1.Epidemiological Characteristics of Malignant Tumors in Cancer Registration Areas of Heilongjiang Province in 2019 and the Trend from 2013 to 2019

[Purpose]To analyze the incidence and mortality of malignant tumors in cancer regis-tration areas of Heilongjiang Province in 2019 and the trend from 2013 to 2019.[Methods]The incidence and mortality data of malignant tumors reported by the Heilongjiang provincial cancer registries from 2013 to 2019 were collected,and the quality of data was assessed.The crude in-cidence/mortality rate,age-standardized incidence/mortality rate by Chinese standard population(ASIRC/ASMRC)and world standard population(ASIRW/ASMRW),0~74 years old cumulative rate were calculated.Joinpoint 4.6.0 software was used to calculate the average annual percentage change(AAPC)of ASIRC/ASMRC for the trend analysis from 2013 to 2019.[Results]In 2019,there were 16 732 new cases of malignant tumors in the cancer registration areas of Heilongjiang Province,including 8 639 males and 8 093 females.The crude incidence rate was 295.37/105,with an ASIRC and ASIRW of 167.10/105 and 164.18/105,respectively.There were 10 988 malig-nant tumor deaths,including 6 540 males and 4 448 females.The crude mortality rate was 193.97/105,with an ASMRC and ASMRW of 101.22/105 and 101.66/105,respectively.The inci-dence and mortality of malignant tumors increased rapidly after the age of 55,and the incidence and mortality of males were slightly higher than those of females.The top five malignant tumors of high incidence were lung cancer,female breast cancer,colorectal cancer,liver cancer and thy-roid cancer,and the top five malignant tumors of high mortality were lung cancer,liver cancer,colorectal cancer,stomach cancer and female breast cancer.From 2013 to 2019,the ASIRC of malignant tumors in cancer registration areas increased from 153.08/105 in 2013 to 167.10/105 in 2019,and the ASMRC increased from 92.22/105 in 2013 to 101.22/105 in 2019,but there was no statistical difference in the change trend.[Conclusion]The incidence and mortality of malignant tumors in Heilongjiang Province remain high.Lung cancer,female breast cancer,colorectal can-cer,liver cancer and stomach cancer should be the focus of cancer prevention and control.
2.Predictive value of reverse shock index multiplied by Glasgow coma scale score for mortality of trauma patients: a Meta analysis

Objective:To systematically evaluate the predictive value of the reverse shock index multiplied by the Glasgow coma scale score (rSIG) for mortality of trauma patients.Methods:A comprehensive literature search was conducted to identify studies on the predictive value of rSIG for mortality of trauma patients in the following databases from inception to April 2025, including CNKI, Wanfang Data, SinoMed, PubMed, Cochrane Library, Web of Science, and Embase. Two investigators independently screened the literature, extracted data, and assessed study quality according to predefined inclusion and exclusion criteria. The Quality assessment of diagnostic accuracy studies-2 (QUADAS-2) tool was used to evaluate the risk of bias in the included studies. Meta analysis was performed using Stata 17.0 software with a bivariate mixed-effects model. The following metrics were used to assess the predictive value of rSIG for mortality in trauma patients, including sensitivity, specificity, positive likelihood ratio (PLR), negative likelihood ratio (NLR), diagnostic odds ratio (DOR), and area under the summary receiver operating characteristic (SROC) curve (AUC). The influence of various factors on the predictive performance of rSIG was examined, including injury type, study design, region, sample size, cut-off value, rSIG measurement time, and outcome measures. Additionally, sensitivity analysis, Fagan′s nomogram, and Deeks′ funnel plot were employed to assess the robustness of the findings, clinical applicability, and publication bias.Results:A total of 15 studies involving 710 612 trauma patients were included, 26 105 of whom were deceased. Meta analysis results showed that rSIG had a pooled sensitivity of 0.78(95% CI 0.71, 0.84), a pooled specificity of 0.78(95% CI 0.68, 0.86), a pooled PLR of 3.60(95% CI 2.46, 5.27), a pooled NLR of 0.28(95% CI 0.22, 0.36), a pooled DOR of 12.70(95% CI 8.10, 19.91), and an AUC of 0.85(95% CI 0.81, 0.87) for predicting mortality of trauma patients. Subgroup analysis identified injury type as one of the major sources of heterogeneity, and the predictive specificity of rSIG was significantly higher in patients with multiple trauma (0.82) than in those with isolated traumatic brain injury (0.65) ( P<0.05). Sensitivity analysis indicated that the findings were robust and stable. Fagan′s nomogram showed that when the pre-test probability was 7%, the post-test probability of death increased to 21% in patients with low rSIG and decreased to 2% in those with high rSIG. Deeks′ funnel plots suggested no significant publication bias among the included studies ( P>0.05). Conclusion:Low rSIG has good predictive performance for mortality of trauma patients and can serve as an effective tool for early and rapid prognosis assessment with superior predictive performance in patients with multiple trauma compared to those with traumatic brain injury.

