1.The incidence and influence factors of moderate to severe cancer-related fatigue in bone tumor chemotherapy patients

Objective To investigate the incidence of moderate to severe cancer-related fatigue in bone tumor chemotherapy patients and its influence factors were analyzed.Methods Totally 265 bone tumor chemotherapy patients who received chemotherapy for the first time in our hospital from January 2021 to June 2023 were selected as the research subjects by convenience sampling method.A general information survey Hamilton anxiety scale,Hamilton depression scale,simplified coping style questionnaire and the Piper fatigue scale revised(PFS-R)were used to investigate the cancer-related fatigue status of bone tumor chemotherapy patients,and the logistic regression analysis was used to analyze its influencing factors.Results Totally 239 patients completed the study.Among 239 bone tumor chemotherapy patients,154 cases had moderate to severe cancer-related fatigue,the incidence rate was 64.44%.The Logistic regression analysis showed that daily sleep time<5 hours,low body weight,and high anxiety and depression scores were risk factors for moderate to severe cancer-related fatigue in bone tumor chemotherapy patients,while active coping strategies was protective factors(all P<0.05).Conclusion The incidence of moderate to severe cancer-related fatigue in bone tumor chemotherapy patients is relatively high.Preintervention is needed for patients with low body weight,short sleep time,anxiety,depression,and negative coping strategies to reduce the occurrence of moderate to severe cancer-related fatigue in patients.

3.Epidemiological characteristics and spatiotemporal distribution of schizophrenia in Ningbo, 2018-2022

Objective:To explore the temporal and spatial distribution characteristics of reported incidence of schizophrenia in Ningbo from 2018 to 2022 and to provide a scientific basis for rational allocation of mental health resources and comprehensive prevention and treatment of schizophrenia.Methods:The reported incidence data of schizophrenia from 2018 to 2022 were collected from Ningbo's mental health information management system, and the reported incidence was calculated by township. The spatial correlation analysis and the spatiotemporal scan analysis were used to study the spatiotemporal distribution of schizophrenia.Results:The reported incidence of schizophrenia decreased from 2018 to 2022, with 4 133 new cases reported, and the annual average reported incidence was 9.76/100 000. Global and local spatial autocorrelation analysis showed positive spatial correlations and hot spots in 2018-2020. The space-time scan analysis showed an incidence cluster in Dongqiao Town, Haishu District, during 2018-2019. The RR was 2.46, and the log-likelihood ratio was 256.89. Conclusions:The reported incidence of schizophrenia in Ningbo has obvious temporal and spatial aggregation, and the high incidence area explored can provide clues for further research on the correlation between environmental factors and the incidence of schizophrenia and has certain guiding significance for the rational allocation of mental health resources in Ningbo.
4.A multicenter, double-blind, randomized controlled clinical trial comparing ergometrine with oxytocin and oxytocin alone for prevention of postpartum hemorrhage at cesarean section

