1.Analysis of clinical,imaging and pathological features of 123 cases of Kikuchi-Fu-jimoto disease

Purpose To investigate the clinical,imaging,and pathological features of Kikuchi-Fujimoto disease(KFD).Methods A retrospective analysis was conducted on 123 pathologically confirmed KFD cases.Clinical and imaging data were collected,and histopathological features were evaluated using HE staining,immunohistochemistry,in situ hybridization for EBER,and molecular analyses(TCR/Ig gene rearrangements by PCR with capillary electro-phoresis).Results Among the 123 patients,the male-to-female ratio was 1∶2,with a median age of 30 years.All patients presented with lymphadenopathy.Among 30 hospitalized patients,63.3%(19/30)had fever,and 23.3%(7/30)had concurrent autoimmune diseases.Of the 12 patients who underwent PET-CT,91.7%(11/12)were sus-pected of malignancy,prompting biopsy recommendations.Among 47 consultation cases,27.7%(13/47)were ini-tially misdiagnosed as lymphoma.Histopathological examination revealed proliferative,necrotic,and xanthomatous phases,which coexisted or occurred independently.The proliferative phase was characterized by atypical lymphocytes and histiocytes,the necrotic phase by abundant eosinophilic fibrin deposits and nuclear debris,and the xanthomatous phase by clusters of foam-like histiocytes.Immunohistochemically analyses revealed that atypical lymphocytes were neg-ative for CD20,CD4,and CD56 but positive for CD3,CD8,TIA1,Granzyme B,and Perforin.Histiocytes expressed CD68,CD163,and MPO,while CD123-positive plasmacytoid dendritic cells were predominantly located around the le-sions and blood vessels.EBER was positive in individual cells in 4 cases.TCR gene rearrangement was positive in 2 cases and suspected positive in 3 cases,while Ig rearrangement was positive and suspected positive in 1 case each.Conclusion KFD exhibits clinical,imaging,and pathological features that can mimic lymphoma,highlighting the im-portance of accurate diagnosis to avoid misdiagnosis and inappropriate treatment.

3.Application of multi-slice spiral CT in the screening of coronary artery lesions in elderly patients with diabetes mellitus

Objective To evaluate the clinical value of multi-slice spiral CT (MSCT)in the screening of coronary artery lesions in elderly patients with diabetes mellitus.Methods MSCT and coronary angiography(CAG)were performed in 136 elderly patients(68 patients with diabetes and 68 patients without diabetes).The number of diseased coronary segments and the plaque type (noncalcified,mixed and calcified)for each patient were determined.The characteristics of coronary lesions were compared between patients with and without diabetes.The sensitivity and specificity of MSCT were evaluated.Results More noncalcified and calcified plaques and few ermixed plaques were observed in patients with diabetes,compared with patients without diabetes(noncalcified plagues:10 cases or 14.7% vs.4 cases or 5.9%;calcified plagues:47 cases or 69.1% vs.39 cases or 57.4%)(P< 0.05).In addition,diabetic patients showed a significantly higher frequency of multivessel disease(P<0.05).Furthermore,MSCT was able to clearly show stenosis and plaque type in the trunk and branches of the coronary artery,with a sensitivity of 91.0%,a specificity of 95.0%,a positive predictive value of 93% and a negative predictive value of 96.0%.Conclusions Diabetes is associated with a higher risk of coronary artery disease.MSCT can effectively detect the characteristics of coronary artery disease in diabetic patients and is an effective,noninvasive,and safe screening method.
4.Diagnostic Value of Dual-phase Enhanced Scan in Detecting Pancreatic Adenocarcinoma with Dual Energy CT

