1.Analysis of learning curve of TiRobot-assisted lumbar pedicle screw fixation based on the cumulative sum test

Objective:To analyze the learning curve of TiRobot-assisted lumbar pedicle screw fixation (LPSF) by cumulative sum (CUSUM) test method.Methods:The clinical data of 50 patients who underwent TiRobot-assisted LPSF from January 2020 to December 2022 in Beijing Friendship Hospital, Capital Medical University were retrospectively analyzed. CUSUM analysis and learning curve fitting were performed with robot usage time as the main indicator with the time for each step refined (robot registration time, path planning time and guide wire placement time), to select the best learning curve fitting model with the R2 value closest to 1. Using the turning point of the learning curve as the boundary, the learning curve was divided into two stages as learning stage and maturity stage, and then the observation indexes were compared between the two stages. Results:All 50 patients successfully completed the surgery without perioperative complications, with a total of 244 pedicle screws implanted. The total robot usage time and robot registration time showed a gradually decreasing trend with the increase of case number, and the learning curves were successfully fitted and reached their peaks at the seventeenth and thirteenth cases respectively. The entire learning process was divided into learning stage (17 cases) and maturity stage (33 cases) based on the turning point of the learning curve of total robot usage time. The path planning time and guide wire placement time did not show significant changes with the increase in the case number. The total robot usage time, robot registration time and the intraoperative blood loss in the learning stage were significantly higher than those in the maturity stage: (35.35 ± 1.58) min vs. (30.61 ± 0.43) min, (20.83 ± 1.56) min vs. (14.94 ± 0.29) min and 400 (150, 500) ml vs. 200 (110, 300) ml, the guide wire placement time of per screw was significantly lower than that in the maturity stage: 2.00 (1.83, 2.34) min/screw vs. 2.33 (2.13, 2.69) min/screw, and there were statistical differences ( P<0.05 or <0.01). There were no statistical difference in the path planning time, path planning time of per screw, guide wire placement time and the accuracy of screw placement between two stages ( P>0.05). Conclusions:TiRobot-assisted LPSF is a new technology with safety and effectiveness, and it has a relatively short learning curve. To achieve technological maturity, at least 17 surgeries are required with accumulated experience, and the robot registration is the main step of the learning process. After reaching maturity stage, the robot usage time is significantly shortened and intraoperative trauma is significantly reduced while the relatively high screw placement accuracy is ensured.
2.Functions and mechanisms of Zn2+-dependent histone deacetylase in plant responses to abiotic stress.

The HDAs (a subfamily of histone deacetylases), a class of Zn2+-dependent histone deacetylases, are highly homologous to the reduced potassium dependency 3 (RPD3) in yeast. HDAs extensively regulate chromosome stability, gene transcription, and protein activity by catalyzing the removal of acetyl group from histone and non-histone lysine residues. HDA-mediated deacetylation is essential for plant growth, development, and responses to abiotic stress. We review the research progress in HDAs regarding the discovery, structures, classification, deacetylation process, and roles in regulating plant responses to abiotic stress. Furthermore, this paper prospects the future research on HDAs, aiming to provide theoretical support for the research on epigenetic regulation mediated by HDAs.

3.Timing of total knee arthroplasty with tourniquet under navigation system:a single-center,retrospective analysis

BACKGROUND:The timing of tourniquet application in traditional total knee arthroplasty has been clearly reported.With the continuous refinement of joint-related techniques,the timing of tourniquet application in total knee arthroplasty under navigation system has rarely been reported.OBJECTIVE:To evaluate the effect of total knee arthroplasty with Brainlab navigation system using full-course and half-course tourniquets.METHODS:From January to December 2022,data of 145 eligible patients who underwent total knee arthroplasty under navigation system in Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine in Hebei Province were retrospectively analyzed and they were divided into the full course group(n=71)and the half course group(n=74)according to the timing of tourniquet release.General data,perioperative conditions,postoperative swelling around the knee joint,postoperative lower extremity deep venous thrombosis,visual analog scale score,Hospital for Special Surgery knee score,and forgotten joint score were collected to assess knee joint function and statistically compared.RESULTS AND CONCLUSION:(1)The operation time of the full course group was shorter than that of the half course group,the incidence of lower extremity deep venous thrombosis was higher,and the postoperative knee joint swelling was more severe(P<0.001,P=0.027,P<0.001).The intraoperative blood loss of the full course group was less than that of the half course group(P<0.001),and the occult blood loss of the full course group was more than that of the half course group(P<0.001),with statistically significant differences.(2)At 1 week after surgery,the visual analog scale score in the full course group was significantly higher than that in the half course group(P<0.001).(3)There was no significant difference in forgotten joint score between the two groups at 3,6 months and 1 year after surgery(P>0.05).(4)It is indicated that in patients undergoing total knee arthroplasty using a navigation system,the half-course group has less occult blood loss,lower postoperative swelling and incidence of postoperative lower extremity deep venous thrombosis,and faster recovery of knee joint function.
4.Health economic analysis and medical cost analysis of children with severe hepatitis B in China: A retrospective study from 2016 to 2022