4.Analysis of Cardiac Involvement in Patients With Fabry Disease in Anhui Region

Objectives:To analyze the cardiac involvement of patients with Fabry disease(FD)in Anhui region.Methods:This retrospective analysis included 48 previously and currently diagnosed FD patients(25 males)in Anhui region,overall patient and gender specific cardiac involvement was analyzed.Results:The median age of FD patients is 28.0(19.0,46.0)years.The cardiac manifestations of patients with FD were most commonly characterized by palpitations/arrhythmias(13/42 cases)and exertional dyspnea(11/42 cases),electrocardiographic changes were most commonly characterized by T-wave inversion(22/42 cases),ST-segment depression(16/42 cases),and left ventricular hypervoltage(18/42 cases),cardiac structural and functional changes were most common in papillary muscle hypertrophy(29/36 cases),bilateral sign(22/37 cases)and left ventricular hypertrophy(21/46 cases),as well as reduced left ventricular global longitudinal strain(26/39 cases).Neuropathic pain(28/43 cases)was the most common extracardiac manifestation of FD patients.FD patients of different gender differed in age at diagnosis(P=0.018),alpha galactosidase A activity(P<0.001),globotriaosylsphingosine(lyso-GL3)levels(P<0.001),enzyme replacement therapy rate(P=0.043),dyshidrosis(P<0.01),and the incidence of angiokeratoma(P=0.004).Correlation analysis showed that genotype was not correlated with enzyme activity or Lyso-GL-3 levels,whereas the Sokolow-Lyon index was positively correlated with Lyso-GL-3 levels(ρ=0.423,P=0.008),and the Sokolow-Lyon indices(septal thickness:ρ=0.562,P<0.001;left ventricle posterior wall thickness:ρ=0.569,P<0.001)and QRS duration(septal thickness:ρ=0.543,P<0.001;left ventricle posterior wall thickness:ρ=0.557,P<0.001)were positively correlated with left ventricular wall thickness.Conclusions:Cardiac involvement in patients with FD in the Anhui region is characterised by palpitations or arrhythmias,accompanied by nonspecific electrocardiographic changes.Echocardiography frequently reveals papillary muscle hypertrophy.The manifestation of cardiac involvement in patients of different genders is similar.

7.Mediating effect of self-efficacy between postoperative pain and kinesiophobia in aged patients with femoral neck fracture

Objective To investigate the influence of self-efficacy and postoperative pain on postoperative kinesiophobia in aged patients with femoral neck fractures and explore the mediating effect of self-efficacy on postoperative pain and kinesiophobia of the patients.Methods A total of 120 aged patients with femoral neck fractures were included in the study.A cross-sectional study was conducted using general questionaire,the rehabilitation self-efficacy scale,digital pain rating scale and Tampa scale for kinesiophobia.Pearson correlation was used to analyse the correlations between postoperative pain,self-efficacy and postoperative kinesiophobia.AMOS21.0 software was used to establish the mediating effect model of self-efficacy between postoperative pain and postoperative kinesiophobia.The mediating effect was verified by Bootstarp confidence interval evaluation.Results All 120 aged patients with femoral neck fractures completed the study.The score of postoperative kinesiophobia was(40.27±12.85).The level of kinesiophobia was negatively correlated with the self-efficacy score(r=-0.571,P<0.01),and positively with the level of postoperative pain(r=0.766,P<0.01).The direct standardized effect of postoperative pain on postoperative panic level was 0.367,the direct standardized effect of self-efficacy on postoperative panic level was-0.485,the direct standardized effect of postoperative pain on self-efficacy was-0.716,the indirect standardized effect of postoperative pain on postoperative panic level was 0.347,and the total standardized effect was 0.714,accounting for 48.60%of the total effect.Conclusions Postoperative pain can directly affect postoperative kinesiophobia or indirectly affect it via self-efficacy in elderly patients with femoral neck fractures.Medical staff should improve the self-efficacy of the aged patients by relieving the postoperative pain and alleviating the postoperative kinesiophobia.
8.Effects of preoperative incentive spirometry combined with active cycle of breathing techniques training in adolescents with scoliosis