Objective:To compare oxytocin combined with ergometrine with oxytocin alone in terms of primary prophylaxis for postpartum hemorrhage (PPH) at the time of cesarean section (CS).Methods:This was a multicenter double-blind randomized controlled interventional study comparing ergometrine combined with oxytocin and oxytocin alone administered at CS. From December 2018 to November 2019, a total of 298 parturients were enrolled in 16 hospitals nationwide. They were randomly divided into experimental group (ergometrine intra-myometrial injection following oxytocin intravenously; 148 cases) and control group (oxytocin intra-myometrial injection following oxytocin intravenously; 150 cases) according to 1∶1 random allocation. The following indexes were compared between the two groups: (1) main index: blood loss 2 hours (h) after delivery; (2) secondary indicators: postpartum blood loss at 6 h and 24 h, placental retention time, incidence of PPH, the proportion of additional use of uterine contraction drugs, hemostatic drugs or other hemostatic measures at 2 h and 24 h after delivery, the proportion requiring blood transfusion, and the proportion of prolonged hospital stay due to poor uterine involution; (3) safety indicators: nausea, vomiting, dizziness and other adverse reactions, and blood pressure at each time point of administration.Results:(1) The blood loss at 2 h after delivery in the experimental group [(402±18) ml] was less than that in the control group [(505±18) ml], and the difference was statistically significant ( P<0.05). (2) The blood loss at 6 h and 24 h after delivery in the experimental group were less than those in the control group, and the differences were statistically significant (all P<0.05). There were no significant differences between the two groups in the incidence of PPH, the proportion of additional use of uterine contraction drugs, hemostatic drugs or other hemostatic measures at 2 h and 24 h after delivery, the proportion requiring blood transfusion, and the proportion of prolonged hospital stay due to poor uterine involution (all P>0.05). (3) Adverse reactions occurred in 2 cases (1.4%, 2/148) in the experimental group and 1 case (0.7%, 1/150) in the control group. There was no significant difference between the two groups ( P>0.05). The systolic blood pressure within 2.0 h and diastolic blood pressure within 1.5 h of drug administration in the experimental group were higher than those in the control group, and the differences were statistically significant ( P<0.05), but the blood pressure of the two groups were in the normal range. Conclusion:The use of ergometrine injection in CS could reduce the amount of PPH, which is safe and feasible.

Objective To investigate the prevalence of iron deficiency(ID)and iron deficiency anemia (IDA) in pregnant women in urban areas of China. Methods The study was a national cross-sectional survey conducted from September 19th, 2016 to November 20th, 2016. According to the classification of the National Bureau of Statistics, all survey sites were set up in 6 regions of the country. Pregnant women were continuously selected using multistage stratified sampling. A total of 12 403 pregnant women were collected and examined for serum ferritin and hemoglobin levels. Results The median serum ferritin level during pregnancy was 20.60 μg/L(11.78-36.98 μg/L), the hemoglobin level was(118±12)g/L. With the progress of pregnancy, the levels of serum ferritin and hemoglobin decreased gradually. The median serum ferritin levels in the first, second trimester and third trimester were 54.30 μg/L(34.48-94.01 μg/L), 28.60 μg/L(16.40-50.52 μg/L), and 16.70 μg/L(10.20-27.00 μg/L)respectively(P<0.01). The mean hemoglobin levels were(127 ± 10)g/L,(119 ± 11)g/L and(117 ± 11)g/L respectively(P<0.01). The prevalence of ID in urban pregnant women was 48.16%(5 973/12 403), and IDA prevalence was 13.87% (1 720/12 403). The prevalence of IDA in the first, second trimester and third trimester were 1.96% (20/1 019), 8.40%(293/3 487)and 17.82%(1 407/7 897), respectively(P<0.01). The prevalence of standardized ID and IDA were significantly different in various regions of China(P<0.01). The standardized prevalence of ID were relatively higher in East China and Northeast China, 57.37% and 53.41% respectively, while it was the lowest in Southwest China, 30.51%. The standardized prevalence of IDA in South Central, Northwest, and East China were relatively high, 21.30%, 16.97% and 17.53% respectively, and the standardized prevalence of IDA in Southwest China was the lowest, 5.44%,the differents in various regions were significant(all P<0.01). Conclusion The current phenomenon of ID and IDA in pregnant women is still very common,and nutrition and health care during pregnancy should be strengthened.
6.Study on the situation regarding endoscope cleaning and disinfection in the department of otolaryngology in Hunan hospitals.

