1.Shear wave elastography for evaluating therapeutic effect of ultrasound-guided drug injection for muscle injury

Objective To observe the value of shear wave elastography(SWE)for evaluating therapeutic effect of ultrasound-guided drug injection for muscle injury.Methods Eighty patients with unilateral muscle injury were retrospectively included,including 40 cases underwent ultrasound-guided drug injection(group A)and 40 cases underwent electromagnetic wave physiotherapy plus external application of Yunnan Baiyao Gao(group B).Pain intensity was assessed using visual analogue scale(VAS)before treatment and 3 weeks after the final treatment,while the Young modulus(E)value of the injured muscle was measured before treatment and 1,2 and 3 weeks after final treatment,and the improvements of VAS scores and E values were compared between groups.Results The total effective rate in group A(35/40,87.50%)was higher than that in group B(21/40,52.50%;P<0.05).Before treatment,no significant difference of VAS score was found between group A(8.07±0.83)and group B(7.88±0.85)(P>0.05).After treatment,VAS scores decreased in both groups(both P<0.05),which in group A(2.30±1.07)was more obviously than that in group B(4.80±0.82)(P<0.05).After treatment,E values of injury muscles increased significantly in both groups(P<0.05),while group A had a greater increase in overall magnitude and overall rate than group B(P<0.05).Conclusion Ultrasound-guided drug injection therapy had significant therapeutic effect for muscle injuries,which could be dynamically monitored with SWE.

3.Establishment of a standardized training assessment indicator system for young teachers in medical colleges and universities

Objective:The establish a standardized training assessment indicator system for young teachers in medical colleges and universities, and to improve the quality of training.Methods:A preliminary assessment indicator system was established through literature analysis and small-sample expert interviews, and then the Delphi method was used to conduct consultation with 40 experts familiar with the standardized training of young teachers in medical colleges and universities. The assessment indicator system was modified based on the opinions of the experts, and consultation was conducted again until basically consistent opinions were obtained from the experts. The analytic hierarchy process was used to assign weights to each indicator, and SPSS 17.0 was used for data analysis and processing.Results:After two rounds of expert consultation, 35 experts agreed with each indicator at a rate of more than 90%, and their opinions were basically consistent. The weight variation coefficient (W value) of each indicator ranged from 0.00 to 0.15, indicating a high degree of concentration. Finally, a standardized training assessment indicator system was established for young teachers, which consisted of 4 first-level indicators, 11 second-level indicators, and 35 third-level indicators.Conclusions:This assessment indicator system provides a basis for scientific evaluation of the quality of standardized training for young teachers in medical colleges and universities and improves the quality and efficiency of standardized training for young teachers by "promoting construction through evaluation and promoting supervision through evaluation".
4.Transvaginal combined with transabdominal oocyte retrieval in IVF/ICSI cycle: a case report

Objective:To improve the technique of oocyte retrieval in in vitro fertilization/intracytoplasmic sperm injection-embryo transfer (IVF/ICSI-ET) cycles and reduce the cancellation rate of IVF/ICSI cycle. Methods:The clinical data of a case of IVF/ICSI cycle using transvaginal combined with abdominal oocyte retrieval was reported.Results:The right ovary of one infertile patient who received ICSI due to male factors was not detected repeatedly by transvaginal ultrasound during ovulation induction. The right lower abdominal pain occurred day 5 after using gonadotropin. We considered that the position of the right ovary was high due to pelvic adhesion after previous appendectomy, and was further confirmed by abdominal ultrasound. Two mature oocytes were obtained from the left and right ovaries respectively. Four embryos were obtained after ICSI insemination, and biochemical pregnancy occurred after transplantation.Conclusion:Transabdominal ultrasound-guided oocyte retrieval is relatively safe and effective, and can obtain similar clinical outcomes as conventional transvaginal ultrasound-guided oocyte retrieval. For IVF/ICSI women with difficulty in obtaining oocytes through vagina, transabdominal ultrasound-guided oocyte retrieval can be tried. During IVF/ICSI treatment cycles, it is necessary to improve the quality of medical history inquiry, pre-cycle evaluation, ovulation induction monitoring, oocyte retrieval and embryo transfer.

6.Role of miRNA in the development and progression of cholangiocarcinoma

Cholangiocarcinoma is the most common malignant tumor of the biliary tract, with a relatively high mortality rate and an incidence rate increasing year by year. Due to atypical symptoms in the early stage and complex anatomical location, it is often difficult for patients with cholangiocarcinoma to be diagnosed in the early stage, and therefore, they often miss the optimal treatment period and tend to have poor prognosis. In recent years, studies have shown that miRNAs play a key role in the pathophysiological process of the development and progression of cholangiocarcinoma. This article reviews the regulatory role of miRNAs in the development, invasion, metastasis, drug resistance, and tumor microenvironment of cholangiocarcinoma and introduces the latest advances in exosome miRNA and cholangiocarcinoma, so as to provide potential treatment strategies for cholangiocarcinoma.
7.Effects of Rabeprazole on the Pharmacokinetics of Clopidogrel and Its Active Metabolite in Healthy Volunteers with Different CYP2C19 Genotypes