Background::Hepatitis B poses a heavy burden for children in China, however, the national studies on the distributional characteristics and health care costs of children with severe hepatitis B is still lacking. This study aimed to analyze the disease characteristics, health economic effects, and medical cost for children with severe hepatitis B in China.Methods::Based on patient information in the Hospital Quality Monitoring System, cases with severe hepatitis B were divided into four groups according to age, and the etiology and symptoms of each group were quantified. The cost of hospitalization was calculated for cases with different disease processes, and severity of disease. The spatial aggregation of cases and the relationship with health economic factors were analyzed by Moran’s I analysis. Results::The total number of children discharged with hepatitis B from January 2016 to April 2022 was 1603, with an average age of 10.5 years. Liver failure cases accounted for 43.48% (697/1603) of total cases and cirrhosis cases accounted for 11.23% (180/1603). According to the grouping of disease progression, there were 1292 cases without associated complications, and the median hospitalization cost was $818.12. According to the spatial analysis, the aggregation of cases was statistically significant at the prefectural and provincial levels in 2019, 2020, and 2021 (all P <0.05). The number of severe cases was negatively correlated with gross domestic product (Moran’s I <0) and percentage of urban population (Moran’s I <0), and positively correlated with the number of pediatric beds per million population (Moran’s I >0). Conclusion::The number of severe hepatitis B cases is low in areas with high gross domestic product levels and high urban population ratios, and health care costs have been declining over the years.
5.Comparison of different training methods for microsurgical vascular anastomosis

Objective:To compare the advantages and disadvantages and training costs of different training methods for microsurgical vascular anastomosis, and to provide a basis for establishing a systematic training model and improving surgeons microsurgical skills and clinical competence.Methods:Doctors came from various primary hospitals and exchange groups from foreign hospitals to China, and several groups of data statistics from 2018-2023 were randomly selected for this study. The microsurgical vascular anastomosis training lasted 10 days, including 1 day of theoretical study and 9 days of practical training. A total of 48 doctors were equally divided into group A (one-person operation), group B (two-person cooperation), and group C (two-person cooperation in the first four days and one-person operation in the last five days). The differences in anastomosis time and number of anastomoses between the groups were analysed by one-way ANOVA using the software GraphPad Prism 8.3.0, with P<0.05 indicating that there were statistically significant differences in the variable data. The cost of the three training methods was investigated, and a questionnaire survey of the trainees was conducted. Results:For all the three groups, the speed of anastomosis and the number of anastomoses increased with the course of training. The duration of single-vessel anastomosis was significantly different between groups A and B as well as between groups A and C at all time points except on day 1 (A1 d vs. B1 d, P=0.335; A1 d vs. C1 d, P=0.064; P<0.05 for all the other time points); groups B and C showed no significant differences on day 1 ( P=0.196) and day 3 ( P=0.115) but had significant differences on days 5, 7, and 9 (all P<0.05) in the duration of anastomosis. The number of anastomoses was not significantly different between A1 d and B1 d ( P=0.081), between A3 d and B3 d ( P=0.160), between B1 d and C1 d ( P=0.695), between B3 d and C3 d ( P=0.520), and between A1 d and C1 d ( P=0.123), with significant differences at the other time points (all P<0.05). The training costs were group A > group C > group B. The training questionnaire showed that the proportion of trainees who applied this technique in their daily work was 100.00% (48/48), the proportion of those who wished to participate in the training of this technique occasionally was 100.00% (48/48), the proportion of participants whose institutions had no relevant training conditions was 37.50% (18/48), the proportion of those whose institutions lacked necessary instruments and equipment was 35.42% (17/48), the proportion of those who had difficulties in access to laboratory animals was 68.75% (33/48), and the proportion of inability to carry out relevant training due to the lack of animal experimentation techniques such as anesthesia, preservation, and euthanasia was 91.67% (44/48), indicating that there is a great need for microsurgical vascular anastomosis training. Conclusions:The three training modes have their own advantages and disadvantages. The A mode is suitable for small-scale training. The B mode is suitable for training with adequate funds, a large number of personnel, and a high use frequency. The C mode is the best choice for microsurgical vascular anastomosis training, in which trainees can not only practice the whole vascular anastomosis process but also cooperative skills for anastomosis.

10.Risk Analysis and Countermeasure Suggestions for Hospital Near-source Cyber-attacks

Purpose/Significance To analyze the risks of near-source cyber-attacks faced by hospitals,and to propose counter-measures.Method/Process Combined with practical work experience,the risk analysis of hospital network architecture,on-site physi-cal environment,personnel behavior and other aspects is carried out from the perspective of near-source cyber-attacker.Then,from the perspective of defender and in combination with regulatory requirements and technical practices,countermeasures and suggestions are proposed.Result/Conclusion 5 main risks are identified,including wireless LAN cracking,exposed wired network sockets,improper configuration of self-service machines,poisoning & phishing,and sensitive information leakage.5 preventive suggestions are put for-ward,including strengthening Wi-Fi management,full coverage of network terminal access,multi-department collaboration in self-service device management,disabling mobile storage media on Intranet terminals,and updating cyber-security education.
