1.Effect of composite electromagnetic stimulation combined with chin tuck against resistance on post-stroke pa-tients with dysphagia

Objective To explore the effect of composite electromagnetic stimulation combined with chin tuck against resistance(CTAR)on post-stroke dysphagia.Methods From January,2021 to December,2023,156 post-stroke patients with pharyngeal dysphagia in the First Peo-ple's Hospital of Yibin were randomly divided into CTAR group(n=51),neuromuscular electrical stimulation(NMES)+repetitive transcranial magnetic stimulation(rTMS)group(n=52)and combination group(n=53).On the basis of routine swallowing function training,they received CTAR,NMES+rTMS and NMES+rTMS+CTAR,respectively,for four weeks.They were assessed with Functional Oral Intake Scale(FOIS),Standardized Swal-lowing Assessment(SSA),Swallowing-Quality of Life(SWAL-QOL),Rosenbek Penetration Aspiration Scale(PAS),video fluoroscopic swallowing study(VFSS)(including the score of VFSS,pharyngeal transit time,and upward and forward displacement of hyoid bone),average surface electromyography(AMEG)of submental and hyoid muscles,before and after treatment.Results The inter-group effect,intra-group effect and interaction effect were significant in SSA,SWAL-QOL and AMEG of the two muscles(F>6.611,P<0.001).The inter-group effect and interaction effect were significant in FOIS,VFSS,pharyngeal transit time,upward displacement of hyoid bone and forward displacement of hyoid bone(F>3.451,P<0.05).Pairwise comparison results showed that pharyngeal transit time was shorter in the combination group than in NMES+rTMS group(P=0.048),forward displacement of hyoid bone was more in the combina-tion group than in CTAR group(P=0.002),and AMEG was higher in the combination group than in CTAR group and NMES+rTMS group(P<0.001).There was significant difference in the score of PAS among three groups(Hc=8.282,P=0.016),and it was the best in the combination group.Conclusion The combination of NMES+rTMS and CTAR is superior to single electromagnetic stimulation or CTAR in the treatment of post-stroke sysphagia.

3.Analysis of the results of clinical skill tests for clinical laboratory in second stage in Beijing

Objective To explore the rationality for the examination mode of residents' clinical laboratory clinical skills in the second stage and to provide scientific and reasonable basis for the second stage training. Method Statistical analysis was made on the results of the assessment of the clinical skills of the residents who took the clinical skills examination in inspection division second stage in Beijing from year 2014 to 2016. The differences between the results of the assessment of the base hospitals and non-base hospitals, three level hospitals and non-three level hospitals and different assessment modules were compared between the three groups. SPSS 17.0 statistical software was used to analyze the data. The com-parison between the measured data was done by independent sample t test, and the comparison between the data was analyzed by chi square test. The test level wasα=0.05. Result From the assessment pass rate, three level hospitals were higher than non-three level hospitals (P=0.01), the base hospital was higher than non-base hospital (P=0.01), and physicians' passing rate was higher than technician'. There was statistical significance (P=0.02, P=0.01) in the auxiliary examination of the written test module of three assessment modules in 2014 and 2015. In 2015 and in 2016, there was significance (P=0.02, P=0.01) in the mean scores of case analysis. Conclusion The second stage clinical skills assessment model is more reasonable, and the non-base and non-three level hospitals should strengthen the training and management of the sec-ond stage.
4.Changes of serum IL-6, IL-10 and IL-1 in patients with Kawasaki disease and its clinical significance

ObjectiveTo observe the serum levels of IL-6,IL-10 and IL-1 in children with Kawasaki disease and its clinical significance.Methods38 childrey with of Kawasaki disease were selected as observction group,then selected 38 cases of normal children as control group.Serum IL-6,IL-10 and IL-1 levels were observed two groups.ResultsObservation group IL-6,IL-10 and IL-1 levels were higher(t =10.3877,15.1010,15.1243,all P<0.05),acute phase of observation group IL-6,IL-10,IL-1,etc.levels were significantly higher than the sub-acute phase patients ( t =9.7594,11.6486,11.6622,all P < 0.05 ) and control group ( t =11.7032,11.7718,11.8267,all P <0.05 ).48h apoptosis rate in the control group( 2.8 ± 0.8 )% was significantly lower than in children with Kawasaki disease(38.3 ±7.9)% (t =38.59,P <0.01 ).Platelets increased[ (464.0 ± 110.2) × 109/L] of the IL-1 level in children with Kawasaki disease(663 ±94)ng/L was significantly higher than those without elevated platelet [ (307.0 ±104.9) × 109 /L ] of children( 492 ± 92 ) ng/L ( t =13.1044,P < 0.05 ).ConclusionThedetection of serum IL-6,IL-10 and IL-1 levels may aid clinical diagnosis of the condition of children with Kawasaki disease,complications and treatment,have important clinical significance.
5.Growth and metabolism of osmo-sensentive yeast Y02724 and high-osmotic resistant yeast Hansel in alginate-chitosan-alginate microcapsules.

To study the effect of the osmotic stress in the microenvironment on the growth and metabolism of the encapsulated cells under aerobic condition, Osmo-sensitive yeast Y02724 and high-osmotic resistant yeast Hansel were used as models to explore the growth and metabolism state of the cells cultivated inalginate-chitosan-alginate (ACA) microcapsules. The changes of the yeast cells' specific growth rate, maximum product quantity and the secretion of ethanol and glycerol were analyzed. For Y02724, the yield of ethanol was increased in the ACA microenvironment compared to suspension cultivation. For Hansel, the maximum growth speed of microencapsulated cultivation had no obvious difference compared to the suspension cultivation. Moreover, after encapsulation, the production of glycerol was decreased for both Y02724 and Hansel compared to suspension cultivation. In conclusion, osmotic stress existed in the ACA microcapsules and affected the growth and metabolism of the cells.