OBJECTIVE:To investigate the effects of rabepr azole on the pharmacokinetic characteristics of clopidogrel and its active metabolite in healthy volunteers with different CYP2C19 genotypes. METHODS :Healthy volunteers were selected as subjects,and then randomly divided into extensive metabolizer (EM)group,intermediate metabolizer (IM)group,and poor metabolizer(PM)group with 8 subjects in each group ,according to their CYP2C19 genotypes by random number table. In single-dose,randomized,open,two-cycle-crossover design ,each group was given Clopidogrel bisulfate tablets 300 mg or Clopidogrel bisulfate tablets 300 mg+Rabeprazole sodium enteric-coated tablets 20 mg. UPLC-MS/MS method was adopted to detect the concentration of clopidogrel and its active metabolite derivative (MP-H4). The pharmacokinetic parameters were calculated and compared by DAS 2.0 software. RESULTS :There was no statistical significance in clinical data as age ,height, body weight ,liver enzymes and serum creatinine among 3 kinds of metabolism subjects (P>0.05). Compared with subjects receiving clopidogrel alone ,cmax and AUC 0-t of clopidogrel of subjects combined with rabeprazole in EM group were increased by 36% and 27%,while those of MP-H 4 were decreased by 34% and 28%(P<0.01);cmax and AUC 0-t of clopidogrel of subjects combined with rabeprazole in IM group were increased by 19% and 18%,while those of MP-H 4 were decreased by 19% and 16% (P<0.05 or P<0.01);there was no statistical significance in cmax and AUC 0-t of clopidogrel and MP-H 4 in PM group after receiving rabeprazole additionally as well as tmax of clopidogrel and MP-H 4 in all metablism subjects ,compared with clopidogrel alone(P>0.05). CONCLUSIONS :Among CYP2C19 EM and IM subjects ,combined use of rabeprazole can significantly increase the exposure of clopidogrel and decrease the exposure of its active metabolite MP-H 4,but has no significant impact on clopidogrel and its active metabolite in CYP2C19 PM subjects.

Objective:
To understand the health literacy and relevant factors of cancer prevention consciousness in Chinese urban residents from 2015 to 2017.
Methods:
A cross-sectional survey was conducted in 16 provinces covered by the Cancer Screening Program in Urban China from 2015 to 2017. A total of 32 257 local residents aged ≥18 years old who could understand the investigation procedure were included in the study by using the cluster sampling method and convenient sampling method. All local residents were categorized into four groups, which contained 15 524 community residents, 8 016 cancer risk assessment/screening population, 2 289 cancer patients and 6 428 occupational population, respectively. The self-designed questionnaire was used to collect the information of demographic characteristics and cancer prevention consciousness focusing on nine common risk factors, including smoking, alcohol, fiber food, food in hot temperature or pickled food, chewing betel nut, helicobacter pylori, moldy food, hepatitis B infection, estrogen, and exercise. The logistic regression model was adopted to identify the influencing factors.
Results:
The overall health literacy of the cancer prevention consciousness was 77.4% (24 980 participants), with 77.4% (12 018 participants), 79.9% (6 406 participants), 77.2% (1 766 participants) and 74.5% (4 709 participants) in each group (

Objective:
To understand the consciousness of the cancer early detection among urban residents and identify the influencing factors from 2015 to 2017.
Methods:
A cross-sectional survey was conducted in 16 provinces covered by the Cancer Screening Program in Urban China from 2015 to 2017. A total of 32 257 local residents aged ≥18 years old who could understand the investigation procedure were included in the study by using the cluster sampling method and convenient sampling method. All local residents were categorized into four groups, which contained 15 524 community residents, 8 016 cancer risk assessment/screening population, 2 289 cancer patients and 6 428 occupational population, respectively. Self-designed questionnaires were used to collect population, socioeconomic indicators, self-cancer risk assessment, regular participation in physical examination and other information. The multivariate logistic regression model was used to identify the factors of people who had not regularly participated in the regular physical examination in the past five years.
Results:
The self-assessment results of 32 357 residents showed that there were 27.54% (8 882) of total study population with self-reported cancer risk, 45.48% (14 671) without cancer risk and 26.98% (8 704) with unclear judgement on their own cancer risk. Among population with cancer risk, 79.84% (7 091) considered physical examination accounted. In the past five years, there were 21 105 (65.43%) residents participated in regular physical examination and 11 148 (34.56%) participated in non-scheduled one, respectively. Objective:
To understand the consciousness of the cancer early diagnosis among urban residents and identify the related factors from 2015 to 2017.
Methods:

All local residents were categorized into four groups, which contained 15 524 community residents, 8 016 cancer risk assessment/screening population, 2 289 cancer patients and 6 428 occupational population, respectively. The general demographic characteristics, the consciousness of the cancer early diagnosis (whether people would have a willingness or encourage their relatives/friends to confirm the abnormal results once which were detected from the physical examination) and other information were collected by using the self-designed questionnaire. The non-conditional logistic regression model was used to identify the relateol factors related to the consciousness of the cancer early diagnosis.
Results:
As for residents with abnormal result from the physical examination, 89.29% (28 802) of residents would choose to seek medical treatment for further diagnosis. If their relatives/friends had abnormal results from the physical examination, 89.55% (28 886) of residents would encourage their relatives/friends to confirm the diagnosis in time. The non-conditional logistic regression model analysis showed that compared with the public institution staff/civil servants, annual household income less than 20 000 CNY, the western region and the cancer risk assessment/screening intervention population, the company staff, annual household income about 40 000 CNY and more, and the residents from the middle and eastern region had a stronger consciousness to seek further diagnosis; while the unemployed residents and community residents were less likely to seek further diagnosis (