OBJECTIVE:
To investigate the situation regarding the cleaning and sterilization of endonasal endoscopes in department of otolaryngology in Hunan Province, and to provide strategy for improving the level of sterilization and management of endonasal endoscopes.
METHODS:
A total of 100 medical institutions were investigated by spot assessment, check and sampling. Data was analyzed by multivariate analysis.
RESULTS:
The qualified rate of rules and regulations for endoscopy was 28.8% in the second-class hospitals and 45% in the top-class hospitals. The qualified rate of environment for endoscopy cleaning and sterilization was 36.3% in the second-class hospitals and 85% in the top-class hospitals. The main problems include lack of independent disinfection room, the space not large enough, and/or lack of ventilation system. The qualified rate of bacterial detection for post-sterilized endoscopes and biopsy forceps was 93.8% in the second-class hospitals and 95.0% in the top-class hospitals, and the main pathogenic bacteria was gram-positive cocci and gram-negative bacilli. The multivariate analysis showed that the influencial factors for endoscope cleaning and disinfection are as follows: staffs responsible for the cleaning and sterilization of otolaryngology endoscopes, the standard for cleaning and disinfection process, and the frequency of endoscope use.
CONCLUSION
The present situation of cleaning and sterilization for otolaryngology endoscopes is better in the top-class hospitals than that in the second-class hospitals. The sterilization and management of otolaryngology endoscopy are needed to be improved, and the staff training is needed, especially in the primary hospitals.

7.Analysis on composition of blood culture pathogenic bacteria and drug resistance in a tertiary general hospital of Xiamen during recent three years

Objective To analyze the distribution and antimicrobial resistance situation of pathogens isolated from blood culture to provide the scientific basis for the accuratel use of antibacterial drugs and preventing and controlling nosocomial acquired blood‐stream infection .Methods The US BACTEK‐FK automatic blood culture instrument and the French VITEK 2 COMPACT auto‐matic bacteria identification/susceptibility system were used to conduct the culture ,identification of blood culture isolated bacterial strains and drug susceptibility test .The results of drug susceptibility test were judged by adopting the 2011 criteria of the Clinical Laboratory and Standards Institute(CLSI) .Results The main isolated bacteria from blood culture for the recent three years were E coli ,staphylococcus aureus and klebsiella pneumonia bacteria ,coagulase negative staphylococcus(CNS) ,etc .The proportions of hos‐pital‐acquired bloodstream infections during these period were 42 .2% ,46 .9% and 54 .1% respectively .The detection rate of methi‐cillin‐resistant staphylococcus aureus (MRSA) was 18 .8% ,which of multiple drug resistant acinetobacter baumannii (MDRAB) was 42 .9% ,which of producing extended spectrum beta lactamase (ESBLs) in E .coli and klebsiella pneumoniae were 71 .8% and 69 .8% respectively .Conclusion The bloodstream infections pathogens in this hospital are mainly Enterobacteriaceae bacteria ,the proportion of hospital‐acquired bloodstream infections increases year by year ;the detection rate of multi‐drug resistant Acinetobact‐er baumanni (MDRAB) is higher ,clinic should pay more attention to the change of blood culture pathogens and their drug resist‐ance trend ,meanwhile nosocomial bloodstream infection should be prevented and controlled .
8.Role of glycogen synthase kinase-3β in trafficking of NR1-containing N-methyl-D-aspartate receptor and NR2B-containing N-methyl-D-aspartate receptor in spinal cord in rats with incisional pain and remifentanil-induced hyperalgesia

