1.Pain after total knee arthroplasty:current status and trend analysis

BACKGROUND:The number of patients receiving total knee arthroplasty has been increasing globally each year.Pain management is a crucial aspect following total knee arthroplasty,as effective pain control can facilitate early mobilization,reduce complications,enhance patient satisfaction,and accelerate the rehabilitation process.OBJECTIVE:To construct a visual map of post-total knee arthroplasty pain,understand the international research status and trends in this field,and provide a reference for future studies.METHODS:Relevant research articles on post-total knee arthroplasty pain were retrieved from the CNKI,WanFang Data,and Web of Science core databases,covering the period from January 2000 to December 2023.The CiteSpace software(version 6.2.3)was used to analyze the annual publication output,authors,institutions,countries,keywords,and references.Utilizing R programming language(version 4.4.1),a database was established to create line charts and bar graphs.RESULTS AND CONCLUSION:(1)Our analysis included 3 796 publications,predominantly in Chinese(3 509 articles)with the remainder in English(287 articles).(2)The United States was the most productive country in English literature,with Harvard University leading institutional output.Guangzhou University of Chinese Medicine was the top publishing institution in Chinese literature.(3)Keyword clustering identified"quality of life,""phobia,"and"acupuncture"as emerging focal points in Chinese literature,while"satisfaction"and"psychological factors"were prominent in English literature over the past five years.Co-occurrence and clustering analysis revealed dense internal connections among institutions,authors,and publications,but sparse external collaborations.(4)The study's bias on visualization analysis may have introduced bias by excluding less influential papers.(5)Regarding research hotspots,domestic research emphasized the efficacy and exploration of analgesic methods,in contrast to international research that focused on pain mechanism subtyping and analgesic drug innovation.Future research is expected to trend towards traditional Chinese medicine for postoperative pain,multimodal analgesia,and the investigation and prevention of pain typing mechanisms.

3.Surgical treatment strategy of thyroid cancer complicated with primary hyperparathyroidism

Objective To explore the surgical treatment strategy for patients with thyroid cancer complicated with primary hyperparathyroidism(PHPT).Methods A retrospective analysis was conducted on the case data of thyroid cancer patients who underwent surgical treatment at Beijing Friendship Hospital,Capital Medical University from January 2016 to January 2022.Among them,22 patients with PHPT were included in study group,and 44 patients were randomly selected from thyroid cancer patients during the same period at a ratio of 1∶2 and included in control group.The operation time,intraoperative blood loss,hospital stay and occurrence of complications of two groups of patients were compared.Results The operation time of patients in study group was significantly longer than that in control group(P<0.01).There was no statistically significant difference in intraoperative blood loss and hospital stay between two groups of patients(P>0.05).There were 5 cases of temporary hypocalcemia in study group,12 cases of temporary hypocalcemia and 1 case of incision infection in control group.There was no statistically significant difference in incidence of postoperative complications between two groups of patients(P>0.05).Conclusion Thyroid cancer complicated with PHPT increases the complexity of treatment.Through adequate preoperative diagnosis and reasonable surgical treatment strategies,the risks of secondary surgery and postoperative complications can be effectively reduced,and the prognosis of patients can be improved.
4.Operational performance of large medical equipment based on the full-process information manage-ment system

This paper analyzes and summarizes the operational performance of four medical linear accelerators and a heli-cal tomotherapy system(Tomo)at a hospital in Guangzhou.We have used the real-time recorded data from the radiotherapy full-process information management system,MOSAIQ Integration Platform(MIP),over the past five years.The analysis includes assessing the social service capacity and adaptability to social demand of large radiotherapy equipment by examining trends and characteristics in the types and numbers of patients treated by different technical categories of equipment.It also evaluates the e-conomic rationality of various equipment types based on their short-term and medium-to-long-term cost-effectiveness characteristics and trends.Additionally,the paper provides references for the renewal and performance assessment of large radiotherapy equip-ment in hospitals,integrating the development trends of radiotherapy technology,changes in patient needs,and the social and e-conomic benefits of different equipment types.
5.Dosimetric comparison of tomotherapy,static IMRT and VMAT techniques in radiotherapy for cervical cancer

