N-acetyl-p-aminophenol （APAP） is an antipyretic analgesic commonly used in clinical practice， and APAP overdose can cause severe liver injury and even death. In recent years， the incidence rate of APAP-induced liver injury （AILI） tends to increase， and it has become the second most common cause of liver transplantation worldwide. Autophagy is a highly conserved catabolic process that removes unwanted cytosolic proteins and organelles through lysosomal degradation to achieve the metabolic needs of cells themselves and the renewal of organelles. A large number of studies have shown that autophagy plays a key role in the pathophysiology of AILI， involving the mechanisms such as APAP protein conjugates， oxidative stress， JNK activation， mitochondrial dysfunction， inflammatory response and apoptosis. This article elaborates on the biological mechanism of autophagy in AILI， in order to provide a theoretical basis for the treatment of AILI and the development of autophagy regulators.

Objective:To summarize the clinical phenotypes and genetic characteristics of Noonan syndrome-like disorder with loose anagen hair 2 (NSLH2) caused by PPP1CB variants. Methods:A case summary.Clinical data, laboratory findings, and imaging studies were collected from a patient with NSLH2 treated at Guangdong Women and Children Hospital in January 2024.Genomic DNA was extracted from the proband and his parents for whole-exome sequencing, with pathogenic variants verified by Sanger sequencing.Literature review was conducted to analyze clinical phenotypes, genetic features, and management approaches in NSLH2.Results:The patient manifested short stature, developmental delay, distinctive facial features, sparse eyebrows, slow hair growth, coarse hair texture, and hypotrichosis.Whole-exome sequencing identified a de novo heterozygous PPP1CB variant (c.146C>G, p.Pro49Arg).Literature review identified 26 reported cases (including this one), with cardinal features including developmental delay, impaired hair growth, abnormal hair texture, characteristic facies, congenital heart disease, short stature, and central nervous system abnormalities. Conclusions:NSLH2 is an autosomal dominant disorder caused by PPP1CB variants with characteristic manifestations.Genetic testing should be considered for children presenting with distinctive facies, developmental delay, impaired hair growth, and abnormal hair texture to establish a definitive diagnosis.

The incidence rate of drug-induced liver injury （DILI） is increasing year by year with unknown mechanisms， and the treatment methods for DILI mainly include drugs， liver support systems， and liver transplantation， all of which have certain limitations. Therefore， the search for safer and more effective treatment methods has become a research hotspot at present. Studies have shown that mesenchymal stem cells and their exosomes can alleviate liver injury by reducing liver inflammation， promoting hepatocyte proliferation and regeneration， inhibiting the apoptosis of hepatocytes， improving oxidative stress， and regulating immunity. This article briefly reviews the role of mesenchymal stem cells and their exosomes in the treatment of DILI， so as to provide a reference for further research.

Autoimmune hepatitis(AIH)is a type of chronic hepatitis caused by the attack of hepatocytes by the autoimmune system,and with the prolongation of disease course,it may gradually progress to liver cirrhosis and even hepatocellular carcinoma.Although great achievements have been made in the understanding and treatment of AIH,its etiology and pathogenesis still remain unclear.T cells play a crucial role in the development and progression of AIH,and by focusing on follicular helper T cells,this article elaborates on the research advances in follicular helper T cells in AIH,in order to provide new ideas and strategies for the clinical treatment of AIH.

Autoimmune hepatitis(AIH)is chronic hepatitis caused by the attack of live cells by the immune system,and at present,the pathogenesis of AIH remains unclear.Inflammasomes are important components of innate immunity and are involved in a variety of pathophysiological processes.Studies have shown that inflammatory response associated with nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3)plays an important role in the pathogenesis of AIH,which mainly mediates the release of proinflammatory factors and pyroptosis,thereby participating in the pathophysiological process of AIH.Therefore,the development and progression of AIH can be delayed by inhibiting the activation of NLRP3 inflammasomes,which provides new ideas for the prevention and treatment of AIH.

Objective To investigate the clinical value of thrombelastogram (TEG) in prediction for the cause of delayed cerebral ischemia (DCI) in patients with aneurysmal subarachnoid hemorrhage (aSAH). Methods In this study, there were 30 patients with DCI (group DCI) and 45 patients without DCI (group No-DCI). TEG was performed in all the patients at post-bleeding day 1, 5, 10 and 15 after aSAH(PBDn, n=1,5,10,15). The changes of reaction time (R value), coagulation time (K value),coagulation angle (α value),maximum thrombus consistency (MA value) and coagulation index (CI value) were examined at different time points after aSAH. Results Thirty of 75 aSAH patients developed DCI and the incidence of DCI was 40 percent. According to linear mixed model, both MA value and CI value were significantly statistical different at different time points within each group (P<0.05,for all) as well as between No-DCI group and DCI group.MA value and CI were significantly statistical different at same time point (P<0.05,for all). The results of logistic regression analysis showed that modified Fisher levelⅢ-Ⅳ,△MA5-1(OR=1.124,P=0.024,95% CI=1.015~1.244), PBD5 MA>70(OR=5.605,P=0.011,95% CI=1.464~21.457)were the independent risk factors of DCI.By using ROC curve to define a threshold for prediction of the occurrence of DCI,the rate of DCI was significantly increased when △MA5-1>3.05.Conclusion aSAH Patients, especially those with DCI have severe hypercoagulation. The MA value in PBD5 has an important predictive value for DCI.

Recently,more and more researches focus on the relationship between bone marrow fat and osteoporosis.From a pathological point of view, osteoporosis is traditionally regarded as the dysfunction of osteoblastic bone formation and osteoclastic bone resorption.Actually, bone marrow adipose tissue that constitutes a large proportion of bone marrow cells is indispensable for the balance of bone metabolism.Bone marrow fat, which is abundant in yellow bone marrow, increases gradually with aging.Bone marrow adipocytes and osteoblasts differentiate from a common progenitor, bone marrow mesenchymal stem cells(BMSCs).It is generally acknowledged that they are reciprocal repellent in the process of BMSC differentiation.Exploring the association between bone marrow adipocytes and osteoporosis is critical for better understanding its role in the pathogenesis and the future treatment of osteoporosis.In this review, we summary the recent progress in the association between bone marrow adipocytes and osteoporosis.
