Objective:To investigate the clinical characteristics and independent risk factors of severe disease in patients with anti-nuclear matrix protein (NXP) 2 antibody-positive juvenile dermatomyositis (JDM).Methods:A retrospective cohort study was conducted, including 219 anti-NXP2 antibody-positive JDM patients admitted to 23 children′s hospitals across China from July 2011 to July 2023. Patients were classified into severe and non-severe groups based on classification criteria for severe dermatomyositis. Demographic characteristics, clinical manifestations, and laboratory parameters were compared between the 2 groups using independent sample t-test, Mann-Whitney U test, or χ2 test. Univariate and multivariate Logistic regression analyses were performed to identify risk factors for severe disease. The receiver operating characteristic curve was employed to calculate optimal cut-off values. Results:Among the 219 patients, 108 were male and 111 were female, with an age at onset of 6.3 (3.5, 9.4) years. The severe group comprised 69 patients, and the non-severe group 150 patients. The severe group had significantly higher rates of fever, heliotrope rash, subcutaneous edema, periorbital edema, anti-Ro52 antibody positivity, as well as elevated levels of ferritin-to-albumin ratio (FAR), creatine kinase (CK), aspartate aminotransferase (AST), and lactate dehydrogenase (LDH) (all P<0.05). Multivariate analysis identified anti-Ro52 antibody positivity ( OR=13.26, 95% CI 1.37-128.29) and elevated FAR ( OR=1.90, 95% CI 1.09-2.31) as independent risk factors for severe anti-NXP2 antibody-positive JDM (both P<0.05). Receiver operating characteristic curve analysis revealed that a FAR cutoff value of 6.82 predicted severe disease with an area under the curve of 0.87 (95% CI 0.81-0.94, P<0.001), sensitivity of 0.85, and specificity of 0.70. All patients received glucocorticoid therapy, and the severe group received higher proportions of steroid pulse therapy, cyclophosphamide, mycophenolate mofetil, intravenous immunoglobulin, biologics, and adjuvant treatments compared to the non-severe group (all P<0.05). In terms of outcomes, 2 patients (2.9%) in the severe group died (due to neurological involvement and intestinal perforation, respectively), while the remaining patients achieved complete clinical response or remission. All patients in the non-severe group achieved remission. Conclusions:The primary clinical features of anti-NXP2 antibody-positive JDM included fever, heliotrope rash, subcutaneous edema, periorbital edema, anti-Ro52 antibody positivity, and elevated levels of CK, AST, LDH, and FAR. Furthermore, anti-Ro52 antibody positivity and a FAR>6.82 were identified as independent risk factors.

Objective To observe the clinical efficacy of fire-needle therapy plus oral administration ofGuizhi Fuling capsules in treating ovarian cysts.Method Seventy-six patients with ovarian cysts were randomized into a treatment group and a control group, 38 cases each. The treatment group was intervened by fire-needle therapy plus oral administration ofGuizhi Fuling capsules, while the control group was intervened by oral administration ofGuizhi Fuling capsules alone. Before and after the treatment, ultrasound B-scanning was adopted to measure the sizes of ovarian cysts, and the therapeutic efficacies were compared between the two groups.Result The sizes of ovarian cysts were significantly changed after the treatment in both groups (P<0.01). The sizes of ovarian cysts in the treatment group were significantly different from those in the control group after the intervention (P<0.05). The total effective rate was 86.8% in the treatment group versus 65.8% in the control group, and the between-group difference was statistically significant (P<0.05).Conclusion Fire acupuncture plus oral administration ofGuizhi Fuling capsules is an effective approach in treating ovarian cysts.