Objective To investigate the role of glycogen synthase kinase-3β (GSK-3β) in the trafficking of NR1-containing N-methyl-D-aspartate receptor (NMDAR) and NR2B-containing NMDAR in the spinal cord in rats with incisional pain (IP) and remifentanil-induced hyperalgesia.Methods Twenty-four male Sprague-Dawley rats,aged 2-3 months,weighing 240-260 g,were randomly divided into 3 groups ( n =8 each):control group (group C),remifentanil group (group R) and GSK-3β inhibitor TDZD-8 group (group TDZD-8).A 1-cm longitudinal incision was made in the plantar surface of the left hindpaw in anesthetized rats.In group C,dimethyl sulfoxide (DMSO) 2 ml/kg was injected and then the normal saline (equal to the volume of remifentanil) was infused for 60 min via the caudal vein.In group R,DMSO 2 ml/kg was injected before IP was made and then remifentanil was infused at a rate of 1.2 μg· kg- 1 ·min- 1 for 60 min.In group TDZD-8,TDZD-8 2 ml/kg was injected before IP was made and then remifentanil was infused at a rate of 1.2 μg·kg-1 ·min-1 for 60 min.Paw withdrawal threshold (PWT) and paw withdrawal latency (PWL) were measured 24 h before infusion of normal saline or remifentanil and at 2,6,24 and 48 h after intravenous injection.The rats were sacrificed after the last measurement of the pain threshold and L4-6 segment of the spinal cord was removed to determine the expression of NMDAR NR1 and NR2B subunits in cell membrane (s) and cytoplasm (i) in the spinal cord by Western blot analysis.The ratios of sNR1/iNR1 and sNR2B/iNR2B were calculated.Results Compared with group C,PWT was significantly decreased and PWL was significantly shortened,the expression of sNR1 and sNR2B was up-regulated,the expression of iNR1 and iNR2B was down-regulated,and the ratios of sNR1/iNR1 and sNR2B/iNR2B were significantly increased in groups R and TDZD-8 (P < 0.05).Compared with group R,PWT was significantly increased and PWL was significantly prolonged,the expression of sNR1 and sNR2B was down-regulated,the expression of iNR1 and iNR2B was up-regulated,and the ratios of sNR1/iNR1 and sNR2B/iNR2B were significantly decreased in group TDZD-8 ( P < 0.05).Conclusion GSK-3β is involved in the regulation of trafficking of NR1-containing NMDAR and NR2B-containing NMDAR from cytoplasm to cell membrane in the spinal cord in rats with IP and hyperalgesia induced by remifentanil.
9.Using recipient free edge fingernails for chimerism analysis following allogeneic hematopoietic stem-cell transplants

ObjectiveTo examine whether DNA extracted from free edge fingernails specimens from patient after hematopoietic stem cell transplantation (allo-HSCT) could be used for short tandem repeat (STR) genotyping and chimerism analyzing,and to observe the chimerism status in fingernails after allo-HSCT.MethodsPeripheral blood,bone marrow,oral mucosa and free edge fingernail specimens were collected from 25 patients which allo-HSCT were performed in Beijing Dao-pei Hospital during Jul.2009 to Sep.2011 and their donor.Genomic DNA was extracted and 15 STR loci genotyping and chimerism analysis were performed.For the first group which including 12 patients,pairs of fingernail and oral mucosa specimens were collected within one month after allo-HSCT and were comparative analyzed.For the second group which including 13 patients,chimerism status in fingernail samples were analyzed 3 months or longer after allo-HSCT,and 3 patients underwent repeated testing at different times.ResultsFor the first group,4 oral mucosa specimens showed donor chimerism with varying degrees,but no donor chimerism was detected.in all of 12 fingernail specimens.For the second group,6.7% to 82.6% donor chimerism was detected in fingernail specimens in 5 out of 13 patients.For the 3 patients underwent repeated testing,donor chimerism was continued negative in one cases,but continued positive in the other 2 cases.ConclusionsFree edge fingernail samples of patients within one month after allo-HSCT can be used for STR typing and chimerism analysis,and it is better than oral mucosa samples.There are cells in allo-HSCT donor graft can differentiate into skin cells,donor derived skin cells chimerism can be formed and persist in some patients.Med,2012,35:23-26)
10.Management of postoperative complications of basicranial tumor resection under nasal endoscopy.

OBJECTIVE:
To explore medical and nursing management of postoperative complications of basicranial tumor resection through nasal endoscopy.
METHOD:
Retrospective analysis was carried out in 178 patients with skull base tumors who underwent nasal endoscopic resection of basicranial tumors under general anesthesia, and controlled hypotension was conducted during the surgery. Different surgical approaches were applied in terms of anatomical regions which the tumor involved.
RESULT:
Postoperative complications occurred in 87 of 178 patients, including cerebrospinal fluid rhinorrhea, intracranial infections, diabetes insipidus, etc. Eighty-five cases with complications recovered through proper medical and nursing care, except for 2 cases of death.
CONCLUSION
Because there is a higher risk for nasal endoscopic basicranial procedure, doctors and nurses are required to master multidisciplinary skills and experiences. Careful and rigorous nursing care of postoperative complications is quite important for successful management of basicranial operative patients.
