantly reduced,the 24 h U-Cu levels were significantly increased,and the NCC levels were significantly reduced (P<0.05,P<0.01). Compared with the control group, the observation group showed better improvement in traditional Chinese medicine syndrome scores, RBDSQ scores, RBDQ-HK scores, and NCC levels (P<0.05,P<0.01). Compared with those before treatment,the total sleep time (TST),sleep efficiency (SE),sleep/REM latency,the proportion of N1/N2/REM stages,arousal index (ARI),and proportion of phasic electromyographic activity (P-EMG-A) were significantly improved in both groups (P<0.05). Compared with the control group after treatment,the observation group showed more significant improvements in the proportion of TST,SE,REM stages,ARI,and P-EMG-A proportion (P<0.05). ConclusionGandou decoction can not only improve the traditional Chinese medicine syndrome of WD patients with dampness heat accumulation accompanied by RBD but also alleviate their RBD symptoms.
9.Value of DUSP6 in peripheral blood mononuclear cells in predicting adverse cardiovascular events after peritoneal dialysis in diabetic nephropathy

Objective:Adverse cardiovascular events are the leading cause of death in peritoneal dialysis patients.Identifying indicators that can predict adverse cardiovascular events in these patients is crucial for prognosis.This study aims to assess the value of dual-specificity phosphatase 6(DUSP6)in peripheral blood mononuclear cells as a predictor of adverse cardiovascular events after peritoneal dialysis in diabetic nephropathy patients. Methods:A total of 124 diabetic nephropathy patients underwent peritoneal dialysis treatment at the Department of Nephrology of the First Affiliated Hospital of Hebei North University from June to September 2022 were selected as study subjects.The levels of DUSP6 in peripheral blood mononuclear cells were determined using Western blotting.Patients were categorized into high-level and low-level DUSP6 groups based on the median DUSP6 level.Differences in body mass index,serum albumin,high-sensitivity C-reactive protein,and dialysis duration were compared between the 2 groups.Pearson,Spearman,and multiple linear regression analyses were performed to examine factors related to DUSP6.Patients were followed up to monitor the occurrence of adverse cardiovascular events,and risk factors for adverse cardiovascular events after peritoneal dialysis were analyzed using Kaplan-Meier and Cox regression. Results:By the end of the follow-up,33(26.61%)patients had experienced at least one adverse cardiovascular event.The high-level DUSP6 group had higher body mass index,longer dialysis duration,and higher high-sensitivity C-reactive protein,but lower serum albumin levels compared to the low-level DUSP6 group(all P<0.05).DUSP6 was negatively correlated with serum albumin levels(r=-0.271,P=0.002)and positively correlated with dialysis duration(rs=0.406,P<0.001)and high-sensitivity C-reactive protein(rs=0.367,P<0.001).Multiple linear regression analysis revealed that dialysis duration and high-sensitivity C-reactive protein were independently correlated with DUSP6 levels(both P<0.05).The cumulative incidence of adverse cardiovascular events was higher in the high-level DUSP6 group than in the low-level DUSP6 group(46.67%vs 7.81%,P<0.001).Cox regression analysis indicated that low serum albumin levels(HR=0.836,95%CI 0.778 to 0.899),high high-sensitivity C-reactive protein(HR=1.409,95%CI 1.208 to 1.644),and high DUSP6(HR=6.631,95%CI 2.352 to 18.693)were independent risk factors for adverse cardiovascular events in peritoneal dialysis patients. Conclusion:Dialysis duration and high-sensitivity C-reactive protein are independently associated with DUSP6 levels in peripheral blood mononuclear cells of diabetic nephropathy patients undergoing peritoneal dialysis.High DUSP6 levels indicate a higher risk of adverse cardiovascular events.
10.Research progress on the impact of diabetes mellitus in male fertility

Diabetes mellitus is a global epidemic characterized by high incidence and mortality rates. It can lead to both acute and chronic systemic complications. Diabetes-induced male reproductive disorders have become a focal point of concern. Numerous studies have shown that hyperglycemia can impair male fertility through various pathways, including erectile and ejaculatory dysfunction, disruption of the hypothalamic-pituitary-gonadal axis, damage to the epididymis and testis, impaired semen quality, abnormalities in testicular and sperm glucose metabolism, and adverse effects on offspring health. The mechanisms involved include oxidative stress, inflammation, autophagy, apoptosis, epigenetic regulation, and mitochondrial damage. Interventions for male fertility impairment include diabetes treatment drugs, antioxidants, Traditional Chinese Medicine and acupuncture, mesenchymal stem cell therapy, exercise and physical training, as well as gut microbiota interventions. This article aims to review the current research on the relationship between diabetes and male fertility, explore the underlying mechanisms leading to male infertility, and identify potential interventions to improve male reproductive health, which holds significant clinical value.