A high performance liquid chromatography-tandem mass spectrometric ( HPLC-MS / MS) method coupled with an immunoaffinity clean-up column was successfully developed for determination of aflatoxins (AFB1 , AFB2 , AFG1 , AFG2 , AFM1 and AFM2 ) and zeranols ( α-zeranol, β-zeranol, α-zearalenol,β-zearalenol, zearalanone and zearalenone ). The sample was extracted with methanol-acetonitrile (20∶ 80, V/ V) after enzymatic digestion by β-glucuronidase / sulfatase, and the extraction solution was passed through glassy fiber filter paper and then diluted with phosphate buffer solution (PBS). The reconstituted solution was cleaned up with IAC-AZ immunoaffinity column, and then analyzed by HPLC-MS / MS in multiple reaction monitoring (MRM) mode. The results indicated that the linear detection range was 0. 03-6. 0 μg / L for AFB2 and AFG2 , and 0. 05-20 μg / L for the rest compounds. The correlation coefficients were above 0. 999. The limits of detection (LOD) and limits of quantitation (LOQ) were 0. 01-0. 03 μg / kg and 0. 04-0. 09 μg / kg, respectively. The recoveries of the aflatoxins and zeranols were in the range of 73. 6% -98. 4% at the spiked levels of 0. 5, 1 and 5 μg / kg, and the relative standard deviations (RSDs) were in the range of 1. 9% -11. 2% . The method was proved to be simple and accurate, and suitable for the rapid determination of aflatoxins and zeranols in animal-originated foods.

Objective To evaluate the clinic value of multiple tumor markers plus PSA in diagnosing prostate cancer.Methods We collected serum samples of 140 prostate cancer patients with average age of 68 (48-82) years,104 benign prostate hyperplasia patients with average age of 70 (52-87) years,and 162 healthy people with average age of 38 (23-49) years.We had detected PSA levels and also the protein expressions of XAGE-1b,SSX-2,AM ACR and AKAP4.In healthy people,the ranges of the normal values with 95％ data range were determined.Multiple tumor markers and PSA were calculated their positive rate,specificity and sensitivity in diagnosing prostate cancer.We had randomly chosen one positive serum,and analyzed the 4 protein expressions by Western bolt.We detected the serum with the four markers plus PSA by Luminex medthod,then drew ROC curve and calculated AUC area according to the results.Results Among all those samples,PSA levels of 266 samples were under 4tμg/L,86 samples were between 4-10 μg/L,and 54 samples were above 10 μg/L.The positive rates of XAGE-1b,SSX-2,AMACR and AKAP4 in prostate cancer patients were 53.6％ (75/140),34.3％ (48/140),27.9％ (39/140),44.3％ (62/140) respectively.The AUC of XAGE-1b,SSX-2,AMACR,AKAP4 and PSA were 0.666,0.615,0.551,0.768,0.675 respectively.The AUC of their combination was 0.887; The specificity and sensitivity of single PSA detection and combination detection were 60.0％,46.2％ and 80.0％,82.2％.Among those whose PSA value was between 4-10 μg/L,the AUC of single PSA detection was 0.505,the specificity and sensitivity of single PSA detection were 43.2％ and 31.8％; the AUC of the combination detection was 0.803; their specificity and sensitivity were 83.7％ and 73.2％.Conclusions Compared with the single PSA detection,the combination of XAGE-1b,SSX-2,AMACR,AKAP4 and PSA has been greatly improved the specificity and sensitivity in prostate cancer detection.This tool still has significant value even in patients with PSA value between 4-10 μg/L.

Objective To study the mechanisms of depression by exploring expressional differences of AQPs in the tissues of chronic stress depression rats.Methods Depression model was replicated by unpredicted chronic stress.20 rats were randomly separated into normal control group and model control group,AQP4 and AQP3,AQP8 expressions on hippocampus and gastrointestinal mucosa of rat model with depression were detected by immunochemical staining method.Results Means of optical density of AQP4 of normal group and model group hippocampus were 0.28 ± 0.02,0.22 ± 0.06 respectively,and the difference between two groups was significant statistically(t value was 2.756,P＜0.05).The expression of AQP3 on gastric mucosa and colonic mucosa between two groups had no significant statistically(t value were 1.814,1.812,P＞0.05).Two groups'means of optical density of AQP3 on small intestinal mucosa were 0.15 ± 0.02,0.17 ± 0.02,and the difference was significant statistically (t value was 2.769,P＜0.05).Two groups'means of AQP8 optical density in gastric mucosa were 0.15± 0.01,0.19 ± 0.04 ;0.16 ± 0.01 and 0.21 ± 0.04 in small intestinal mucosa;0.16 ± 0.01 and 0.22 ± 0.04 in colonic mucosa,and the differences were significant statistically(t values were 3.139,5.113,4.534,P＜0.05,P＜0.01,P＜0.01).Conclusion The expression of AQP4 on depression model rat hippocampus are lower than those of the normal group ; and the expression of AQP3 on gastric mucosa and colonic mucosa are no change obviously,but it(')s up-regulation on small intestinal mucosa.The expressions of AQP8 on gastric mucosa and small intestinal mucosa and colonic mucosa are up-regulating.