Objective:To explore the effect of preoperative incentive spirometry combined with active cycle of breathing techniques (ACBT) training on pulmonary function recovery after scoliosis surgery in adolescents.Methods:From June 2019 to June 2021, random sampling was used to select 88 patients with scoliosis admitted to Beijing Chaoyang Hospital affiliated to Capital Medical University as the study subject. The patients were divided into a study group and a control group using a random number table method, with 44 patients in each group. The control group received preoperative ACBT training, while the study group conducted preoperative incentive spirometry combined with ACBT training. The respiratory function and blood gas indicators, including forced expiratory volume (FEV1) , forced vital capacity in one second (FVC) , vital capacity, and blood oxygen saturation (SpO 2) , were compared between the two groups before and two weeks after intervention. The Borg Scale and the Tilburg Frailty Indicator were used to evaluate the degree of dyspnea and physical frailty in the two groups of patients, and the incidence of complications and treatment satisfaction were compared between the two groups to analyze the effect of preoperative incentive spirometry combined with ACBT training on pulmonary function recovery after scoliosis surgery in adolescents. Results:Two weeks after operation, the FEV1/FVC ratio and SpO 2 in the two groups were lower than those before intervention, with a statistical difference ( P<0.05) ; The FEV1/FVC ratio and SpO 2 in the study group were statistically higher than those in the control group ( P<0.01) . The vital capacity of the control group decreased two weeks after operation compared to that before intervention, and the vital capacity of the study group was higher than that of the control group, with statistically significant differences ( P<0.05) . Two weeks after operation, the Borg Scale score of patients in both groups was higher than that before intervention, and that of the study group was lower than that of the control group, with statistically significant differences ( P<0.05) . There was no statistical difference in the incidence of postoperative complications between the two groups ( P>0.05) . The treatment satisfaction in the study group was 97.73% (43/44) , higher than 86.36% (38/44) in the control group, with a statistically significant difference ( P<0.05) . Conclusions:Preoperative incentive spirometry combined with ACBT training can promote postoperative pulmonary function recovery, alleviate frailty symptoms, and improve patient satisfaction in patients with scoliosis, which is worthy of clinical practice.
9.Internet-based assessment technology for age-related cognitive impairment

With the population aging,the number of elderly with Alzheimer's disease or other types of dementia is increasing,which seriously threatens the health and quality of life in elderly.The early screening and detection of dementia among high-risk population and appropriate interventional measures would delay the disease course,and maintain patients' self-care abilities and dignity.Computerized neuropsychological assessment devices are economic and efficient,and suitable for screening in large population.This article reviews the theory,characteristics and application of internet-based neuropsychological assessment for age-related cognitive impairment,as well as the prospect in the future studies.
10.Different off-loading mattresses with different repositioning intervals for preventing pressure ulcers in criti-cal patients:A randomized controlled trial

Objective The incidence rate of pressure ulcer is high in critical patients and off-loading mattresses and reposi-tioning are known as effective interventions for the prevention of pressure ulcers .However, evidence is lacking for selection of the right type of mattresses and suitable interval of repositioning .This study was to compare the effects of two types of off-loading mattresses with two different repositioning intervals in preventing pressure ulcers in critical patients . Methods According to the design of this ran-domized controlled trial , we made a training plan concerning the participants , methods of intervention and comparison , criteria and methods of observation , and methods of recording , and trained 26 nurses from 7 hospitals .Using non-inferiority design and the method of stratified blocked randomization , we divided 1194 patients with the risk of pressure ulcer into a trial group ( n=596) and a control group ( n=598) , a viscoelastic sponge mattress with every-four-hours repositioning used for the former and an automatic aeration mat-tress with every-two-hours repositioning for the latter , both for 7 successive days .We examined the patients every day , recorded the in-cidence and stages of pressure ulcer , and compared the data obtained between the two groups of patients . Results The total inci-dence rate of pressure ulcer was 1.09%(13/1194), significantly lower in the trial than in the control group (0.34%[2/596] vs 1.84%[11/598], P=0.012). Conclusion A viscoelastic sponge mattress with every-four-hours repositioning is superior to an automatic aeration mattress with every-two-hours repositioning and therefore is preferred to the latter in preventing the incidence of pressure ulcer in critical patients in the ICU .