Purpose To explore the diagnostic value of multiple image technique derived from dual-energy CT in arterial+parenchymal phase scan mode in detecting pancreatic adenocarcinoma so as to provide more valuable information for clinical treatment.Materials and Methods Thirty two patients with pancreatic adenocarcinoma proved pathologically underwent dual-phase scan with dual-source CT.Linear blend image,non-linear blend image and iodine map were acquired.The absolute enhancement value of tumor (AEV),the relative enhancement value of tumor (REV),enhancement ratio (ER) of tumor to pancreatic parenchyma,and the image contrast to noise ratio (CNR) were also calculated,so that the diagnostic value and the ability image in two phases to display the pancreatic adenocarcinoma lesions could be assessed.Results On arterial phase,the differences in AEV,REV and CNR value were significant among the three groups images (P<0.05).On parenchymal phase,the difference in REV,ER,and CNR value were also significant among the three groups (P<0.05).When the three sequences on the 2 phases were compared with each other,the differences in AEV and REV value of tumor tissues were significant among the groups (P<0.05).Moreover,the differences of ER value in linear blend image and CNR in the iodine map were significant in dual phase enhancement (P<0.05).Conclusion Dualenergy CT enhanced scan mode on dual phase combined with multiple sequences can improve the sensitivity in detection of pancreatic adenocarcinoma lesions.
5.Appraisal preventive effects of meridian stretch stocking on postgynecological pelvic surgery to LEDVT

Objective To explore the effects and influences of meridian stretch stocking for prevention of lower extremity deep vein thrombosis ( LEDVT ) after gynecological pelvic surgery. Methods A total of 210 patients,who conducted pelvic surgery at Department of Obstetrics and Gynecology, Wuhan Third Hospital from April 2010 to February 2013, were randomly divided into routine group (Group A), the common stretch stocking group ( Group B) and meridian stretch stocking group ( Group C) , 70 cases in each group,according to their check-in time. The patients of Group A followed routine nursing methods, and the patients of Group B complied with normal stretch stocking while the patients of Group C adopted meridian stretch stocking. The usage of stretch stockings was on the basis of conventional nursing methods and applied immediately after the surgery. Then a series of comparisons were made in terms of lower extremity venous blood flow velocity, foot dorsum skin temperature and the incidence of LEDVT, coagulation and fibrinolytic function, etc. Results Group C possessed the lowest LEDVT incidence in three groups and the differences were statistically significant (χ2 =6. 667,P<0. 05). Meanwhile Group C had the fastest posterior divisions superficial vein and popliteal fossa vein blood flow velocity with statistical significance compared with the other two groups (P<0. 01). The lower limb dorsal skin temperature of Group C was higher than that of Group A and Group B after two and four hours′ surgery ( P<0. 01). There was no statistical difference in blood coagulation and fibrinolytic function of the three groups (P>0. 05). Conclusions Meridian stretch stocking can advance venous blood flow velocity, improve lower limb dorsal skin temperature and reduce LEDVT incidence of patients with gynecological pelvic surgery.
6.Randomly comparative study on the therapeutic effects of Fufangkushen injection and Aidi injection on improving quality of life for patients with advanced lung cancer

Objective To explore and compare the therapeutic effects of Fufangkushen injection and Aidi injection on improving quality of life for patients with advanced lung cancer. Methods 60 patients in late stage of lung cancer with pathological diagnosis were split into two groups randomly. 30 patients in Kushen group received Fufangkushen injection plus base therapy, 30 patients in Aidi group received Aidi injection plus base therapy.The data was analyzed on term of tumour size,clinical symptoms,Karnofsky score,cancer marker CA125,CEA,before and after treatment in two groups.The therapeutic effects were evaluated after two circles of treatment. Results Compared the short term therapeutic effect, Kushen group had the stability rate for 83.3 %(25/30),and 80.0 %(24/30) for Aidi group.There was no significant difference.(P>0.05).Compare the clinical therapeutic effect,Kushen group had the improving rate for 83.3 %(25/30),and 80.0 %(24/30) for Aidi group.There was no significant difference(P>0.05).Compare the Karnofsky score,two groups had equal increasing stability rate (P>0.05). Compare decreasing ratio of CEA and CA125 after treatment in two groups,there was no significant difference (P>0.05). Conclusion Fufangkushen injection and Aidi injection both have acceptable therapeutic effects in the treatment of patients with later stage lung cancer.The result is equal. As well as they have some other characters themselves to improve lung cancer related symptoms. Fufangkushen injection is better to improve symptoms of heat toxin, and Aidi injection is better to improve symtoms of deficiency Qi. For Fufangkushen injection, it can also adjust disorder of liver function and relieve pain.