OBJECTIVE: To test the expression of NF-kappaB/p65, MMP-3 and MMP-9 in nasopharyngeal carcinoma (NPC), and analyze their relationship and the clinical significance. METHOD: All samples were examined for the expression of NF-kappaB/p65, MMP-3 and MMP-9 by streptavidin-peroxidase immunohistochemistry. Chi2-test and Spearman-test were used to exam the correlation within them and the expression difference between nasopharyngeal carcinoma and normal nasopharyngeal mucosa. RESULT: Positive expression rate of NF-kappaB/p65 in nasopharyngeal carcinoma was 78.0%; positive expressions rate of MMP-3 and MMP-9 in nasopharyngeal carcinoma were 75.6%. There was positive relationship between NF-kappaB/p65, MMP-3, MMP-9 and N stage. The expression of NF-kappaB/p65 showed a significant positive correlation with the expression of MMP-3 and MMP-9. MMP-3 also had a significant positive correlation with MMP-9. CONCLUSION The result suggests that NF-kappaB/p65, MMP-3 and MMP-9 are highly expressed in nasopharyngeal carcinoma cells. Their expression is correlated with lymph nodes metastasis, but do not have relationship with sex, age, pathological classification and clinical stage. Each of them has positive correlation with the others.

Objective To compare the sensitivity and specificity of mucin 7(Muc7) mRNA test with urine cytology in detection bladder cancer.Methods In 86 patients suspected with bladder cancer,RT-PCR for Muc7 mRNA and urine cytology were conducted in the same urine samples.Fif-ty-two patients with bladder transitional cell carcinoma were confirmed histologically.The sensitivity and specificity of Muc7 mRNA and urine cytology were analyzed.Results The overall sensitivity,specificity and false positive of Mue7 mRNA test were 84.6%,85.2% and 14.7% respectively.Those of urine cytology were 34.6%,91.2% and 8.8% respectively.There were no significant differences between urine cytology and Muc7 mRNA test in the specificity and false positive; however,Muc7 mRNA had significantly higher sensitivity than urine cytology in detection of bladder cancer.Conclusion The sensitivity of Muc7 mRNA test is superior to urine cytology in detection of bladder cancer.

OBJECTIVE To investigate the relation of cyclooxygenase-2 (COX-2) and vascular endothelial growth factor (VEGF) expression with cervical lymph metastases in papillary thyroid carcinoma. METHODS The expression of COX-2 and VEGF in 79 specimens of papillary thyroid carcinoma were evaluated with SP immunohistochemical methods. In all the 79 cases, there were 46 cases with cervical lymph node metastases and 33 cases without cervical lymph node metastasis. RESULTS The positive expression rates of COX-2 and VEGF in the cases with cervical metastases were 81.6 % and 86.8 % respectively, and in the cases without cervical lymph metastases were 54.5 % and 66.7 % respectively. Objective:To study the a feasibility of modified pharyngoplasty in the treatment of velopharyngeal insufficiency.Methods:30 cases of velopharyngeal insufficiency were treated with a modified pharyngoplasty followed by speech training.The effects of the treatment were evaluated by blowing test (BT),chinese articulation inteligibility test (CIT) and hypernasality observation(HO).Results:Before and after treatment BT was 1～26 and 8～40,CIT(%) 20～60 and 47～82,HO(cases) 30 and 3,respectively.Conclusions:The modified pharyngoplastic surgery is feasible in the treatment of velophayngeal insufficiency.