Objective To compare the dosimetric disparities among static intensity-modulated radiotherapy(IMRT),volumetric modulated arc therapy(VMAT),and tomotherapy(TOMO)techniques in cervical cancer radiotherapy for providing data support for clinical decision-making scheme of radiotherapy.Methods The clinical data of 19 cervical cancer patients,treated at the Affiliated Cancer Hospital and Institute of Guangzhou Medical University from February to May in 2024,were analyzed.Three plans were devised for each case using IMRT,VMAT,and TOMO techniques,followed by dosimetric evaluation in terms of various metrics such as dose volume parameters of the target areas as well as organs-at-risk(OAR),conformity index(CI),homogeneity index(HI),and delivery time.Results All 3 plans met the clinical prescription requirements for the target areas.Compared with static IMRT and VMAT,TOMO had significantly lower Dmean and Dmaxof PCTV and PGTVnd.For OAR,TOMO demonstrated significant advantages over IMRT and VMAT in the Dmean of the bladder,the Dmean,Dmax,V30,V40of the rectum,the Dmean,Dmax,V20,V30of left and right femoral heads,and the Dmean,V20,V50of the pelvis(P<0.05).In addition,the TOMO group showed significantly higher CI for both PCTV and PGTVnd as compared with IMRT and VMAT groups,and lower PGTVnd HI than IMRT group(all P<0.05).Although there was trivial difference among 3 groups in term of PCTV HI,TOMO group performed slightly better than the other two groups.Notably,VMAT technique had the shortest treatment time.Conclusion In various treatment modalities for cervical cancer,TOMO is superior to IMRT and VMAT in terms of target dose coverage,OAR dose distribution,CI,and HI.However,VMAT has the highest efficiency.

9.Cytomegalovirus Encephalitis Presenting With Palpitations as the Initial Symptom After Hematopoietic Stem Cell Transplantation to Treat Acute Lymphoblastic Leukemia in a Child:A Case Report

Hematopoietic stem cell transplantation(HSCT)patients may develop cytomegalovirus encephalitis,a rare but life-threatening complication with a mortality rate exceeding 90%.Cytomegalovirus encephalitis is rare and does not have specific clinical manifestations,which makes clinical diagnosis more difficult.This article reports a pediatric case of acute lymphoblastic leukemia(ALL)with early onset(81 d after transplantation)cytomegalovirus encephalitis after HSCT,with palpitations presenting as the initial symptom.The patient exhibited rapid clinical progression and the treatment was ineffective.This case report can help clinicians gain further understanding of the high-risk factors and clinical features of central nervous system complications following HSCT.When encountering unexplained tachycardia,clinicians should still be vigilant for the possibility of intracranial infection even in the absence of typical neurological manifestations associated with encephalitis.Early recognition and management may help improve the outcomes of such patients.

【Objective】 To understand the current situation of blood components distribution in domestic prefecture-level blood stations through analyzing the components distribution data of 24 prefecture-level blood stations in China. 【Methods】 The data of components distribution of 24 blood stations from 2017 to 2020 as well as the data of blood deployment of 24 blood stations from 2019 to 2020 were collected and analyzed. 【Results】 From 2017 to 2020, positive annual growth in red blood cells, plasma and cryoprecipitate was observed in 22, 19 and 15 out of the 24 blood stations, and the annual growth median rate of above three components was 5.24%, 3.80% and 3.25%, respectively. Among the 24 prefecture-level blood stations, 23 carried out the preparation of cryoprecipitate. 【Conclusion】 The distribution of red blood cells, cryoprecipitate and plasma in prefecture-level blood stations is increasing year by year. However, there is a overstock of plasma, and most blood stations need blood employment.
