Acute lung injury (ALI) is a critical respiratory emergency caused by various non-cardiogenic factors, including severe pulmonary and extrapulmonary infections, lung contusion, and sepsis. Its pathological features are centered on alveolar-capillary barrier dysfunction, uncontrolled inflammatory responses, and impaired alveolar fluid clearance, manifesting as refractory hypoxemia and respiratory distress, with a poor clinical prognosis. Modern studies have revealed that mitochondrial dysfunction and glucose metabolic reprogramming in macrophages, characterized by hyperactivated glycolysis, drive pro-inflammatory polarization and represent a key mechanism leading to excessive inflammatory responses and disease progression in ALI. Therefore, targeting macrophage glycolysis may be a potential therapeutic strategy for ALI. The Plain Questions: Special Discussion on Channels and Vessels in Huangdi's Internal Classic states that "The spleen disseminates essence upward to the lungs". When the spleen fails in transport and transformation, the distribution of refined nutrients becomes disordered, resulting in the endogenous generation of turbid pathogens that obstruct the pulmonary collaterals, causing failure of lung dispersion and descent and leading to dyspneic reversal. This forms the dynamic pathological pattern of ''deficiency in the root with excess in the manifestation'', thereby proposing the core pathogenesis of ALI as ''spleen deficiency with transport dysfunction and turbid pathogens obstructing the lungs''. This process is intrinsically associated with the inflammatory cascade triggered by disordered macrophage glucose metabolism. Taking the theory of ''spleen failing to disperse essence'' as the entry point, the present study systematically elucidates the progressive pathological mechanism of "energy metabolism disorder-cytokine storm'' in ALI from the perspectives of the ''spleen deficiency-turbid obstruction'' pathogenesis and macrophage glycolysis-pro-inflammatory polarization. In addition, we summarize Chinese medicine compound formulas and monomeric components that ''restore spleen transport and eliminate turbid pathogens'', and explore their potential mechanisms in the treatment of ALI by regulating macrophage metabolic phenotypes to improve the state of ''spleen failing to disperse essence''. This provides a molecular basis for elucidating the ''spleen-lung'' energy metabolism dialogue, deepens understanding of the multi-target regulatory mechanisms of herbal prescriptions, and promotes the deep integration of classical theory with modern medicine.

Objective: To evaluate the efficacy of the left spermatic vein transposition to the great saphenous vein in treating left varicocele (VC) secondary to nutcracker syndrome (NCS). Methods: Clinical data of 8 patients treated during Feb.2020 and Feb.2023 in our hospital were retrospectively analyzed.A meticulous preoperative evaluation of the vascular status of the spermatic vein and the great saphenous vein was performed using color Doppler ultrasound.A spermatic vein-great saphenous vein shunt surgery was performed in patients who were strictly selected. The clinical symptoms and hemodynamics of renal vein were compared before and after operation. Results: The median age of patients was 23.5(18-33) years. There was a notable reduction in post-exercise scrotal and lower back pain in all patients, and the score of scrotal pain decreased to 0 in 7 patients. The median quantification of urinary protein was 352.8(54.4-687.3) mg prior to surgical intervention, which significantly diminished to 125.5(25.9-255.1) mg 6 months after operation.Notably,3 cases of preoperative positive urine occult blood tests were undetectable in the subsequent postoperative assessments. The median peak blood flow velocity at the site of stenosis in the left renal vein measured at 74.4(48.7-117.6) cm/s preoperatively, subsequently reduced to 45.1(25.5-61.2) cm/s postoperatively.During the 6-month follow-up,no recurrence of varicocele, vascular anastomotic stenosis or thrombosis were observed. Conclusion: Our research indicates that spermatic vein to great saphenous vein bypass is safe and feasible in the treatment of left varicocele secondary to nutcracker syndrome for strictly selected patients, which can effectively alleviate renal vein congestion without significant complications.

Objective This study aims to explore the correlation between the grade of testicular tissue damage and clinical features which is used to evaluate the viability of testicular tissue after the orchidopexy for testicular torsion. Methods We conducted a retrospective, multicenter analysis of pathological specimens obtained from patients who under-went unilateral orchiectomy due to testicular torsion.Tissue viability was reassessed by the Mikuz grading scale. The corre-lations among tissue viability, duration of torsion symptoms, the angle of torsion, preoperative Doppler ultrasound image and intraoperative testicular resuscitation methods were analyzed. Results Eighty-five pathological specimens were en-rolled from 4 medical centers. The age range was 18 years(IQR 14-33),with a median torsion angle of 480°(IQR 180°-1080°).The interval from the torsion onset to surgical intervention was 18 hours(IQR 9-84).In the subgroup analysis of different torsion durations, the distribution of Grade I injuries was as follows. The subgroup within 12 hours ac-counted for 37.5%.The subgroup between 12 to 24 hours accounted for 14.28%.And the subgroup between 24 to 48 hours ac-counted for 6.45%.The subgroup exceeding 48 hours only accounted for 3.13%.Four patients received orchiectomy due to pre-operative ultrasound image indicating necrosis, correlating with Mikuz grade 3 tissue damage. The decision was made based on the failure of color recovery of affected testis in 57 cases, revealing 12.28%with grade 1 and 35.89%with grade Ⅱ damage.Twenty-four cases were identified as an Arda score of 3,with 4.16%showing grade 1 and 20.83%showing grade Ⅱ Mikuz damage. The method of Arda's grading could be used to determine the vitality of testicular tissue more accurately compared to the post-resusci-tation color observation method(P=0.032).Conclusion Pathological reassessment provides a precise determination of tis-sue viability in the affected testis. In some instances, the testicular tissue damage can be found potentially reversible.

OBJECTIVETo evaluate the risk of nasopharyngeal carcinoma (NPC) through EB virus antibody profile by enzyme linked immunosorbent assay (ELISA).
METHODSEBNA 1/IgA, EBNA 1/IgG and zta/IgG by ELISA and VCA/IgA by immmunoenzymatic method were detected in 121 NPC patients and 332 healthy subjects (HS) in the Pearl river estuary.
RESULTSThe sensitivity rates were 85%, 83% and 79% for EBNA 1/IgA, EBNA 1/IgG and zta/IgG, all three of which if combined was the highest 92%. The specificity rates were 86%, 86% and 80% for EBNA 1/IgA, EBNA 1/IgG and zta/IgG, all three of which if combined was also the highest 93%. According to the level of odds ratio, nasopharyngeal carcinoma risk could be divided into 3 groups: low, moderate and high-risk groups. 93% of HS had low risk of NPC with the odds ratio 0.0 to 0.3. 0.4% of HS had high risk of NPC with the odds ratio of 137.9%.
CONCLUSIONELISA is more objective than the traditional immunoenzymatic method in the detection and diagnosis of NPC. The combination of EBNA 1/IgA, EBNA 1/IgG and zta/IgG is able to evaluate the risk of NPC.
